目的评价可视球囊扩张在经皮肾镜取石术(PCNL)中的可行性和安全性。方法回顾性分析2017年3月-2018年3月应用可视球囊扩张导管建立通道行PCNL的20例肾结石患者的病例资料。结果 20例均成功应用可视球囊建立皮肾通道,通道建立时间为5.0~1... 目的评价可视球囊扩张在经皮肾镜取石术(PCNL)中的可行性和安全性。方法回顾性分析2017年3月-2018年3月应用可视球囊扩张导管建立通道行PCNL的20例肾结石患者的病例资料。结果 20例均成功应用可视球囊建立皮肾通道,通道建立时间为5.0~13.5 min,平均(7.2±2.1)min,手术时间为30.0~120.0 min,平均(59.1±21.9)min。术后血尿(ClavienⅠ)2例,经保守治疗好转。发热(ClavienⅡ)1例,经抗生素治疗好转。1例残留结石行体外冲击波治疗。术后1个月无石率和手术成功率分别为90.00%和95.00%,术后3个月无石率为100.00%。结论可视球囊有助于提高建立皮肾通道的准确性和提高手术成功率。 展开更多

孕期肾绞痛是非产科急症住院的常见原因之一。因及时准确诊断困难,且涉及孕妇和胎儿健康风险,治疗妊娠期泌尿系结石对泌尿外科和产科医师仍是一个挑战。随着诊断和治疗技术的进步,尤其是内镜技术的进步,治疗观念和方法均有了转变。明确... 孕期肾绞痛是非产科急症住院的常见原因之一。因及时准确诊断困难,且涉及孕妇和胎儿健康风险,治疗妊娠期泌尿系结石对泌尿外科和产科医师仍是一个挑战。随着诊断和治疗技术的进步,尤其是内镜技术的进步,治疗观念和方法均有了转变。明确结石手术治疗成功率较高,应用也逐渐增加,多学科参与有助于降低手术风险;明确结石手术治疗能够提高孕期生活质量、避免产后再次手术等。泌尿科医师应该充分认识孕期泌尿系结石病的重要性和复杂性,实践中推荐多学科参与,并行个体化治疗。为了提高临床医师对妊娠期泌尿系结石的认知和临床诊治能力,在Pubmed和万方数据库进行相关文献搜索,对妊娠期泌尿系结石的成因、诊断、药物和手术治疗等现状进行综述。 展开更多

With the utilization of underground space,backward erosion piping(BEP)has been observed in many underground structures(e.g.,shield tunnels)founded on sandy aquifers.However,due to invisibility,the geometry of the eroded pipe and its spatial evolution with time during the piping process was still not clear.In this study,we developed a Hele-Shaw cell to visualize the dynamic progression of BEP.With imaging process technology,we obtained a typical process of BEP(the erosion process can be divided into a piping progression phase and a piping stabilization phase),quantitatively characterized the formation of erosion pipes,and compared the patterns of erosion(e.g.,the erosion area A and the maximum erosion radius R)that spontaneously develop under different fluxes of water.The most interesting finding is that the sand grains in a thicker Hele-Shaw model are easier to dislodge,which is possibly due to the granular system in a thicker model having more degrees of freedom,reducing the stability of the sand grains. 展开更多
