Due to the interaction of the Pacific plate, Eurasian plate and Philippine plate, its tectonic environ...The Sea of Japan is located in the southeast margin of Eurasia, in the triangle area of the western Pacific Ocean. Due to the interaction of the Pacific plate, Eurasian plate and Philippine plate, its tectonic environment is complex, forming a typical trench-arc-basin system. At present, 148 oil and gas fields have been discovered in Japan, with an oil and gas resource of 255.78×10^(6) t, showing a good prospect for oil and gas exploration. Based on the previous research and the recently collected geological and geophysical data, the characteristics of tectonic-sedimentary evolution and geothermal field in the basins around the Sea of Japan are analyzed. The results show that the tectonic evolution of the basin is mainly controlled by plate subduction and back-arc oceanic crust expansion, and it mainly undergone four tectonic-sedimentary evolution stages: Subduction period, basin development period, subsidence period and compression deformation period. The overall heat flow value of Japan Sea is high, and it is distributed annularly along Yamato Ridge. The geothermal heat flow value is about 50–130 MW/m^(2), and the average heat flow is75.9±19.8 MW/m^(2), which has a typical “hot basin ”. The high heat flow background provides unique thermal evolution conditions for hydrocarbon generation, which leads to the high temperature and rapid evolution. The authors summarized as “early hydrocarbon generation, rapid maturity and shallow and narrow hydrocarbon generation window”. The type of oil and gas is mainly natural gas, and it mainly distributed in Neogene oil and gas reservoirs. The trap types are mainly structural traps, lithologic traps and composite traps. In addition, the pre-Neogene bedrock oil and gas reservoirs also show a good exploration prospect. The resource prospecting indicates that Niigata Basin, Ulleung Basin and kitakami Basin are the main target areas for future exploration and development.展开更多

In early studi...Both platinum-based doublet chemotherapy(PBC) and epidermal growth factor receptor tyrosine kinase inhibitors(EGFR-TKIs) prolong the survival of patients with advanced non-small cell lung cancer(NSCLC). In early studies, most patients underwent PBC as first-line treatment, but not all patients could afford EGFR-TKIs as second-line treatment. To understand the impact of PBC and EGFR-TKIs on NSCLC prognosis, we evaluated the association between the receipt of both regimens and overall survival(OS). Using MEDLINE and EMBASE, we identified prospective, randomized, controlled phase III clinical trials in advanced NSCLC that met the inclusion criteria: in general population with advanced NSCLC, the percentage of patients treated with both PBC and EGFR-TKIs was available in the trial and OS was reported. After collecting data from the selected trials, we correlated the percentage of patients treated with both PBC and EGFR-TKIs with the reported OS, using a weighted analysis. Fifteen phase III clinical trials—involving 11,456 adult patients in 32 arms—were included in the analysis, including 6 trials in Asian populations and 9 in non-Asian(predominantly Caucasian) populations. The OS was positively correlated with the percentage of patients treated with both PBC and EGFR-TKIs(r = 0.797, P < 0.001). The correlation was obvious in the trials in Asian populations(r = 0.936, P < 0.001) but was not statistically significant in the trials in predominantly Caucasian populations(r = 0.116, P = 0.588). These results suggest that treatment with PBC and EGFR-TKIs may provide a survival benefit to patients with advanced NSCLC, highlighting the importance of having both modalities available for therapy.展开更多

AIM to evaluate the association of body mass index(b MI) with the overall survival of pancreatic ductal adenocarcinoma(PDAC) patients.METHODS A retrospective analysis of PDAC patients diagnosed in the National Cancer ...AIM to evaluate the association of body mass index(b MI) with the overall survival of pancreatic ductal adenocarcinoma(PDAC) patients.METHODS A retrospective analysis of PDAC patients diagnosed in the National Cancer Center of China between January 1999 and December 2014 was performed. these patients were categorized into four b MI groups(< 18.5, 18.5-22.9, 23-27.4 and ≥ 27.5 kg/m2). χ2 tests for comparison of the proportions of categorical variables, and Student's t-test or Mann-Whitney test for continuous variables were employed. Survival analysis was performed with the Kaplan-Meyer method. their HRs of mortality and 95%CIs were estimated using the Cox proportional hazards model.RESULTS With a median age of 59.6 years(range: 22.5-84.6 years), in total 1783 PDAC patients were enrolled in this study. their mean usual b MI was 24.19 ± 3.53 for the whole cohort. More than half of the patients(59.3%) experienced weight loss during the disease onset and progression. Compared with healthy-weight individuals, newly diagnosed patients who were overweight or obese had more severe weight loss during their disease onset and progression(P < 0.001). Individuals who were overweight or obese were associated with positive smoking history(P < 0.001). A significant difference in comorbidity of diabetes(P = 0.044) and coronary artery disease(P < 0.001) was identified between high b MI and normal-weight patients. After a median follow-up of 8 mo, the survival analysis showed no association between b MI and the overall survival(P = 0.90, n = 1783). When we stratified the whole cohort by pancreatic cancer stage, no statistically significant association between b MI and overall survival was found for resectable(P = 0.99, n = 217), unresectable locally advanced(P = 0.90, n = 316) and metastatic patients(P = 0.88, n = 1250), respectively. the results did not change when we used the b MI at diagnosis.CONCLUSION Our results showed no significance of b MI for the overall survival of PDAC patients.展开更多
Gemcitabine has high activity against nasopharyngeal carcinoma(NPC).The level of ribonucleotide reductase subunit M1(RRM1) expression is closely related to the efficacy of gemcitabine on non-small cell lung cancer and...Gemcitabine has high activity against nasopharyngeal carcinoma(NPC).The level of ribonucleotide reductase subunit M1(RRM1) expression is closely related to the efficacy of gemcitabine on non-small cell lung cancer and pancreatic cancer.However,the expression of RRM1 and its association with sensitivity to gemcitabine-based chemotherapy in advanced NPC is not known.In this study,we retrospectively collected 48 formalin-fixed,paraffin-embedded NPC tissues to evaluate the expression of RRM1 using immunohistochemistry.All patients were diagnosed and treated with gemcitabine-based chemotherapy at Sun Yat-sen University Cancer Center.RRM1 expression was positive in 17(35%) patients.RRM1 expression was not associated with sex,age,performance status,WHO histological type,number of distant metastases,previous treatment,or cycles of gemcitabine-based chemotherapy(P> 0.05).The progression-free survival of the RRM1-positive group was shorter than that of the RRM1-negative group(5 months vs.7 months,P = 0.036),and the response rate of the RRM1-positive group was somewhat lower than that of the RRM1-negative group(51.6% vs.35.3%,P = 0.278).There was no significant difference in median survival between the RRM1-positive and RRM1-negative groups(22 months vs.19 months,P = 0.540).Our results show that RRM1-negative expression is related with longer progression-free survival in advanced NPC patients treated with gemcitabine-based regimens.展开更多

A packing algorithm, considering the am- monium perchlorate (AP) and aluminum (A1) particles as spheres or discs is developed to match the size distribution and volume fraction of solid propellants. A homogenization theory is employed to compute the mean stress and strain of a representative volume element (RVE). Using the mean results, a suitable size of RVE is decided. Without consider- ing the interfaces between particles and matrix, several nu- merical simulations of the relaxation of propellants are per- formed. The relaxation effect and the nonlinear mechanical behavior of propellants which are dependent on the applied loads are discussed. A new technology named surface-based cohesive behavior is proposed to describe the phenomenon of particle dewetting consisting of two ingredients: a damage initiation criterion and a damage evolution law. Several ex- amples considering contact damage behavior are computed and also nonlinear behavior caused by damaged interfaces is discussed in this paper. Furthermore the effects of the criti- cal contact stress, initial contact stiffness and contact failure distance on the damaged interface model have been studied.展开更多

Individuals who underwent pancreatic resection between January2004 and December 2013 were identified and included in the study. Persons were classified as having a normal weight if their BMI was < 24 kg/m2 and overweight/obese if their BMI was ≥ 24 kg/m2 as defined by the International Life Sciences Institute Focal Point in China. A χ 2 test(for categorical variables) or a t test(for continuous variables) was used to examine the differences in patients' characteristics between normal weight and overweight/obese groups. Multiple logistic regression models were used to assess the associationsof postoperative complications, operative difficulty,length of hospital stay, and cost with BMI, adjusting for age, sex, and type of surgery procedures.RESULTS: A total of 362 consecutive patients with data available for BMI calculation underwent pancreatic resection for benign or malignant disease from January1, 2004 to December 31, 2013. Of the 362 patients,156 were overweight or obese and 206 were of normal weight. One or more postoperative complications occurred in 35.4% of the patients following pancreatic resection. Among patients who were overweight or obese, 42.9% experienced one or more complications,significantly higher than normal weight(29.6%)individuals(P = 0.0086). Compared with individuals who had normal weight, those with a BMI ≥ 24.0kg/m2 had higher delayed gastric emptying(19.9% vs5.8%, P < 0.0001) and bile leak(7.7% vs 1.9%, P =0.0068). There were no significant differences seen in pancreatic fistula, gastrointestinal hemorrhage, reoperation,readmission, or other complications. BMI did not show a significant association with intraoperative blood loss, operative time, length of hospital stay, or cost.CONCLUSION: Higher BMI increases the risk for postoperative complications after pancreatectomy in the Chinese population. The findings require replication in future studies with larger sample sizes.展开更多

The experimental setup and optimal operating parameters for the clock are introduced.The current fractional frequency stability is measured to be 1.3 × 10^-12/■ up to 20s and reaches 3.1 × 10^-13 at 200 s. We have thoroughly investigated the related noise sources that affect clock frequency stability at the 1s and 100s levels. The investigation shows that the laser frequency noise limits the clock frequency stability significantly. The clock performance can be further improved by technically upgrading the laser frequency stabilization setup.展开更多

AIM: To investigate the prognostic value of metastaticlymph node ratio(MLNR) in extrahepatic cholangiocarcinoma(ECC) patients undergoing radical resection.METHODS: Seventy-eight patients with ECC were enrolled.Associa...AIM: To investigate the prognostic value of metastaticlymph node ratio(MLNR) in extrahepatic cholangiocarcinoma(ECC) patients undergoing radical resection.METHODS: Seventy-eight patients with ECC were enrolled.Associations between various clinicopathologic factors and prognosis were investigated by KaplanMeier analyses.The Cox proportional-hazards model was used for multivariate survival analysis.RESULTS: The overall three- and five-year survival rates were 47.26% and 23.99%, respectively.MLNR of 0, 0-0.2, 0.2-0.5, and > 0.5 corresponded to fiveyear survival rates of 28.59%, 21.60%, 18.84%, and 10.03%, respectively.Univariate analysis showed that degree of tumor differentiation, lymph node metastasis, MLNR, tumor-node-metastasis(TNM) stage, and margin status were closely associated with postoperative survival in ECC patients(P < 0.05).Multivariate analysis showed that MLNR and TNM stage were independent prognostic factors after pancreaticoduodenectomy(HR = 2.13, 95%CI: 1.45-3.11; P < 0.01; and HR = 1.97, 95%CI: 1.17-3.31; P = 0.01, respectively).The median survival time for MLNR > 0.5, 0.2-0.5, 0-0.2, and 0 was 15 mo, 24 mo, 23 mo, and 35.5 mo, respectively.There were statistical differences in survival time between patients with different MLNR(χ2 = 15.38; P < 0.01).CONCLUSION: MLNR is an independent prognostic factor for ECC patients after radical resection and is useful for predicting postoperative survival.展开更多

In some cases, vascular resection and reconstruction are required for complete resection of pancreatic tumors. However, the optimum surgical method for venous management is controversial. Resection of the SMV without reconstruction during surgery for pancreatic tumors is rarely reported. Here we present the case of a 58-year-old woman with a giant pancreatic mucinous cystadenoma adhering to the SMV, who underwent an en bloc tumor resection, including the main trunk of the SMV and the spleen. No venous reconstruction was performed during surgery. No ischemic changes occurred in the bowel. The presence of several welldeveloped collateral vessels was shown by 3-dimensional computed tomography examination. The patient had an uneventful postoperative period and was discharged. This case indicated that the main trunk of the SMV can be resected without venous reconstruction if adequate collateralization has formed.展开更多
Introduction:An increasing number of targeted drugs have been tested for the treatment of nasopharyngeal carcinoma(NPC).However,targeted therapy-related oncogenic mutations have not been fully evaluated.This study aim...Introduction:An increasing number of targeted drugs have been tested for the treatment of nasopharyngeal carcinoma(NPC).However,targeted therapy-related oncogenic mutations have not been fully evaluated.This study aimed to detect targeted therapy-related oncogenic mutations in NPC and to determine which targeted therapy might be potentially effective in treating NPC.Methods:By using the SNaPshot assay,a rapid detection method,19 mutation hotspots in 6 targeted therapy-related oncogenes were examined in 70 NPC patients.The associations between oncogenic mutations and clinicopathologic factors were analyzed.Results:Among 70 patients,12(17.1%) had mutations in 5 oncogenes:7(10.0%) had v-kit Hardy-Zuckerman 4 feline sarcoma viral oncogene homolog(KIT) mutation,2(2.8%) had epidermal growth factor receptor(EGFR) mutation,1(1.4%) had phosphatidylinositol-4,5-bisphosphate 3-kinase,catalytic subunit alpha(PIK3CA) mutation,1(1.4%) had Kirsten rat sarcoma viral oncogene homolog(KRAS) mutation,and 1(1.4%) had simultaneous EGFR and v-Raf murine sarcoma viral oncogene homolog B1(BRAF) mutations.No significant differences were observed between oncogenic mutations and clinicopathologic characteristics.Additionally,these oncogenic mutations were not associated with tumor recurrence or metastasis.Conclusions:Oncogenic mutations are present in NPC patients.The efficacy of targeted drugs on patients with the related oncogenic mutations requires further validation.展开更多

A dielectric mode filter is installed on top of the VCSEL. The dielectric layer (SiO2) is deposited and patterned to modify the mirror reflectivity across the oxide aperture via antiphase reflections. This mode selection is nondestructive and universally applicable for other structures under single transverse mode. Destructive etching techniques (dry/wet) or epitaxial regrowth are also not required. This method simplifies the preparation process and improves the repeatability of the device. Measurements show that under continuous-wave current injection, the side-mode suppression ratio exceeds 30 dB.展开更多
The complete orbital and spin period evolutions of the double neutron star(NS)system PSR J0737-3039 are simulated from birth to coalescence,which include the two observed radio pulsars classified as primary NS PSR J07...The complete orbital and spin period evolutions of the double neutron star(NS)system PSR J0737-3039 are simulated from birth to coalescence,which include the two observed radio pulsars classified as primary NS PSR J0737-3039 A and companion NS PSR J0737-3039 B.By employing the characteristic age of PSR J0737-3039 B to constrain the true age of the double pulsar system,the initial orbital period and initial binary separation are obtained as 2.89 h and 1.44 x 106 km,respectively,and the coalescence age or the lifetime from the birth to merger of PSR J0737-3039 is obtained to be 1.38×10^(8)yr.At the last minute of coalescence,corresponding to the gravitational wave frequency changing from 20 Hz to1180 Hz,we present the binary separation of PSR J0737-3039 to be from 442 km to 30 km,while the spin periods of PSR J0737-3039 A and PSR J0737-3039 B are 27.10 ms and 4.63 s,respectively.From the standard radio pulsar emission model,before the system merged,the primary NS could still be observed by a radio telescope,but the companion NS had crossed the death line in the pulsar magnetic-field versus period(B-P)diagram at which point it is usually considered to cease life as a pulsar.This is the first time that the whole life evolutionary simulation of the orbit and spin periods for a double NS system is presented,which provides useful information for observing a primary NS at the coalescence stage.展开更多

So far among the nineteen pairs of detected double neutron star(DNS) systems, it is a usual fact that the first-born recycled pulsar is detected, however the youngest DNS system PSR J1906+0746, with the characteristic...So far among the nineteen pairs of detected double neutron star(DNS) systems, it is a usual fact that the first-born recycled pulsar is detected, however the youngest DNS system PSR J1906+0746, with the characteristic age of 113 kyr, is one of the three detected DNS as a non-recycled and second-born NS, which is believed to be formed by an electron capture or a low energy ultra-stripped iron core-collapse supernova(SN) explosion. The SN remnant around PSR J1906+0746 is too dim to be observed by optical telescopes, then its x-ray flux limit has been given by Chandra. A reference pulsar PSR J1509-5850 with the young characteristic age of 154 kyr was chosen as an object of comparison, which has an SN remnant observed by Chandra and is believed to be formed by iron core SN explosion. We impose a restriction on the maximum kinetic energy of electron-capture(EC) SN explosion that induces the formation of PSR J1906+0746. The estimated result is(4–8)×10^(50) erg(1 erg= 10-7 J), which is consistent with that of the published simulations of the EC process, i.e., a lower value than that of the conventional iron core SN explosion of(1–2)×10^(51) erg. As suggested, EC process for NS formation is pertained to the subluminous type Ic SN by the helium star with ONeMg core, thus for the first time we derived the kinetic energy of EC SN explosion of DNS, which may be reconciled with the recent observation of type Ic SN 2014 ft with kinetic energy of 2 × 10^(50) erg.展开更多
Fast radio bursts(FRBs) are extremely strong radio flares lasting several milliseconds,most of which come from unidentified objects at a cosmological distance.They can be apparently repeating or not.In this paper,we a...Fast radio bursts(FRBs) are extremely strong radio flares lasting several milliseconds,most of which come from unidentified objects at a cosmological distance.They can be apparently repeating or not.In this paper,we analyzed 18 repeaters and 12 non-repeating FRBs observed in the frequency bands of 400–800 MHz from Canadian Hydrogen Intensity Mapping Experiment(CHIME).We investigated the distributions of FRB isotropic-equivalent radio luminosity,considering the K correction.Statistically,the luminosity distribution can be better fitted by Gaussian form than by power-law.Based on the above results,together with the observed FRB event rate,pulse duration,and radio luminosity,FRB origin models are evaluated and constrained such that the gamma-ray bursts(GRBs) may be excluded for the non-repeaters while magnetars or neutron stars(NSs) emitting the supergiant pulses are preferred for the repeaters.We also found the necessity of a small FRB emission beaming solid angle(about 0.1 sr) from magnetars that should be considered,and/or the FRB association with soft gamma-ray repeaters(SGRs) may lie at a low probability of about 10%.Finally,we discussed the uncertainty of FRB luminosity caused by the estimation of the distance that is inferred by the simple relation between the redshift and dispersion measure(DM).展开更多
Objective:Based on the current evidence system to evaluate the effect of traditional Chinese medicine compound on the clinical efficacy of infertility patients with kidney deficiency.Methods:Search the China Knowledge...Objective:Based on the current evidence system to evaluate the effect of traditional Chinese medicine compound on the clinical efficacy of infertility patients with kidney deficiency.Methods:Search the China Knowledge Resources Database(CNKI),Chinese Science and Technology Journal Database(VIP),China Academic Journal Database(Wanfang),PubMed,EMBASE,Ovid.The included studies are randomized controlled trials.The experimental group is Chinese medicine compound or Chinese medicine compound combined with western medicine,and the control group is western medicine.Two reviewers independently extracted and evaluated the literature data according to the Cochrane manual standards,and used Rev Man5.3 software for data synthesis and Meta-analysis.Results:32 studies were selected,a total of 2715 cases of infertility patients.The clinical pregnancy rate of the experimental group and the control group[n=29,OR(95%CI)=2.47(2.07,2.96),P<0.00001],the clinical total effective rate[n=20,OR(95%CI)=3.77(2.83,5.03),P<0.00001],TCM syndrome score[n=14,MD(95%CI)=4.34(3.08,5.61),P<0.00001],ovulation rate[n=20,OR(95%CI)=2.21(1.85,2.63),P<0.00001],the occurrence rate of two-way BBT[n=11,OR(95%CI)=2.65(2.02,3.48),P<0.00001].The GRADE scoring results show that the level of evidence is low and very low.Conclusion:Traditional Chinese medicine compound can increase the clinical pregnancy rate and clinical total effective rate of infertility with kidney deficiency syndrome,improve the scores of Chinese medical syndromes,increase its ovulation rate and the occurrence rate of BBT bidirectional type,and confirm the clinical efficacy of Chinese medicine compound from the perspective of evidence-based medicine.展开更多

文摘The Sea of Japan is located in the southeast margin of Eurasia, in the triangle area of the western Pacific Ocean. Due to the interaction of the Pacific plate, Eurasian plate and Philippine plate, its tectonic environment is complex, forming a typical trench-arc-basin system. At present, 148 oil and gas fields have been discovered in Japan, with an oil and gas resource of 255.78×10^(6) t, showing a good prospect for oil and gas exploration. Based on the previous research and the recently collected geological and geophysical data, the characteristics of tectonic-sedimentary evolution and geothermal field in the basins around the Sea of Japan are analyzed. The results show that the tectonic evolution of the basin is mainly controlled by plate subduction and back-arc oceanic crust expansion, and it mainly undergone four tectonic-sedimentary evolution stages: Subduction period, basin development period, subsidence period and compression deformation period. The overall heat flow value of Japan Sea is high, and it is distributed annularly along Yamato Ridge. The geothermal heat flow value is about 50–130 MW/m^(2), and the average heat flow is75.9±19.8 MW/m^(2), which has a typical “hot basin ”. The high heat flow background provides unique thermal evolution conditions for hydrocarbon generation, which leads to the high temperature and rapid evolution. The authors summarized as “early hydrocarbon generation, rapid maturity and shallow and narrow hydrocarbon generation window”. The type of oil and gas is mainly natural gas, and it mainly distributed in Neogene oil and gas reservoirs. The trap types are mainly structural traps, lithologic traps and composite traps. In addition, the pre-Neogene bedrock oil and gas reservoirs also show a good exploration prospect. The resource prospecting indicates that Niigata Basin, Ulleung Basin and kitakami Basin are the main target areas for future exploration and development.
文摘Both platinum-based doublet chemotherapy(PBC) and epidermal growth factor receptor tyrosine kinase inhibitors(EGFR-TKIs) prolong the survival of patients with advanced non-small cell lung cancer(NSCLC). In early studies, most patients underwent PBC as first-line treatment, but not all patients could afford EGFR-TKIs as second-line treatment. To understand the impact of PBC and EGFR-TKIs on NSCLC prognosis, we evaluated the association between the receipt of both regimens and overall survival(OS). Using MEDLINE and EMBASE, we identified prospective, randomized, controlled phase III clinical trials in advanced NSCLC that met the inclusion criteria: in general population with advanced NSCLC, the percentage of patients treated with both PBC and EGFR-TKIs was available in the trial and OS was reported. After collecting data from the selected trials, we correlated the percentage of patients treated with both PBC and EGFR-TKIs with the reported OS, using a weighted analysis. Fifteen phase III clinical trials—involving 11,456 adult patients in 32 arms—were included in the analysis, including 6 trials in Asian populations and 9 in non-Asian(predominantly Caucasian) populations. The OS was positively correlated with the percentage of patients treated with both PBC and EGFR-TKIs(r = 0.797, P < 0.001). The correlation was obvious in the trials in Asian populations(r = 0.936, P < 0.001) but was not statistically significant in the trials in predominantly Caucasian populations(r = 0.116, P = 0.588). These results suggest that treatment with PBC and EGFR-TKIs may provide a survival benefit to patients with advanced NSCLC, highlighting the importance of having both modalities available for therapy.

文摘Gastrointestinal stromal tumors(GISTs)are mesenchymal tumors that arise from the gastrointestinal tract.In rare cases,these tumors are found in intra-abdominal sites unrelated to the gastrointestinal tract,such as the mesentery,omentum and retroperitoneum.However,pancreatic extra-gastrointestinal stromal tumors are extremely rare,with only 14 previous cases reported.A 61-year-old man with no clinical symptoms had a routine check-up,during which an abdominal mass located in the pancreas tail was detected.Abdominal surgery was performed with resection of the pancreas tail and the spleen,and he was diagnosed with lowrisk GISTs.Another 60-year-old man with no clinical symptoms underwent Computed tomography which revealed a well-demarcated tumor,6 cm in diameter,in the head of the pancreas.He was diagnosed with pancreatic GISTs.Here,we describe two rare cases of pancreatic GISTs and review the cases previously reported in the literature.

文摘AIM to evaluate the association of body mass index(b MI) with the overall survival of pancreatic ductal adenocarcinoma(PDAC) patients.METHODS A retrospective analysis of PDAC patients diagnosed in the National Cancer Center of China between January 1999 and December 2014 was performed. these patients were categorized into four b MI groups(< 18.5, 18.5-22.9, 23-27.4 and ≥ 27.5 kg/m2). χ2 tests for comparison of the proportions of categorical variables, and Student's t-test or Mann-Whitney test for continuous variables were employed. Survival analysis was performed with the Kaplan-Meyer method. their HRs of mortality and 95%CIs were estimated using the Cox proportional hazards model.RESULTS With a median age of 59.6 years(range: 22.5-84.6 years), in total 1783 PDAC patients were enrolled in this study. their mean usual b MI was 24.19 ± 3.53 for the whole cohort. More than half of the patients(59.3%) experienced weight loss during the disease onset and progression. Compared with healthy-weight individuals, newly diagnosed patients who were overweight or obese had more severe weight loss during their disease onset and progression(P < 0.001). Individuals who were overweight or obese were associated with positive smoking history(P < 0.001). A significant difference in comorbidity of diabetes(P = 0.044) and coronary artery disease(P < 0.001) was identified between high b MI and normal-weight patients. After a median follow-up of 8 mo, the survival analysis showed no association between b MI and the overall survival(P = 0.90, n = 1783). When we stratified the whole cohort by pancreatic cancer stage, no statistically significant association between b MI and overall survival was found for resectable(P = 0.99, n = 217), unresectable locally advanced(P = 0.90, n = 316) and metastatic patients(P = 0.88, n = 1250), respectively. the results did not change when we used the b MI at diagnosis.CONCLUSION Our results showed no significance of b MI for the overall survival of PDAC patients.
文摘Gemcitabine has high activity against nasopharyngeal carcinoma(NPC).The level of ribonucleotide reductase subunit M1(RRM1) expression is closely related to the efficacy of gemcitabine on non-small cell lung cancer and pancreatic cancer.However,the expression of RRM1 and its association with sensitivity to gemcitabine-based chemotherapy in advanced NPC is not known.In this study,we retrospectively collected 48 formalin-fixed,paraffin-embedded NPC tissues to evaluate the expression of RRM1 using immunohistochemistry.All patients were diagnosed and treated with gemcitabine-based chemotherapy at Sun Yat-sen University Cancer Center.RRM1 expression was positive in 17(35%) patients.RRM1 expression was not associated with sex,age,performance status,WHO histological type,number of distant metastases,previous treatment,or cycles of gemcitabine-based chemotherapy(P> 0.05).The progression-free survival of the RRM1-positive group was shorter than that of the RRM1-negative group(5 months vs.7 months,P = 0.036),and the response rate of the RRM1-positive group was somewhat lower than that of the RRM1-negative group(51.6% vs.35.3%,P = 0.278).There was no significant difference in median survival between the RRM1-positive and RRM1-negative groups(22 months vs.19 months,P = 0.540).Our results show that RRM1-negative expression is related with longer progression-free survival in advanced NPC patients treated with gemcitabine-based regimens.
文摘In the present work, a computational frame- work is established for multiscale modeling and analysis of solid propellants. A packing algorithm, considering the am- monium perchlorate (AP) and aluminum (A1) particles as spheres or discs is developed to match the size distribution and volume fraction of solid propellants. A homogenization theory is employed to compute the mean stress and strain of a representative volume element (RVE). Using the mean results, a suitable size of RVE is decided. Without consider- ing the interfaces between particles and matrix, several nu- merical simulations of the relaxation of propellants are per- formed. The relaxation effect and the nonlinear mechanical behavior of propellants which are dependent on the applied loads are discussed. A new technology named surface-based cohesive behavior is proposed to describe the phenomenon of particle dewetting consisting of two ingredients: a damage initiation criterion and a damage evolution law. Several ex- amples considering contact damage behavior are computed and also nonlinear behavior caused by damaged interfaces is discussed in this paper. Furthermore the effects of the criti- cal contact stress, initial contact stiffness and contact failure distance on the damaged interface model have been studied.

文摘AIM: To examine the impact of body mass index(BMI)on outcomes following pancreatic resection in the Chinese population.METHODS: A retrospective cohort study using prospectively collected data was conducted at the Cancer Hospital of the Chinese Academy of Medical Sciences, China National Cancer Center. Individuals who underwent pancreatic resection between January2004 and December 2013 were identified and included in the study. Persons were classified as having a normal weight if their BMI was < 24 kg/m2 and overweight/obese if their BMI was ≥ 24 kg/m2 as defined by the International Life Sciences Institute Focal Point in China. A χ 2 test(for categorical variables) or a t test(for continuous variables) was used to examine the differences in patients' characteristics between normal weight and overweight/obese groups. Multiple logistic regression models were used to assess the associationsof postoperative complications, operative difficulty,length of hospital stay, and cost with BMI, adjusting for age, sex, and type of surgery procedures.RESULTS: A total of 362 consecutive patients with data available for BMI calculation underwent pancreatic resection for benign or malignant disease from January1, 2004 to December 31, 2013. Of the 362 patients,156 were overweight or obese and 206 were of normal weight. One or more postoperative complications occurred in 35.4% of the patients following pancreatic resection. Among patients who were overweight or obese, 42.9% experienced one or more complications,significantly higher than normal weight(29.6%)individuals(P = 0.0086). Compared with individuals who had normal weight, those with a BMI ≥ 24.0kg/m2 had higher delayed gastric emptying(19.9% vs5.8%, P < 0.0001) and bile leak(7.7% vs 1.9%, P =0.0068). There were no significant differences seen in pancreatic fistula, gastrointestinal hemorrhage, reoperation,readmission, or other complications. BMI did not show a significant association with intraoperative blood loss, operative time, length of hospital stay, or cost.CONCLUSION: Higher BMI increases the risk for postoperative complications after pancreatectomy in the Chinese population. The findings require replication in future studies with larger sample sizes.

文摘A Cs vapor cell-based atomic clock that uses a lin‖lin pumping scheme with dispersion detection is reported. This atomic clock shows potential for high performance because of its high contrast pumping scheme, and for miniaturization because of its simple architecture. The experimental setup and optimal operating parameters for the clock are introduced.The current fractional frequency stability is measured to be 1.3 × 10^-12/■ up to 20s and reaches 3.1 × 10^-13 at 200 s. We have thoroughly investigated the related noise sources that affect clock frequency stability at the 1s and 100s levels. The investigation shows that the laser frequency noise limits the clock frequency stability significantly. The clock performance can be further improved by technically upgrading the laser frequency stabilization setup.
文摘After introducing the mechanical structure and working principle of electric power steering (EPS), theEPS dynamic model and the state space equation are established. Based on this model, a PID genetic algorithmcontrol strategy for EPS is put forward, i.e. , use the powerful search ability of genetic algorithm to optimize theparameters of the PID controller, the PID controller can achieve a better control effect. By using Matlab/Simulinksimulation and analysis, the simulation results show that the control strategy makes EPS fast response and stableoutput, the performance has been effectively improved. Therefore, it has laid the theoretical foundation to developproducts.
文摘AIM: To investigate the prognostic value of metastaticlymph node ratio(MLNR) in extrahepatic cholangiocarcinoma(ECC) patients undergoing radical resection.METHODS: Seventy-eight patients with ECC were enrolled.Associations between various clinicopathologic factors and prognosis were investigated by KaplanMeier analyses.The Cox proportional-hazards model was used for multivariate survival analysis.RESULTS: The overall three- and five-year survival rates were 47.26% and 23.99%, respectively.MLNR of 0, 0-0.2, 0.2-0.5, and > 0.5 corresponded to fiveyear survival rates of 28.59%, 21.60%, 18.84%, and 10.03%, respectively.Univariate analysis showed that degree of tumor differentiation, lymph node metastasis, MLNR, tumor-node-metastasis(TNM) stage, and margin status were closely associated with postoperative survival in ECC patients(P < 0.05).Multivariate analysis showed that MLNR and TNM stage were independent prognostic factors after pancreaticoduodenectomy(HR = 2.13, 95%CI: 1.45-3.11; P < 0.01; and HR = 1.97, 95%CI: 1.17-3.31; P = 0.01, respectively).The median survival time for MLNR > 0.5, 0.2-0.5, 0-0.2, and 0 was 15 mo, 24 mo, 23 mo, and 35.5 mo, respectively.There were statistical differences in survival time between patients with different MLNR(χ2 = 15.38; P < 0.01).CONCLUSION: MLNR is an independent prognostic factor for ECC patients after radical resection and is useful for predicting postoperative survival.
文摘In this paper, we investigate the control problem of autonomous bay parking system. We choose a referenced parking lot and define a suitable parking spot based on some measurements at various places. A kinetic model is set up for the convenience of analysis and simulation. The pose of the car during the parking procedure can be determined by the initial pose, the backward speed, and the steering angle of the wheel. Then, both a fuzzy speed controller and a fuzzy steering controller are designed for the bay parking. Finally, simulation results show the effectiveness of our designed controllers.
文摘Pancreatic tumors, with peri-pancreatic main vascular invasion, especially the superior mesenteric vein(SMV) or the portal vein, are very common. In some cases, vascular resection and reconstruction are required for complete resection of pancreatic tumors. However, the optimum surgical method for venous management is controversial. Resection of the SMV without reconstruction during surgery for pancreatic tumors is rarely reported. Here we present the case of a 58-year-old woman with a giant pancreatic mucinous cystadenoma adhering to the SMV, who underwent an en bloc tumor resection, including the main trunk of the SMV and the spleen. No venous reconstruction was performed during surgery. No ischemic changes occurred in the bowel. The presence of several welldeveloped collateral vessels was shown by 3-dimensional computed tomography examination. The patient had an uneventful postoperative period and was discharged. This case indicated that the main trunk of the SMV can be resected without venous reconstruction if adequate collateralization has formed.

文摘Introduction:An increasing number of targeted drugs have been tested for the treatment of nasopharyngeal carcinoma(NPC).However,targeted therapy-related oncogenic mutations have not been fully evaluated.This study aimed to detect targeted therapy-related oncogenic mutations in NPC and to determine which targeted therapy might be potentially effective in treating NPC.Methods:By using the SNaPshot assay,a rapid detection method,19 mutation hotspots in 6 targeted therapy-related oncogenes were examined in 70 NPC patients.The associations between oncogenic mutations and clinicopathologic factors were analyzed.Results:Among 70 patients,12(17.1%) had mutations in 5 oncogenes:7(10.0%) had v-kit Hardy-Zuckerman 4 feline sarcoma viral oncogene homolog(KIT) mutation,2(2.8%) had epidermal growth factor receptor(EGFR) mutation,1(1.4%) had phosphatidylinositol-4,5-bisphosphate 3-kinase,catalytic subunit alpha(PIK3CA) mutation,1(1.4%) had Kirsten rat sarcoma viral oncogene homolog(KRAS) mutation,and 1(1.4%) had simultaneous EGFR and v-Raf murine sarcoma viral oncogene homolog B1(BRAF) mutations.No significant differences were observed between oncogenic mutations and clinicopathologic characteristics.Additionally,these oncogenic mutations were not associated with tumor recurrence or metastasis.Conclusions:Oncogenic mutations are present in NPC patients.The efficacy of targeted drugs on patients with the related oncogenic mutations requires further validation.

文摘We establish a novel method of controlling the transverse modes of vertical cavity surface emitting lasers (VCSELs) to achieve 1 mW single-fundamental-mode lasing. A dielectric mode filter is installed on top of the VCSEL. The dielectric layer (SiO2) is deposited and patterned to modify the mirror reflectivity across the oxide aperture via antiphase reflections. This mode selection is nondestructive and universally applicable for other structures under single transverse mode. Destructive etching techniques (dry/wet) or epitaxial regrowth are also not required. This method simplifies the preparation process and improves the repeatability of the device. Measurements show that under continuous-wave current injection, the side-mode suppression ratio exceeds 30 dB.

文摘The complete orbital and spin period evolutions of the double neutron star(NS)system PSR J0737-3039 are simulated from birth to coalescence,which include the two observed radio pulsars classified as primary NS PSR J0737-3039 A and companion NS PSR J0737-3039 B.By employing the characteristic age of PSR J0737-3039 B to constrain the true age of the double pulsar system,the initial orbital period and initial binary separation are obtained as 2.89 h and 1.44 x 106 km,respectively,and the coalescence age or the lifetime from the birth to merger of PSR J0737-3039 is obtained to be 1.38×10^(8)yr.At the last minute of coalescence,corresponding to the gravitational wave frequency changing from 20 Hz to1180 Hz,we present the binary separation of PSR J0737-3039 to be from 442 km to 30 km,while the spin periods of PSR J0737-3039 A and PSR J0737-3039 B are 27.10 ms and 4.63 s,respectively.From the standard radio pulsar emission model,before the system merged,the primary NS could still be observed by a radio telescope,but the companion NS had crossed the death line in the pulsar magnetic-field versus period(B-P)diagram at which point it is usually considered to cease life as a pulsar.This is the first time that the whole life evolutionary simulation of the orbit and spin periods for a double NS system is presented,which provides useful information for observing a primary NS at the coalescence stage.

文摘Molecular dynamics simulation of a sympathetically-cooled ^(113)Cd^(+)ion crystal system is achieved.Moreover,the relationship between ions’axial temperature and different electric parameters,including radio frequency voltage and endcap voltage is depicted.Under stable trapping condition,optimum radio frequency voltage,corresponding to minimum temperature and the highest cooling efficiency,is obtained.The temperature is positively correlated with end-cap voltage.The relationship is also confirmed by a sympathetically-cooled ^(113)Cd^(+) microwave clock.The pseudo-potential model is used to illustrate the relationship and influence mechanism.A reasonable index,indicating ions’temperature,is proposed to quickly estimate the relative ions’temperature.The investigation is helpful for ion crystal investigation,such as spatial configuration manipulation,sympathetic cooling efficiency enhancement,and temporal evolution.

文摘So far among the nineteen pairs of detected double neutron star(DNS) systems, it is a usual fact that the first-born recycled pulsar is detected, however the youngest DNS system PSR J1906+0746, with the characteristic age of 113 kyr, is one of the three detected DNS as a non-recycled and second-born NS, which is believed to be formed by an electron capture or a low energy ultra-stripped iron core-collapse supernova(SN) explosion. The SN remnant around PSR J1906+0746 is too dim to be observed by optical telescopes, then its x-ray flux limit has been given by Chandra. A reference pulsar PSR J1509-5850 with the young characteristic age of 154 kyr was chosen as an object of comparison, which has an SN remnant observed by Chandra and is believed to be formed by iron core SN explosion. We impose a restriction on the maximum kinetic energy of electron-capture(EC) SN explosion that induces the formation of PSR J1906+0746. The estimated result is(4–8)×10^(50) erg(1 erg= 10-7 J), which is consistent with that of the published simulations of the EC process, i.e., a lower value than that of the conventional iron core SN explosion of(1–2)×10^(51) erg. As suggested, EC process for NS formation is pertained to the subluminous type Ic SN by the helium star with ONeMg core, thus for the first time we derived the kinetic energy of EC SN explosion of DNS, which may be reconciled with the recent observation of type Ic SN 2014 ft with kinetic energy of 2 × 10^(50) erg.

文摘Fast radio bursts(FRBs) are extremely strong radio flares lasting several milliseconds,most of which come from unidentified objects at a cosmological distance.They can be apparently repeating or not.In this paper,we analyzed 18 repeaters and 12 non-repeating FRBs observed in the frequency bands of 400–800 MHz from Canadian Hydrogen Intensity Mapping Experiment(CHIME).We investigated the distributions of FRB isotropic-equivalent radio luminosity,considering the K correction.Statistically,the luminosity distribution can be better fitted by Gaussian form than by power-law.Based on the above results,together with the observed FRB event rate,pulse duration,and radio luminosity,FRB origin models are evaluated and constrained such that the gamma-ray bursts(GRBs) may be excluded for the non-repeaters while magnetars or neutron stars(NSs) emitting the supergiant pulses are preferred for the repeaters.We also found the necessity of a small FRB emission beaming solid angle(about 0.1 sr) from magnetars that should be considered,and/or the FRB association with soft gamma-ray repeaters(SGRs) may lie at a low probability of about 10%.Finally,we discussed the uncertainty of FRB luminosity caused by the estimation of the distance that is inferred by the simple relation between the redshift and dispersion measure(DM).

文摘Objective:Based on the current evidence system to evaluate the effect of traditional Chinese medicine compound on the clinical efficacy of infertility patients with kidney deficiency.Methods:Search the China Knowledge Resources Database(CNKI),Chinese Science and Technology Journal Database(VIP),China Academic Journal Database(Wanfang),PubMed,EMBASE,Ovid.The included studies are randomized controlled trials.The experimental group is Chinese medicine compound or Chinese medicine compound combined with western medicine,and the control group is western medicine.Two reviewers independently extracted and evaluated the literature data according to the Cochrane manual standards,and used Rev Man5.3 software for data synthesis and Meta-analysis.Results:32 studies were selected,a total of 2715 cases of infertility patients.The clinical pregnancy rate of the experimental group and the control group[n=29,OR(95%CI)=2.47(2.07,2.96),P<0.00001],the clinical total effective rate[n=20,OR(95%CI)=3.77(2.83,5.03),P<0.00001],TCM syndrome score[n=14,MD(95%CI)=4.34(3.08,5.61),P<0.00001],ovulation rate[n=20,OR(95%CI)=2.21(1.85,2.63),P<0.00001],the occurrence rate of two-way BBT[n=11,OR(95%CI)=2.65(2.02,3.48),P<0.00001].The GRADE scoring results show that the level of evidence is low and very low.Conclusion:Traditional Chinese medicine compound can increase the clinical pregnancy rate and clinical total effective rate of infertility with kidney deficiency syndrome,improve the scores of Chinese medical syndromes,increase its ovulation rate and the occurrence rate of BBT bidirectional type,and confirm the clinical efficacy of Chinese medicine compound from the perspective of evidence-based medicine.
