目的分析术中体位管理对胃黏膜下巨大肿瘤患者在内镜下全层切除术(EFTR)中的影响。方法回顾性分析2017年12月-2019年7月在该院消化内科被诊断为胃黏膜下巨大肿瘤(肿瘤直径≥2.0 cm)并接受EFTR的患者。术中调整患者体位以避免肿瘤被胃腔... 目的分析术中体位管理对胃黏膜下巨大肿瘤患者在内镜下全层切除术(EFTR)中的影响。方法回顾性分析2017年12月-2019年7月在该院消化内科被诊断为胃黏膜下巨大肿瘤(肿瘤直径≥2.0 cm)并接受EFTR的患者。术中调整患者体位以避免肿瘤被胃腔内液体淹没,并分析患者的术中体位与肿瘤位置的关系以及对EFTR术后效果的影响。结果共37例患者纳入研究,术中体位包括:左侧卧位、仰卧位、俯卧位和右侧卧位。根据肿瘤的位置,采取对应的术中体位使肿瘤处于高位,术中视野暴露良好,镜下止血、缝合以及其他操作不受胃腔内液体的影响。所有患者均成功实施EFTR,未见大出血、肿瘤掉入腹腔、显性腹膜炎征象。结论EFTR治疗胃黏膜下巨大肿瘤时,使肿瘤处于高位的术中体位管理应受到重视。通过术前CT确认肿瘤位置或内镜下注水后观察手术视野,均可为患者体位的选择提供依据。该体位管理简单易行,不增加医疗费用,具有降低内镜下操作难度、增加手术安全性和缩短手术时间的优势。 展开更多

To solve the problem of immune incompatibility, nuclear transplantation has been envisaged as a means to produce cells or tissues for human autologous transplantation. Here we have derived embryonic stem cells by the transfer of human somatic nuclei into rabbit oocytes. The number of blastocysts that developed from the fused nuclear transfer was comparable among nuclear donors at ages of 5, 42, 52 and 60 years, and nuclear transfer (NT) embryonic stem cells (ntES cells) were subsequently derived from each of the four age groups. These results suggest that human somatic nuclei can form ntES cells independent of the age of the donor. The derived ntES cells are human based on karyotype, isogenicity, in situ hybridization, PGR and immunocytochemistry with probes that distinguish between the various species. The ntES cells maintain the capability of sustained growth in an undifferentiated state, and form embryoid bodies, which, on further induction, give rise to cell types such as neuron and muscle, as well as mixed cell populations that express markers representative of all three germ layers. Thus, ntES cells derived from human somatic cells by NT to rabbit eggs retain phenotypes similar to those of conventional human ES cells, including the ability to undergo multilineage cellular differentiation. 展开更多

In order to reduce or avoid the fluctuations from interface breakup, a meandering microchannel with curved multi-bends(44 turns) is fabricated, and investigations of scaling bubble/slug length in Taylor flow in a rectangular meandering microchannel are systematically conducted. Based on considerable experimental data,quantitative analyses for the influences of two important characteristic times, liquid phase physical properties and aspect ratio are made on the prediction criteria for the bubble/slug length of Taylor flow in a meandering microchannel. A simple principle is suggested to predict the bubble formation period by using the information of Rayleigh time and capillary time for six gas–liquid systems with average deviation of 10.96%. Considering physical properties of the liquid phase and cross-section configuration of the rectangular mcirochannel,revised scaling laws for bubble length are established by introducing Ca, We, Re and W/h whether for the squeezing-driven or shearing-driven of bubble break. In addition, a simple principle in terms of Garstecki-type model and bubble formation period is set-up to predict slug lengths. A total of 107 sets of experimental data are correlated with the meandering microchannel and operating range: 0.001 b CaTPb 0.05, 0.06 b WeTPb 9.0,18 b ReTPb 460 using the bubble/slug length prediction equation from current work. The average deviation between the correlated data and the experimental data for bubble length and slug length is about 9.42% and9.95%, respectively. 展开更多

The large amount of repeats, especially high copy repeats, in the genomes of higher animals and plants makes whole genome assembly (WGA) quite difficult. In order to solve this problem, we tried to identify repeats and mask them prior to assembly even at the stage of genome survey. It is known that repeats of different copy number have different probabilities of appearance in shotgun data, so based on this principle, we constructed a statistical model and inferred criteria for mathematically defined repeats (MDRs) at different shotgun coverages. According to these criteria, we developed software MDRmasker to identify and mask MDRs in shotgun data. With repeats masked prior to assembly, the speed of assembly was increased with lower error probability. In addition, clone-insert size affects the accuracy of repeat assembly and scaffold construction. We also designed length distribution of clone-inserts using our model. In our simulated genomes of human and rice, the length distribution of repeats is different, so their optimal length distributions of clone-inserts were not the same. Thus with optimal length distribution of clone-inserts, a given genome could be assembled better at lower coverage. 展开更多
