Scalp angiosarcoma with cervical lymph node metastasis is particularly rare. This article describes a rare case of angiosarcoma of the scalp, presenting as neck inflammation. Imaging procedures such as computed tomography (CT), magnetic resonance image (MRI) and ultrasonography (US) were not sufficient to diagnose this case. A needle biopsy provided an effective and accurate diagnosis of cervical lymph node metastasis. Additional observation and physical examination was required to diagnose the origin of the primary cancerous lesion. Once the angiosarcoma diagnosis was confirmed histologically, sequential weekly and monthly docetaxel (DTX) treatment was effective in preventing reoccurrence. Nonetheless, the optimization of angiosarcoma treatment remains a future goal. Although patients generally describe pain and swelling at the primary lesion site, this patient complained only of painful neck inflammation, without any indication of pain or swelling of the scalp. A revised diagnostic protocol should note that cervical lymph node metastasis of unknown primary origin may result from angiosarcoma of the scalp. 展开更多

Objective: This multicenter clinical study was to assess the clinical usability of an oral moisture-checking device in detecting the dry mouth patients and evaluating the optimal measurement site. Materials and Methods: The study group comprised 250 patients with dry mouth and 241 healthy volunteer subjects at 13 medical centers. This device was used to measure the moisture degrees of the lingual mucosa and the buccal mucosa. Subjective oral dryness, objective oral dryness, and saliva flow rates were also compared between the two groups. For statistical analysis, receiver-operating characteristic analysis was performed to calculate the area under the curve (AUC). Results: The moisture degree of the lingual mucosa was significantly lower in the dry mouth group (27.2 ± 4.9) than that in the healthy group (29.5 ± 3.1, AUC = 0.653). When a lingual mucosa moisture degree of 31.0 or higher was defined as normal, less than 27.0 as dry mouth, and 27.0 to less than 31.0 as borderline zone of dry mouth, both the sensitivity and the specificity for the diagnosis of dry mouth were close to 80%. Conclusion: These results suggest that the oral moisture-checking device is a usable screening device for dry mouth. 展开更多

Parotid lymph node (LN) metastasis occurs most commonly from cutaneous squamous cell carcinoma (CSCC) of the head and neck, but rarely from oral squamous cell carcinoma (OSCC). Here, we present a rare case of metastasis to the parotid LN in a patient with tongue cancer. Although usual extent of modified radical neck dissection that includes resection of the tail of the parotid gland is below the line of the mandibular angle and the mastoid process was performed, the parotid LN metastasis occurred. The patient was treated with partial parotidectomy and chemoradiotherapy. One year and two months later, there was no evidence of local recurrence, although multiple lung metastases were observed. We also review the literature on parotid LN metastasis from OSCC. 展开更多

Salivary duct carcinoma (SDC) is a rare, high-grade, aggressive malignancy, as having a morphologic resemblance to ductal carcinoma of the breast. We present here an atypical SDC of the parotid gland in a 63-year-old man that was diagnosed from immunohistochemical findings. The immunohistochemistry was similar in the parotid lesion and pulmonary metastases in autopsy specimens, but only the latter showed a cribriform pattern and comedolike necrosis. Although the parotid tumor was treated with concurrent chemoradiotherapy and adjuvant chemotherapy for over 21 months, the patient died of respiratory failure caused by advanced pulmonary metastases. 展开更多
