Validation of the index for the core competence of nurses leading discharge planning for older patients in China

Objective:With the continuous advancement of aging in China,the number of older inpatients has increased sharply.Older patients have a high demand for planning their discharge services.Nurses serve as the leader of discharge planning for patients;there is a lack of reliable evaluation tool to evaluate the core competitiveness of nurses who implement discharge planning for older patients in China.The purpose of this study was to validate the index for the core competence of nurses who lead discharge planning for older patients developed by a project team through the Delphi method in the early stage.Methods:A cross-sectional questionnaire survey with 3-stage stratified sampling was used to select 1075 nurses from 17 public general hospitals in Ningxia,China.Results:The index consists of 4 first-level indicators,13 second-level indicators,and 57 third-level indicators.The results show that 57 third-level indicators had good discrimination.With exploratory factor analysis(EFA),4 common factors that explained 72.79%of the total variance were extracted.The Cronbach's a was 0.98,and the retest reliability within a 14-d interval was 0.86.The confirmatory factor analysis(CFA)results show that the fit of the index structure was good.The criterion validity was 0.73.Conclusions:The index presented excellent psychometric proper ties and can be used to measure the core competence of nurses in implementing discharge planning for older patients in China. 展开更多

Background:Acute pancreatitis(AP),known for its rapid onset and significant incidence and mortality rates,presents a clinical challenge due to the limited availability of effective treatments and preventive measures.Anemarsaponin B(ASB)has emerged as a potential therapeutic agent,demonstrating capabilities in reducing immune inflammation,positioning it as a promising candidate for AP treatment.Methods:We investigated the effects of ASB on AP in mice,induced by caerulein and lipopolysaccharide(LPS).Peripheral blood samples were collected 24 h post-induction with caerulein to assess of key biomarkers including lipase,amylase,TNF-α,IL-1β,IL-6,SOD,and GSH-Px.A range of techniques such as immunohistochemistry staining,immunofluorescence staining,Western blotting,and quantitative Polymerase Chain Reaction(q-PCR),were employed to measure the expression of critical genes.Additionally,pancreas samples from the mice were harvested for microbiome and metabolome sequencing,with the data analyzed to understand the impact of ASB on AP.Results:Our study revealed that,compared to the sham group,the AP group exhibited significantly higher serum levels of lipase,amylase,and cytokines,while levels of SOD and GSH Px were notably lower.Treatment with ASB led to a substantial decrease in the levels of lipase,amylase,and cytokines,and an increase in SOD and GSH-Px levels.q-PCR analysis of pancreatic histiocytes corroborated these serum findings.Hematoxylin and Eosin(H&E)staining indicated significant alterations in the pathological changes in the pancreas,lungs,and small intestine of the AP model due to ASB.Immunofluorescence assays demonstrated that ASB alleviated the apoptosis of pancreatic histiocytes in the AP model.Western Blot and histological analyses showed that ASB reduced the phosphorylation of TAK,p38,JNK,and ERK proteins,as well as the levels of TRAF6 protein in the AP model.Furthermore,metabolomic and gut microbiota analysis identified 27 differential metabolites and 34 differential species.The combined metabolome and microbiome analysis suggested an association between certain microbes(e.g.,unclassified-Saprospiraceae and unclassified-Micavibrionales)and metabolites(e.g.,LysoPE(0:0/20:0),PC(DiMe(13,5)/PGJ2)),and Heptanoic acid,indicating potential pathways through which ASB may exert its therapeutic effects in AP.Conclusions:ASB exhibits therapeutic efficacy in treating AP induced by caerulein combined with lipopolysaccharide(LPS),primarily through modulating the mitogenactivated protein kinase(MAPK)signaling pathway.This discovery offers fresh perspectives for AP drug development,underscoring the potential of targeting specific cellular pathways.Additionally,the intricate interplay observed between the gut microbiota and metabolites following ASB treatment highlights novel therapeutic targets,suggesting that manipulating the gut microbiome and metabolome could be a viable strategy in AP management.These findings pave the way for further research into comprehensive treatment approaches that incorporate both pharmacological intervention and microbiota modulation. 展开更多

试验旨在探讨桑叶等复合粉剂对灵山麻鸡生长性能、消化酶活性、肠道菌群及血清免疫指标的影响。选取200只56日龄体重相近、健康的灵山麻鸡,随机分为4组,每组5个重复,每个重复10只鸡。对照组饲喂基础饲粮,试验1组、试验2组和试验3组分别... 试验旨在探讨桑叶等复合粉剂对灵山麻鸡生长性能、消化酶活性、肠道菌群及血清免疫指标的影响。选取200只56日龄体重相近、健康的灵山麻鸡,随机分为4组,每组5个重复,每个重复10只鸡。对照组饲喂基础饲粮,试验1组、试验2组和试验3组分别在基础饲粮中添加2.0%、1.0%和0.5%桑叶等复合粉剂。试验期42 d。结果显示,与对照组相比,试验2组麻鸡平均日增重升高了12.65%(P<0.05),料重比显著降低(P<0.05);各试验组大肠杆菌、沙门氏菌的数量及腹泻率均显著降低(P<0.05),十二指肠胰蛋白酶、淀粉酶、脂肪酶及盲肠纤维素酶活性显著提高(P<0.05),血清免疫球蛋白G(IgG)含量分别提高了40.69%、49.88%和25.72%(P<0.05),血清总蛋白(TP)、白蛋白(ALB)和球蛋白(GLB)含量显著升高(P<0.05),白球比(A/G)显著降低(P<0.05)。研究表明,桑叶等复合粉剂能够提高灵山麻鸡生长性能、消化酶活性,降低腹泻率,改善灵山麻鸡的血清生化指标和免疫功能,桑叶等复合粉剂适宜添加量为1.0%。 展开更多

[目的]为中药在养禽业上“减抗”“替抗”的推广应用提供参考。[方法]选取56日龄341 g左右的灵山麻鸡200羽,随机分为4组,每组10羽鸡、每组5个重复,对照组饲喂基础日粮,试验组在基础日粮上分别添加0.5%、1.0%、2.0%木棉花超微粉,饲养试验为42 d,测定试验鸡的生长性能、腹泻率、肠道菌群、血清免疫球蛋白等指标。[结果]与对照组相比,木棉花超微粉对试验鸡的平均日采食量、血清免疫球蛋白IgA、IgM均无显著影响,但均能不同程度地提高平均日增质量,中剂量试验组增加最多(12.67%);试验组灵山麻鸡血清中IgG含量分别显著提高36.13%、55.35%、26.18%,且中剂量试验组效果最好。[结论]在饲料中添加木棉花超微粉可促进灵山麻鸡生长,同时降低肠道菌群的丰度和腹泻率,显著提高血清免疫球蛋白IgG的含量,且以1%木棉花超微粉效果最佳。 展开更多

Product detection based on state abstraction technologies in the software product line(SPL)is more complex when compared to a single system.This variability constitutes a new complexity,and the counterexample may be valid for some products but spurious for others.In this paper,we found that spurious products are primarily due to the failure states,which correspond to the spurious counterexamples.The violated products correspond to the real counterexamples.Hence,identifying counterexamples is a critical problem in detecting violated products.In our approach,we obtain the violated products through the genuine counterexamples,which have no failure state,to avoid the tedious computation of identifying spurious products dealt with by the existing algorithm.This can be executed in parallel to improve the efficiency further.Experimental results showthat our approach performswell,varying with the growth of the system scale.By analyzing counterexamples in the abstract model,we observed that spurious products occur in the failure state.The approach helps in identifying whether a counterexample is spurious or genuine.The approach also helps to check whether a failure state exists in the counterexample.The performance evaluation shows that the proposed approach helps significantly in improving the efficiency of abstraction-based SPL model checking. 展开更多

采用人工紫外光老化和自然老化2种方法对天然橡胶(NR)进行老化研究,利用傅里叶变换红外光谱、核磁共振波谱及紫外-可见光吸收光谱测试法对老化产物进行表征,探究了NR的老化机理。结果表明,天然橡胶抗紫外光老化能力较差,在紫外光下照射3... 采用人工紫外光老化和自然老化2种方法对天然橡胶(NR)进行老化研究,利用傅里叶变换红外光谱、核磁共振波谱及紫外-可见光吸收光谱测试法对老化产物进行表征,探究了NR的老化机理。结果表明,天然橡胶抗紫外光老化能力较差,在紫外光下照射3 h,其分子结构即发生明显改变,而在自然老化的条件下,天然橡胶经过30 d结构才发生变化。天然橡胶的老化机理:双键α-H最先被活化,随后发生氧化生成醛酮等氧化产物,同时双键被加成,烯氢含量减少;老化过程中,氧化降解与交联反应同时存在,老化前期以降解为主,后期产生交联。以核磁谱图为依据对烯氢的氧化百分比进行定量分析,建立了本实验条件下2种老化方程,并得出了自然老化时间与人工紫外光老化时间的关系,天然橡胶经紫外光老化7 d约相当于自然老化下3个月的老化效果。 展开更多

不同组合微生物菌剂对牛粪堆肥效果的影响

为了探讨不同的微生物菌剂对牛粪堆肥的影响,该试验以80%牛粪+20%玉米秸秆为堆肥材料,设计4个堆肥处理,前3个处理组分别加入0.05%的菌剂A(黑曲霉+煎盘梭菌+普通高温放线菌)、0.05%菌剂B(黑曲霉+煎盘梭菌+热纤梭菌)和0.05%菌剂C(黑曲霉+... 为了探讨不同的微生物菌剂对牛粪堆肥的影响,该试验以80%牛粪+20%玉米秸秆为堆肥材料,设计4个堆肥处理,前3个处理组分别加入0.05%的菌剂A(黑曲霉+煎盘梭菌+普通高温放线菌)、0.05%菌剂B(黑曲霉+煎盘梭菌+热纤梭菌)和0.05%菌剂C(黑曲霉+煎盘梭菌),处理组4为不加菌剂的对照组,每个处理3个重复,经过31 d堆肥处理,以发酵温度、pH值、有机质含量、全氮含量和种子发芽指数5个指标进行跟踪测定。结果表明:菌剂A、B、C处理仅用1 d时间堆肥温度就升至50℃以上,高温阶段能持续时间分别为15 d、14 d、11 d便能完成发酵;发酵过程中菌剂A、B、C处理pH值基本低于对照组,pH值的变化呈现为下降、上升、下降至平稳的趋势。堆制第30 d与第0 d相比,所有处理的全氮含量比0 d时增加,对照组、菌剂A、B、C处理分别增加为11.01%、29.36%、33.94%和24.77%;所有处理组有机质含量均下降,对照组、菌剂A、B、C处理分别下降12.34%、26.14%、31.55%、19.88%。菌剂A、B、C处理的小白菜种子发芽指数达到74.63%以上,菌剂B处理最高达到97.73%。综合以上各项评价指标,菌剂B堆肥发酵效果优于菌剂A、C,菌剂B更适合牛粪堆肥发酵。 展开更多

[目的]研究饮水中添加乳酸对三黄鸡的生长性能和抗病力的影响。[方法]选取1日龄三黄鸡5 000羽,随机分为5组,1 000只/组。在饮水中加入乳酸调节饮水pH值,使得A组、B组、C组和D组的饮水pH值分别为2.0、3.0、4.0和5.0,每天试验鸡饮用酸化水4 h,对照组为正常饮水组。测定各组试验鸡体重和采食量,并记录每日病死数,计算平均日增重、饲料转化率和病死率。[结果]①平均日增重:1~35日龄段,C组试验鸡的平均日增重提高最明显,提高3.33%。36日龄后,仅B组和对照组相同水平,其他组均降低。②采食量和饲料转化率:B组、C组与对照组相比差异显著,A组、D组差异不显著。③病死率:A组、B组、C组、D组比对照组病死率分别减少3.7%、4.2%、4.7%和2.1%。[结论]在三黄鸡的饲养试验中,乳酸调节饮水pH值为4.0时能有效提高饲料转化率,明显提高1~35日龄段三黄鸡的平均日增重,明显降低三黄鸡的病死率,对于肉鸡促进生长和抗病力具有良好的应用前景。 展开更多

目的探讨水下内镜切除术在治疗消化道肿瘤中的疗效及安全性。方法通过美国国立医学图书馆搜索引擎PubMed,分别以"underwater endoscopic mucosal resection","underwater endoscopic submucosal dissection","... 目的探讨水下内镜切除术在治疗消化道肿瘤中的疗效及安全性。方法通过美国国立医学图书馆搜索引擎PubMed,分别以"underwater endoscopic mucosal resection","underwater endoscopic submucosal dissection","underwater peroral endoscopic myotomy"为关键词,检索出相关文献,汇总病例并进行分析。结果共检索出40篇相关文献,纳入452例病例,其中水下内镜下黏膜切除术(UEMR)413例,水下内镜黏膜下剥离术(UESD)37例,水下经口内镜下肌切开术(UPOEM)2例。413例UEMR患者一共有468个病灶,切除成功率为98.5%,其中整块切除率达61.1%,整块切除者的R0切除率为94.3%;37例UESD患者共有39个病灶,所有病灶均经UESD完整剥离;2例UPOEM患者成功行水下黏膜下隧道建立及肌切开。全组术后并发症较低,以出血多见,其他包括穿孔、十二指肠腔狭窄、低钠血症、吸入性肺炎和自限性息肉切除术后综合征。结论水下内镜切除技术是一项较为安全有效的治疗手段,易于掌握且操作方便。今后仍需要更多的临床研究以及长期随访资料,以明确水下内镜切除技术与常规内镜下黏膜切除术(EMR)或内镜黏膜下剥离术(ESD)相比的优势及适应证,便于临床应用及推广。 展开更多

以1,4-环己二酮-2,5-二甲酸二甲酯(DMSS)为起始原料,采用两步法成功制备了2,5-二甲氧基对苯二甲酸二甲酯(DMDMT)。分别以DMDMT和不同碳链的脂肪二醇为单体,在三氧化二锑的催化作用下,采用熔融共聚酯交换法合成了4种由脂肪族二元醇-芳香... 以1,4-环己二酮-2,5-二甲酸二甲酯(DMSS)为起始原料,采用两步法成功制备了2,5-二甲氧基对苯二甲酸二甲酯(DMDMT)。分别以DMDMT和不同碳链的脂肪二醇为单体,在三氧化二锑的催化作用下,采用熔融共聚酯交换法合成了4种由脂肪族二元醇-芳香族二元酸构成的聚酯。经过溶解、沉淀、离心等分离手段,得到目标聚酯a^d。通过红外光谱、核磁共振氢谱、凝胶渗透色谱、差示扫描量热分析和热重分析对其进行表征和分析。聚酯a^d的PDI在1.95~2.70之间,数均相对分子质量(Mn)在10000~15200之间。研究了聚合物中分子主链柔顺性对聚合物的热性能的影响,其中,聚酯a的熔点(Tm)较高,达到90℃,其初始分解温度(T5%)高达363℃;随着脂肪族碳链的碳数增加,所合成的聚酯a^d的熔点和初始分解温度(T5%)下降,这为新型脂肪族-芳香族聚酯的分子设计与合成提供了有益的参考。 展开更多

AIM To investigate the incidence and risk factors of portosplenomesenteric vein thrombosis(PSMVT) in the early stage of severe acute pancreatitis(SAP).METHODS Patients with SAP in a tertiary care setting from January 2014 to December 2016 were retrospectively reviewed. All contrast-enhanced computed tomography(CT) studies were reassessed and reviewed. Clinical outcome measures were compared between SAP patients with and without PSMVT in the early stage of the disease. Univariate and multivariate logistic regression analyses were sequentially performed to assess potential risk factors for the development of PSMVT in SAP patients. A receiver operating characteristic(ROC) curve was generated for the qualifying independent risk factors.RESULTS Twenty-five of the one hundred and forty(17.86%) SAP patients developed PSMVT 6.19 ± 2.43 d after acute pancreatitis(AP) onset. PSMVT was confirmed by contrast-enhanced CT. Multivariate stepwise logistic regression analyses showed that Balthazar's CT severity index(CTSI) scores [odds ratio(OR): 2.742; 95% confidence interval(CI): 1.664-4.519; P = 0.000], hypoalbuminemia(serum albumin level < 25 g/L)(OR: 32.573; 95%CI: 2.711-391.353; P = 0.006) and gastrointestinal wall thickening(OR: 4.367, 95%CI: 1.218-15.658; P = 0.024) were independent risk factors for PSMVT developed in patients with SAP. The area under the ROC curve for Balthazar's CTSI scores was 0.777(P = 0.000), the sensitivity was 52%, and the specificity was 93% at a cut-off value of 5.5.CONCLUSION High Balthazar's CTSI scores, hypoalbuminemia and gastrointestinal wall thickening are independent risk factors for PSMVT developed in the early stage of SAP. 展开更多

BACKGROUND Gastrointestinal(GI)dysfunction is a common and important complication of acute pancreatitis(AP),especially in patients with severe AP.Despite this,there is no consensus means of obtaining a precise assessment of GI function.AIM To determine the association between acute gastrointestinal injury(AGI)grade and clinical outcomes in critically ill patients with AP.METHODS Patients with AP admitted to our pancreatic intensive care unit from May 2017 to May 2019 were enrolled.GI function was assessed according to the AGI grade proposed by the European Society of Intensive Care Medicine in 2012,which is mainly based on GI symptoms,intra-abdominal pressure,and feeding intolerance in the first week of admission to the intensive care unit.Multivariate logistic regression analysis was performed to assess the association between AGI grade and clinical outcomes in critically ill patients with AP.RESULTS Among the 286 patients included,the distribution of patients with various AGI grades was 34.62%with grade I,22.03%with grade II,32.52%with grade III,and 10.84%with grade IV.The distribution of mortality was 0%among those with grade I,6.35%among those with grade II,30.11%among those with grade III,and 61.29%among those with grade IV,and AGI grade was positively correlated with mortality(χ2=31.511,P<0.0001).Multivariate logistic regression analysis showed that age,serum calcium level,AGI grade,persistent renal failure,and persistent circulatory failure were independently associated with mortality.Compared with the Acute Physiology and Chronic Health Evaluation II score(area under the curve:0.739 vs 0.854;P<0.05)and Ranson score(area under the curve:0.72 vs 0.854;P<0.01),the AGI grade was more useful for predicting mortality.CONCLUSION AGI grade is useful for identifying the severity of GI dysfunction and can be used as a predictor of mortality in critically ill patients with AP. 展开更多

This paper proposes a novel method of lane detection,which adopts VGG16 as the basis of convolutional neural network to extract lane line features by cavity convolution,wherein the lane lines are divided into dotted lines and solid lines.Expanding the field of experience through hollow convolution,the full connection layer of the network is discarded,the last largest pooling layer of the VGG16 network is removed,and the processing of the last three convolution layers is replaced by hole convolution.At the same time,CNN adopts the encoder and decoder structure mode,and uses the index function of the maximum pooling layer in the decoder part to upsample the encoder in a counter-pooling manner,realizing semantic segmentation.And combined with the instance segmentation,and finally through the fitting to achieve the detection of the lane line.In addition,the currently disclosed lane line data sets are relatively small,and there is no distinction between lane solid lines and dashed lines.To this end,our work made a lane line data set for the lane virtual and real identification,and based on the proposed algorithm effective verification of the data set achieved by the increased segmentation.The final test shows that the proposed method has a good balance between lane detection speed and accuracy,which has good robustness. 展开更多
