目的探讨北京地区人群血清多种维生素含量与变应性鼻炎的相关性。方法随机选取2017年11月-2018年12月就诊于解放军总医院第一医学中心耳鼻咽喉头颈外科医学部的北京地区变应性鼻炎患者214例(变应性鼻炎组)和健康查体志愿者41例(对照组)...目的探讨北京地区人群血清多种维生素含量与变应性鼻炎的相关性。方法随机选取2017年11月-2018年12月就诊于解放军总医院第一医学中心耳鼻咽喉头颈外科医学部的北京地区变应性鼻炎患者214例(变应性鼻炎组)和健康查体志愿者41例(对照组),抽血后进行维生素A、维生素B1、维生素B2、维生素B6、维生素B9、维生素B12、维生素C、维生素D、维生素E的检测。结果变应性鼻炎组患者血液样本的维生素B6[Md(IQR):38.88(35.54,40.52)nmol/L vs 37.01(27.56,39.57)nmol/L]、维生素B12[Md(IQR):504.83(443.43,574.64)pg/ml vs 464.58(411.81,533.32)pg/ml]和维生素D[Md(IQR):49.50(40.52,61.30)nmol/L vs 43.50(36.08,50.85)nmol/L]的含量总体高于对照组,差异均有统计学意义(P<0.05),三种维生素的中位数均在正常范围内。两组维生素A、维生素B1、维生素B2、维生素B9、维生素C和维生素E含量的差异均无统计学意义(P>0.05)。结论变应性鼻炎患者血液中的维生素B6、维生素B12及维生素D含量高于对照组,但是否与变应性鼻炎相关,还需进一步研究。展开更多

Previous studies on the synergistic effe...It is known that aminoglycoside antibiotics can damage the vestibular and auditory sensory epithelia, and the loop diuretics can enhance the ototoxic effect of aminoglycosides. Previous studies on the synergistic effect of these two types of drugs have used mice, guinea pigs and cats, but not rats. The aim of this study was to determine this synergistic effects in rat cochleae. Rats received intravenous injections of different doses of furosemide and/or intramuscular injections of kanamycin sulfate. Au- ditory brainstem response (ABR), scanning electron microscopy (SEM) and immunocytochemistry were used to determine the effects of drug administration. In the group receiving combined administration of furosemide and kanamycin, the ABR thresh- old showed significant elevation 3 days after drug administration, greater than single drug administration. The hair cells showed various degrees of injury from the apical turn to the basal turn of the cochlea and from the outer hair cells to the inner hair cells. Neuron fibers of the hair cells showed significant loss 7 days after the drug administration, but the number of spiral ganglia did not decrease and supporting cells showed no signs of injury. Our study suggest that combined administration of fu- rosemide and kanamycin has an synergistic ototoxic effect, and can result in hair cell loss and hearing loss in rats.展开更多
目的:分析抗中性粒细胞胞浆抗体(anti-neutrophil cytoplasmic antibody,ANCA)相关性血管炎耳部受累的临床特点及治疗预后。方法:回顾性分析解放军总医院第一医学中心诊治的40例ANCA相关性血管炎伴耳部症状患者的临床表现、实验室检查...目的:分析抗中性粒细胞胞浆抗体(anti-neutrophil cytoplasmic antibody,ANCA)相关性血管炎耳部受累的临床特点及治疗预后。方法:回顾性分析解放军总医院第一医学中心诊治的40例ANCA相关性血管炎伴耳部症状患者的临床表现、实验室检查、纯音测听、声导抗检查、颞骨CT及治疗预后。结果:以耳部表现为首发症状者11例(27.5%),其中最常见的耳部症状为听力下降,其余有耳闷、耳溢液以及耳鸣。40例患者中,有35例出现听力下降,传导性耳聋19例(47.5%)、感音神经性耳聋9例(22.5%)、混合性耳聋7例(17.5%);5例患者仅有耳闷、耳鸣,听力检查无异常(12.5%)。40例患者均有多器官受累,最常见于呼吸系统;且ANCA检查均阳性;治疗包括全身使用糖皮质激素、免疫抑制剂、生物制剂。经治疗后,3例听力恢复正常且无波动(7.5%),22例症状减轻(55.0%),6例听力反复波动(15.0%),9例无明显改善(22.5%)。结论:ANCA相关性血管炎中耳炎(otitis media with AAV,OMAAV)疾病改变是动态过程:传导性耳聋(分泌性中耳炎)可以是前期表现,后期可出现双耳混合性耳聋。耳鼻喉科医生在诊治分泌性中耳炎反复发作的患者时,需考虑OMAAV诊断,多系统症状问诊及ANCA检查可帮助鉴别,早期全身用药及免疫抑制剂或生物制剂的应用可帮助患者听力恢复。展开更多
