BACKGROUND Toxic epidermal necrolysis(TEN)is a life-threatening dermatological emergency mainly induced by drug hypersensitivity reactions.Standard management includes discontinuation of culprit drug and application of immunomodulatory therapy.However,mortality remains high due to complications like septic shock and multiorgan failures.Innovative approaches for skin care are crucial.This report introduces borneol-gypsum,a traditional Chinese drug but a novel dressing serving as an adjuvant of TEN therapy,might significantly improve skin conditions and patient outcomes in TEN.CASE SUMMARY A 38-year-old woman diagnosed with eosinophilic granulomatosis with polyangiitis experienced gangrenous complications and motor nerve involvement.After initial treatment of high-dose corticosteroids and cyclophosphamide,symptom of foot drop improved,absolute eosinophil counts decreased,while limb pain sustained.Duloxetine was added to alleviate her symptom.Subsequently,TEN developed.Additional topical application of borneol-gypsum dressing not only protected the skin lesions from infection but also significantly eased localized pain.This approach demonstrated its merit in TEN management by promoting skin healing and potentially reducing infection risks.CONCLUSION Borneol-gypsum dressing is a promising adjuvant that could significantly improve TEN management,skin regeneration,and patient comfort. 展开更多

目的探讨经导管肝动脉化疗栓塞术(TACE)联合鸦胆子油乳液静脉滴注对原发性肝癌患者的临床疗效及对血管内皮生长因子(VEGF)表达水平的影响。方法选取2012年3月-2013年3月该院收治的肝癌患者88例,使用随机数字表法分为研究组及对照组,每组... 目的探讨经导管肝动脉化疗栓塞术(TACE)联合鸦胆子油乳液静脉滴注对原发性肝癌患者的临床疗效及对血管内皮生长因子(VEGF)表达水平的影响。方法选取2012年3月-2013年3月该院收治的肝癌患者88例,使用随机数字表法分为研究组及对照组,每组各44例,对照组患者单用TACE治疗,研究组采用TACE加鸦胆子油乳液静脉滴注。所有患者治疗至少3个疗程并随访6~37个月。比较两组患者治疗后的临床疗效、不良反应发生率、中位疾病进展时间(TTP)及总生存率,同时比较治疗前后肝功能、血浆内毒素及VEGF的水平。结果观察组的治疗总有效率及中位TTP高于对照组(P<0.01);治疗后,观察组的不良反应发生率低于对照组(P<0.01);两组肝功能指标、血浆内毒素及VEGF的水平均升高,但观察组低于对照组(P<0.01);两组1~3年的总生存率相比差异无统计学意义(P>0.05)。结论鸦胆子油乳液辅助TACE治疗原发性肝癌可改善患者的近期疗效和肝功能,延长TPP时间,降低血浆内毒素、VEGF的水平,是一种安全有效的方法。 展开更多

目的分析经脐单孔腹腔镜术(UOTLA)治疗小儿复杂性阑尾炎的疗效。方法总结2012年1月-2015年10月在该院治疗的78例复杂性阑尾炎患者临床资料,其中单孔腹腔镜手术的44例患者为UOTLA组,开腹手术治疗的34例患者为开腹阑尾切除术组(OA),对两... 目的分析经脐单孔腹腔镜术(UOTLA)治疗小儿复杂性阑尾炎的疗效。方法总结2012年1月-2015年10月在该院治疗的78例复杂性阑尾炎患者临床资料,其中单孔腹腔镜手术的44例患者为UOTLA组,开腹手术治疗的34例患者为开腹阑尾切除术组(OA),对两组患者的手术时间、术后住院时间、术后腹腔脓肿、切口感染、早期炎性肠梗阻和疼痛程度等指标进行统计分析。结果化验指标中C反应蛋白(CRP)在两组中无明显差异,外周血白细胞总数(WBC)在腹腔镜组下降较开腹组明显;手术相关指标中,UOTLA组手术时间较OA组缩短,但差异无统计学意义[(66.59±33.24)vs(72.86±30.36)min,P>0.05],术后住院时间更短[(8.21±1.67)vs(9.21±2.01)d,P<0.05]。术后腹腔脓肿UOTLA组3例,OA组1例(P>0.05);切口感染:UOTLA组6例,OA组9例(P>0.05);早期炎性肠梗阻:UOTLA组1例,OA组5例(P>0.05);疼痛程度比较,UOTLA组较OA组术后恢复正常活动时间明显缩短(P<0.05),UOTLA组的平均住院费用与OA组比较,差异无统计学意义[(10 639.37±2 970.92)vs(10 765.04±2 902.64)元,P>0.05]。结论 UOTLA治疗小儿复杂性阑尾炎不但具有创伤小、恢复快、住院时间短和美容效果等优点,而且并没有明显增加手术费用,术后并发症也没有明显增加,是一种安全有效的手术方式,可用于治疗小儿化脓穿孔性阑尾炎及坏疽穿孔性阑尾炎等相对复杂的阑尾炎。 展开更多

AIM:To evaluate the feasibility and efficacy of endoscopic submucosal dissection(ESD) for foregut neuroendocrine tumors(NETs).METHODS:From April 2008 to December 2010,patients with confirmed histological diagnosis of foregut NETs were included.None had regional lymph node enlargement or distant metastases to the liver or lung on preoperative computerized tomography scanning or endoscopic ultrasonography(EUS).ESD was attempted under general anesthesia.After making several marking dots around the lesion,a mixture solution was injected into the submucosa.The mucosa was incised outside the marking dots.Dissection of the submucosal layer beneath the tumor was performed under direct vision to achieve complete en bloc resection of the specimen.Tumor features,clinicopathological characteristics,complete resection rate,and complications were evaluated.Foregut NETs were graded as G1,G2,or G3 on the basis of proliferative activity by mitotic count or Ki-67 index.All patients underwent regular follow-up to evaluate for any local recurrence or distant metastasis.RESULTS:Those treated by ESD included 24 patients with 29 foregut NETs.The locations of the 29 lesions are as follows:esophagus(n = 1),cardia(n = 1),stomach(n = 23),and duodenal bulb(n = 4).All lesions were found incidentally during routine upper gastrointestinal endoscopy for other indications,and none had symptoms of carcinoid syndrome.Preoperative EUS showed that all tumors were confined to the submucosa.Among the 24 gastric lesions,16 lesions in 11 patients were type I gastric NETs arising in chronic atrophic gastritis with hypergastrinemia,while the other 8 solitary lesions were type Ⅲ because of absence of atrophic gastritis in these cases.All of the tumors were removed in an en bloc fashion.The average maximum diameter of the lesions was 9.4 mm(range:2-30 mm),and the procedure time was 20.3 min(range:10-45 min).According to the World Health Organization 2010 classification,histological evaluation determined that 26 lesions were NET-G1,2 gastric lesions were NET-G2,and 1 esophageal lesion was neuroendocrine carcinoma(NEC).Complete resection was achieved in 28 lesions(28/29,96.6%),and all of them were confined to the submucosa in histopathologic assessment with no lymphovascular invasion.The remaining patient with NEC underwent additional surgery because the resected specimens revealed angiolymphatic and muscularis invasion,as well as incomplete resection.Delayed bleeding occurred in 1 case 3 d after ESD,which was managed by endoscopic treatment.There were no procedure-related perforations.During a mean follow-up period of 24.4 mo(range:12-48 mo),local recurrence occurred in only 1 patient 7 mo after initial ESD.This patient successfully underwent repeat ESD.Metastasis to lymph nodes or distal organs was not observed in any patient.No patients died during the study period.CONCLUSION:ESD appears to be a safe,feasible,and effective procedure for providing accurate histopathological evaluations and curative treatment for eligible foregut NETs. 展开更多

In this study, we hypothesized that an increase in integrin αβand its co-activator vascular endothelial growth factor play important neuroprotective roles in ischemic injury. We performed ischemic preconditioning with bilateral common carotid artery occlusion for 5 minutes in C57BL/6J mice. This was followed by ischemic injury with bilateral common carotid artery occlusion for 30 minutes. The time interval between ischemic preconditioning and lethal ischemia was 48 hours. Histopathological analysis showed that ischemic preconditioning substantially diminished damage to neurons in the hippocampus 7 days after ischemia. Evans Blue dye assay showed that ischemic preconditioning reduced damage to the blood-brain barrier 24 hours after ischemia. This demonstrates the neuroprotective effect of ischemic preconditioning. Western blot assay revealed a significant reduction in protein levels of integrin αβ, vascular endothelial growth factor and its receptor in mice given ischemic preconditioning compared with mice not given ischemic preconditioning 24 hours after ischemia. These findings suggest that the neuroprotective effect of ischemic preconditioning is associated with lower integrin αβand vascular endothelial growth factor levels in the brain following ischemia. 展开更多

背景上世纪发现幽门螺杆菌(Helicobacter pylori, H. pylori)后,人们发现消化性溃疡90%以上的人均伴有H. pylori感染.然而近年来的流行病学研究显示, H. pylori与消化性溃疡并非联系那么密切,越来越多的H. pylori阴性胃溃疡在世界各地发... 背景上世纪发现幽门螺杆菌(Helicobacter pylori, H. pylori)后,人们发现消化性溃疡90%以上的人均伴有H. pylori感染.然而近年来的流行病学研究显示, H. pylori与消化性溃疡并非联系那么密切,越来越多的H. pylori阴性胃溃疡在世界各地发现,且近年来呈逐年上升趋势.这可能与近年来H. pylori根治疗法的广泛应用,H. pylori感染率的下降有关,也可能是H. pylori阴性胃溃疡与H. pylori阳性溃疡具有独立的病理生理机制.目的探讨艾司奥美拉唑联合康复新治疗H. pylori阴性胃溃疡的疗效及炎性因子表达影响.方法选择天津医科大学第二医院2017-05/2018-05消化内科门诊及住院收治的胃溃疡患者共128例作为研究对象,按照随机数字法将其分为观察组和对照组,观察组对照组给予艾司奥美拉唑治疗,观察组在对照组的基础上给予康复新治疗,评价两者临床疗效、测量炎性因子白细胞介素-1β(interleukin-1β,IL-1β),白细胞介素-6(interleukin-6,IL-6)及肿瘤坏死因子-α(tumor necrosis factor-α, TNF-α)、γ-干扰素(interferon-γ,IFN-γ)及C反应蛋白(C-reactiveprotein,CRP)水平,临床症状评分,观察有无不良反应和复发情况.结果组内比较,两组患者经治疗后临床症状积分均较治疗前明显下降(P<0.05),治疗后观察组各临床症状积分及总临床症状积分均较对照组明显下降(P<0.05);治疗后IL-1β、IL-6、TNF-α、IFN-γ、CRP均较治疗前明显下降(P<0.05),观察组IL-1β、IL-6、TNF-α、IFN-γ、CRP均显著低于对照组(P <0.05);观察组总有效率为(93.75%)显著高于对照组有效率(73.44%);两组不良反应和溃疡复发情况无统计学差异(P>0.05).结论艾司奥美拉唑联合康复新治疗H. pylori阴性胃溃疡的疗效明显,可显著降低炎性因子水平,促进溃疡的早期愈合,改善患者的预后. 展开更多

Objective:Cultural competence has gradually attracted attention from many countries,including China.This study was undertaken to determine the cultural competence of registered nurses in Shanghai,China,and to identify the cultural competence among registered nurses in Pudong New Area,Shanghai.Methods:Qualitative interviews were conducted in combination with a quantitative survey.Fifteen clinical nurses were interviewed,and 1088 clinical nurses were recruited for the survey with cultural competence scale for registered nurses,based on the results of the qualitative and quantitative studies.Results:The overall level of cultural competence among registered nurses in Shanghai's Pudong New Area was moderate.Among the seven dimensions,cultural encounter had the highest score,followed by cultural practice,cultural awareness,cultural desire,cultural skill,cultural experience,and cultural knowledge.Age,level of hospital care,mastery of secondary level,and studying overseas were the influencing factors.Conclusions:Hospitals and universities should be aware of the importance of studying cultural competence.Cultural competencerelated courses should be increased,and various forms of training should be undertaken to enhance the interest of nurses. 展开更多

Objective:The clinical effectiveness of blood-heat psoriasis treated by Qingre Liangxue decoction and ultrasound imaging changes of lesions was observed.Methods:Patients with blood-heat psoriasis were selected and randomly grouped into two groups:the treatment group taking the Qingre Liangxue decoction orally,the control group taking the Compound Glycyrrhizin Tablets orally,both groups applying topical emollient lotion for a 12-week treatment period.Ultrasound images of target lesions and healthy skin were collected at different treatment times.Results:The epidermal thickness thought,the dermal density incremented as well as the subepidermal low-echogenic band(SLEB)width narrowed in both groups during the treatment.Conclusion:Qingre Liangxue decoction has signified efficiency in the treatment of blood-heat psoriasis,and high-frequency ultrasound has the advantages of non-invasive,timely and objective in the evolution. 展开更多

Background:Takayasu arteritis-induced renal arteritis (TARA), commonly seen in Takayasu arteritis (TA), has become one of the main causes of poor prognosis and early mortality in patients with TA. TARA progressing into Takayasu arteritis-induced renal artery stenosis (TARAS), could lead to severe complications including malignant hypertension, cardiac-cerebral vascular disease, and ischemic nephropathy. Since there existed no guidelines on treatments, this study aimed to review the comprehensive treatments for TARA.Methods:We searched systematically in databases including PubMed, Ovid-Medline, EMBASE, Web of Science, China National Knowledge Infrastructure, Wanfang, and SinoMed, from inception to May 2018. Literature selection, data extraction, and statistical analysis were performed.Results:Eighty-two literatures were recruited focusing on medical treatments ( n = 34) and surgical treatments ( n = 48). We found that combined medical treatments of glucocorticoids and conventional synthetic disease-modifying anti-rheumatic drugs could reach high rates of remission in patients with TARA, and biological disease-modifying anti-rheumatic drugs were preferred for refractory patients. After remission induction, surgical treatment could help reconstruct renal artery and recover renal function partly. Percutaneous transluminal angioplasty was the first choice for patients with TARAS, while open surgery showed a good longterm survival. Conclusions:Patients with TARA should benefit both from medical treatments and from surgical treatments comprehensively and sequentially. Multidisciplinary team coordination is recommended especially in patients with severe complications. 展开更多

The fatigue performance and fracture mechanism of laser welded twinning induced plasticity(TWIP)steel joint were investigated experimentally based on the evolution of microstructure and micromechanical properties.The optical microscopy was used to analyze the evolution of microstructure.The variation of composition and phase structure of fusion zone were detected by energy dispersive X-ray and X-ray diffraction spectrometers.The micromechanical behaviors of the various zones were characterized using nanoindentation.The static tensile test and high cycle fatigue test were performed to evaluate the mechanical properties of welded joint and base metal.The microstructures,tensile properties and fatigue strength of base metal as well as welded metal were analyzed.The fatigue fracture surfaces of base metal and welded joint were observed by means of scanning electron microscopy,in order to identify fatigue crack initiation sites and propagation mechanisms.Moreover,the fatigue fracture characteristics and mechanisms for the laser welded TWIP steel joints were analyzed. 展开更多

The controllable construction of light-emitting organometallic supramolecular materials integrating stimulus responses and biological applications is still a major challenge today.In this work,emissive organometallic assemblies based on AgI/AuI and mesoionic carbene-decorated tetraphenylethene(MIC-TPE)ligands have been synthesized and characterized.Photophysical studies showed that these organometallic assemblies[M_(4)(2)_(2)](OTf)_(4) not only fluoresce in various dilute solutions,resulting from the rigidification of MIC-TPE ligands upon complexation,but also show tunable fluorescence wavelengths and intensity behaviors in mixed solvent systems and at varying temperatures while in solution,and mechanical pressure stimulus-responsive fluorescent features in the solid state.Furthermore,the organometallic cages[M_(4)(2)_(2)](OTf)_(4)(M=Ag and Au)were successfully employed for cell imaging and showed the potential of anticancer activity,making them promising candidates for cancer theranostics.This work provides a simple and efficient method to prepare highly emissive and stimulus-responsive organometallic materials and paves the way for related biological applications such as cancer cell imaging and targeted therapy. 展开更多

Summary of main observation and conclusion Nonporous molecular single crystals of coordinatively unsaturated 16-electron metallacycle are found to undergo highly selective-yet reversible-binding of CO over CH4,H2,N2,CO2,various volatile organic compounds and water,through binding of the molecule to its metal centers.Carbon monoxide is taken up by black crystals and subsequently liberated from the corresponding orange CO adduct by reversible metal coordination,the structural evidence obtained from in situ X-ray diffraction.The single-crystallinity of the metallacycle is retained even after several cycles.The material,when adsorbed on silica,shows remarkable sensitivity and selectivity for CO,allowing visual CO sensing in the solid state.The metallacycle offer the possibility of serving as readily regenerable scavenger for removing trace CO from gas mixtures with N2. 展开更多

NiMo-based materials have been identified as potential candidates of Pt/C electrocatalysts for hydrogen evolution reaction(HER)due to appropriate binding energy to hydrogen,and good resistance to corrosive environments.However,little work has been carried out to enhance the catalytic performance in large-scale water-alkali electrolysis.The NoMo amorphous coating,as a highefficient and cost-effective catalyst toward HER,was synthesized by a facile electrodeposition strategy in this study.The effects of the pH value of electrolyte on the structure and HER activity of NiMo coating were investigated.The as-prepared NiMo_((pH10))exhibited the highest HER activity with overpotentials of 63.9 and 157.1 mV(vs.RHE,with 80%potential drop due to electrical resistance(iR)compensation)at the current density of-10 mA·cm^(-2)and-100 mA·cm^(-2).This NiMo_((pH10))coating also had excellent long-term durability of up to100 h stable operation under the constant current density of-100 mA·cm^(-2).The rapid HER kinetics and outstanding endurance can be ascribed to the NiMo compact coating with amorphous structures as well as good contact between NiMo coating and Ni foam substrate,endowing it grand feasibility in practical industrial applications. 展开更多

Poly(2-oxazoline)(POx)is a kind of polymeric amides that can be viewed as conformational isomers of polypeptides with excellent cyto-and hemo-compatibility,and is promising to be used as drug carriers.However,the drug loading capacity(DLC)of POx for many drugs is still low except several hydrophobic ones including paclitaxel(PTX).Herein,we prepared a series of amphiphilic POx block copolymers with various functional groups,and investigated the relationship between functional structures and the DLC.Functional POxs with benzyl,carboxyl,and amino groups in the side-chain were synthesized based on a poly(2-methyl-2-oxazoline)-block-poly(2-buty1-2-oxazoline-co-2-buteny1-2-oxazoline)(PMeOx-P(nBuOx-co-ButenOx),PMBEOx)precursor,followed by click reaction between vinyl and the 2-phenylethanethiol,thioglycolic acid and cysteamine.Using thin-film hydration method,eight commonly used drugs with various characteristics were encapsulated within these functional POx polymers.We found that amine-containing drugs were more easily encapsulated by POx with carboxyl groups,while amine functionalities in POx enhanced the loading capacity of drugs with carboxyl groups.In addition,n-n interactions resulted in enhanced DLC of most drugs,except several hydrophobic drugs with aromatic to total carbon ratios less than 0.5.In general,we could successfully encapsulate all the selected drugs with a DLC%over 10%using properly selected functional POxs.The above results confirm that the DLC of polymeric carriers can be adjusted by modifying the functional groups,and the prepared series of functional POxs provide an option for various drug loadings. 展开更多

As a new type of high manganese steel, the twinning induced plasticity (TWIP) steels have attracted a growing interest in the automotive industry due to their good performance. Thin plates of TWIP steel were welded by laser beam welding (LBW) and gas tungsten arc welding (GTAW). The microstructure result shows that GTAW joint has obvious heat-affected zone (HAZ), while the HAZ of LBW joint is almost invisible. The X-ray diffraction result shows that the phase compositions of both joints are austenitic and no phase transition occurs. Energy disper- sive spectrometry result shows that there is violent evaporation of Mn element in LBW joint, while the proportion of Mn element in GTAW joint is almost unchanged. Tensile tests and micro-hardness measurements were performed to take into account the mechanical properties of joints manufactured by the two different processes. The micro-hard- ness profiles of both joints present a typical saddle distribution, and the hardness of GTAW seam is lower than that of LBW seam. The failure positions of LBW joints are all located in base metal while the GTAW joints are all at the weld toe due to the softening of HAZ. By means of scanning electron microscopy, a typical ductile fracture is observed in LBW joint, while a brittle fracture with quasi-cleavage fracture characteristic is observed in GTAW joint. 展开更多

The field of supramolecular assemblies has developed rapidly in the last few decades,thanks in a large part to their diverse applications.These assemblies have been mostly based on Werner-type coordination motifs in which metal centres are coordinated by nitrogen or oxygen donors.Recently,N-heterocyclic carbene(NHC)ligands have been employed as carbon donors not only because of their appealing structures but also due to the extensive applications in catalysis,biomedicine and material science of the resulting assemblies.During the last decade,NHC-based supramolecular assemblies have witnessed rapid growth and extensive application in molecular recognition,luminescent materials and catalysis.For different topological systems,a diverse selection of poly-NHC precursors and synthetic strategies is crucial to precisely control the synthesis of supramolecular architectures.Several synthetic strategies have been developed to synthesise two-dimensional(2D)molecular metallacycles and three-dimensional(3D)metallacages from a wide range of poly-NHC precursors,including a straightforward one-pot strategy,supramolecular transmetalation,stepwise synthesis,an improved one-pot strategy involving self-sorting behaviour of 3D metallacages and a subtle variation strategy of poly-NHC ligand precursors.This review offers a summary of the synthetic strategies applied for the construction of different poly-NHC-based supramolecular assemblies,particularly emphasizes recent progress in the synthesis of large and complex supramolecular assemblies from poly-NHC precursors,and further attention is given to their application in postsynthetic modifications(PSMs),host-guest chemistry,luminescent properties and biomedical applications. 展开更多
