BACKGROUND Lower extremity fractures are mainly treated by surgical reduction,but this operation is often affected by the patient’s level of agitation and the type of anesthesia used.The main treatment for lower-extr...BACKGROUND Lower extremity fractures are mainly treated by surgical reduction,but this operation is often affected by the patient’s level of agitation and the type of anesthesia used.The main treatment for lower-extremity fractures is operative reduction.However,operations can often be affected by both agitation and the degree of anesthesia.Therefore,it is of great importance to develop an effective anesthesia program to effectively ensure the progress of surgery.AIM To discuss the effect of ultrasound-guided nerve block combined with dexmedetomidine anesthesia in lower extremity fracture surgery.METHODS A total of 120 hospital patients with lower extremity fractures were selected for this retrospective study and divided into an observation group(n=60)and a control group(n=60)according to the anesthesia scheme;the control group received ultrasound-guided nerve block;the observation group was treated with dextromethomidine on the basis of the control group,and the mean arterial pressure,heart rate(HR),and blood oxygen saturation were observed in the two groups.RESULTS The mean arterial pressure of T1,T2 and T3 in the observation group were 94.40±7.10,90.84±7.21 and 91.03±6.84 mmHg,significantly higher than that of the control group(P<0.05).The observation group’s HR at T1 was 76.60±7.52 times/min,significantly lower than that of the control group(P<0.05);The observation group’s HR at T2 and T3 was 75.40±8.03 times/min and 76.64±7.11 times/min,significantly higher than that of the control group(P<0.05).The observation group’s visual analog score at 2 h,6 h and 12 h after operation was 3.55±0.87,2.84±0.65 and 2.05±0.40.the recovery time was 15.51±4.21 min,significantly lower than that of the control group(P<0.05).Six hours post-anesthesia,epinephrine and norepinephrine in the observation group were 81.10±21.19 pg/mL and 510.20±98.27 pg/mL,significantly lower than that of the control group(P<0.05),and the mini-mental state exam score of the observation group was 25.51±1.15,significantly higher than that in the control group(P<0.05).CONCLUSION Ultrasound-guided nerve block combined with dexmedetomidine has a good anesthetic effect in the operation of lower limb fractures and has little effect on the hemodynamics of patients.展开更多

The flow field is analyzed with emphases laid on the heat transport as well as its rela- tion with momentum transport. When turbulent coherent structures are suppressed by SWO, the turbulent transports are significantly changed, however the momentum and heat transports change in the same manner, which gives the evi- dence of inherently consistent transport mechanisms between momentum and heat in turbulent boundary layers. The Reynolds analogies of all the flow cases are quite good, which confirms again the fact that the transport mechanisms of momentum and heat are consistent, which gives theoreti- cal support for controlling the wall heat flux control by using the drag reducing techniques.展开更多

These corrections can be used in model applications in which explicit expressions, rather than numerical solutions are needed. The comparison with an interpolation formula proposed by Batchelor, showed that the latter gives surprisingly precise results. The modification of the same method to obtain analytical corrections to the scaling law, taking into account the possible corrections induced by intermittency, is also proposed.展开更多

To the authors' knowledge, most solutions of 2D or 3D DPL models available in the literature are obtained by numerical methods, and there are few exact solutions up to now. The exact solutions in this paper can be used as benchmarks to validate numerical solutions and to develop numerical schemes, grid generation methods and so forth. In addition, they are of theoretical significance since they correspond to physically possible situations. The main goal of this paper is to obtain some possible exact explicit solutions of the dual-phase lag heat conduction equation as the benchmark solutions for computational heat transfer, rather than specific solutions for some given initial and boundary conditions. Therefore, the initial and boundary conditions are indeterminate before derivation and can be deduced from the solutions afterwards. Actually, all solutions given in this paper can be easily proven by substituting them into the governing equation.展开更多
BACKGROUND Familial hypercholesterolemia(FH)is an autosomal dominant disorder that is characterized by severely increased low-density lipoprotein(LDL)cholesterol levels.At the same time,elevated LDL levels accelerated...BACKGROUND Familial hypercholesterolemia(FH)is an autosomal dominant disorder that is characterized by severely increased low-density lipoprotein(LDL)cholesterol levels.At the same time,elevated LDL levels accelerated the development of coronary heart disease.Several classes of drugs are currently in use to treat FH.Proprotein convertase subtilisin/kexin type 9 inhibitor(PCSK9i)is novel one of these.CASE SUMMARY This manuscript reports a case of FH that responded modestly after treatment with PCSK9i and statin drugs.Of even more concern is that the patient frequently admitted to the hospital during a 12-year follow-up period.Subsequently,we identified a heterozygous mutation,1448G>A(W483X)of the LDL receptor(LDLR)in this patient.The serum levels of PCSK9(proprotein convertase subtilisin/kexin type 9)in the patient was 71.30±26.66 ng/mL,which is close the average level reported in the literature.This LDLR mutation affects LDLR metabolism or structure,which may make it unsuitable for use of PCSK9i.CONCLUSION Our outcome demonstrates that LDLR-W483X represents a partial loss-of-function LDLR and may contribute to PCSK9i ineffective. In the meanwhile, additional measures aretherefore required (particularly with gene sequencing or change the treatment plan) must beinitiated as early as possible. Genetic testing for clinically challenging cases who do not respond toPCSK9i therapy is very helpful.展开更多
Objective:To investigate the expressions of microRNA-155 (miR-155) and histone deacetylase 2 (HDAC2) in peripheral blood mononuclear cells (PBMC) of patients with sudden deafness (SSNHL) and their clinical significanc...Objective:To investigate the expressions of microRNA-155 (miR-155) and histone deacetylase 2 (HDAC2) in peripheral blood mononuclear cells (PBMC) of patients with sudden deafness (SSNHL) and their clinical significances.Methods:112 patients with SSNHL who were treated in our hospital from February 2017 to September 2018 were selected as the study subjects, and they were referred to as sudden deafness group, and another 115 healthy examinees in the same period were compared and studied as normal group. The relative expression levels of miR-155, HDAC2, ICAM-1, TNF-α and interleukin-8 (IL-8) in PBMC of all subjects were detected by real-time fluorescence quantitative analysis (qRT-PCR). Pearson method was used to analyze the relationships between miR-155, HDAC2 and the expressions of ICAM-1, TNF-α, IL-8, and miR-155 and the expression of HDAC2 in PBMC of SSNHL patients. Logistic regression analysis was used to analyze the risk factors of SSNHL. The predictive diagnostic values of miR-155 and HDAC2 in PBMC for SSNHL were evaluated. Results:The relative expression levels of miR-155, ICAM-1, TNF-α and IL-8 in PBMC of sudden deafness group were significantly higher than those of normal group (P<0.05), while the relative expression levels of HDAC2 were significantly lower than those of normal group (P<0.05). In SSNHL patients, miR-155 were positively correlated with the expression levels of ICAM-1, TNF-α and IL-8 (P<0.05). The expression level of HDAC2 was negatively correlated with the expression levels of TNF-α and IL-8 (P<0.05). The expression level of miR-155 in PBMC of SSNHL patients was negatively correlated with HDAC2 (P<0.05). MiR-155, ICAM-1, TNF-α and IL-8 were risk factors for SSNHL (P<0.05), while HDAC2 was protective factor for SSNHL (P<0.05). The areas under curve (AUC) of miR-155 and HDAC2 in PBMC for SSNHL diagnosis were 0.855 and 0.835 respectively, the truncation values of which were 1.449 and 0.959 respectively, at this moment, the sensitivities were 75.0% and 81.3% respectively, and the corresponding specificities were 86.1% and 70.1% respectively. The AUC of combined diagnosis of SSNHL was 0.927, and the sensitivity and specificity were 86.7% and 85.3% respectively.Conclusions:The expressions of miR-155 is high and HDAC2 is low in PBMC of SSNHL patients, which are negatively correlated, and both of them may participate in the occurrence and development of SSNHL through mutual influence, which is a risk factor of SSNHL. The combination of the two can effectively improve the predictive diagnostic value of SSNHL.展开更多
Fluorescent analysis of bone provides valuable insights into bone structures.However,conventional dyes suffer from low specificity on bone tissue,small stokes shift,short fluorescent lifetime,and aggregation-caused qu...Fluorescent analysis of bone provides valuable insights into bone structures.However,conventional dyes suffer from low specificity on bone tissue,small stokes shift,short fluorescent lifetime,and aggregation-caused quenching effect,which result in low efficacy and artifacts.In this work,we design an aggregation-induced emission(AIE)-active iridium(III)complex(Ir-BP2)as a highly selective,convenient,nondestructiveness,and dual-mode staining agent for bone analysis.Ir-BP2 containing phosphonate groups selectively binds to hydroxyapatites,the main component of bone matrix,and exhibits turn-on AIE phosphorescence with prolonged lifetime.Ir-BP2 exhibits promising biosafety and offers higher accuracy in staining calcium deposits than conventional Alizarin Red S staining assay when it is employed in real-time monitoring of osteogenesis differentiation process.A ready-to-use staining spray of Ir-BP2 is fabricated.By using fluorescent imaging and lifetime imaging,Ir-BP2 staining provides valuable insights into bone microstructure analysis,microdamage diagnosis,and bone growth state identification.Further,Ir-BP2 is successfully applied on a human spine vertebra for diagnosing bone invasiveness of eosinophilic granuloma,validating its clinical practice.This work presents a powerful tool in bone analysis and will lead to new approaches for the diagnosis and treatment of bone-related diseases.展开更多
