错配修复功能缺陷(deficient mismatch repair,dMMR)是目前公认的预测免疫检查点抑制剂(immune checkpoint inhibitors,ICIs)疗效的生物标志物,国内外指南已推荐dMMR实体瘤患者一线治疗选择免疫治疗。在直肠癌中,仅5%患者为dMMR/高度微... 错配修复功能缺陷(deficient mismatch repair,dMMR)是目前公认的预测免疫检查点抑制剂(immune checkpoint inhibitors,ICIs)疗效的生物标志物,国内外指南已推荐dMMR实体瘤患者一线治疗选择免疫治疗。在直肠癌中,仅5%患者为dMMR/高度微卫星不稳定(microsatellite instability-high,MSI-H),绝大多数为“免疫沙漠型”或错配修复功能完整(mismatch repair proficient,pMMR)/微卫星稳定(microsatellite stabilization,MSS),即对ICIs反应欠佳。因此免疫药物与新辅助放化疗的协同作用是近些年基础和临床研究的热点,并且有较多Ⅱ/Ⅲ期免疫全新辅助治疗(immuno-total neoadjuvant therapy,iTNT)相关临床试验涌现,局部进展期直肠癌(locally advanced rectal cancer,LARC)管理模式也开始进入非手术治疗时代。dMMR/MSI-H型LARC患者新辅助免疫疗法效果显著,有望让患者在后续治疗中豁免手术和放化疗从而选择观察等待策略;对于pMMR/MSS型LARC,目前初步认定ICIs从初始作为后线姑息方案前移到一线选择策略以及继续开展大型临床试验。但iTNT在LARC中应用最佳模式尚无定论,但短程放疗序贯新辅助化疗联合免疫药物可能获得良好短期疗效。寻找其它新的生物标志物,可能会鉴定出部分对iTNT敏感的pMMR/MSS型患者(尤其是低位直肠癌)。未来LARC治疗策略需结合患者临床复发危险度分层和器官保留意愿两方面考虑,实现分层精准治疗。本文将对LARC中iTNT的相关研究背景、基础和临床研究进展以及存在问题等进行综述。 展开更多

Risk factors of atherosclerosis in elderly senile obstructive sleep apnea hypopnea syndrome patients with hypertension and type 2 diabetes

Objective: To explore the related factors of atherosclerosis in senile obstructive sleep apnea hypopnea syndrome (OSAHS) patients. Methods: 129 outpatients and inpatients with senile hypertension and type 2 diabetes were chosen from Department of Geri-atrics during May of 2012 to January of 2014. The sleep breath monitoring was performed to each patient in the early stage. According to the presence or absence of OSAHS, they were divided into OSAHS (n = 60) and non-OSAHS (n = 69) groups. And 30 healthy elderly subjects recruited in Physical Examination Department of our hospital were included in the control group. The parameters of age, gender, height, body mass index, hypersensitive C-reactive protein (Hs-CRP), homocysteine (Hcy), triglyc-eride (TG), total cholesterol (TC), high density lipoprotein-cholesterol (HDL-C), low density lipoprotein-cholesterol (LDL-C) and carotid intima-media thickness (CIMT) were recorded. The comparison of Hs-CRP, Hcy, TG, TC, HDL-C, LDL-C and CIMT between OSAHS and non-OSAHS groups was made;and the comparison with the normal control group was made to analyze the correlations of CIMT to the above-mentioned indexes. Results: The levels of Hs-CRP [(3.40 ± 0.91) mg/L, (2.57 ± 1.01) mg/L], Hcy [(16.50 ± 2.89) μmol/L, (11.20 ± 2.32) μmol/L], CIMT [(1.06 ± 0.14) mm, (0.93 ± 0.13) mm], TG [(2.52 ± 1.40) mmol/L, (2.51 ± 2.02) mmol/L], TC [(5.74 ± 1.16) mmol/L, (5.49 ± 1.09) mmol/L] and LDL-C [(3.15 ± 0.47) mmol/L, (3.05 ± 0.70) mmol/L] in both OSAHS and non-OSAHS groups were higher than those in normal control group [Hs-CRP: (2.06 ± 1.22) mg/L, Hcy: (6.04 ± 1.85) μmol/L, CIMT: (0.65 ± 0.07) mm, TG: (0.99 ± 0.24) mmol/L, TC: (3.63 ± 0.71) mmol/L and LDL-C: (2.47 ± 0.27) mmol/L];the levels of Hs-CRP, Hcy, CIMT and plaque score in OSAHS group were higher than those in non-OSAHS group [plague score: (1.92 ± 1.03) vs. (1.35 ± 0.97) points], the difference was of statistical significance (OSAHS group vs. control group, t = 5.88, 18.15, 15.62, 5.91, 9.17, 7.47;non-OSAHS group vs. control group, t = 2.20, 10.73, 11.51, 4.09, 8.56, 4.45;OSAHS group vs. non-OSAHS group, t = 4.84, 11.72, 5.54, 3.91, all p < .05). Related analyses showed that CIMT and plaque score in OSAHS group were correlated to age, BMI, TG, TC, LDL-C, Hs-CRP and Hcy respectively. Conclusions: OSAHS may elevate the level of inflammatory cytokines and cause atherosclerosis. And the changes of CIMT, Hs-CRP and Hcy are in parallel. Hcy also has an identically important clinical significance in the progress of atherosclerosis. 展开更多

Casualties are frequent in high-risk environments,particularly in high-risk chemical and high-temperature fire environments,due to improper protection or accidents.While wearable sensors can offer real-time biomechanical monitoring in fire and chemical environments,they cause discomfort,contain toxic heavy metals,and lack resistance to fire and acid/alkali.Herein,a facile approach to fabricating metal-free fire/acid/alkali-resistant poly(m-phenylene isophthalamide)fiber and carbon nanofiber composite triboelectric nanogenerator(PMIA/CNF-TENG)was demonstrated.The PMIA/CNF-TENG shows the advantages of textile construction including flexibility,waterproofing,and moisture permeability.It also exhibits unique functions,such as ultrahigh fire/temperature resistance,strong acid and alkali protection,the ability to monitor human signals in real time with self-power,handwritten input for danger signals,and sudden risk perception.The PMIA/CNF-TENG pos-sessed an open-circuit voltage(V_(OC))retention rate of 96.8%even at 250℃,thereby showing considerably higher thermal stability than conventional flame-retardant TENGs.When moved from room temperature to a simulated fire environment,the biomotion-generated VOC increased by 136.7%for bending the elbow and by over 900%for hand input,indicating good fire-sensing capability.In addition,output signal strength by solid-liquid contact depended on the solution type and cor-responded to the laws—NaOH>HNO_(3)>H_(2)SO_(4)>H_(2)O,indicating potential applications in chemical splash detection and active acid-alkali liquid identification.Moreover,the PMIA/CNF-TENG could be built into wireless intelligent sensing systems to achieve remote biomotion and risk perception. 展开更多

A population of 117 doubled haploid(DH)lines derived from the cross of Zhaiyeqing 8(indica)6 Jingxi 17(japonica)was employed to map quantitative trait loci(QTL)underlying four physiological traits related to chlorophyll contents of the flag leaf.There were significantly positive correlations among chlorophyll a,chlorophyll b and chlorophyll a+b content.Chlorophyll a/b ratio was significantly negatively correlated with chlorophyll b content.These four traits were normally distributed with transgressive segregation,suggesting that they were controlled by multiple minor genes.A total of 11 QTLs were detected for the four traits and they lay on six chromosomes.Each of them explained 9.2%–19.6%of the phenotypic variations,respectively.Of these,two QTLs controlling chlorophyll a content were mapped on chromosomes 2 and 5;four QTLs underlying chlorophyll b content were mapped on chromosomes 2,3,5 and 9;three QTLs underlying chlorophyll a+b amount were mapped on chromosomes 3,5 and 9;two QTLs underlying chlorophyll a/b ratio were mapped on chromosomes 6 and 11.The intrinsic relationship among the four traits and the practical implication in rice breeding are discussed. 展开更多
