目的分析2014—2018年辽宁省男男性行为(men who have sex with men,MSM)人群抗逆转录病毒治疗失败病例HIV-1基因型耐药特征,探讨失败的原因和药物敏感性,指导临床个性化诊疗。方法募集2014—2018年辽宁省MSM人群接受抗逆转录病毒治疗...目的分析2014—2018年辽宁省男男性行为(men who have sex with men,MSM)人群抗逆转录病毒治疗失败病例HIV-1基因型耐药特征,探讨失败的原因和药物敏感性,指导临床个性化诊疗。方法募集2014—2018年辽宁省MSM人群接受抗逆转录病毒治疗半年后的病例,对HIV-1病毒载量≥1000 copies/mL的样本用RT-PCR法进行HIV-1 pol基因检测。对获得的pol基因序列进行亚型判定用Stanford University HIV-1耐药数据库进行基因型耐药解析。用SPSS软件进行统计分析。结果2014—2018年共发现辽宁省MSM人群抗逆转录病毒治疗病毒学失败病例400例,CRF01_AE为最主要流行毒株,约占83.8%;其中267例(66.8%)存在耐药相关突变并对一线药物存在低度及以上耐受。最主要受累群体为25~45岁年龄段病例,72.0%的病例在治疗后0.5~2.0年被检出,87.6%的耐药病例(234/267)同时对核苷类和非核苷类逆转录酶抑制剂耐药。结论抗逆转录病毒治疗失败病例主要由耐药相关突变引起的;在扩大治疗防控策略背景下,加强初始治疗病例依从性教育,减少治疗后耐药发生,必要时可以加强治疗后随访检测,识别治疗失败病例,及时优化治疗方案,提高治疗成功率。展开更多
