A classroom-based parent interview was designed and implemented in an undergraduate psychiatric mental health nursing class to fill the gap between nursing students and parents of child or adolescent patients with mental health issues faced during clinical. The goals of this learning activity were to increase understanding of what parents experience when dealing with their child’s mental health problems and to increase student engagement and attention. The class using this learning activity consisted of three parts: 1) an assigned pre-class reading;2) a mini-lecture;and 3) a parent interview presentation. Students were pre-assigned a reading chapter and faculty-developed interview questions. During the mini-lecture, important knowledge related to mental health care of children was assessed using CourseKey software. After the mini-lecture, the parent guest speaker delivered her presentation about herself, her child’s strengths and abilities, her child’s mental health problems and their impact, and her family’s strengths based on the interview question prompts for about 30 minutes, followed by 20 minutes of a Q & A session. The set of interview questions was developed by faculty based on the competency questions of the Child Behavior Check List. Despite some limitations, this classroom-based parent interview using a flipped classroom model was found to be a meaningful learning strategy by increasing student engagement and attention, increasing retention of knowledge learned in class, and filling that gap in clinical. 展开更多

Co-Occurring Polysubstance Use and Physical Disease of Persons with Mental Disorders in Residential Treatment Program

The purpose of this study was to describe the prevalence of co-occurring polysubstance use and to examine the differences in physical disease between polysubstance users and single/no substance users with mental disorders. This retrospective cross-sectional study included 1949 clinical records of psychiatric patients from 11 residential treatment programs between 2007 and 2011. Demographic variables, psychiatric diagnoses, and data on substance use and physical disease were obtained from the clinical records. Chi-square analyses were used to examine substance use difference in the prevalence of each physical disease category. This study found that the prevalence of co-occurring polysubstance use was 53.5%. Chi-square analyses identified that co-occurring polysubstance users reported more respiratory, digestive, musculoskeletal, and HIV/AIDS diseases but less endocrine diseases than single/no substance users. Therefore, integrated treatment programs for treating patients with co-occurring substance use and physical disease should be developed and expanded for this high-risk group. 展开更多

This paper reviewed genetic, neurodevelopmental, and neurobiological perspectives to understand the etiology and relevant treatment approaches of schizophrenia. Although genetic and neurodevelopmental theories provide a substantial contribution to the etiology of schizophrenia, neurobiological one has its dominant stance in which it can explain causative mechanisms of schizophrenia, identify targets for treatment, and predict outcomes well. According to the neurobiology, onset and course of schizophrenia are well supported by the alterations in neurotransmitters such as dopamine or serotonin. Based on this mechanism, antipsychotics have been widely used as one of the treatment approaches for schizophrenia. By following up the degree of patients’ responses to antipsychotics, treatment outcomes could be evaluated or predicted. Although neurobiology seldom provides information about identification or prevention of risk factors about schizophrenia compared to genetic and neurodevelopmental approaches, it has well-established scientific foundations, ability to guide treatment, applicability across age and culture, and usefulness as a research framework. 展开更多

This paper discussed various philosophical and ethical perspectives on how to know recovery from schizophrenia. According to the empirical approach, recovery can be measured by objective and standardized tools which evaluate the severity of psychiatric symptoms or levels of social and vocational functioning. These measurements have minimum scores or sets of criteria of what recovery looks like, so this approach may be useful to evaluate patient’s treatment outcomes by healthcare providers. On the contrary, phenomenological approach stands qualitative research methods to understand an individual’s subjective, lived, or unique experiences while he or she lives with schizophrenia. Historical perspective holds that a point of view on recovery has been changed from negative to positive as our social perspectives for schizophrenia changes. Feminist perspective suggests that recovery can be understood by exploring female’s subjective experiences like successful marriage life or having responsibility for children. Ethical perspectives regarding the meaning of recovery were also addressed. Each philosophical and ethical perspective guides different research methodology and methods. Therefore, to employ appropriate methodology and methods, understanding philosophical and ethical backgrounds would be important. 展开更多

Predictors of Co-Occurring Substance Use among Asian Americans in Residential Treatment Programs

Although severe and chronic mental disorders are common among Asian Americans in residential treatment programs, little has been known about the prevalence and predictors of co-occurring substance use in this population. The purpose of this study was to examine predictors of co-occurring substance use among Asian Americans with mental disorders in residential treatment programs. This cross-sectional study included 375 clinical records of Asian Americans from residential treatment programs between 2007 and 2011. Demographic variables, principal psychiatric diagnoses, and data on alcohol, stimulant, and marijuana use were obtained from the clinical records. Separate binary logistic regression analyses were used to examine the demographic and diagnostic contributions to the risk of each type of substance use. Findings of this study indicated that the prevalence of co-occurring substance use was about 53% in Asian Americans with mental disorders. Binary logistic regression analyses revealed that male gender, older age, and depressive disorder predicted more alcohol use, but homelessness and schizophrenia predicted less alcohol use. Male gender, homelessness, and smoking predicted more stimulant use. Male gender and younger age predicted more marijuana use. Based on the findings of this study, awareness about co-occurring substance use problems of ethnic minority psychiatric clients should be increased and appropriate substance use prevention and treatment programs should be developed and provided for high-risk groups. 展开更多

Nucleic acid vaccines have attracted enormous attention for resolving the limitations of conventional vaccines using live attenuated viruses. Because nucleic acid vaccines can be produced rapidly in response to the emergence of new virus strains, they are more appropriate for the control of urgent epidemic and pandemic issues. In particular, messenger RNA (mRNA) vaccines have evolved as a new type of nucleic acid vaccines in accordance with their superior protein expression and a lack of mutagenesis as compared with DNA vaccines. Using mRNA vaccines, large amounts of target proteins can be expressed in immune cells for efficient immunization. For instance, antigen-specific vaccination is a feasible option involving the expression of specific antigens in antigen-presenting cells. Immunological reactions are modulated by expressing several proteins associated with stimulation or maturation of immune cells. In addition, mRNA vaccines can stimulate innate immunity through specific recognition by pattern recognition receptors. On the basis of these remarkable properties, mRNA vaccines have been used for prophylactic and therapeutic applications. This review highlights the role of mRNA vaccines as prophylactic vaccines for prevention of future infections and as therapeutic vaccines for cancer immunotherapy.
