水母征对完全性前置胎盘合并胎盘植入性疾病不良围产结局的提示意义 被引量:1

目的研究超声检查时发现水母征对完全性前置胎盘合并胎盘植入性疾病(placenta accreta spectrum disorders,PAS)不良围产结局的提示意义。方法回顾性研究2020年1月至2023年2月在南京医科大学第一附属医院经剖宫产分娩、诊断为完全性前... 目的研究超声检查时发现水母征对完全性前置胎盘合并胎盘植入性疾病(placenta accreta spectrum disorders,PAS)不良围产结局的提示意义。方法回顾性研究2020年1月至2023年2月在南京医科大学第一附属医院经剖宫产分娩、诊断为完全性前置胎盘合并PAS的72例单胎孕妇的超声图像。根据是否存在水母征,将研究对象分为水母征组[15例(20.8%)]和非水母征组[57例(79.2%)]。分析2组孕妇的一般资料和围产结局。不良围产结局包括患者接受腹主动脉球囊阻断、子宫动脉栓塞或子宫切除术,发生产后出血,以及其新生儿收住新生儿重症监护病房(neonatal intensive care unit,NICU)等。采用两独立样本t检验、Mann-Whitney U检验和χ^(2)检验(或Fisher精确概率法)进行统计学分析。结果(1)水母征组的产次[(1.6±0.7)次与(1.2±0.6)次,t=2.01]和产前胎盘植入评分[(12.3±1.5)分与(8.6±2.9)分,t=6.59]均高于非水母征组(P值均<0.05)。非水母征组包括粘连型胎盘植入14例(24.6%)、植入型胎盘植入40例(70.2%)、穿透型胎盘植入3例(5.3%);水母征组包括植入型胎盘植入9例和穿透型胎盘植入6例,无粘连型病例。2组各类型PAS分布差异有统计学意义(Fisher精确概率法,P<0.001)。(2)水母征组的腹主动脉球囊阻断率[15/15与38.6%(22/57),χ^(2)=17.92]、子宫动脉栓塞率[3/15与1.8%(1/57),Fisher精确概率法]、术中出血量[指放置球囊的病例,(1973±1057)ml与(1211±576)ml,t=2.55]、需要输血率[15/15与63.2%(36/57),Fisher精确概率法]、输悬浮红细胞量[指放置球囊的病例,4.0 U(2.0~23.0 U)与2.5 U(0.0~11.0 U),Z=-2.53]和术后住院时间[(9.7±2.4)d与(7.5±2.2)d,t=3.36]均高于非水母征组,而分娩孕周[(33.6±1.5)周与(35.2±1.8)周,t=-3.24]、新生儿1和5 min Apgar评分[1 min:8分(3~10分)与9分(4~10分),Z=-2.46;5 min:9分(7~10分)与10分(6~10分),Z=-2.02]均小于非水母征组(P值均<0.05)。2组的急诊手术率、术后24 h出血量、新生儿出生体重及新生儿收住NICU比例的差异无统计学意义。2组均无切除子宫或死亡病例。结论超声检查发现水母征的完全性前置胎盘合并PAS患者更易于出现不良围产结局,应更予重视。 展开更多

Chitosan oligosaccharides(COSs)have been reported to possess a broad range of activities such as antitumor,antioxidant and neuroprotective activities.In this study,the protective efects and mechanisms of peracetylated chitosan oligosaccharides(PACOs)against Aβ-induced cognitive defcits were investigated in Sprague–Dawley(SD)rats.PACOs treatment signifcantly improved the learning and memory function of Alzheimer’s disease(AD)rats and attenuated the neuron cell damage caused by Aβ.PACOs also markedly reduced the levels of lactate dehydrogenase(LDH)and Malondialdehyde(MDA)and decreased the phosphorylation of Tau protein to inhibit oxidative injury and infammatory responses in AD rats.Further studies indicated that PACOs may promote the repair of Aβinduced nerve damage and inhibit neuronal apoptosis mainly through regulating PI3K/Akt/GSK3βsignaling pathway.Consistently,the transcriptome analysis verifed that the diferentially expressed genes(DEGs)were mainly involved in neuron development and the PI3K-Akt signaling pathway.Taken together,peracetylated chitosan oligosaccharides(PACOs)have the potential to be developed into novel anti-AD agents targeting the cellular PI3K/Akt/GSK3βsignaling pathway. 展开更多
