A hollow cylinder torsional sh...This paper aims to comprehensively analyze the influence of the principal stress angle rotation and intermediate principal stress on loess's strength and deformation characteristics. A hollow cylinder torsional shear apparatus was utilized to conduct tests on remolded samples under both normal and frozen conditions to investigate the mechanical properties and deformation behavior of loess under complex stress conditions. The results indicate significant differences in the internal changes of soil particles, unfrozen water, and relative positions in soil samples under normal and frozen conditions, leading to noticeable variations in strength and strain development.In frozen state, loess experiences primarily compressive failure with a slow growth of cracks, while at normal temperature, it predominantly exhibits shear failure. With the increase in the principal stress angle, the deformation patterns of the soil samples under different conditions become essentially consistent, gradually transitioning from compression to extension, accompanied by a reduction in axial strength. The gradual increase in the principal stress axis angle(α) reduces the strength of the generalized shear stress and shear strain curves.Under an increasing α, frozen soil exhibits strain-hardening characteristics, with the maximum shear strength occurring at α = 45°. The intermediate principal stress coefficient(b) also significantly impacts the strength of frozen soil, with an increasing b resulting in a gradual decrease in generalized shear stress strength. This study provides a reference for comprehensively exploring the mechanical properties of soil under traffic load and a reliable theoretical basis for the design and maintenance of roadbeds.展开更多
BACKGROUND Multiple primary carcinomas(MPCs)are defined as two or more independent primary cancers that occur simultaneously or sequentially in the same individual.Synchronous MPCs are rarer than solitary cancers or m...BACKGROUND Multiple primary carcinomas(MPCs)are defined as two or more independent primary cancers that occur simultaneously or sequentially in the same individual.Synchronous MPCs are rarer than solitary cancers or metachronous MPCs.Accurate diagnoses of synchronous MPCs and the choice of treatment are critical for successful outcomes in these cases.CASE SUMMARY A 64-year-old patient presented with dysphagia,without obvious cause.A diagnosis of synchronous esophageal squamous cell carcinoma and colon adenocarcinoma with liver metastasis was confirmed based on examination and laboratory results.After multi-disciplinary consultations,combination chemotherapy(a 3-wk cycle with oxaliplatin 212 mg administered on day 1 and capecitabine 1.5 g twice daily on days 1-14)and esophageal cancer radiotherapy were initiated.Based on the results of genetic testing,we switched to a regimen of leucovorin+fluorouracil+oxaliplatin and cetuximab regimen for 8 cycles.Subsequently,capecitabine and bevacizumab were administered until the most recent follow-up,at which the tumor remained stable.CONCLUSION Successful cetuximab chemotherapy treatment provides a reference for the nonoperative and homogeneous treatment of different pathological types of synchronous MCPs.展开更多
Background:Patients with type 2 diabetes are at high risk for developing multiple chronic complications.However,there is a lack of studies of the cumulative number of diabetic complications in China.Methods:A retrospe...Background:Patients with type 2 diabetes are at high risk for developing multiple chronic complications.However,there is a lack of studies of the cumulative number of diabetic complications in China.Methods:A retrospective cohort study was performed from 2009 to 2021.Type 2 diabetes patients who were first diagnosed after the age of 35 years between January 1,2009,and December 31,2017,were included.Five states were defined according to the number of chronic complications:no(S0),one(S1),two(S2),three(S3),and four or more complications(S4).A multi-state Markov model was constructed to estimate transition probability,transition intensity,mean sojourn time,and the possible factors for each state.Results:The study included 32653 type 2 diabetes patients(mean age,59.59 years;15929(48.8%)male),and mean follow-up time of 7.75 years.In all,4375 transitions were observed.The 12-year transition probability of from state S0 to S1 was the lowest at 16.4%,while that from S2 to S3 was the highest,at 45.6%.Higher fasting blood glucose,lower high-density lipoprotein cholesterol,higher total cholesterol,and an unhealthy diet were associated with higher risk of progression from S0 to S1.Being female,less than 60 years old,weekly physical activity,and vegetarian diet decreased this risk.Being female and less than 60 years old reduced the likelihood of transition from S1 to S2,whereas lower high-density lipoprotein cholesterol increased this likelihood.Conclusions:Following the occurrence of two complications in type 2 diabetes patients,the risk for accumulating a third complication within a short time is significantly increased.It is important to take advantage of the stable window period when patients have fewer than two complications,strengthen the monitoring of blood glucose and blood lipids,and encourage patients to maintain good living habits to prevent further deterioration.展开更多
Electrocatalytic N_(2) reduction reaction(NRR)represents an appealing solution for sustainable ammonia production,whereas exploring high-efficiency NRR catalysts is highly desired but extremely challenging.Herein,we c...Electrocatalytic N_(2) reduction reaction(NRR)represents an appealing solution for sustainable ammonia production,whereas exploring high-efficiency NRR catalysts is highly desired but extremely challenging.Herein,we combine Ti_(3) C_(2)T_(x)-MXene quantum dots(MQDs)with porous Cu nanosheets to design a novel heterostructured MQDs/Cu as an effective and durable NRR catalyst.Impressively,MQDs/Cu showed a synergistically enhanced NRR activity with an NH_(3) yield of 78.5 lg h^(-1) mg^(-1)(-0.5 V)and a Faradaic efficiency of 21.3%(-0.4 V),far superior to pure MQDs and Cu,and outperforming the majority of the state-of-the-art NRR catalysts.Density function theory computations demonstrated that the synergy of MQDs and Cu enabled the creation of interfacial Cu-Ti dimer as dual-active-centers to strongly activate the absorbed N 2 and promote the*N_(2)H formation,consequently resulting in the much reduced energy barriers and greatly enhanced NRR performance.展开更多
Sentinel-1A/B data are crucial for retrieving numerical information about surface phenomena and processes.Coregistration of terrain observation by progressive scans(TOPS)data is a critical step in its application.TOPS...Sentinel-1A/B data are crucial for retrieving numerical information about surface phenomena and processes.Coregistration of terrain observation by progressive scans(TOPS)data is a critical step in its application.TOPS data must be fundamentally co-registered with an accuracy of 0.001 pixels.However,various decorrelation factors due to natural vegetation and seasonal effects affect the coregistration accuracy of TOPS data.This paper proposed an enhanced spectral diversity coregistration method for dual-polarimetric(PolESD)Sentinel-1A/B TOPS data.The PolESD method suppresses speckle noise based on a unified non-local framework in dual-pol Synthetic Aperture Radar(SAR),and extracts the phase of the optimal polarization channel from the denoised polarimetric interferometric coherency matrix.Compared with the traditional ESD method developed for single-polarization data,the PolESD method can obtain more accurate coherence and phase and get more pixels for azimuth-offset estimation.In bare areas covered with low vegetation,the number of pixels selected by PolESD is more than the Boxcar method.It can also correct misregistration more effectively and eliminate phase jumps in the burst edge.Therefore,PolESD will help improve the application of TOPS data in low-coherence scenarios.展开更多
Variable frequency,a new pattern of pulse hydraulic fracturing,is presented for improving permeability in coal seam.A variable frequency pulse hydraulic fracturing testing system was built,the mould with triaxial load...Variable frequency,a new pattern of pulse hydraulic fracturing,is presented for improving permeability in coal seam.A variable frequency pulse hydraulic fracturing testing system was built,the mould with triaxial loading was developed.Based on the monitor methods of pressure sensor and acoustic emission,the trials of two patterns of pulse hydraulic fracturing of single frequency and variable frequency were carried out,and at last fracturing mechanism was analyzed.The results show that the effect of variable frequency on fracture extension is better than that of single frequency based on the analysis of macroscopic figures and AE.And the shortage of single frequency is somewhat remedied when the frequency is variable.Under variable frequency,the pressure process can be divided into three stages:low frequency band,pressure stability band and high frequency band,and rupture pressure of the sample is smaller than that of the condition of single frequency.Based on the Miner fatigue theory,the effect of different loading sequences on sample rupture is discussed and the results show that it is better to select the sequence of low frequency at first and then high frequency.Our achievements can give a basis for the improvement and optimization of the pulse hydraulic fracturing technology.展开更多
AIM: To investigate the relationship between cholecystectomy and fatty liver disease(FLD) in a Chinese population.METHODS: A total of 32428 subjects who had voluntarily undergone annual health checkups in the Second A...AIM: To investigate the relationship between cholecystectomy and fatty liver disease(FLD) in a Chinese population.METHODS: A total of 32428 subjects who had voluntarily undergone annual health checkups in the Second Affiliated Hospital of Nanjing Medical University from January 2011 to May 2013 were included in this study. Basic data collection, physical examination, laboratory examination, and abdominal ultrasound examination were performed.RESULTS: Subjects undergoing cholecystectomy were associated with greater age, female sex, higher body mass index, and higher levels of systolic blood pressure, diastolic blood pressure, fasting plasma glucose, total cholesterol, and triglycerides. However, no significant differences were found in high-density lipoprotein cholesterol, low-density lipoprotein cholesterol, alanine aminotransferase, aspartate aminotransferase, gammaglutamyl transpeptidase, albumin, and serum uric acid. The overall prevalence of FLD diagnosed by ultrasonography was high at 38.4%. The prevalence of FLD was significantly higher for subjects who had undergone cholecystectomy(46.9%) than those who had not undergone cholecystectomy(38.1%; χ2 test, P < 0.001). Cholecystectomy was positively associated with FLD(OR = 1.433, 95%CI: 1.259-1.631). However, after adjusting for possible factors associated withFLD, multivariate regression analysis showed that the association between cholecystectomy and FLD was not statistically significant(OR = 1.096; 95%CI: 0.939-1.279). CONCLUSION: According to our study results, cholecystectomy may not be a significant risk factor for FLD.展开更多

This research is to determine whether hER-α36 can enhance or weaken chemosensitivity to docetaxel in breast cancer cell line MCF-7(ERα66 positive). Methods: RT-PCR was used to detect the expressions of ERα66 and ERa36 in the two human breast cancer cell lines MCF-7(MCF-7/ERα66) and MCF-7 transfected with ERa36(MCF-7/ERα36). The two cell lines were treated with docetaxel(0-100umol/L), and cell growth and apoptosis were evaluated using MTT (3-(4,5-dimethylthiazol- 2-yl)-2,5-diphenyl tetrazolium bromide) assay (using adriamycin (0-50umol/L) as the control) and flowcytometry. Western blot analysis was used to measure the effect of docetaxel on phosphor-ERKl/2 expression in the two cell lines. Results: The expressions of ERct36 and ERα66 were detectable in both MCF-7/ERα66 and MCF-7/ERα36 cell lines, while the expression of ERα36 in MCF-7/ER36 cells was higher. Both docetaxel and adriamycin inhibited the proliferation of both cells lines in a dose and time dependent manner. In comparison with MCF-7/ERα36 cell line, the MCF-7/ERα66 cells produced greater growth inhibition and apoptosis after treatment with docetaxel, but there was no significant difference in growth inhibition between the two cell lines treated with adriamycin; The MCF-7/ERα36 cell line resulted in a significant activation (phosphorylation) of ERK1/2 after treatment with docetaxel in a dose-dependent manner, but in the MCF-7/ERα66 cell line , a decrease in the level of phosphor- ERK1/2 expression was observed as the dose of docetaxel increased. Conclusion: ERa36 may be an agent that weakens chemosensitivity to docetaxel in breast cancer, probably by activating the expression of ERKI/2.展开更多
BACKGROUND Hepatoid adenocarcinoma of the lung(HAL) is an extremely rare malignant tumor,and many patients with HAL exhibit high levels of alpha-fetoprotein(AFP) expression.Currently,there is no standardized treatment...BACKGROUND Hepatoid adenocarcinoma of the lung(HAL) is an extremely rare malignant tumor,and many patients with HAL exhibit high levels of alpha-fetoprotein(AFP) expression.Currently,there is no standardized treatment strategy for advanced HAL and its prognosis is poor.CASE SUMMARY We report a 55-year-old man with unresectable AFP-related HAL.The largest cross-sectional area of the mass in the upper lobe of the left lung at the beginning of treatment was 8.46 cm × 6.53 cm.The patient’s serum AFP level was 9283 ng/m L.The mass increased in size to 8.86 cm × 8.21 cm after two courses of platinum-based combination chemotherapy and immunotherapy,and serum AFP reached its highest level(71232.2 ng/m L).The patient was treated with sorafenib(400 mg twice daily,per os).Forty days later,the mass was reduced to 5.63 cm × 5.29 cm and serum AFP level dropped to 786.8 ng/m L.The patient achieved partial remission for > 9 mo with sorafenib and an excellent biomarker response,as well as survival > 13 mo,which is among the longest reported for unresectable stage IV HAL.CONCLUSION This is the first report to document successful treatment of unresectable AFPrelated HAL with single-agent sorafenib after multiline therapy.展开更多
BACKGROUND Acute chest pain(ACP)is very common among patients presenting to emergency departments.Nevertheless,ACP caused by esophageal foreign body is relatively rarely reported.CASE SUMMARY A 56-year-old man sufferi...BACKGROUND Acute chest pain(ACP)is very common among patients presenting to emergency departments.Nevertheless,ACP caused by esophageal foreign body is relatively rarely reported.CASE SUMMARY A 56-year-old man suffering from chest pain(increased pain for the last 9 h)was admitted to our hospital on October 25,2015.After undergoing physical examination and laboratory blood testing,he was diagnosed with acute anterior myocardial infarction.Consequently,the patient underwent emergency percutaneous coronary angiography;however,no myocardial infarction signs were observed.Later on,the patient experienced respiration failure and therefore was transferred to intensive care unit.Cardiac ultrasound showed pericardial effusion,which was considered as the cause of shock.He then underwent pericardium puncture drainage and the circulation temporarily improved.Nevertheless,persistent pericardial bleeding,unclear bleeding causes,and clot formation induced poor drainage led to worsening of cardiac tamponade symptoms.Consequently,the patient underwent emergency exploratory thoracotomy,which revealed a fish bone causing pericardial bleeding.The bone was removed,and the damaged blood vessels were mended.Eventually,the patient was discharged in good clinical condition.CONCLUSION For patients with chest pain,it is necessary to consider the possibility of foreign body in the esophagus or even in the heart.Careful history taking and the corresponding inspection can help to avoid unnecessary damage and safeguard patients from unnecessary pain.展开更多

The results show that the kinetic curves of methane desorption based on PI and HI are consistent with each other, and the diffusion model can best describe the characteristics of meth- ane desorption. Initial velocity, diffusion capacity and ultimate desorption amount of methane desorption after P! are greater than those after HI, and the ultimate desorption amount increases by 16.7-39.7%. Methane decay rate over the time is less than that of the HI. The PI influences the diffusion model param- eters, and it makes the mass transfer Biot number B'_i decrease and the mass transfer Fourier series F'_0 increase. As a result, PI makes the methane diffusion resistance in the coal smaller, methane diffusion rate greater, mass transfer velocity faster and the disturbance range of methane concentration wider than HI. Therefore, the effect of methane desorption based on PI is better than that of HI.展开更多

However, no significant differences were found in high-density lipoprotein cholesterol, low-density lipoprotein cholesterol, alanine aminotransferase, aspartate aminotransferase, gammaglutamyl transpeptidase, albumin, and serum uric acid. The overall prevalence of FLD diagnosed by ultrasonography was high at 38.4%. The prevalence of FLD was significantly higher for subjects who had undergone cholecystectomy(46.9%) than those who had not undergone cholecystectomy(38.1%; χ2 test, P < 0.001). Cholecystectomy was positively associated with FLD(OR = 1.433, 95%CI: 1.259-1.631). However, after adjusting for possible factors associated withFLD, multivariate regression analysis showed that the association between cholecystectomy and FLD was not statistically significant(OR = 1.096; 95%CI: 0.939-1.279). CONCLUSION: According to our study results, cholecystectomy may not be a significant risk factor for FLD.
文摘Objective: hER-α36 is a variant of estrogen receptor-a, identified and cloned by a team of American. This research is to determine whether hER-α36 can enhance or weaken chemosensitivity to docetaxel in breast cancer cell line MCF-7(ERα66 positive). Methods: RT-PCR was used to detect the expressions of ERα66 and ERa36 in the two human breast cancer cell lines MCF-7(MCF-7/ERα66) and MCF-7 transfected with ERa36(MCF-7/ERα36). The two cell lines were treated with docetaxel(0-100umol/L), and cell growth and apoptosis were evaluated using MTT (3-(4,5-dimethylthiazol- 2-yl)-2,5-diphenyl tetrazolium bromide) assay (using adriamycin (0-50umol/L) as the control) and flowcytometry. Western blot analysis was used to measure the effect of docetaxel on phosphor-ERKl/2 expression in the two cell lines. Results: The expressions of ERct36 and ERα66 were detectable in both MCF-7/ERα66 and MCF-7/ERα36 cell lines, while the expression of ERα36 in MCF-7/ER36 cells was higher. Both docetaxel and adriamycin inhibited the proliferation of both cells lines in a dose and time dependent manner. In comparison with MCF-7/ERα36 cell line, the MCF-7/ERα66 cells produced greater growth inhibition and apoptosis after treatment with docetaxel, but there was no significant difference in growth inhibition between the two cell lines treated with adriamycin; The MCF-7/ERα36 cell line resulted in a significant activation (phosphorylation) of ERK1/2 after treatment with docetaxel in a dose-dependent manner, but in the MCF-7/ERα66 cell line , a decrease in the level of phosphor- ERK1/2 expression was observed as the dose of docetaxel increased. Conclusion: ERa36 may be an agent that weakens chemosensitivity to docetaxel in breast cancer, probably by activating the expression of ERKI/2.
文摘BACKGROUND Hepatoid adenocarcinoma of the lung(HAL) is an extremely rare malignant tumor,and many patients with HAL exhibit high levels of alpha-fetoprotein(AFP) expression.Currently,there is no standardized treatment strategy for advanced HAL and its prognosis is poor.CASE SUMMARY We report a 55-year-old man with unresectable AFP-related HAL.The largest cross-sectional area of the mass in the upper lobe of the left lung at the beginning of treatment was 8.46 cm × 6.53 cm.The patient’s serum AFP level was 9283 ng/m L.The mass increased in size to 8.86 cm × 8.21 cm after two courses of platinum-based combination chemotherapy and immunotherapy,and serum AFP reached its highest level(71232.2 ng/m L).The patient was treated with sorafenib(400 mg twice daily,per os).Forty days later,the mass was reduced to 5.63 cm × 5.29 cm and serum AFP level dropped to 786.8 ng/m L.The patient achieved partial remission for > 9 mo with sorafenib and an excellent biomarker response,as well as survival > 13 mo,which is among the longest reported for unresectable stage IV HAL.CONCLUSION This is the first report to document successful treatment of unresectable AFPrelated HAL with single-agent sorafenib after multiline therapy.

文摘BACKGROUND Acute chest pain(ACP)is very common among patients presenting to emergency departments.Nevertheless,ACP caused by esophageal foreign body is relatively rarely reported.CASE SUMMARY A 56-year-old man suffering from chest pain(increased pain for the last 9 h)was admitted to our hospital on October 25,2015.After undergoing physical examination and laboratory blood testing,he was diagnosed with acute anterior myocardial infarction.Consequently,the patient underwent emergency percutaneous coronary angiography;however,no myocardial infarction signs were observed.Later on,the patient experienced respiration failure and therefore was transferred to intensive care unit.Cardiac ultrasound showed pericardial effusion,which was considered as the cause of shock.He then underwent pericardium puncture drainage and the circulation temporarily improved.Nevertheless,persistent pericardial bleeding,unclear bleeding causes,and clot formation induced poor drainage led to worsening of cardiac tamponade symptoms.Consequently,the patient underwent emergency exploratory thoracotomy,which revealed a fish bone causing pericardial bleeding.The bone was removed,and the damaged blood vessels were mended.Eventually,the patient was discharged in good clinical condition.CONCLUSION For patients with chest pain,it is necessary to consider the possibility of foreign body in the esophagus or even in the heart.Careful history taking and the corresponding inspection can help to avoid unnecessary damage and safeguard patients from unnecessary pain.

文摘In order to understand the kinetic characteristics of coal gas desorption based on the pulsating injection (PI), the research experimentally studied the kinetic process of methane desorption in terms of the PI and hydrostatic injection (HI). The results show that the kinetic curves of methane desorption based on PI and HI are consistent with each other, and the diffusion model can best describe the characteristics of meth- ane desorption. Initial velocity, diffusion capacity and ultimate desorption amount of methane desorption after P! are greater than those after HI, and the ultimate desorption amount increases by 16.7-39.7%. Methane decay rate over the time is less than that of the HI. The PI influences the diffusion model param- eters, and it makes the mass transfer Biot number B'_i decrease and the mass transfer Fourier series F'_0 increase. As a result, PI makes the methane diffusion resistance in the coal smaller, methane diffusion rate greater, mass transfer velocity faster and the disturbance range of methane concentration wider than HI. Therefore, the effect of methane desorption based on PI is better than that of HI.
