AIM:To evaluate potential risk factors in the development of ulcerative colitis(UC) in China.METHODS:A total of 1308 patients with UC and 1308 age-matched and sex-matched controls were prospectively studied in China.T...AIM:To evaluate potential risk factors in the development of ulcerative colitis(UC) in China.METHODS:A total of 1308 patients with UC and 1308 age-matched and sex-matched controls were prospectively studied in China.The UC cases were collected from 17 hospitals in China from April 2007 to April 2010.Uniform questionnaires were designed to investigate risk factors including smoking,appendectomy,stress,socio-economic conditions,nonsteroidal antiinflammatory drugs(NSAIDs),oral contraceptives,diet,breastfeeding,infections and family sanitary conditions.Group comparisons by each factor were done using simple logistic regression analysis.Conditional logistic regression was used for multivariate analysis.RESULTS:By univariate analysis,the variables predictive of UC included feeling stress,light and heavy alcoholic drinking,spicy food,sugar consumption and infectious diarrhea,while heavy tea intake and tap water consumption were protective against UC.On multivariate analysis,the protective factor for UC was tap water consumption [odds ratios(OR) = 0.424,95%CI:0.302-0.594,P < 0.001];while the potential risk factors for UC were heavy sugar consumption(OR = 1.632,95%CI:1.156-2.305,P < 0.001),spicy food(light intake:OR = 3.329,95%CI:2.282-4.857,P < 0.001;heavy intake:OR = 3.979,95%CI:2.700-5.863,P < 0.001),and often feeling stress(OR = 1.981,95%CI:1.447-2.711,P < 0.001).Other factors,such as smoking habit,appendectomy,breastfeeding,a history of measles,rural or urban residence,education,oral contraceptives,and NSAID use have not been found to have a significant association with the development of UC in the present study.CONCLUSION:Our study showed tap water consumption was a protective factor for UC,while spicy food,heavy sugar consumption and often feeling stress were risk factors for UC in this Chinese population.展开更多

METHODS: Diclofenac (2.5 mg/kg) was administered once daily for 3 d orally. A control group received the vehicle by gavage. Rebamipide (100 mg/kg, 200 mg/kg, 400 mg/kg) was administered intragastrically once a day for 3 d 4 h after diclofenac administration. Intestinal permeability was evaluated by Evans blue and the FITC-dextran method. The ultrastructure of the mucosal barrier was evaluated by transmission electron microscopy (TEM). Mitochondrial function including mitochondrial swelling, mitochondrial membrane potential, mitochondrial nicotinamide adenine dinucleotide-reduced (NADH) levels, succinate dehydrogenase (SDH) and ATPase activities were measured. Small intestinal mucosa was collected for assessment of malondialdehyde (MDA) content and myeloperoxidase (MPO) activity. RESULTS: Compared with the control group, intestinal permeability was significantly increased in the diclofenac group, which was accompanied by broken tight junctions, and significant increases in MDA content and MPO activity. Rebamipide significantly reduced intestinal permeability, improved inter-cellular tight junctions, and was associated with decreases in intestinal MDA content and MPO activity. At the mitochondrial level, rebamipide increased SDH and ATPase activities, NADH level and decreased mitochondrial swelling. CONCLUSION: Increased intestinal permeability induced by diclofenac can be attenuated by rebamipide, which partially contributed to the protection of mitochondrial function.展开更多
AIM:To explore the association of MYO9B gene polymorphisms with clinical phenotypes and intestinal permeability of individuals with inflammatory bowel disease(IBD)in China.METHODS:A total of 442 IBD patients and 402 h...AIM:To explore the association of MYO9B gene polymorphisms with clinical phenotypes and intestinal permeability of individuals with inflammatory bowel disease(IBD)in China.METHODS:A total of 442 IBD patients and 402 healthy volunteers were genotyped for two single nucleotides(rs962917 and rs1545620)using the ligase detection reaction and polymerase chain reaction.Allelic and genotype frequency analyses were performed for the two groups.Intestinal permeability was evaluated using lactulose(L)and mannitol(M)excretion.The association of MYO9B gene polymorphisms with intestinal permeability between the normal and high intestinal permeability groups was analyzed.RESULTS:Overall,there was no significant difference in the genotypic and allelic frequencies of MYO9B between IBD patients and controls.Although no association was found with ulcerative colitis in the comparison between the subgroups,the frequencies of rs962917and rs1545620 were different in the Crohn’s disease(CD)subgroup with ileocolitis(CC vs CT and TT,P=0.014;and AA vs AC and CC,P=0.022,respectively).rs1545620 variants appear to be the genetic susceptibility factor for perianal disease in CD patients(AA vs AC CC,P=0.029).In addition,the L/M ratio was significantly higher in IBD patients than in controls(0.065±0.013 vs 0.020±0.002,P=0.02),but no association was found between the MYO9B gene and the L/M ratio in IBD patients.CONCLUSION:MYO9B gene polymorphisms may influence the sub-phenotypic expression of CD in China.No association between these MYO9B polymorphisms and intestinal permeability in IBD patients was found.展开更多
Mesalamine suppositories have been used widely for the treatment of distal ulcerative colitis and considered to be safer than systemic administration for its limited systemic absorption.However,previous studies have s...Mesalamine suppositories have been used widely for the treatment of distal ulcerative colitis and considered to be safer than systemic administration for its limited systemic absorption.However,previous studies have shown that mesalamine suppository occasionally causes severe hypersensitivity reactions including fever,rashes,colitis exacerbation and acute eosinophilic pneumonia.Here we present a 25-year-old woman with ulcerative colitis with bloody diarrhea accompanied by abdominal pain and fever which were aggravated after introduction of mesalamine suppositories.In light of symptom exacerbation of ulcerative colitis,increased inflammatory injury of colon mucosa shown by colonoscopy and elevated peripheral eosinophil count after mesalamine suppositories administration,and the Naranjo algorithm score of 10,the possibility of hypersensitivity reaction to mesalamine suppositories should be considered,warning us to be aware of this potential reaction after administration of mesalamine formulations even if it is the suppositories.展开更多
The implementation of the Grain for Green Program(GGP)has changed the development track of the agricultural eco-economic system in China.In response to the results of a lag study that investigated the coupling between...The implementation of the Grain for Green Program(GGP)has changed the development track of the agricultural eco-economic system in China.In response to the results of a lag study that investigated the coupling between the GGP and the agricultural eco-economic system in a loess hilly region,we used a structural equation model to analyze the survey data from 494 households in Ansai,a district of Yan’an City in Shaanxi Province of China in 2015.The model clarified the direction and intensity of the coupling between the GGP and the agricultural eco-economic system.The coupling benefits were derived through linkages between the program and various chains in the agricultural eco-economic system.The GGP,the agroecosystem of Ansai and their potential coupling effects were in a state of general coordination.The agroecosystem directly affected the coupling effect,with the standardized path coefficient of 0.87,indicating that the agroecosystem in Ansai at this stage provided basic material support for the coupling between the GGP and the agricultural eco-economic system.The direct path coefficient of agroeconomic system impacted on the coupling effect was-0.76,indicating that partial contradictions occurred between the agroeconomic system and the coupling effect.Therefore,although the current agroecosystem in Ansai should be provided sufficient agroecological resources for the benign coupling between the program and the agricultural eco-economic system,agricultural development failed to effectively transform agroecological resources into agricultural economic advantages in this region,which resulted in a relative lag in the development of the agricultural economic system.Thus,the coupling between the GGP and the agricultural eco-economic system was poor.To improve the coupling and the sustainable development of the agricultural eco-economic system in cropland retirement areas,the industrial structure needs to be diversified,the agricultural resources(including agroecological resources,agricultural economic resources and agricultural social resources)need to be rationally allocated,and the chain structure of the agricultural eco-economic system needs to be continuously improved.展开更多

文摘目的研究俯卧位手术患者面部受压部位的压力与术中压力性损伤的相关性。方法选取2018年6月—12月医院收治的行手术治疗的患者201例,剔除手术时间<2 h者8例,共纳入193例。通过测量工具测出俯卧位下患者面部各个受压部位的压力,并依据面部发生压力性损伤与否将患者分成2组,比较2组患者相关指标,分析面部压力性损伤的相关因素。结果患者面部受压部位压力3-97 mm Hg,其中额部压力最大达到(56.93±15.44)mm Hg,额部压力与下颌和左、右颧骨部位压力差异有统计学意义(P<0.01)。头面部发生压力性损伤患者31例,共32处,其中以额部最多,有25处。发生压力性损伤患者和未发生压力性损伤患者的额部压力、白蛋白水平、俯卧位时间、失血量和手术时间比较,差异有统计意义(P<0.01)。结论通过压力的测量为俯卧位手术患者避免头面部压力性损伤提供了客观依据,可以通过压力测量来指导俯卧位放置。
