精细化管理模式在疾控机构的应用效果研究

目的探讨精细化管理模式在疾控机构的应用效果,以期提高疾控机构的管理效率和防控效果。方法选取2021年1月—2022年12月南通市疾控中心的工作人员30名为研究对象,2021年1月—2021年12月采取传统管理模式(传统组),2022年1月—2022年12月... 目的探讨精细化管理模式在疾控机构的应用效果,以期提高疾控机构的管理效率和防控效果。方法选取2021年1月—2022年12月南通市疾控中心的工作人员30名为研究对象,2021年1月—2021年12月采取传统管理模式(传统组),2022年1月—2022年12月采取精细化管理模式(精细组),对比不同管理模式下的管理质量评分和管理满意度。结果精细组管理质量评分高于传统组,差异有统计学意义(P<0.05)。精细组管理满意度为100.00%高于传统组的73.33%,差异有统计学意义(P<0.05)。结论精细化管理模式在疾控机构的应用效果显著,能够有效提升疾控机构的管理质量和防控效果,提高管理满意度。 展开更多

AIM:To investigate the function of microRNA-143(miR-143)in gastric cancer and explore the target genes of miR-143.METHODS:A quantitative real-time reverse transcription-polymerase chain reaction(RT-PCR)analysis was performed to evaluate miR-143 expression in gastric cancer cell lines.After transfecting gastric cancer cells with miR-143-5p and miR-143-3p precursors,Alamar blue and apoptosis assays were used to measure the respective proliferation and apoptosis rates.Cyclooxygenase-2(COX-2)expression was determined by realtime RT-PCR and Western blot assays after miR-143transfection.Reporter plasmids were constructed,and a luciferase reporter assay was used to identify the miR-143 binding site on COX-2.RESULTS:Both miR-143-5p and miR-143-3p were significantly downregulated in multiple gastric cancer cell lines.Forced miR-143-5p and miR-143-3p expression in gastric cancer cells produced a profound cytotoxic effect.MiR-145-5p transfection into gastric cancer cells resulted in a greater growth inhibitory effect(61.23%±3.16%vs 46.58%±4.28%,P<0.05 in the MKN-1cell line)and a higher apoptosis rate(28.74%±1.93%vs 22.13%±3.31%,P<0.05 in the MKN-1 cell line)than miR-143-3p transfection.Further analysis indicated that COX-2 expression was potently suppressed by miR-143-5p but not by miR-143-3p.The activity of a luciferase reporter construct that contained the 3’-untranslated region(UTR)of COX-2 was downregulated by miR-143-5p(43.6%±4.86%,P<0.01)but not by miR-143-3p.A mutation in the miR-145-5p binding site completely ablated the regulatory effect on luciferase activity,which suggests that there is a direct miR-145-5p binding site in the 3’-UTR of COX-2.CONCLUSION:Both miR-143-5p and miR-143-3p function as anti-oncomirs in gastric cancer.However,miR-143-5p alone directly targets COX-2,and it exhibits a stronger tumor suppressive effect than miR-143-3p. 展开更多

AIM:To compare fluoroscopic, endoscopic and guide wire assistance with ultraslim gastroscopy for placement of nasojejunal feeding tubes. METHODS:The information regarding nasojejunal tube placement procedures was retrieved using the gastrointestinal tract database at Tongji Hospital affiliated to Tongji Medical College. Records from 81 patients who underwent nasojejunal tubes placement by different techniques between 2004 and 2011 were reviewed for procedure success and tube-related outcomes. RESULTS:Nasojejunal feeding tubes were successfully placed in 78 (96.3%) of 81 patients. The success rate by fluoroscopy was 92% (23 of 25), by endoscopic technique 96.3% (26 of 27), and by guide wire assistance (whether via transnasal or transoral insertion)100% (23/23, 6/6). The average time for successful placement was 14.9 ± 2.9 min for fluoroscopic placement, 14.8 ± 4.9 min for endoscopic placement, 11.1 ± 2.2 min for guide wire assistance with transnasal gastroscopic placement, and 14.7 ± 1.2 min for transoral gastroscopic placement. Statistically, the duration for the third method was significantly different (P < 0.05) compared with the other three methods. Transnasal placement over a guidewire was significantly faster (P < 0.05) than any of the other approaches. CONCLUSION:Guide wire assistance with transnasal insertion of nasojejunal feeding tubes represents a safe, quick and effective method for providing enteral nutrition. 展开更多

To analyze the effects of width and thickness of each composition element of mixed steel U-rib-stiffened plates on the welding residual stress distribution, the distribution of the U-rib and the plate residual stress was calculated using a simplified calculation method. The method involved welding the mixed steel U-rib-stiffened plates for a structure with different sizes and different strength ratios of U-rib to plate. Based on a welding residual stress numerical simulation method validated by the blind hole method test, the distribution law of the mixed steel U-rib stiffened plate was studied. The results showed that the change of plate width has little impact on the welding residual stress and that the ratio of the thicknesses of the plate to U-rib stiffeners, the thickness of the plate, and the thickness of the U-rib has a great influence on the distribution of the welding residual stress. The thickness of plate and steel strength also greatly influenced the distribution width of the residual tensile stress. While analyzing the compression capacity of U-rib-stiffened plates, the simplified distribution of welding residual stress was used. 展开更多

Ulcerative colitis (UC) is a chronic inflammatory disease and its involvement area in colon is influenced by a complex network of gene interactions. We analyzed the weighted gene co-expression networks in mieroarray dataset from colonic mucosa of patients with UC and identified one gene co-expression module that was highly associated with the progression of involved area in UC colon (Pearson coefficient=0.81, P〈0.0001). In total, 523 hub genes in this module were found to be involved in immune system process after enrichment analysis in Gene Ontology. By the STRING and Cytoscape analysis, we observed that interleukin-8 (IL- 8) and matrix metalloproteinase-9 (MMP-9) were centered in the network of hub genes. We then detected the expression of IL-8 and MMP-9 in mucosa from left-sided colon of patients using quantitative PCR and immunofluorescence assay respectively. Both quantitative PCR and immunofluorescence assay revealed the expression levels of IL-8 and MMP-9 were significantly different among the healthy controls, left-sided colitis group and pancolitis group (P〈0.05). IL-8 and MMP-9 were detected with an enhanced expression in pancolitis as compared with leftsided colitis and healthy controls, respectively (P〈0.05). This study demonstrates that immune system process is indispensable in the progression of disease in colon, and identifies that IL-8 and MMP-9 play potential critical roles for the progression. 展开更多

Use of Plastic Correction Formula to Improve Accuracy of Welding Residual Stress Test with Blind-Hole Method 被引量:2

The blind-hole method is the most widely used approach to experimentally determine the distribution of residual stress. This paper aims to improve test accuracy of welding residual stress and conducts an experimental study on the strain release factors involved when using the blind-hole method for Q235 and Q345, two steels commonly used in building structures. The ranges of strain release factors A and B in the elastic stage, the effects of strain release factors on residual stress calculated values, and the plastic corrected strain release factors are analyzed considering of the effect of plastic deformation around the blind hole on measurement accuracy. Finally, a simplified calculation formula to determine strain release factors is proposed for use with the blind-hole method. Results show that in the elastic stage, strain release factor A for Q235 and Q345 ranges from-0.399 to-0.525 and strain release factor B from-0.791 to-0.960. Changing the strain release factors A and B shows that calculated residual tensile stress varies in relation to a decrease in both factor values. However, there is a increase in calculated residual compressive stress with a decrease in the strain release factor A value, but there is an decrease with a decrease in strain release factor B value. Calculated residual stress applied to elastic strain release factors is compared with that applied to amended plastic strain release factors for Q235 steel. The maximum deviation between calculated residual stress and test stress is reduced from 21.1 to 1.0%,and for Q345 steel from 26.5 to 1.2%. It is thus evident that the plastic correction formula proposed in this paper can be used in calculations when conducting a residual stress test. 展开更多

To alleviate deck fatigue failure and regular pavement damage,which are congenital deficiencies of highway steel bridge deck structure,this paper proposes a newtype of composite bridge deck,consisting of steel tubular connectors and steel-reactive powder concrete (RPC). Push-out tests were conducted to study the newdeck's shear performance. During the experimental process,specimens were divided into two groups which are composed of steel tubular connectors with or without penetrative bars set in. Then,researchers analyzed destroyed models and mechanisms of the composite structure under shear forces. Results showed that test models in two groups,once destroyed,displayed similar shear fracture,which appeared on the lower margin of the steel tubular wall along the welds. Meanwhile,RPC under the connector,for varied tests,was crushed at the same stage,although the large shear and bending deformation just occurred on connectors with penetrative bars. Additionally,shear capacity of specimens with penetrative bars,compared with the ones without bars,unexpectedly decreased by 20%,but the structural ductility was 1.75 times as much,and the ductility coefficients of specimens were all larger than 3.5,demonstrating certain deformation capacity. 展开更多

Although the exact etiology of inflammatory bowel disease(IBD)remains unclear,exaggerated immune response in genetically predisposed individuals has been reported.Th1 and Th17 cells mediate IBD development.Macrophages produce IL-12 and IL-23 that share p40 subunit encoded by IL12B gene as heteromer partner to drive Th1 and Th17 differentiation.The available animal and human data strongly support the pathogenic role of IL-12/IL-23 in IBD development and suggest that blocking p40 might be the potential strategy for IBD treatment.Furthermore,aberrant alteration of some cytokines expression via epigenetic mechanisms is involved in pathogenesis o f IBD.In this study,we analyzed core promoter region of IL12B gene and investigated whether IL12B expression could be regulated through targeted epigenetic modification with gene editing technology.Transcription activator-like effectors(TALEs)are widely used in the field of genome editing and can specifically target DNA sequence in the host genome.We synthesized the TALE DNA-binding domains that target the promoter of human IL12B gene and fused it with the functional catalytic domains of epigenetic enzymes.Transient expression of these engineered enzymes demonstrated that the TALE-DNMT3A targeted the selected IL12B promoter region,induced loci-specific DNA methylation,and down-regulated IL-12B expression in various human cell lines.Collectively,our data suggested that epigenetic editing of IL12B through methylating DNA on its promoter might be developed as a potential therapeutic strategy for IBD treatment. 展开更多

Steel and ultra⁃high performance concrete(UHPC)composite decks are effective at reducing fatigue cracking and asphalt pavement damage.The shear behavior of innovative open steel tube(OST)connectors in steel⁃UHPC composite decks was investigated by conducting push⁃out tests.The test parameter is the presence of reinforcement in the deck.The load⁃slip curves and shear behavior of the push⁃out specimens were obtained and discussed.The test results indicate that as compared with plain concrete specimens,the limit slip of reinforced specimens decreased by 32%and the shear stiffness increased by 10%,but the ultimate shear capacity was almost the same.The use of UHPC influenced the failure process as it was observed that the OST connector was sheared off at its lower semi⁃tube,followed by the pull⁃out failure of the upper semi⁃tube.A finite element model was verified by tests and was then used to analyze the deformation and failure behaviors of the composite deck with open tubes.The model demonstrates that there is a stress concentration zone at the connector root,and the lower semi⁃tube is the main component that is subject to loads. 展开更多

[ Method ] Sixteen traps ( 4 lines x 4 rows) were placed according to the specifications of 25. 8 m ×25.8 m ( 15. 0 trap/hm2 ), 31. 6 m× 31.6 m ( 10.0 trap/hm2 ) and 36.5 m × 36.5m ( 7.5 trap/hm2 ). Meanwhile, a control area larger than 2 hm2 was set up with three repetitions. [ Result] Without consideration of impact of wind direc- tion, the amount of trapping moth increased with the increasing controlled area of single trap, but difference of trapping amount of moth was not significant under three different density treatments. Three placement densities could effectively control S. liture in tobacco fields, thereby reduce the use of pesticides and protect the natural enemies and reduce the control cost. [Conclusion] The suitable density of chemical pheromone traps to control S. litura was 31. 6 m x31. 6 m (10.0 trap/hm2 ). 展开更多

Posttranslational modifications increase the complexity and functional diversity of proteins in response to complex external stimuli and internal changes.Among these,protein lipidations which refer to lipid attachment to proteins are prominent,which primarily encompassing five types including S-palmitoylation,N-myristoylation,S-prenylation,glycosylphosphatidylinositol(GPl)anchor and cholesterylation.Lipid attachment to proteins plays an essential role in the regulation of protein trafficking,localisation,stability,conformation,interactions and signal transduction by enhancing hydrophobicity.Accumulating evidence from genetic,structural,and biomedical studies has consistently shown that protein lipidation is pivotal in the regulation of broad physiological functions and is inextricably linked to a variety of diseases.Decades of dedicated research have driven the development of a wide range of drugs targeting protein lipidation,and several agents have been developed and tested in preclinical and clinical studies,some of which,such as asciminib and lonafarnib are FDA-approved for therapeutic use,indicating that targeting protein lipidations represents a promising therapeutic strategy.Here,we comprehensively review the known regulatory enzymes and catalytic mechanisms of various protein lipidation types,outline the impact of protein lipidations on physiology and disease,and highlight potential therapeutic targets and clinical research progress,aiming to provide a comprehensive reference for future protein lipidation research. 展开更多

装配式贝雷梁杆系的受力性能精细化数值模拟方法研究

为解决贝雷梁实体模型在有限元分析中建模困难、计算代价大的弊端,采用有限元分析软件ANSYS,基于已验证的贝雷梁实体有限元模型,提出一种针对装配式贝雷梁的杆系模型,并与《装配式公路钢桥多用途使用手册》中的贝雷梁试验进行比对,验证... 为解决贝雷梁实体模型在有限元分析中建模困难、计算代价大的弊端,采用有限元分析软件ANSYS,基于已验证的贝雷梁实体有限元模型,提出一种针对装配式贝雷梁的杆系模型,并与《装配式公路钢桥多用途使用手册》中的贝雷梁试验进行比对,验证线性状态下贝雷梁实体模型与杆系模型的准确性。通过非线性屈曲分析,比较两种模型在非线性状态下的差异性,通过修正得出正确的杆系有限元模型,并采用修正后的杆系模型代替实体模型进行不同工况下的极限承载力研究,探明初始几何缺陷对贝雷梁受力性能的影响,提出了贝雷梁极限承载力验算中几何缺陷的参考幅值。结果表明:相同荷载作用下实体模型在线性阶段的位移值略大于杆系模型,跨中最大位移相差4%,但杆系模型和实体模型在各测点的位移值都与试验结果接近,且误差范围小于2%。杆系模型和实体模型的内力基本吻合,截面平均等效应力最大误差为2.78%;不同侧向支撑间距下两种模型在非线性阶段得到的失稳模式高度一致,极限承载力差值在2.4%以内,荷载-位移曲线基本吻合,破坏模式基本一致,且杆系模型的荷载-位移曲线与试验曲线吻合较好,由此可以证明,杆系模型可代替实体模型进行极限承载力分析;贝雷梁的侧向支撑间距对结构的破坏模式起决定性作用,贝雷梁的侧向支撑间距不小于7.5 m时,结构发生整体失稳,侧向支撑间距小于6 m时,贝雷梁发生局部失稳,失稳模式为跨中单元的上弦杆发生面外弯曲破坏,且出现了明显的局部失稳变形,弯曲变形最大部分的Mises应力达到屈服;当贝雷梁发生整体失稳时,整体几何缺陷对结构的承载力影响较大,而当结构发生局部失稳时,需要特别注意结构的局部几何缺陷;稳定承载力对整体缺陷的幅值较为敏感,而面内局部缺陷对贝雷梁的稳定承载力影响很小,面外局部几何缺陷对贝雷梁的稳定承载力影响较为显著,考虑面外初弯曲幅值为1/1000的初始几何缺陷时,弦杆、竖杆及斜杆的面外局部缺陷使稳定承载力分别下降6.1%、1.3%和1.2%,而面内缺陷仅使其降低了1.0%、1.0%和0.5%。故在选用贝雷片时应格外注意弦杆的面外变形。 展开更多
