糖尿病视网膜病变(diabetic retinopathy,DR)是糖尿病患者最常见的微血管并发症,已成为全球工作年龄人群视力障碍的主要原因之一。DR的发生是多种机制共同作用的结果,越来越多的研究表明昼夜节律及生物钟基因与DR的发病密切相关。昼夜... 糖尿病视网膜病变(diabetic retinopathy,DR)是糖尿病患者最常见的微血管并发症,已成为全球工作年龄人群视力障碍的主要原因之一。DR的发生是多种机制共同作用的结果,越来越多的研究表明昼夜节律及生物钟基因与DR的发病密切相关。昼夜节律是受生物钟基因调控,以24h为周期,与外界明暗变化相一致的生理过程,生物钟调控机体的各种生理活动。昼夜节律紊乱通过影响糖尿病患者的血糖水平和眼部的生理稳态等诱导DR的发生,生物钟基因可能通过调控DR的氧化应激反应、炎症反应、视网膜自噬节律、线粒体功能障碍以及内皮祖细胞功能等各方面参与DR的发病。本文介绍了昼夜节律的产生及调控机制,以及视网膜内在昼夜节律生物钟,深入探讨了昼夜节律系统以及生物钟基因在DR发生发展过程中的影响,旨在为DR的预防及治疗提供参考。 展开更多

目的:探讨象限分类法在1型阈值前期、阈值期早产儿视网膜病变中的应用。方法:收集2017-04/2018-01来南昌大学附属眼科医院治疗的1型阈值前期、阈值期早产儿视网膜病变患儿32例进行回顾性病例分析。以象限法将这些早产儿视网膜病变病例... 目的:探讨象限分类法在1型阈值前期、阈值期早产儿视网膜病变中的应用。方法:收集2017-04/2018-01来南昌大学附属眼科医院治疗的1型阈值前期、阈值期早产儿视网膜病变患儿32例进行回顾性病例分析。以象限法将这些早产儿视网膜病变病例中的主要病变位置及范围分为三类:仅分布在一个象限(鼻上、鼻下、颞上、颞下)记录为“1个象限”;分布在两个象限者(颞上+颞下、鼻上+鼻下、鼻上+颞上、鼻下+颞下、鼻上+颞下、鼻下+颞上)记录为“2个象限”,分布超过两个象限者记录为“>2个象限”。通过象限法来观察这些病例的临床特点以及玻璃体腔注射雷珠单抗治疗后的治愈及复发情况。结果:纳入研究的32例62眼早产儿视网膜病变患儿中,仅分布在1个象限的有2眼(3%),单次玻璃体腔注药治愈构成比100%;分布在2个象限的共25眼(40%),单次玻璃体腔注药治愈构成比100%;分布>2个象限的共35眼(56%),单次玻璃体腔注药治愈构成比74%,复发构成比26%。结论:1型阈值前期、阈值期早产儿视网膜病变的主要病变部位大多数分布在颞侧上下象限,而且分布的象限数越多,病情越严重,玻璃体腔注射雷珠单抗治疗的复发可能性越高。 展开更多

Comparison of visual outcomes with implantation of trifocal versus bifocal intraocular lens after phacoemulsification: a Meta-analysis 被引量:10

AIM: To compare a trifocal intraocular lens (IOL) and a bifocal IOL implantation in improving visual function after cataract surgery. METHODS: Eligible literatures were systematically searched through EMBASE and P... AIM: To compare a trifocal intraocular lens (IOL) and a bifocal IOL implantation in improving visual function after cataract surgery. METHODS: Eligible literatures were systematically searched through EMBASE and PubMed databases. The inclusion criteria were prospective comparative clinical trials on cataract surgery comparing trifocal IOL with bifocal IOL implantation that assessed visual acuity, contrast sensitivity and subjective vision quality. The effects were computed as standardized mean differences and pooled using fixed-effect or random-effect models. RESULTS: Four prospective randomized controlled trials (RCTs) and five cohorts provided data were included by a systematic review, comprising 265 eyes implanted with trifocal IOLs and 264 eyes implanted with bifocal IOLs. Monocular distance visual acuity (VA) showed a statistically significant but small difference that favored trifocal IOLs (MD=.0.06; 95%CI, -0.10 to -0.02; Z=2.90, P=0.004 for uncorrected distance VA, and MD- -0.02; 95%CI, -0.03 to .0.00; Z=2.02, P=0.04 for corrected distance VA), but the data did not suggest that the effect of trifocal IOL implantation would clinically outperform bifocal IOL implantation. There was no significant difference in monocular near VA (MD=.0.01; 95%CI, -0.07 to 0.04; Z=0.42, P=-0.68 for distance-corrected near VA, and MD=-0.01; 95%CI, -0.06 to 0.03; Z=0.55, P=0.58 for corrected near VA) or refraction between two groups. Contrast sensitivity and subjective visual quality had no conclusive results. CONCLUSION: All results indicate that trifocal IOL and bifocal IOL had similar levels of monocular distance and near VA. 展开更多

AIM:To investigate the efficacy and safety of Honghua preserved amniotic membrane(AM)for preventing scar formation of the filtering bleb in a rabbit model of glaucoma trabeculectomy surgery.METHODS:Totally 36 rabbits(36 eyes)were randomly divided into 3 groups:the experimental group(ocular trabeculectomy in combination with Honghua preserved AM transplantation),the control group(ocular trabeculectomy surgery in combination with AM implantation),and the blank group(single trabeculectomy).Clinical observations[including intraocular pressure(IOP),filtering blebs and complications],MassonTrichrome staining,real-time quantitative reverse transcription-polymerase chain reaction(real-time PCR),Western blot were performed on different time points(D1,D7,D14,D21 and D56)after the surgery.RESULTS:After operated for 14d,there were statistically significant differences in the filtering blebs compared to the situation before operation(P【0.05),whereas no statistically difference on that among three groups(P】0.05).After 21d,the IOP of experimental group was lowest(P【0.05).There was significant difference between control group and blank group(P【0.05).On postoperative D14,the mean number of fibroblasts in the experimental group was significantlylower(40.6±10.2)compared to those in the control group(54.4±10.8)and blank group(68.2±11.6)(P【0.05,respectively).The mean numbers of the macrophage in the experimental and control groups were respcitively significantly lower versus the blank group(P【0.05,P【0.05,respectively).Compared to that in blank group,the level of transforming growth factor-β(TGF-β1)expression in sclera and conjunctival areas was reduced in the experimental and control groups on protein and mRNA level(P【0.05),but not significant difference between these two groups(P】0.05).CONCLUSION:The trabeculectory surgery with Honghua preserved AM can control IOP,sustain the functional filtration bleb,inhibit the proliferation of fibroblasts and open the filtrating pathway on the rabbit glaucoma models. 展开更多

·AIM:To compare the efficacy and safety of intravitreal aflibercept with dexamethasone implant in the treatment of macular edema(ME)associated with diabetic retinopathy(DR)or retinal vein occlusion(RVO).·MET... ·AIM:To compare the efficacy and safety of intravitreal aflibercept with dexamethasone implant in the treatment of macular edema(ME)associated with diabetic retinopathy(DR)or retinal vein occlusion(RVO).·METHODS:A comprehensive search of studies comparing dexamethasone and aflibercept in patients with ME was conducted at PubMed,Embase,and Cochrane Central Register of Controlled Trials from the beginning of library to April 16,2021.Extracting the data including bestcorrected visual acuity(BCVA),central retinal thickness(CRT),number of injections and serious adverse events(SAEs)from the final qualified articles.RevMan 5.3 software was used for Meta-analysis of the included studies.·RESULTS:Totally 7 studies with 369 eyes were included.The causes of ME in the final screening study included RVO and DR.Compared with the aflibercept treatment group,the BCVA of the dexamethasone implant treatment group showed no significant difference in the follow-up for 3 mo[mean difference(MD):-0.05,95%confidence interval(CI):-0.11,0.02;P=0.17]and 12 mo(MD:-0.01,95%CI:-0.38,0.37;P=0.98),but it was slightly worse than the aflibercept group at 6 mo(MD:0.12.95%CI:0.03,0.21;P=0.008).In terms of CRT reduction,there was no significant difference between the two groups at 3 mo(MD:-28.14,95%CI:-79.95,23.67;P=0.29),6 mo(MD:27.67,95%CI:-84.89,140.24;P=0.63),and 12 mo(MD:-59.00,95%CI:-127.37,9.37;P=0.09).However,dexamethasone implant had fewer injections,but more adverse events such as elevated intraocular pressure(IOP)and cataract.·CONCLUSION:Intravitreal injection of aflibercept and dexamethasone implant can both effectively increase BCVA and reduce CRT.Compared with aflibercept,dexamethasone implant is not inferior in improving vision and reducing CRT in the initial treatment period(3 mo)and long-term treatment period(12 mo).Besides,it has fewer injections and more likely to cause elevated IOP and cataract. 展开更多

Efficacy of wet-lab training versus surgical-simulator training on performance of ophthalmology residents during chopping in cataract surgery 被引量:5

AIM: To analyze whether wet-lab training(WLT) or surgical-simulator training(SST) is better for ophthalmology residents to master the chopping technique.METHODS: Sixty ophthalmology residents(in their second year) and three cataract surgeons participated in the study. The residents were randomly separated into two groups, WLT group and SST group. The residents in WLT group were asked to perform 10 trials of chopping using pig eyes and scored by the surgeons, and then they performed and scored using simulator for one time. The residents in SST group underwent 10 trials of chopping using simulator, and the simulator scored each trail. Then, this group were asked to perform the chopping using pig eyes and scored by the surgeons. At last, we investigated the residents’ satisfaction about the training.RESULTS: The demographic characteristics had no significant differences between the two groups. Recorded by the simulator, the residents in SST group got significantly higher overall score(83.90±1.31) than WLT group(78.73±1.92, P=0.03). And the residents in SST group got less corner area injured, and they spend less time than WLT group(P<0.05). Moreover, the residents in WLT group used more ultrasonic energy value than SST group(P=0.03). However, scored by the surgeons, the residents in two groups got nearly the same overall score. The residents in WLT group performed better on the frequencies of posterior capsule torn and incisional stress(P=0.03, 0.008, respectively). In the survey, the residents in two groups held the same opinion that the training was helpful and they strongly recommended this training. And all of them enjoyed the training, and enjoyed being randomized in their own group. However, with respect to the realistic character, the residents thought that WLT was better than SST(P<0.001).CONCLUSION: Both of the Eyesi surgical-stimulator and the wet-lab improve the residents’ chopping ability and each has its own advantages. The combination of the two training ways could be considered to be a part of the training curriculum for new residents. 展开更多

AIM:To investigate the association of Manganese superoxide dismutase(Mn SOD) Val16 Ala polymorphism with diabetic retinopathy(DR).METHODS:Pub Med,Embase,China National Knowledge Infrastructure,and Wanfang databases were searched.The pooled odds ratios(ORs) and 95% confidence intervals(CIs) were calculated to evaluate the strength of the association.Subgroup,sensitivity,and cumulative analyses were performed.Publication bias was also analyzed.RESULTS:Eight studies were included in the pooled analysis.The Mn SOD Val16 Ala polymorphism was associated with the risk of DR under the dominant model(OR=0.66,95%CI=0.48-0.91,P〈0.0001),this result was demonstrated to be relatively stable in cumulative analysis.No significant publication bias was found.This polymorphism was also associated with the risk of DR in Caucasians under the dominant model(OR=0.64,95%CI=0.42-0.97,P=0.04,) and in Asians under the recessive model(OR=0.31,95%CI=0.11-0.88,P=0.03).CONCLUSION:These findings suggest that the Mn SOD Val16 Ala polymorphism is a risk factor for DR,and that more attention should be paid to carriers of these susceptibility genes. 展开更多

Photograph, slit-lamp observation on various eye showed normal corneal transparence.RESULTS: A strong α-Gal expression in invertebrates and some vertebrates ocular tissue, but no α-Gal binding in birds, fish and mammal. α-Gal expression change in the development of mice ocular surface tissue (except sclera) and display genus dependency in the different murine ocular surface tissue.CONCLUSION: This study identified specific α-Gal epitopes binding area in the ocular surface of several species and may solve the problem that naive ocular surface may be used as natural α-Gal gene knockout model/high risk immunologic rejection model or ocular surface scaffold material. 展开更多

Effectiveness of peripheral defocus spectacle lenses in myopia control: a Meta-analysis and systematic review 被引量:1

AIM: To evaluate the effectiveness of peripheral defocus spectacle lenses(PDLs) in myopia control. METHODS: Literature retrieval on Pub Med, Cochrane Library, Embase, and Web of Science databases, and the search time limit was from the establishment of each database to December 29, 2021 were conducted. Change of spherical equivalent refraction(SER) and axial change(AL) were extracted from the literatures that met the inclusion criteria, and Rev Man5.3 software was used for Meta-analysis. RESULTS: A total of 4 randomized controlled trials(RCTs) were included in this Meta-analysis, involving 770 myopic children. The results showed that PDLs could delay the progression of myopia in children with myopia compared with single vision spectacle lenses(SVLs;WMD=0.21 D, 95%CI: 0.01, 0.41, P=0.04). However, there was no significant difference in controlling the growth of axial length(AL) in myopic children(WMD=-0.10 mm, 95%CI:-0.21, 0.01, P=0.07). The results of the effectiveness of myopia control between the two spectacle lenses showed that PDLs were more effective in controlling the progression of myopia(OR=5.73, 95%CI: 2.58, 12.70, P<0.001) and delaying the growth of AL(OR=44.25, 95%CI: 8.84, 221.58, P<0.001) than SVLs, and the differences were statistically significant. CONCLUSION: PDLs can control the progression of myopia compared with SVLs, but cannot delay the growth of AL, and the effectiveness of PDLs in myopia control better than SVLs. 展开更多

AIM:To investigate whether inhibition of acetylcholinesterase(AChE)by donepezil ameliorate aberrant retinal neovascularization(RNV)and abnormal glial activation in oxygen-induced retinopathy(OIR).METHODS:A mouse model of RNV was induced in postnatal day 7(P7)mice by exposure to 75%oxygen.Donepezil was administrated to P12 mice by intraperitoneal injection.Expression and localization of AChE in mouse retinas were determined by immunofluorescence.RNV was evaluated by paraffin sectioning and hematoxylin and eosin(HE)staining.Activation of retinal M uller glial cells were examined by immunoblot of glial fibrillary acidic protein(GFAP).rMC-1.a retinal Muller cell line,was used for in vitro study.Expression of hypoxia-induced factor 1α(HIF-1α)and vascular endothelial growth factor(VEGF)were determined by Western-blot analysis,enzyme-linked immunosorbent assay(ELISA)or immunostaining.RESULTS:Aberrant RNV and glial activation was observed after OIR.Of note,retinal AChE was mainly expressed by retinal Muller glial cells and markedly increased in OIR mice.Systemic administration of donepezil significantly reduced RNV and abnormal glial activation in mice with OIR.Moreover,ischemia-induced HIF-1αaccumulation and VEGF upregulation in OIR mouse retinas and cultured rMC-1 were significantly inhibited by donepezil intervention.CONCLUSION:AchE is implicated in RNV with OIR.Inhibition of AChE by donepeizl is likely to be a potential therapeutic approach for retinal neovascular diseases. 展开更多

AIM:To evaluate efficacy of intravitreal ranibizumab(IVR)therapy for aggressive posterior retinopathy of prematurity(ROP),threshold ROP disease and type 1 pre-threshold ROP.METHODS:A retrospective analysis was performed on 40 patients(76 eyes)who had IVR as the primary treatment for ROP from April 2017 to January 2018.According to disease pathogenic features,the 76 eyes were divided into three groups:aggressive posterior ROP(AP-ROP)group(16 eyes),threshold ROP group(28 eyes)and type 1 pre-threshold ROP group(32 eyes).The characteristics of patients and lesions situation before the first intravitreal injection,and posttreatment fundus outcomes determined by wide-angle Ret Cam fundus imaging were recorded.RESULTS:The birth weight and postmenstrual age of first IVR treatment in AP-ROP,threshold ROP,and type 1 pre-threshold ROP groups were significant difference(1087.50±246.78,1103.75±168.30,1257.03±210.82 g,P=0.005;34.50±1.46,36.89±2.97,36.50±2.36 wk,P=0.008),while the gestational age was not difference(28.00±2.00,28.54±1.90,28.59±1.43 wk,P=0.510).The retina hemorrhage ratio(with/without:14/2,8/20,5/27),iris neovascularization or vascular engorgement ratio(with/without:12/4,11/17,6/26),and the zone I(inside/outside:16/0,2/26,5/27)in AP-ROP,threshold ROP,and type 1 pre-threshold ROP group were difference significantly(all P<0.05).The regression rates were 37.5%,92.86%,and 100%,and the recurrence rates were 62.5%,7.14%,and 0 in AP-ROP,threshold ROP,and type 1 pre-threshold ROP group,respectively(both P<0.05).The recurrence eyes were cured by secondary IVR or retinal laser photocoagulation.CONCLUSION:IVR is an effective treatment for all types of ROP.The regression of AP-ROP is significantly lower than type 1 pre-threshold and threshold disease.Birth weight,retinal hemorrhage,iris neovascularization or vascular engorgement and lesions located in zone I may be associated with AP-ROP recurrence and retreatment,which should be noted in follow-up. 展开更多

AIM:To compare the dif ferences of choroidal neovascularization(CNV)measurements between sweptsource and spectral-domain optical coherence tomography angiography(SS-OCTA and SD-OCTA)in neovascular agerelated macular degeneration(nAMD)and the imaging reliability of the two devices.METHODS:Prospective comparative study.SS-OCTA and SD-OCTA were used to scan the same eye with the modes of 3×3 and 6×6 mm2 centered on the neovascularization.Only qualified images were chosen and the border of CNV was manually delineated by two graders independently.The area of CNV(ACNV),vascular perfusion density(PD),and vessel length density(VLD)within the delineation were calculated using Image J.The differences of CNV measurements between the two OCTA devices were compared using Bland-Altman analysis.The agreement between the two graders on the measurements of each device was compared using the intraclass correlation coefficient(ICC).RESULTS:A total of 18 patients(22 eyes)with nAMD were included.The measurements of ACNV,PD,and VLD were 7.247±4.586 and 4.901±3.741 mm^(2),43.202±9.636 and 34.904±10.489,6.339±1.228 and 5.908±1.741 mm^(-1) for SS-OCTA and SD-OCTA,respectively.The differences between the two devices were 2.346±3.030 mm^(2)(Z=-3.782,P<0.0001),8.298±14.160(Z=-2.419,P=0.016),and 0.431±2.114 mm^(-1)(Z=-0.828,P=0.408)for ACNV,PD and VLD,respectively.The ICC between two graders were 0.893(P<0.001),0.902(P<0.001),0.885(P<0.001)for ACNV,PD,VLD in SS-OCTA,and 0.971(P<0.001),0.976(P<0.001),0.973(P<0.001)in SD-OCTA,respectively.CONCLUSION:Both OCTA devices have high imaging reliability.Compared with SD-OCTA,SS-OCTA has a larger ACNV measurements,but doesn’t show better resolution of internal vessels of CNV and well signal strength. 展开更多

The pure TiO2 and Fe salts [Fe(C2O4)3,5H2O]-doped TiO2 electrodes were prepared by the hydrothermal method. The pure TiO2 or Fe-doped TiO2 slurry was coated onto the fluorine-doped tin oxide glass substrate by the Doctor Blade method and then sintered at 450 ℃. The Mott-Schottks, plot indicates that the fiat band potential of TiO2 was shifted positively after Fe-doped TiO2. The positive shift of the fiat band potential improves the driving force of injected electrons from the LUMO of the dye to the conduction band of TiO2. This study shows that photovoltaic efficiency increased by 22.9% from 6.07% to 7.46% compared to pure TiO2, and the fill factors increased from 0.53 to 0.63. 展开更多
