This study aimed to...Background: Cavernous transformation of the portal vein(CTPV) due to portal vein obstruction is a rare vascular anomaly defined as the formation of multiple collateral vessels in the hepatic hilum. This study aimed to investigate the imaging features of intrahepatic portal vein in adult patients with CTPV and establish the relationship between the manifestations of intrahepatic portal vein and the progression of CTPV. Methods: We retrospectively analyzed 14 CTPV patients in Beijing Tsinghua Changgung Hospital. All patients underwent both direct portal venography(DPV) and computed tomography angiography(CTA) to reveal the manifestations of the portal venous system. The vessels measured included the left portal vein(LPV), right portal vein(RPV), main portal vein(MPV) and the portal vein bifurcation(PVB). Results: Nine males and 5 females, with a median age of 40.5 years, were included in the study. No significant difference was found in the diameters of the LPV or RPV measured by DPV and CTA. The visualization in terms of LPV, RPV and PVB measured by DPV was higher than that by CTA. There was a significant association between LPV/RPV and PVB/MPV in term of visibility revealed with DPV( P = 0.01), while this association was not observed with CTA. According to the imaging features of the portal vein measured by DPV, CTPV was classified into three categories to facilitate the diagnosis and treatment. Conclusions: DPV was more accurate than CTA for revealing the course of the intrahepatic portal vein in patients with CTPV. The classification of CTPV, that originated from the imaging features of the portal vein revealed by DPV, may provide a new perspective for the diagnosis and treatment of CTPV.展开更多
RESTful API fuzzing is a promising method for automated vulnerability detection in Kubernetes platforms.Existing tools struggle with generating lengthy,high-semantic request sequences that can pass Kubernetes API gate...RESTful API fuzzing is a promising method for automated vulnerability detection in Kubernetes platforms.Existing tools struggle with generating lengthy,high-semantic request sequences that can pass Kubernetes API gateway checks.To address this,we propose KubeFuzzer,a black-box fuzzing tool designed for Kubernetes RESTful APIs.KubeFuzzer utilizes Natural Language Processing(NLP)to extract and integrate semantic information from API specifications and response messages,guiding the generation of more effective request sequences.Our evaluation of KubeFuzzer on various Kubernetes clusters shows that it improves code coverage by 7.86%to 36.34%,increases the successful response rate by 6.7%to 83.33%,and detects 16.7%to 133.3%more bugs compared to three leading techniques.KubeFuzzer identified over 1000 service crashes,which were narrowed down to 7 unique bugs.We tested these bugs on 10 real-world Kubernetes projects,including major providers like AWS(EKS),Microsoft Azure(AKS),and Alibaba Cloud(ACK),and confirmed that these issues could trigger service crashes.We have reported and confirmed these bugs with the Kubernetes community,and they have been addressed.展开更多
Network embedding aspires to learn a low-dimensional vector of each node in networks,which can apply to diverse data mining tasks.In real-life,many networks include rich attributes and temporal information.However,mos...Network embedding aspires to learn a low-dimensional vector of each node in networks,which can apply to diverse data mining tasks.In real-life,many networks include rich attributes and temporal information.However,most existing embedding approaches ignore either temporal information or network attributes.A self-attention based architecture using higher-order weights and node attributes for both static and temporal attributed network embedding is presented in this article.A random walk sampling algorithm based on higher-order weights and node attributes to capture network topological features is presented.For static attributed networks,the algorithm incorporates first-order to k-order weights,and node attribute similarities into one weighted graph to preserve topological features of networks.For temporal attribute networks,the algorithm incorporates previous snapshots of networks containing first-order to k-order weights,and nodes attribute similarities into one weighted graph.In addition,the algorithm utilises a damping factor to ensure that the more recent snapshots allocate a greater weight.Attribute features are then incorporated into topological features.Next,the authors adopt the most advanced architecture,Self-Attention Networks,to learn node representations.Experimental results on node classification of static attributed networks and link prediction of temporal attributed networks reveal that our proposed approach is competitive against diverse state-of-the-art baseline approaches.展开更多
Background Individuals with diabetes have a significantly higher risk of developing chronic kidney disease(CKD)and higher levels of social isolation and loneliness compared with those without diabetes.Recently,the Ame...Background Individuals with diabetes have a significantly higher risk of developing chronic kidney disease(CKD)and higher levels of social isolation and loneliness compared with those without diabetes.Recently,the American Heart Association highlighted the importance of considering social determinants of health(SDOH)in conjunction with traditional risk factors in patients with diabetes.Aims To investigate the associations of loneliness and social isolation with incident CKD risk in patients with diabetes in the UK Biobank.Methods A total of 18972 patients with diabetes were included in this prospective study.Loneliness and Social Isolation Scales were created based on self-reported factors.An adjusted Cox proportional hazard model was used to investigate the associations of loneliness and social isolation with CKD risk among patients with diabetes.The relative importance in predicting CKD was also calculated alongside traditional risk factors.Results During a median follow-up of 10.8 years,1127 incident CKD cases were reported.A higher loneliness scale,but not social isolation,was significantly associated with a 25%higher risk of CKD,independent of traditional risk factors,among patients with diabetes.Among the individual loneliness factors,the sense of feeling lonely emerged as the primary contributing factor to the elevated risk of CKD.Compared with individuals not experiencing feelings of loneliness,those who felt lonely exhibited a 22%increased likelihood of developing CKD.In addition,feeling lonely demonstrated greater relative importance of predicting CKD compared with traditional risk factors such as body mass index,smoking,physical activity and diet.Conclusions This study indicates the significant relationship between loneliness and CKD risk among patients with diabetes,highlighting the need to address SDOH in preventing CKD in this population.展开更多

BACKGROUND Obstruction or fullness after feeding is common in gastric cancer(GC)patients,affecting their nutritional status and quality of life.Patients with digestive obstruction are generally in a more advanced stag...BACKGROUND Obstruction or fullness after feeding is common in gastric cancer(GC)patients,affecting their nutritional status and quality of life.Patients with digestive obstruction are generally in a more advanced stage.Existing methods,including palliative gastrectomy,gastrojejunostomy,endoluminal stent,jejunal nutrition tube and intravenous chemotherapy,have limitations in treating these symptoms.AIM To analyze the efficacy of continuous gastric artery infusion chemotherapy(cGAIC)in relieving digestive obstruction in patients with advanced GC.METHODS This study was a retrospective study.Twenty-nine patients with digestive obstruction of advanced GC who underwent at least one cycle of treatment were reviewed at The Second Affiliated Hospital of Zhejiang University School of Medicine.The oxaliplatin-based intra-arterial infusion regimen was applied in all patients.Mild systemic chemotherapy was used in combination with local treatment.The clinical response was evaluated by contrast-enhanced computed tomography using Response Evaluation Criteria In Solid Tumors(RECIST)criteria.Digestive tract symptoms and toxic effects were analyzed regularly.A comparison of the Karnofsky Performance Status(KPS)score and Stooler’s Dysphagia Score before and after therapy was made.Univariate survival analysis and multivariate survival analysis were also performed to explore the key factors affecting patient survival.RESULTS All patients finished cGAIC successfully without microcatheter displacement,as confirmed by arteriography.The median follow-up time was 24 mo(95%CI:20.24-27.76 mo).The overall response rate was 89.7%after cGAIC according to the RECIST criteria.The postoperative Stooler’s Dysphagia Score was significantly improved.Twentytwo(75.9%)of the 29 patients experienced relief of digestive obstruction after the first two cycles,and 13(44.8%)initially unresectable patients were then considered radically resectable.The median overall survival time(mOS)was 16 mo(95%CI:9.32-22.68 mo).Patients who received radical surgery had a significantly longer mOS than other patients(P value<0.001).Multivariate Cox regression analysis indicated that radical resection after cGAIC,intravenous chemotherapy after cGAIC,and immunotherapy after cGAIC were independent predictors of mOS.None of the patients stopped treatment because of adverse events.CONCLUSION cGAIC was effective and safe in relieving digestive obstruction in advanced GC,and it could improve surgical conversion possibility and survival time.展开更多
BACKGROUND It is not clear whether sacubitril/valsartan is beneficial for patients with heart failure(HF)with reduced ejec-tion fraction(HFrEF)and low systolic blood pressure(SBP).This study aimed to investigate the e...BACKGROUND It is not clear whether sacubitril/valsartan is beneficial for patients with heart failure(HF)with reduced ejec-tion fraction(HFrEF)and low systolic blood pressure(SBP).This study aimed to investigate the efficacy and tolerability of sacu-bitril/valsartan in HFrEF patients with SBP<100 mmHg.METHODS&RESULTS An observational study was conducted on 117 patients,40.2%of whom had SBP<100 mmHg wit-hout symptomatic hypotension,and 59.8%of whom had SBP≥100 mmHg in an optimized HF follow-up management system.At the 6-month follow-up,52.4%of patients with SBP<100 mmHg and 70.0%of those with SBP≥100 mmHg successfully rea-ched the target dosages of sacubitril/valsartan.A reduction in the concentration of N-terminal pro-B-type natriuretic peptide was similar between patients with SBP<100 mmHg and SBP≥100 mmHg(1627.5 pg/mL and 1340.1 pg/mL,respectively;P=0.75).The effect of sacubitril/valsartan on left ventricular ejection fraction was observed in both SBP categories,with a 10.8%increase in patients with SBP<100 mmHg(P<0.001)and a 14.0%increase in patients with SBP≥100 mmHg(P<0.001).The effects of sac-ubitril/valsartan on SBP were statistically significant and inverse across both SBP categories(P=0.001),with an increase of 7.5 mmHg in patients with SBP<100 mmHg and a decrease of 11.5 mmHg in patients with SBP≥100 mmHg.No statistically signi-ficant differences were observed between the two groups in terms of the occurrence of symptomatic hypotension,deteriorating re-nal function,hyperkalemia,angioedema,or stroke.CONCLUSIONS Within an optimized HF follow-up management system,sacubitril/valsartan exhibited excellent tolerability and prompted left ventricular reverse remodeling in patients with HFrEF who presented asymptomatic hypotension.展开更多

However,the problem of the poor applicability of low-frequency measurements in these tanks has not yet been solved. Therefore,we propose a low-frequency acoustic measurement method based on sound-field correction(SFC) in an enclosed space that effectively solves the problem of measuring the sound power from complex sound sources below the Schroeder cutoff frequency in a non-anechoic tank. Using normal mode theory, the transfer relationship between the mean-square sound pressure in an underwater enclosed space and the free-field sound power of the sound source is established, and this is regarded as a correction term for the sound field between this enclosed space and the free field. This correction term can be obtained based on previous measurements of a known sound source. This term can then be used to correct the mean-square sound pressure excited by any sound source to be tested in this enclosed space and equivalently obtain its free-field sound power. Experiments were carried out in a non-anechoic water tank(9.0 m × 3.1 m × 1.7 m) to confirm the validity of the SFC method. Through measurements with a spherical sound source(whose free-field radiation characteristics are known),the correction term of the sound field between this water tank and the free field was obtained. On this basis, the sound power radiated from a cylindrical shell model under the action of mechanical excitation was measured. The measurement results were found to have a maximum deviation of 2.9 d B from the free-field results. These results show that the SFC method has good applicability in the frequency band above the first-order resonant frequency in a non-anechoic tank. This greatly expands the potential low-frequency applications of non-anechoic tanks.展开更多

Background: Augmented reality(AR) technology is used to reconstruct three-dimensional(3D) images of hepatic and biliary structures from computed tomography and magnetic resonance imaging data, and to superimpose the v...Background: Augmented reality(AR) technology is used to reconstruct three-dimensional(3D) images of hepatic and biliary structures from computed tomography and magnetic resonance imaging data, and to superimpose the virtual images onto a view of the surgical field. In liver surgery, these superimposed virtual images help the surgeon to visualize intrahepatic structures and therefore, to operate precisely and to improve clinical outcomes.Data Sources: The keywords "augmented reality", "liver", "laparoscopic" and "hepatectomy" were used for searching publications in the Pub Med database. The primary source of literatures was from peer-reviewed journals up to December 2016. Additional articles were identified by manual search of references found in the key articles.Results: In general, AR technology mainly includes 3D reconstruction, display, registration as well as tracking techniques and has recently been adopted gradually for liver surgeries including laparoscopy and laparotomy with video-based AR assisted laparoscopic resection as the main technical application. By applying AR technology, blood vessels and tumor structures in the liver can be displayed during surgery,which permits precise navigation during complex surgical procedures. Liver transformation and registration errors during surgery were the main factors that limit the application of AR technology.Conclusions: With recent advances, AR technologies have the potential to improve hepatobiliary surgical procedures. However, additional clinical studies will be required to evaluate AR as a tool for reducing postoperative morbidity and mortality and for the improvement of long-term clinical outcomes. Future research is needed in the fusion of multiple imaging modalities, improving biomechanical liver modeling,and enhancing image data processing and tracking technologies to increase the accuracy of current AR methods.展开更多

METHODS...AIM: To present our trial using a combination of the human acellular dermal matrix (HADM) implant and an interpositional omentum flap to repair giant abdominal wall defects after extensive tumor resection. METHODS: Between February and October of 2007, three patients with giant defects of the abdominal wall after extensive tumor resection underwent reconstruction with a combination of HADN and omentum flap. Postoperative morbidities and signs of herniation were monitored. RESULTS: The abdominal wall reconstruction was successful in these three patients, there was no severe morbidity and no signs of herniation in the follow-up period. CONCLUSION: The combination of HADM and omentum flap offers a new, safe and effective alternative to traditional forms in the repair of giant abdominal wall defects. Further analysis of the long-term outcome and more cases are needed to assess the reliability of this technique.展开更多
The lack of communication infrastructure in the ocean inevitably leads to coverage blind zones.In addition to high-throughput marine satellites,unmanned aerial vehicles(UAVs)can be used to provide coverage for these b...The lack of communication infrastructure in the ocean inevitably leads to coverage blind zones.In addition to high-throughput marine satellites,unmanned aerial vehicles(UAVs)can be used to provide coverage for these blind zones along with onshore base stations.In this paper,we consider the use of UAV for maritime coverage enhancement.Particularly,to serve more ships on the vast oceanic area with limited spectrum resources,we employ non-orthogonal multiple access(NOMA).A joint power and transmission duration allocation problem is formulated to maximize the minimum ship throughput,with the constraints on onboard communication energy.Different from previous works,we only assume the slowly time-varying large-scale channel state information(CSI)to reduce the system cost,as the large-scale CSI is locationdependent and can be obtained according to a priori radio map.To solve the non-convex problem,we decompose it into two subproblems and solve them in an iterative way.Simulation results show the effectiveness of the proposed solution.展开更多

The photoaging fibroblast model wasestablished by ultraviolet B (UVB) irradiation. The toxic effects of different concentrations of DOP were detected usingMTT. Senescent cells were detected using a β-galactosidase kit. Reactive oxygen species (ROS) in cells were detectedusing a flow cytometer. The expression of matrix metalloproteinase-1 (MMP-1), type I collagen C-terminal peptide(CICP), and transforming growth factor β-1 (TGF-β1) in spent culture medium was detected by ELISA. The resultsshowed that the low concentration of DOP (20, 40, 80 μg/mL) had no cytotoxicity on fibroblasts. After 60 mJ/cm2UVBirradiation, the number of aging β-gal-positive cells increased, the levels of CICP and TGF-β1 in spent culture mediumdecreased, while the levels of MMP-1 and ROS increased. After administration of DOP on photoaging fibroblasts, thenumber of aging β-gal-positive cells decreased, the levels of ROS and MMP-1 decreased, and the levels of TGF-β1 andCICP increased. This experiment suggests that DOP has the effect of removing ROS induced by UVB, regulating thebalance of collagen production and degradation, and protecting photoaging human skin fibroblasts.展开更多

Methods The medical records of patients with HAE admitted to Peking Union ...Objective To determine the safety and efficacy of fresh frozen plasma (FFP) iniusion for the treat- ment of hereditary angioedema (FIAE). Methods The medical records of patients with HAE admitted to Peking Union Medical College Fiospital who had received FFP infusion during 2004 and 2010 were reviewed and PubMed database iFom 1966 to the present were searched using the following key words: hereditary angioedema and fresh frozen plasma. The patient's age, sex, body location of HAE attacks, the dose of FFP infusion, time of beginning to improvenaent, time to complete remission, complication, C 1 inhibitor activity, and outcome were analyzed. Results A total of 13 enrolled patients (7 male and 6 female) received 16 times of FFP infusion, in- cluding 2 patients undergoing FFP infusion in Peking Union Medical College Hospital and 11 patients re- ported in the literature. The mean dosage of FFP infusion was 586±337 mL. Two cases suffered from wors- ening abdominal pain and one case experienced skin rash. Only I patient had no improvement in symptom owing to transfusion related reaction. There was a definite improvement in symptom 49± 19 minutes after beginning FFP infusion. The remission time decreased from 61.7±27.0 hours to 3.3 (2.0, 12.0) hours after FFP infusion. FFP infusion was effective for both type I and type Ⅱ HAE. Conclusion FFP seems to be safe and effective for acute attacks of HAE.展开更多

In this study,an environmentally friendly and non-toxic route to synthesize lignin-based non-isocyanate poly(imine-hydroxyurethane)s networks was explored.Specifically,the NH_(2)-terminated polyhydroxyurethanes(NPHUs)...In this study,an environmentally friendly and non-toxic route to synthesize lignin-based non-isocyanate poly(imine-hydroxyurethane)s networks was explored.Specifically,the NH_(2)-terminated polyhydroxyurethanes(NPHUs)prepolymer was first synthesized from bis(6-membered cyclic carbonate)(BCC)and diamine via the ring-opening reaction.Subsequently,the corresponding ligninbased non-isocyanate polyurethanes(NIPUs)with tunable properties were synthesized from NPHUs and levulinate lignin derivatives containing ketone groups via the Schiff base reaction.The structural,mechanical,and thermal properties of NIPUs with different stoichiometric feed ratios of BCC and levulinate lignin were characterized by Fourier transform infrared spectroscopy(FT-IR),nuclear magnetic resonance(NMR),differential scanning calorimetry(DSC),dynamic mechanical analysis(DMA),and thermogravimetric analysis(TGA).The results indicated that the tensile strength,Young's modulus,toughness,storage modulus,glass transition temperature,and thermal stability of lignin-based NIPUs gradually increased with increasing lignin content,and the highest Young's modulus of 41.1 MPa was obtained when lignin content reached 45.53%.With good reprocessing properties,this synthetic framework of ligninbased NIPUs also provides sustainable non-isocyanate-based substitutions to traditional polyurethane networks.展开更多

The classification of CTPV, that originated from the imaging features of the portal vein revealed by DPV, may provide a new perspective for the diagnosis and treatment of CTPV.

文摘RESTful API fuzzing is a promising method for automated vulnerability detection in Kubernetes platforms.Existing tools struggle with generating lengthy,high-semantic request sequences that can pass Kubernetes API gateway checks.To address this,we propose KubeFuzzer,a black-box fuzzing tool designed for Kubernetes RESTful APIs.KubeFuzzer utilizes Natural Language Processing(NLP)to extract and integrate semantic information from API specifications and response messages,guiding the generation of more effective request sequences.Our evaluation of KubeFuzzer on various Kubernetes clusters shows that it improves code coverage by 7.86%to 36.34%,increases the successful response rate by 6.7%to 83.33%,and detects 16.7%to 133.3%more bugs compared to three leading techniques.KubeFuzzer identified over 1000 service crashes,which were narrowed down to 7 unique bugs.We tested these bugs on 10 real-world Kubernetes projects,including major providers like AWS(EKS),Microsoft Azure(AKS),and Alibaba Cloud(ACK),and confirmed that these issues could trigger service crashes.We have reported and confirmed these bugs with the Kubernetes community,and they have been addressed.

文摘Network embedding aspires to learn a low-dimensional vector of each node in networks,which can apply to diverse data mining tasks.In real-life,many networks include rich attributes and temporal information.However,most existing embedding approaches ignore either temporal information or network attributes.A self-attention based architecture using higher-order weights and node attributes for both static and temporal attributed network embedding is presented in this article.A random walk sampling algorithm based on higher-order weights and node attributes to capture network topological features is presented.For static attributed networks,the algorithm incorporates first-order to k-order weights,and node attribute similarities into one weighted graph to preserve topological features of networks.For temporal attribute networks,the algorithm incorporates previous snapshots of networks containing first-order to k-order weights,and nodes attribute similarities into one weighted graph.In addition,the algorithm utilises a damping factor to ensure that the more recent snapshots allocate a greater weight.Attribute features are then incorporated into topological features.Next,the authors adopt the most advanced architecture,Self-Attention Networks,to learn node representations.Experimental results on node classification of static attributed networks and link prediction of temporal attributed networks reveal that our proposed approach is competitive against diverse state-of-the-art baseline approaches.

文摘Background Individuals with diabetes have a significantly higher risk of developing chronic kidney disease(CKD)and higher levels of social isolation and loneliness compared with those without diabetes.Recently,the American Heart Association highlighted the importance of considering social determinants of health(SDOH)in conjunction with traditional risk factors in patients with diabetes.Aims To investigate the associations of loneliness and social isolation with incident CKD risk in patients with diabetes in the UK Biobank.Methods A total of 18972 patients with diabetes were included in this prospective study.Loneliness and Social Isolation Scales were created based on self-reported factors.An adjusted Cox proportional hazard model was used to investigate the associations of loneliness and social isolation with CKD risk among patients with diabetes.The relative importance in predicting CKD was also calculated alongside traditional risk factors.Results During a median follow-up of 10.8 years,1127 incident CKD cases were reported.A higher loneliness scale,but not social isolation,was significantly associated with a 25%higher risk of CKD,independent of traditional risk factors,among patients with diabetes.Among the individual loneliness factors,the sense of feeling lonely emerged as the primary contributing factor to the elevated risk of CKD.Compared with individuals not experiencing feelings of loneliness,those who felt lonely exhibited a 22%increased likelihood of developing CKD.In addition,feeling lonely demonstrated greater relative importance of predicting CKD compared with traditional risk factors such as body mass index,smoking,physical activity and diet.Conclusions This study indicates the significant relationship between loneliness and CKD risk among patients with diabetes,highlighting the need to address SDOH in preventing CKD in this population.

文摘Intrahepatic arterioportal fistulas(APFs)are abnormal hepatic artery and portal vein(PV)communications that develop as a result of congenital malformation,trauma,ruptured hepatic aneurysm,cirrhosis,tumor-related changes,biopsy,chemotherapy or iatrogenic causes[1,2].The most common symptoms are gastrointestinal bleeding and ascites secondary to portal hypertension;other symptoms include abdominal pain,pyrexia,edema,back pain and jaundice[3].The main goal of therapy is to decrease the portal pressure with variceal bleeding being the absolute indi-cation for surgical management.Transarterial embolization(TAE)should be the first choice to treat APFs,while resection,portocaval shunt and even transplantation may cure APFs in the case of TAE failure[4].In previous reports,caudate lobe-sparing subtotal hep-atectomy(CLSSH)has been applied for the treatment of primary hepatolithiasis and hepatocellular carcinoma[5,6].As far as we know,this is the first report describing CLSSH as treatment for an extensive intrahepatic APF,which involved segments 2 to 8,with corresponding hypertrophy of the caudate lobe.

文摘BACKGROUND Obstruction or fullness after feeding is common in gastric cancer(GC)patients,affecting their nutritional status and quality of life.Patients with digestive obstruction are generally in a more advanced stage.Existing methods,including palliative gastrectomy,gastrojejunostomy,endoluminal stent,jejunal nutrition tube and intravenous chemotherapy,have limitations in treating these symptoms.AIM To analyze the efficacy of continuous gastric artery infusion chemotherapy(cGAIC)in relieving digestive obstruction in patients with advanced GC.METHODS This study was a retrospective study.Twenty-nine patients with digestive obstruction of advanced GC who underwent at least one cycle of treatment were reviewed at The Second Affiliated Hospital of Zhejiang University School of Medicine.The oxaliplatin-based intra-arterial infusion regimen was applied in all patients.Mild systemic chemotherapy was used in combination with local treatment.The clinical response was evaluated by contrast-enhanced computed tomography using Response Evaluation Criteria In Solid Tumors(RECIST)criteria.Digestive tract symptoms and toxic effects were analyzed regularly.A comparison of the Karnofsky Performance Status(KPS)score and Stooler’s Dysphagia Score before and after therapy was made.Univariate survival analysis and multivariate survival analysis were also performed to explore the key factors affecting patient survival.RESULTS All patients finished cGAIC successfully without microcatheter displacement,as confirmed by arteriography.The median follow-up time was 24 mo(95%CI:20.24-27.76 mo).The overall response rate was 89.7%after cGAIC according to the RECIST criteria.The postoperative Stooler’s Dysphagia Score was significantly improved.Twentytwo(75.9%)of the 29 patients experienced relief of digestive obstruction after the first two cycles,and 13(44.8%)initially unresectable patients were then considered radically resectable.The median overall survival time(mOS)was 16 mo(95%CI:9.32-22.68 mo).Patients who received radical surgery had a significantly longer mOS than other patients(P value<0.001).Multivariate Cox regression analysis indicated that radical resection after cGAIC,intravenous chemotherapy after cGAIC,and immunotherapy after cGAIC were independent predictors of mOS.None of the patients stopped treatment because of adverse events.CONCLUSION cGAIC was effective and safe in relieving digestive obstruction in advanced GC,and it could improve surgical conversion possibility and survival time.

文摘BACKGROUND It is not clear whether sacubitril/valsartan is beneficial for patients with heart failure(HF)with reduced ejec-tion fraction(HFrEF)and low systolic blood pressure(SBP).This study aimed to investigate the efficacy and tolerability of sacu-bitril/valsartan in HFrEF patients with SBP<100 mmHg.METHODS&RESULTS An observational study was conducted on 117 patients,40.2%of whom had SBP<100 mmHg wit-hout symptomatic hypotension,and 59.8%of whom had SBP≥100 mmHg in an optimized HF follow-up management system.At the 6-month follow-up,52.4%of patients with SBP<100 mmHg and 70.0%of those with SBP≥100 mmHg successfully rea-ched the target dosages of sacubitril/valsartan.A reduction in the concentration of N-terminal pro-B-type natriuretic peptide was similar between patients with SBP<100 mmHg and SBP≥100 mmHg(1627.5 pg/mL and 1340.1 pg/mL,respectively;P=0.75).The effect of sacubitril/valsartan on left ventricular ejection fraction was observed in both SBP categories,with a 10.8%increase in patients with SBP<100 mmHg(P<0.001)and a 14.0%increase in patients with SBP≥100 mmHg(P<0.001).The effects of sac-ubitril/valsartan on SBP were statistically significant and inverse across both SBP categories(P=0.001),with an increase of 7.5 mmHg in patients with SBP<100 mmHg and a decrease of 11.5 mmHg in patients with SBP≥100 mmHg.No statistically signi-ficant differences were observed between the two groups in terms of the occurrence of symptomatic hypotension,deteriorating re-nal function,hyperkalemia,angioedema,or stroke.CONCLUSIONS Within an optimized HF follow-up management system,sacubitril/valsartan exhibited excellent tolerability and prompted left ventricular reverse remodeling in patients with HFrEF who presented asymptomatic hypotension.
文摘Food-dependent,exercise-induced anaphylaxis(FDEIA)is a potentially life-threatening disorder that often occurs with exercise,and patients typically have eaten a specific food within hours before disease onset.This disease is exceedingly rare,with a prevalence of 0.02%.No well-recognized prevention or treatment strategy has been available for FDEIA except avoiding triggers strictly.Here we report an 11-year-old boy with a history of recurrent anaphylaxis of unknown etiology more than 10 times within two years.As the anaphylactic symptoms had not been controlled after traditional treatments,the patient was given subcutaneous injection of dupilumab seven times within 33 weeks.During dupilumab treatments,the patient was exposed to culprit mushrooms plus exercises at least twice a month but without notable anaphylaxis.Thus,dupilumab may improve the allergic reactions in FDEIA patients.

文摘Similar to air reverberation chambers, non-anechoic water tanks are important acoustic measurement devices that can be used to measure the sound power radiated from complex underwater sound sources using diffusion field theory. However,the problem of the poor applicability of low-frequency measurements in these tanks has not yet been solved. Therefore,we propose a low-frequency acoustic measurement method based on sound-field correction(SFC) in an enclosed space that effectively solves the problem of measuring the sound power from complex sound sources below the Schroeder cutoff frequency in a non-anechoic tank. Using normal mode theory, the transfer relationship between the mean-square sound pressure in an underwater enclosed space and the free-field sound power of the sound source is established, and this is regarded as a correction term for the sound field between this enclosed space and the free field. This correction term can be obtained based on previous measurements of a known sound source. This term can then be used to correct the mean-square sound pressure excited by any sound source to be tested in this enclosed space and equivalently obtain its free-field sound power. Experiments were carried out in a non-anechoic water tank(9.0 m × 3.1 m × 1.7 m) to confirm the validity of the SFC method. Through measurements with a spherical sound source(whose free-field radiation characteristics are known),the correction term of the sound field between this water tank and the free field was obtained. On this basis, the sound power radiated from a cylindrical shell model under the action of mechanical excitation was measured. The measurement results were found to have a maximum deviation of 2.9 d B from the free-field results. These results show that the SFC method has good applicability in the frequency band above the first-order resonant frequency in a non-anechoic tank. This greatly expands the potential low-frequency applications of non-anechoic tanks.

文摘Background: Augmented reality(AR) technology is used to reconstruct three-dimensional(3D) images of hepatic and biliary structures from computed tomography and magnetic resonance imaging data, and to superimpose the virtual images onto a view of the surgical field. In liver surgery, these superimposed virtual images help the surgeon to visualize intrahepatic structures and therefore, to operate precisely and to improve clinical outcomes.Data Sources: The keywords "augmented reality", "liver", "laparoscopic" and "hepatectomy" were used for searching publications in the Pub Med database. The primary source of literatures was from peer-reviewed journals up to December 2016. Additional articles were identified by manual search of references found in the key articles.Results: In general, AR technology mainly includes 3D reconstruction, display, registration as well as tracking techniques and has recently been adopted gradually for liver surgeries including laparoscopy and laparotomy with video-based AR assisted laparoscopic resection as the main technical application. By applying AR technology, blood vessels and tumor structures in the liver can be displayed during surgery,which permits precise navigation during complex surgical procedures. Liver transformation and registration errors during surgery were the main factors that limit the application of AR technology.Conclusions: With recent advances, AR technologies have the potential to improve hepatobiliary surgical procedures. However, additional clinical studies will be required to evaluate AR as a tool for reducing postoperative morbidity and mortality and for the improvement of long-term clinical outcomes. Future research is needed in the fusion of multiple imaging modalities, improving biomechanical liver modeling,and enhancing image data processing and tracking technologies to increase the accuracy of current AR methods.

文摘AIM: To present our trial using a combination of the human acellular dermal matrix (HADM) implant and an interpositional omentum flap to repair giant abdominal wall defects after extensive tumor resection. METHODS: Between February and October of 2007, three patients with giant defects of the abdominal wall after extensive tumor resection underwent reconstruction with a combination of HADN and omentum flap. Postoperative morbidities and signs of herniation were monitored. RESULTS: The abdominal wall reconstruction was successful in these three patients, there was no severe morbidity and no signs of herniation in the follow-up period. CONCLUSION: The combination of HADM and omentum flap offers a new, safe and effective alternative to traditional forms in the repair of giant abdominal wall defects. Further analysis of the long-term outcome and more cases are needed to assess the reliability of this technique.

文摘The lack of communication infrastructure in the ocean inevitably leads to coverage blind zones.In addition to high-throughput marine satellites,unmanned aerial vehicles(UAVs)can be used to provide coverage for these blind zones along with onshore base stations.In this paper,we consider the use of UAV for maritime coverage enhancement.Particularly,to serve more ships on the vast oceanic area with limited spectrum resources,we employ non-orthogonal multiple access(NOMA).A joint power and transmission duration allocation problem is formulated to maximize the minimum ship throughput,with the constraints on onboard communication energy.Different from previous works,we only assume the slowly time-varying large-scale channel state information(CSI)to reduce the system cost,as the large-scale CSI is locationdependent and can be obtained according to a priori radio map.To solve the non-convex problem,we decompose it into two subproblems and solve them in an iterative way.Simulation results show the effectiveness of the proposed solution.
文摘The purpose of this study was to explore the protective effect of Dendrobium officinale polysaccharides (DOP) onphotoaging human skin fibroblasts and its specific mechanism of action. The photoaging fibroblast model wasestablished by ultraviolet B (UVB) irradiation. The toxic effects of different concentrations of DOP were detected usingMTT. Senescent cells were detected using a β-galactosidase kit. Reactive oxygen species (ROS) in cells were detectedusing a flow cytometer. The expression of matrix metalloproteinase-1 (MMP-1), type I collagen C-terminal peptide(CICP), and transforming growth factor β-1 (TGF-β1) in spent culture medium was detected by ELISA. The resultsshowed that the low concentration of DOP (20, 40, 80 μg/mL) had no cytotoxicity on fibroblasts. After 60 mJ/cm2UVBirradiation, the number of aging β-gal-positive cells increased, the levels of CICP and TGF-β1 in spent culture mediumdecreased, while the levels of MMP-1 and ROS increased. After administration of DOP on photoaging fibroblasts, thenumber of aging β-gal-positive cells decreased, the levels of ROS and MMP-1 decreased, and the levels of TGF-β1 andCICP increased. This experiment suggests that DOP has the effect of removing ROS induced by UVB, regulating thebalance of collagen production and degradation, and protecting photoaging human skin fibroblasts.
文摘Objective To determine the safety and efficacy of fresh frozen plasma (FFP) iniusion for the treat- ment of hereditary angioedema (FIAE). Methods The medical records of patients with HAE admitted to Peking Union Medical College Fiospital who had received FFP infusion during 2004 and 2010 were reviewed and PubMed database iFom 1966 to the present were searched using the following key words: hereditary angioedema and fresh frozen plasma. The patient's age, sex, body location of HAE attacks, the dose of FFP infusion, time of beginning to improvenaent, time to complete remission, complication, C 1 inhibitor activity, and outcome were analyzed. Results A total of 13 enrolled patients (7 male and 6 female) received 16 times of FFP infusion, in- cluding 2 patients undergoing FFP infusion in Peking Union Medical College Hospital and 11 patients re- ported in the literature. The mean dosage of FFP infusion was 586±337 mL. Two cases suffered from wors- ening abdominal pain and one case experienced skin rash. Only I patient had no improvement in symptom owing to transfusion related reaction. There was a definite improvement in symptom 49± 19 minutes after beginning FFP infusion. The remission time decreased from 61.7±27.0 hours to 3.3 (2.0, 12.0) hours after FFP infusion. FFP infusion was effective for both type I and type Ⅱ HAE. Conclusion FFP seems to be safe and effective for acute attacks of HAE.

文摘To alleviate the energy crisis and global warming,photothermal catalysis is an attractive way to effi ciently convert CO_(2)and renewable H_(2) into value-added fuels and chemicals.However,the catalytic performance is usually restricted by the trade-off between the dispersity and light absorption property of metal catalysts.Here we demonstrate a simple SiO 2-protected metal-organic framework pyrolysis strategy to fabricate a new type of integrated photothermal nanoreactor with a comparatively high metal loading,dispersity,and stability.The core-satellite structured Co@SiO_(2)exhibits strong sunlight-absorptive abil-ity and excellent catalytic activity in CO_(2)hydrogenation,which is ascribed to the functional separation of diff erent sizes of Co nanoparticles.Large-sized plasmonic Co nanoparticles are mainly responsible for the light absorption and conversion to heat(nanoheaters),whereas small-sized Co nanoparticles with high intrinsic activities are responsible for the catalysis(nanoreactors).This study provides a new concept for designing effi cient photothermal catalytic materials.

文摘In this study,an environmentally friendly and non-toxic route to synthesize lignin-based non-isocyanate poly(imine-hydroxyurethane)s networks was explored.Specifically,the NH_(2)-terminated polyhydroxyurethanes(NPHUs)prepolymer was first synthesized from bis(6-membered cyclic carbonate)(BCC)and diamine via the ring-opening reaction.Subsequently,the corresponding ligninbased non-isocyanate polyurethanes(NIPUs)with tunable properties were synthesized from NPHUs and levulinate lignin derivatives containing ketone groups via the Schiff base reaction.The structural,mechanical,and thermal properties of NIPUs with different stoichiometric feed ratios of BCC and levulinate lignin were characterized by Fourier transform infrared spectroscopy(FT-IR),nuclear magnetic resonance(NMR),differential scanning calorimetry(DSC),dynamic mechanical analysis(DMA),and thermogravimetric analysis(TGA).The results indicated that the tensile strength,Young's modulus,toughness,storage modulus,glass transition temperature,and thermal stability of lignin-based NIPUs gradually increased with increasing lignin content,and the highest Young's modulus of 41.1 MPa was obtained when lignin content reached 45.53%.With good reprocessing properties,this synthetic framework of ligninbased NIPUs also provides sustainable non-isocyanate-based substitutions to traditional polyurethane networks.
