Th...Introduction: Primary cutaneous apocrine carcinoma (PCAC) is a rare skin malignant tumor that originates from areas with a high concentration of apocrine glands. The incidence of PCAC in the neck is relatively low. The age of onset in PCAC ranges from 5 to 70 years old. Clinically, PCAC typically shows up as painless nodules or lumps. In immunohistochemistry, positive expression of CK-7, GCDFP-15, AR, and myoepithelial markers are helpful in the diagnosis of PCAC. This case report pertains to a 59-year-old male of Asian descent. Five years ago, a painless mass was discovered behind his right ear, which grew slowly. This patient was identified as PCAC based on clinical symptoms, pathology, immunohistochemistry, and imaging characteristics. PCAC involved the parotid gland in this case;it is easy to be misdiagnosed as a primary malignant tumor of the parotid gland on imaging. Furthermore, a thorough set of clinical, imaging, pathological, and immunohistochemical examinations must be performed to make a diagnosis because it is challenging to differentiate PCAC from metastatic breast carcinoma. Conclusion: A well-developed multidisciplinary examination is essential because PCAC can be challenging to diagnose and differentiate.展开更多
[Objectives]To investigate the therapeutic effect of Feixinning Capsules on acute exacerbation of chronic obstructive pulmonary disease(AECOPD)complicated with right heart failure.[Methods]A total of 60 cases of AECOP...[Objectives]To investigate the therapeutic effect of Feixinning Capsules on acute exacerbation of chronic obstructive pulmonary disease(AECOPD)complicated with right heart failure.[Methods]A total of 60 cases of AECOPD patients with right heart failure and phlegm-heat syndrome were randomly selected as the study subjects,and were divided into the Feixinning group(n=30)and the control group(n=30).The Feixinning group was treated with Feixinning Capsules combined with conventional western medicine,and the control group was treated with conventional western medicine.The pulmonary function,CRP,CAT score,BODE index item index,and EQ-5D index were compared between the two groups before and after treatment.[Results]After treatment,the pulmonary function,CRP,CAT score,6MWD,mMRC,BODE score and EQ-5D index in the Feixinning group were significantly improved compared with the control group(P<0.05),and there was no statistical difference in the change of BMI index(P>0.05).[Conclusions]Feixinning Capsules can effectively improve the clinical symptoms and quality of life of patients with AECOPD complicated with right heart failure and phlegm-heat syndrome.展开更多
Steady structures originating from dynamic self-assembly have begun to show their advantages in new generation materials, and pose challenges to equilibrium self-assembly. In view of the important role of confinement ...Steady structures originating from dynamic self-assembly have begun to show their advantages in new generation materials, and pose challenges to equilibrium self-assembly. In view of the important role of confinement in self-assembly, here, we propose a new type of confinement leading to dynamic steady structures, which opens a new window for the conventional confinement.In our model, we consider the self-assembly of ellipsoids in 2D circular confinement via the boundary performing periodically stretching and contracting oscillation. Langevin dynamics simulations reveal the achievement of non-equilibrium steady structures under appropriate boundary motions, which are novel smectic structures with stable topological defects. Different from the confinement with a static boundary, ellipsoids close to the boundary have variable orientations depending on the boundary motion.Order-order structural transitions, accompanied by the symmetry change and varied defect number, occur with the change of oscillating amplitude and/or frequency of the boundary. Slow and fast dynamics are distinguished according to whether structural rearrangements and energetic adjustment happen or not. The collective motion of confined ellipsoids, aroused by the work performed on the system, is the key factor determining both the structure and dynamics of the self-assembly. Our results not only achieve novel textures of circular confined liquid crystals, but also inspire us to reconsider the self-assembly within the living organisms.展开更多
