目的对基础胰岛素联合一线口服降糖药(二甲双胍/磺脲类药物)治疗效果不佳的2型糖尿病(T2DM)患者的不同治疗策略进行伞形评价,为患者推荐最佳治疗策略提供科学依据。方法计算机检索PubMed、Embase、Cochrane Library、SinoMed、CNKI、Wan... 目的对基础胰岛素联合一线口服降糖药(二甲双胍/磺脲类药物)治疗效果不佳的2型糖尿病(T2DM)患者的不同治疗策略进行伞形评价,为患者推荐最佳治疗策略提供科学依据。方法计算机检索PubMed、Embase、Cochrane Library、SinoMed、CNKI、WanFang Data和VIP数据库,搜集基础胰岛素联合一线口服降糖药治疗T2DM效果不佳的系统评价(SR)/Meta分析(MA),其原始研究为随机对照试验(RCT),检索时限均从建库至2023年9月13日,由2名研究者独立进行文献筛选、数据提取和文献质量评估,采用R 4.2.2软件中的metaumbrella包进行伞形评价。结果共纳入1篇SR和8篇MA,包括72个RCT,24095例患者,涉及的后续治疗措施有胰高糖素样肽1受体激动剂(GLP-1RA)、二肽基肽酶Ⅳ抑制剂(DPP-4i)、钠葡萄糖共转运蛋白2抑制剂(SGLT-2i)、噻唑烷二酮类药(TZD)等。结果显示,与安慰剂相比,GLP-1RA[WMD=-3.67,95%CI(-5.81,-1.53),P=0.001]、DPP-4i[WMD=-5.56,95%CI(-7.40,-3.73),P<0.001]和SGLT-2i[WMD=-5.34,95%CI(-9.56,-1.13),P=0.013]均可显著降低糖化血红蛋白(%)水平;阳性药物比较中,GLP-1RA vs.甘精胰岛素利司那肽固定比例复方制剂(IGlarLixi)[WMD=7.49,95%CI(7.01,7.92),P<0.001],GLP-1RA vs.德谷胰岛素利拉鲁肽注射液(IDegLira)[WMD=0.83,95%CI(0.11,1.54),P=0.023],显示GLP-1RA的降血糖效果不及IGlarLixi和IDegLira。与安慰剂相比,GLP-1RA可显著减轻体重(kg)[WMD=-3.46,95%CI(-5.30,-1.63),P<0.001];阳性药物比较中,GLP-1RA vs.IGlarLixi[WMD=-8.40,95%CI(-8.86,-7.93),P<0.001],GLP-1RA vs.IDegLira[WMD=-0.53,95%CI(-0.99,-0.07),P=0.025],显示GLP-1RA的降体重效果优于IGlarLixi和IDegLira。与安慰剂相比,TZD[OR=1.51,95%CI(1.11,2.05),P=0.009]和GLP-1RA[OR=1.24,95%CI(1.03,1.49),P=0.023]均会增加低血糖发生风险;阳性药物比较中,GLP-1RA vs.IDegLira[OR=1.22,95%CI(1.04,1.43),P=0.014],显示GLP-1RA导致的低血糖发生风险高于IDegLira。结论后续治疗使用GLP-1RA、DPP-4i、SGLT-2i能更好地控制血糖,且GLP-1RA有降低体重优势,但使用GLP-1RA的患者低血糖风险较高。 展开更多

采用磁控溅射法在不同基底温度下制备Co−TiO2纳米复合薄膜。所合成的纳米复合薄膜由TiO2非晶基体和分散其中的Co颗粒组成,呈铁磁和超顺磁共存的特性。随着基底温度从室温升高到400℃,Co颗粒尺寸逐渐增大,氧化程度明显降低。因此,在Co含... 采用磁控溅射法在不同基底温度下制备Co−TiO2纳米复合薄膜。所合成的纳米复合薄膜由TiO2非晶基体和分散其中的Co颗粒组成,呈铁磁和超顺磁共存的特性。随着基底温度从室温升高到400℃,Co颗粒尺寸逐渐增大,氧化程度明显降低。因此,在Co含量相同的情况下,随着基底温度的升高(从室温升高至400℃),饱和磁化强度从0.13增加到0.43 T。在高基底温度下,一部分Co颗粒聚集形成导电路径,导致电阻率从1600快速下降至76μΩ·m。即使在电阻率低至76μΩ·m的情况下,依然能获得Co−TiO2的磁电阻性能。结果表明,纳米复合薄膜在室温下具有低Co氧化、高磁化强度和磁阻特性。 展开更多

Objective To identify serum diagnosis or progression biomarkers in patients with lung cancer using protein chip profiling analysis. Method Profiling analysis was performed on 450 sera collected from 213 patients with lung cancer, 19 with pneumonia, 16 with pulmonary tuberculosis, 65 with laryngeal carcinoma, 55 with laryngopharyngeal carcinoma patients, and 82 normal individuals. A new strategy was developed to identify the biomarkers on chip by trypsin pre-digestion. Results Profiling analysis demonstrated that an 11.6kDa protein was significandy elevated in lung cancer patients, compared with the control groups (P〈0.001). The level and percentage of 11.6kDa protein progressively increased with the clinical stages Ⅰ-Ⅳ and were also higher in patients with squamous cell carcinoma than in other subtypes. This biomarker could be decreased after operation or chemotherapy. On the other hand, 11.6kDa protein was also increased in 50% benign diseases of lung and 13% of other cancer controls. After trypsin pre-digestion, a set of new peptide biomarkers was noticed to appear only in the samples containing a 11.6kDa peak. Further identification showed that 2177Da was a fragment of serum amyloid A (SAA, MW 11.6kDa). Two of the new peaks, 1550Da and 1611Da, were defined from the same protein by database searching. This result was further confirmed by partial purification of 11.6kDa protein and MS analysis. Conclusion SAA is a useful biomarker to monitor the progression of lung cancer and can directly identify some biomarkers on chip. 展开更多

In this paper,the metastableβTB8 titanium alloy with nanocrystallineαphase is achieved by electric pulse treatment.The morphology evolution and variant selection of nanocrystallineαphase in metastableβTB8titanium alloy were investigated by using scanning electron microscope(SEM),electron backscattered diffraction(EBSD)and transmission electron microscope(TEM)analysis.The results indicated that the morphologies of the nanocrystallineαphase were mainly triangular clusters and needle-like at the pressure of 0 MPa.With increasing pressure from 20 to 50 MPa,the volume fraction of needlelikeαphase decreased,and a large amount of V-shapedαphase formed in the interior ofβgrains.Based on the EBSD data,the parentβphase was reconstructed by MTEX software.In the interior of theβgrains,12 variants can form for the samples electric pulse treated at 0 and20 MPa,while only 3 and 6 variants can form for the samples electric pulse treated at 30 and 50 MPa.In the grain boundary of theβgrains,one or more grain boundaryαvariants can be generated for the samples electric pulse treated at different pressures as long as one of the neighborβgrains follows the Burgers orientation relationship. 展开更多

Background:The global pandemic coronavirus disease 2019(COVID-19)has become a major public health problem and presents an unprecedented challenge.However,no specific drugs were currently proven.This study aimed to evaluate the comparative efficacy and safety of pharmacological interventions in patients with COVID-19.Methods:Medline,Embase,the Cochrane Library,and clinicaltrials.gov were searched for randomized controlled trials(RCTs)in patients infected with severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)/SARS-CoV.Random-effects network metaanalysis within the Bayesian framework was performed,followed by the Grading of Recommendations Assessment,Development,and Evaluation system assessing the quality of evidence.The primary outcome of interest includes mortality,cure,viral negative conversion,and overall adverse events(OAEs).Odds ratio(OR)with 95%confidence interval(CI)was calculated as the measure of effect size.Results:Sixty-six RCTs with 19,095 patients were included,involving standard of care(SOC),eight different antiviral agents,six different antibiotics,high and low dose chloroquine(CQ_HD,CQ_LD),traditional Chinese medicine(TCM),corticosteroids(COR),and other treatments.Compared with SOC,a significant reduction of mortality was observed for TCM(OR=0.34,95%CI:0.20–0.56,moderate quality)and COR(OR=0.84,95%CI:0.75–0.96,low quality)with improved cure rate(OR=2.16,95%CI:1.60–2.91,low quality for TCM;OR=1.17,95%CI:1.05–1.30,low quality for COR).However,an increased risk of mortality was found for CQ_HD vs.SOC(OR=3.20,95%CI:1.18–8.73,low quality).TCM was associated with decreased risk of OAE(OR=0.52,95%CI:0.38–0.70,very low quality)but CQ_HD(OR=2.51,95%CI:1.20–5.24)and interferons(IFN)(OR=2.69,95%CI:1.02–7.08)vs.SOC with very low quality were associated with an increased risk.Conclusions:COR and TCM may reduce mortality and increase cure rate with no increased risk of OAEs compared with standard care.CQ_HD might increase the risk of mortality.CQ,IFN,and other antiviral agents could increase the risk of OAEs.The current evidence is generally uncertain with low-quality and further high-quality trials are needed. 展开更多
