目的比较开放、完全腹腔镜和简易手助器腹腔镜脾切除术加贲门周围血管离断术治疗肝硬化门脉高压症伴胃底食管静脉曲张的临床疗效。方法回顾性分析2014年1月至2016年12月苏州大学附属第一医院施行的22例开放脾切除加贲门周围血管离断术(...目的比较开放、完全腹腔镜和简易手助器腹腔镜脾切除术加贲门周围血管离断术治疗肝硬化门脉高压症伴胃底食管静脉曲张的临床疗效。方法回顾性分析2014年1月至2016年12月苏州大学附属第一医院施行的22例开放脾切除加贲门周围血管离断术(开放组)、18例完全腹腔镜脾切除加贲门周围血管离断术(完全腹腔镜组)和15例简易手助器腹腔镜脾切除加贲门周围血管离断术(简易手助器组)患者的临床资料,比较三组的手术时间、术中出血量、术后住院时间、引流管拔除时间、术后镇痛时间和术后并发症等资料。结果开放组、完全腹腔镜组和简易手助器组平均手术时间分别为(147±36)min、(236±57)min和(157±30)min(F=24.601,P<0.01),中位术中出血量分别为115 m L、212 m L和128 m L(H=8.622,P<0.05);组间比较显示,简易手助器组手术平均时间和中位术中出血量均少于完全腹腔镜组(均数差78.87 min,P<0.01;Bonferroni校正,P<0.05)。三组术后引流管拔除时间分别为(9.1±5.0)d、(6.4±5.2)d和(5.3±1.9)d(F=3.645,P<0.05),术后镇痛时间分别为(4.2±0.9)d、(2.8±1.7)d和(2.8±1.1)d(F=8.739,P<0.01),简易手助器组引流管拔除时间和术后镇痛时间均少于开放组(均数差分别为3.80 d和1.43 d,P<0.05)。简易手助器组无中转开腹,完全腹腔镜组有5例中转开腹(Fisher确切概率法,P<0.05)。三组均无围手术期死亡病例,其术后住院时间及脾脏长径、术后并发症发生率差异均无统计学意义(P>0.05)。结论简易手助器行腹腔镜脾切除术加贲门周围血管离断术具有完全腹腔镜手术的微创性,且兼具术中出血少、手术时间短、中转开腹率低等优势,降低了手术风险和难度;且此手助器制作简易,利于该术式在基层医院中推广应用。展开更多
Shuttle effect,poor conductivity and large volume expansion are the main factors that hinder the practical application of sulfur cathodes.Currently,rational structure designing of carbon-based sulfur hosts is the most...Shuttle effect,poor conductivity and large volume expansion are the main factors that hinder the practical application of sulfur cathodes.Currently,rational structure designing of carbon-based sulfur hosts is the most effective strategy to address the above issues.However,the preparation process of carbon-based sulfur hosts is usually complex and costly.Therefore,it is necessary to develop an efficient and cost-effective method to fabricate carbon hosts for high-performance sulfur cathodes.Herein,we reported the fabrication of a bio-derived nitrogen doped porous carbon materials(BNPC)via a molten-salt method for high performance sulfur cathodes.The long-range-ordered honeycomb structure of BNPC is favorable for the trapping of polysulfide(PS)species and accommodates the volumetric variation of sulfur during cycling,while the high graphitization degree of BNPC favors the redox kinetics of sulfur cathodes.Moreover,the nitrogen doping content not only enhances the electrical conductivity of BNPC,but also provides ample anchoring sites for the immobilization of PS,which plays a key role in suppressing the shuttle effect.As a result,the S@BNPC cathode exhibits a high initial specific capacity of 1189.4 mA·h/g at 0.2C.After 300 cycles,S@BNPC still maintains a capacity of 703.2 mA·h/g which corresponds to a fading rate of 0.13%per cycle after the second cycle.This work offers vast opportunities for the large-scale application of high performance carbon-based sulfur hosts.展开更多

Their structures have been confirmed by IR, MS and UV spectrometries. When iodine was used as the dope to the conjugated compounds, the electrical conductivities (EC) of the doped conjugated compounds were increased by several orders of magnitude. The thermal stability of these two compounds investigated by TGA shows a good result, which guarantees the correct result of EC when the compounds are heated. As can be seen from the CV characterization of the electrochemical properties of these two compounds, the azomethine diamine and p-aminophenyl-9-ethylcarbazolyl azomethine possess electrochemical activity, which arises from the heteroatom of molecules.展开更多

T...We report the observation of the splitting of whispering-gallery mode in the silica spherical resonator. To characterize the fused-silica spherical resonator,an evanescent fiber taper coupling technique is employed. The coupling system consists of a fiber taper with a diameter of about 2.8 microns and a fused-silica spherical resonator with a diameter of about 143 microns. By injecting the laser power into the fiber taper,a number of narrow absorption peaks with equal peak spacing of 3.7 nm are observed in a wide spectral range. After replacing the resonator by the other one with the shape being slightly modified,whispering-gallery mode splitting in the spherical resonator is observed. The wavelength spacing between adja-cent peaks is 0.32 nm,much smaller than that of the previous one.展开更多
