电子胃镜下斑马导丝转换技术用于空肠营养管置入术的安全性及有效性研究

目的探讨标准型胃镜下斑马导丝转换技术用于空肠营养管置入术的安全性及有效性。方法选取承德市中心医院2017年1月-2021年12月胃镜下放置螺旋型鼻肠管的患者80例,随机分为实验组与对照组,各40例。实验组使用标准型胃镜将斑马导丝置入十... 目的探讨标准型胃镜下斑马导丝转换技术用于空肠营养管置入术的安全性及有效性。方法选取承德市中心医院2017年1月-2021年12月胃镜下放置螺旋型鼻肠管的患者80例,随机分为实验组与对照组,各40例。实验组使用标准型胃镜将斑马导丝置入十二指肠远端后,保持导丝位置相对固定,退出内镜,应用一次性导尿管将斑马导丝置换至鼻腔的方法代替经鼻胃镜下置入导丝技术;对照组使用标准型胃镜下钳夹推送置入螺旋型鼻肠管技术。结果实验组较对照组操作时间短,两组患者比较,差异有统计学意义(P<0.05);实验组鼻咽部不适及腹痛腹胀发生率明显低于对照组,两组患者比较,差异有统计学意义(P<0.05);两组患者一次性置管成功率、误吸及导丝移位发生率比较,差异均无统计学意义(P>0.05)。结论采用斑马导丝转换技术将斑马导丝置换至鼻腔的方法与传统内镜钳夹法相比,具有操作简单和并发症少等优势,且安全有效,可为广大基层医院提供技术支持,从而为消化内镜营养管置入术提供新思路。 展开更多

内镜自动染色水汽装置与常规染色方法在内镜化学染色中的应用效果比较 被引量:1

目的研发内镜自动染色水汽装置,对内镜水汽瓶进行改造,使内镜水汽与染色剂整合在一个装置内,通过转换钮控制内镜水汽和染色剂都分别通过内镜水汽管道进入消化道。并与常规染色方法进行对比,以此来验证内镜自动染色水汽装置的临床效果。... 目的研发内镜自动染色水汽装置,对内镜水汽瓶进行改造,使内镜水汽与染色剂整合在一个装置内,通过转换钮控制内镜水汽和染色剂都分别通过内镜水汽管道进入消化道。并与常规染色方法进行对比,以此来验证内镜自动染色水汽装置的临床效果。方法分为自动染色组与常规染色组两组,每组100例,选取靛胭脂及复方碘溶液作为化学染色剂,对比两组染色剂的使用量和操作时间。结果自动染色组碘液使用剂量和操作时间明显少于常规染色组[碘液使用剂量分别为(9.53±1.01)和(10.73±1.82)mL,操作时间分别为(16.10±2.46)和(25.45±2.43)s],两组比较,差异均有统计学意义(P<0.05);自动染色组靛胭脂使用剂量和操作时间明显少于常规染色组[靛胭脂使用剂量分别为(8.75±1.20)和(9.95±1.43)mL,操作时间分别为(8.60±1.35)和(10.45±2.01)s],两组比较,差异均有统计学意义(P<0.05)。结论内镜自动染色水汽装置可以用于内镜下化学染色,使内镜下染色既高效又便捷。 展开更多

Effect of rituximab combined with CVAD regimen on serum VEGF and β2-MG levels in patients with primary gastrointestinal B-cell lymphoma

Objective:To investigate the clinical effect of rituximab combined with CVAD regimen in patients with primary gastrointestinal B-cell lymphoma and serum vascular endothelial growth factor (VEGF) andβ2 microglobulin (β2-MG) The impact of the level.Methods:Eighty-four patients with primary gastrointestinal B-cell lymphoma treated from May 2014 to December 2015 were enrolled. Based on the random number table, all the patients were divided into a control group (n=42) and an observation group (n=42). The control group was treated with CVAD. The observation group was treated with rituximab on the basis of the control group. The effect of the patients was evaluated after 3 courses of treatment. The patients were followed up for 3 years after treatment. US RECIST 1.1 was used to evaluate the short-term efficacy on the patients;VEGF, TNF receptor-associated factor 6 (TRAF6) and B-cell lymphoma factor-6 (Bcl-6) levels were measured by enzyme-linked immunosorbent assay;β2-MG level test was implemented to compare the short-term efficacy, biochemical indicators, incidence of toxic side effects and long-term survival rate of the two groups. Results: The short-term efficacy rate of the observation group was 76.19%, which was higher than that of the control group (50.00%) (P<0.05). The levels of VEGF, TRAF6, Bcl-6, andβ2-MG were lower in the observation group after 3 courses of treatment than that in the control group (P<0.05);there was no significant difference in the incidence of neutropenia, gastrointestinal reactions, sepsis, infection, infusion-related reactions and cardiovascular events between the observation group and the control group (P>0.05);The 1-year long-term survival rate after treatment was not statistically significant (P>0.05). The long-term survival rate of the observation group was higher than that of the control group at 2 and 3 years after treatment (P<0.05).Conclusion: The combination of rituximab and CVAD in patients with primary gastrointestinal B-cell lymphoma can improve short-term efficacy, lower VEGF andβ2-MG levels, and lower incidence of side effects. It can improve the long-term survival rate of patients and is worthy of promotion and application. 展开更多

Revealing the photoluminescence(PL)origin and mechanism is a most vital but challenging topic of carbon dots.Herein,confined-domain crosslink-enhanced emission(CEE)effect was first studied by a well-designed model system of carbonized polymer dots(CPDs),serving as an important supplement to CEE in the aspect of spatial interactions.The“addition-condensation polymerization”strategy was adopted to construct CPDs with substituents exerting different degrees of steric hindrance.The effect of confined-domain CEE on the structure and luminescence properties of CPDs have been systematically investigated by combining characterizations and theoretical calculations.Such tunable spatial interactions dominated the coupling strength of the luminophores in one particle,and eventually resulted in the modulated PL properties of CPDs.These findings provide insights into the structural advantages and the PL mechanism of CPDs,which are of general significance to the further development of CPDs with tailored properties. 展开更多

A reversible dual-responsive luminescent material was introduced by our group to show the simultaneous color and lifetime switching in response to external stimuli.Pristine crystalline powder of(E)-2-(benzo[d]thiazol-2-yl)-3-(pyren-1-yl)acrylonitrile(Py-BZTCN)shows the orderedπ–πstacking with only near-monomer-normal orange-yellow fluorescence,but it exhibits red emission with thermally activated delayed fluorescence(TADF)after grinding,which can be reversibly recovered by heating or fuming treatment.Grinding disturbs the orderedπ–πstacking of pristine powder,leading to the formation of small aggregates with compressed distance and increased overlap ofπ–πstacking between adjacent molecules.The cause of switching was verified by single-crystal X-ray diffraction experiments of two corresponding crystals.This strongπ–πinteraction effectively promotes the excited-state energy splitting and substantially decreases the singlet–triplet energy gap(ΔEST)of aggregates,resulting in the red TADF emission of aggregates through reverse intersystem crossing.This finding proposes a new route to realizing the TADF emission of aggregates through strong intermolecular interactions based on non-TADF monomer,thereby enabling a novel high-contrast dualresponsive luminescence switching. 展开更多
