目的研究血管活性肠肽(VIP)及其受体在嗜酸性粒细胞性胃肠炎(EG)儿童胃黏膜中的表达,并探讨其临床意义。方法选取2008年11月-2018年12月天津市儿童医院消化科收治的50例EG儿童作为研究对象(EG组);同时选取胃镜下病理活检嗜酸性粒细胞(EO... 目的研究血管活性肠肽(VIP)及其受体在嗜酸性粒细胞性胃肠炎(EG)儿童胃黏膜中的表达,并探讨其临床意义。方法选取2008年11月-2018年12月天津市儿童医院消化科收治的50例EG儿童作为研究对象(EG组);同时选取胃镜下病理活检嗜酸性粒细胞(EOS)<10个/高倍视野(HPF),总免疫球蛋白E(IgE)正常的同年龄段急性肠胃炎儿童50例作为对照组。比较两组患儿外周血EOS计数、总IgE水平,胃镜检查其黏膜变化。采用逆转录聚合酶链反应(RT-PRC)检测两组患儿VIP及VPAC1、VPAC2 mRNA的相对表达水平并进行比较。利用Logistic回归模型分析EG的相关因素。结果EG组外周血EOS、EOS%、IgE水平高于对照组(P<0.05);EG组胃镜检查黏膜充血水肿、糜烂、溃疡、结节样隆起或增生例数多于对照组(P<0.05)。EG组儿VIP及VPAC1、VPAC2 mRNA的表达水平低于对照组(P<0.05)。Logistic回归分析结果表明,VIP及VPAC1、VPAC2 mRNA是EG的独立影响因素(P<0.05)。结论VIP、VPAC1及VPAC2是EG发生的独立影响因素,可能与儿童胃黏膜EG的发生、发展相关,有助于早期诊断。 展开更多

AIM:To investigate the relationship between 579 G>T polymorphisms in the DNMT3B gene, which is involved in de novo methylation and associated with the risk of esophagus cancer (EC) in Chinese. METHODS:DNMT3B 579 G>T genotypes were determined by PCR-RFLP in 194 EC patients and 210 healthy controls matched for age and sex, who did not receive radiotherapy or chemotherapy for newly diagnosed and histopathologically confirmed EC. RESULTS:In control subjects, the frequency of T/T and G/T genotypes, and T and G alleles was 81.4%, 18.1%, 90.05% and 9.55%, respectively. The distribution of genotypes and allelotypes in the EC patients was not significantly different from that in the controls. When stratified by sex and age, there was still no significant association between the risks of EC and GT and GG genotypes. This study also showed a distinct difference in the distribution of DNMT3B and single nucleotide polymorphism (SNP) between Chinese and Koreans.CONCLUSION:DNMT3B 579 G>T polymorphism may not be a stratification marker to predict the susceptibility to EC, at least in Chinese. DNMT3B promoter SNP is diverse in ethnic populations. 展开更多

Coexistent Kaposi sarcoma and post-transplant lymphoproliferative disorder in the same lymph nodes after pediatric liver transplantation:A case report

BACKGROUND Kaposi sarcoma and post-transplant lymphoproliferative disorder have been occasionally reported in post-liver transplant patients.However,the simultaneous occurrence of these two diseases in the same lymph nodes is very rare.CASE SUMMARY We report the case of a 19-mo-old boy,who presented with intermittent fever and enlarged cervical lymph nodes after liver transplantation.Six cervical lymph nodes were biopsied,and the histopathological examinations revealed multifocal hyperplasia of spindle cells around small blood vessels,extravasated erythrocytes,and heavy infiltration of plasma cells in the cortex and medulla of the lymph nodes.The immunohistochemical analyses of spindle cells revealed positive expression of CD34,CD31,erythroblast transformation-specific-related gene,friend leukemia integration 1,and human herpesvirus-8.The lymphoproliferative lesions expressed CD38,CD138,and multiple myeloma 1.Epstein-Barr encoded RNA in situ hybridization demonstrated Epstein-Barr virus-positive lymphoid cells.Finally,we diagnosed the coexistence of Kaposi sarcoma and post-transplant lymphoproliferative disorder(plasmacytic hyperplasia)in the same lymph nodes.Treatment strategy included anti-CD20 monoclonal antibody(rituximab)and discontinuation of the immunosuppressant therapies.Lymph node biopsies during follow-up examinations revealed lymphoid hyperplasia.CONCLUSION The rare coexistence of Kaposi sarcoma and post-transplant lymphoproliferative disorder in the same lymph nodes post-liver transplantation possibly associates with immunodeficiency and Epstein-Barr virus and human herpesvirus-8 coinfection. 展开更多

Torsades de pointes episode in a woman with high-grade fever and inflammatory activation: A case report

BACKGROUND QT interval prolongation can induce torsades de pointes(TdP),a potentially fatal ventricular arrhythmia.Recently,an increasing number of non-cardiac drugs have been found to cause QT prolongation and/or TdP onset.Moreover,recent findings have demonstrated the key roles of systemic inflammatory activation and fever in promoting long-QT syndrome(LQTS)and TdP development.CASE SUMMARY A 30-year-old woman was admitted with a moderate to high-grade episodic fever for two weeks.The patient was administered with multiple antibiotics after hospitalization but still had repeating fever and markedly elevated C-reactive protein.Once after a high fever,the patient suddenly lost consciousness,and electrocardiogram(ECG)showed transient TdP onset after frequent premature ventricular contraction.The patient recovered sinus rhythm and consciousness spontaneously,and post-TdP ECG revealed a prolonged QTc interval of 560 ms.The patient’s clinical manifestations and unresponsiveness to the antibiotics led to the final diagnosis of adult-onset Still’s disease(AOSD).There was no evidence of cardiac involvement.After the AOSD diagnosis,discontinuation of antibiotics and immediate initiation of intravenous dexamethasone administration resulted in the normal temperature and QTc interval.The genetic analysis identified that the patient and her father had heterozygous mutations in KCNH2(c.1370C>T)and AKAP9(c.7725A>C).During the 2-year follow-up period,the patient had no recurrence of any arrhythmia and maintained normal QTc interval.CONCLUSION This case study highlights the risk of systemic inflammatory activation and antibiotic-induced TdP/LQTS onset.Genetic analysis should be considered to identify individuals at high risk of developing TdP. 展开更多

Objective:Through epidemiological investigation,to investigate the etiology and influencing factors of primary gout and hyperuricemia(HUA).Methods:Using case-control methods,designed questionnaires on gout and/or HUA related factors(living habits,eating habits and environmental factors,etc).Logistic regression analysis of the survey results was performed with SPSS 19.0 software.Results:(1)Univariate Logistic regression analysis:11 risk factors with statistical significance(P<0.05),often eat sauce food,often eat Fried food,often eat asparagus,often eat nuts,often drink,smoking,drink water<1000ml per day,residential temperature≤17℃in winter,work environment average temperature≤17℃in winter,often exposure to toxic and harmful substances,often use induction cooker or microwave oven(P<0.05),the OR values were 2.063,1.701,3.242,1.560,2.055,1.338,5.700,5.330,2.305,2.821,1.701;3 protective factors,occasionally eat fresh fruit,wake up between 5 and 7 am,use the computer less than 1 hour per day(P<0.05),OR values were 0.723,0.611 and 0.558,respectively.(2)Multivariate Logistic regression analysis was performed for all factors:6 risk factors,The risk factors were often drink,drink water<1000ml per day,residential temperature≤17℃in winter,work environment average temperature≤17℃in winter,use mobile network cellphones and use unicom network cellphones(P<0.05),OR values are 2.008,4.240,4.034,1.889,2.207 and 1.891,respectively;2 protective factors:occasionally eat agaric and use the computer less than 1 hour per day(P<0.05),OR values were 0.551 and 0.536,respectively.Conclusions:Proper improvement of diet structure,adjustment of life and behavior,and avoidance of environmental risk factors can effectively reduce uric acid level,control the incidence of HUA and gout,and improve health level. 展开更多
