目的分析乙型肝炎病毒(hepatitis B virus,HBV)相关肝硬化病人前S1抗原(Pre-S1 antigen,PreS1Ag)、乙型肝炎表面抗原(hepatitis B surface antigen,HBsAg)水平及PreS1Ag/HBsAg的比值,对HBV相关肝硬化病人肝癌发生风险的影响。方法2019年...目的分析乙型肝炎病毒(hepatitis B virus,HBV)相关肝硬化病人前S1抗原(Pre-S1 antigen,PreS1Ag)、乙型肝炎表面抗原(hepatitis B surface antigen,HBsAg)水平及PreS1Ag/HBsAg的比值,对HBV相关肝硬化病人肝癌发生风险的影响。方法2019年3月~2021年4月合肥市第一人民医院收治HBV相关肝硬化肝癌病人61例,未发生肝癌的HBV相关肝硬化病人61例,检测所有受试者PreS1Ag、HBsAg水平及PreS1Ag/HBsAg比值,建立Logistic回归预测模型分析HBV相关肝硬化病人PreS1Ag、HBsAg水平及PreS1Ag/HBsAg比值对肝癌发生风险的影响,采用ROC曲线、决策曲线对模型进行预测效力及临床实用性评价。结果HBV相关肝硬化病人PreS1Ag、HBsAg及PreS1Ag/HBsAg比值分别为(112.36±20.47)ng/ml、(3.67±0.29)log IU/ml和(1.14±0.20);Logistic回归分析显示,年龄、HBsAg、PreS1Ag、PreS1Ag/HBsAg是HBV相关肝硬化病人肝癌发生风险的危险因素,ROC曲线分析,多因素Logistic回归分析结果建立预测模型,结果显示,HBsAg、PreS1Ag、PreS1Ag/HBsAg在预测HBV相关肝硬化病人肝癌发生均具临床价值。结论年龄、HBsAg、PreS1Ag、PreS1Ag/HBsAg是HBV相关肝硬化病人肝癌发生危险因素,HBsAg、PreS1Ag、PreS1Ag/HBsAg预测HBV相关肝硬化病人肝癌发生均具临床价值。展开更多

However, the relationship between altered glycosylation patterns and the onset and development of reproductive disorders is unclear, mainly limited by the lack of in situ imaging techniques for spermatozoa glycosylation. We developed an efficient and highly specific spermatozoa glycan imaging technique based on the robust chemoselective labeling of sialic acid(Sia) and N-acetyl-D-galactosamine(Gal/GalNAc). We further proposed a “tandem glycan chemoselective labeling” strategy to achieve simultaneous imaging of two types of glycans on spermatozoa. We applied the developed method to the spermatozoa from oligozoospermic patients and diabetic mice and found that these spermatozoa showed higher levels of Sia and Gal/Gal NAc expression than the normal groups. Moreover, spermatozoa from diabetic mice showed a severe decrease in number, viability, and forward motility, suggesting that in vivo glucose metabolism disorders may lead to an elevated level of spermatozoa glycosylation and have a correlation with the development of oligoasthenotspermia. Our work provides a research tool to reveal the relationship between glycosylation modification and spermatozoa quality, and a promising clue for the development of glycan-based reproductive markers.展开更多
目的:探讨快速康复理念(enhanced recovery after surgery,ERAS)联合基于现象学分析法的健康干预在肝胆胰疾病患者围术期的应用效果。方法:选取我院2021年1月―2023年1月肝胆胰疾病手术患者161例,根据非随机临床同期对照研究以及患者自...目的:探讨快速康复理念(enhanced recovery after surgery,ERAS)联合基于现象学分析法的健康干预在肝胆胰疾病患者围术期的应用效果。方法:选取我院2021年1月―2023年1月肝胆胰疾病手术患者161例,根据非随机临床同期对照研究以及患者自愿原则分为对照组76例(常规干预)和干预组85例(快速康复理念联合基于现象学分析法的健康干预)。对比两组术后首次排便时间、下床活动时间、住院时间、住院费用、术后疼痛程度[视觉模拟评分(VAS)]、并发症发生情况、心理困扰程度和满意率。结果:干预组首次排便时间、下床活动时间、住院时间均短于对照组,住院费用少于对照组;术后3 d及8 d干预组VAS评分低于对照组;干预组术后并发症总发生率低于对照组;术后1个月干预组中重度心理困扰程度占比低于对照组;术后1个月后干预组总满意率高于对照组。结论:快速康复理念联合基于现象学分析法的健康干预在肝胆胰疾病患者围术期的应用效果显著,能促进患者术后恢复,减轻患者治疗经济负担,缓解患者术后疼痛及不良心理状态,降低术后并发症发生率,患者满意率高,值得临床应用。展开更多
