Treatment of pulmonary mucosa-associated lymphoid tissue lymphoma with clarithromycin

To the editor：Mucosa-associated lymphoid tissue （MALT） lymphoma is a low grade B-cell extra-nodal lymphoma from margtnal zone.Lung location is rare and represents 3%-4% of all cases.1 There is no uniform treatment for pulmonary-MALT （p-MALT） lymphoma.This is the report of p-MALT without any extra-pulmonary lesions of MALT lymphoma that was regressed with clarithromycin （CAM） （500 mg/d） treatment in China.

Bosentan and antiretroviral therapy in an HIV/HBV/HCV coinfected Chinese patient with HIV-related pulmonary arterial hypertension

To the editor： Pulmonary arterial hypertension （PAH） is an infrequent but nevertheless, serious life threatening complication of human immunodeficiency virus （HIV） infection. People infected with HIV have a high prevalence of chronic hepatitis B virus （HBV） and/or hepatitis C virus （HCV）. Bosentan is effective in patients with HIV-related PAH （HRPH）;1 but hardly used on Chinese patients so far. In this rare case, we present successful, long-term coadministration of bosentan and antiretroviral therapy in an HIV/HBV/HCV coinfected Chinese patient with HRPH.

Aortoduodenal fistula after endovascular aortic repair of an infected abdominal aortic aneurysm

To the editor： We report a case of aortoduodenal fistula presenting as right lower limb ischemia and infection after endovascular aortic repair （EVAR） of an infected abdominal aortic aneurysm （AAA）. A right above knee amputation was performed for the septic embolism of the ilio-femoral arteries before the stem-graft infection was finally diagnosed. Then an extra-anatomic revascularization of the left lower limb, following by explantation of the infected stent-graft and rebuilding of the gastro-intestinal continuity were done.

Asymptomatic hydrocephalus and subdural hygroma after successful resection of hypothalamic hamartoma with central precocious puberty

To the editor： A 12-month-old boy presenting with a three-month history of precocious puberty was admitted to our department in December 2009. Apart from an unremarkable medical history, he was found to have pubic hair and noted to be more aggressive than other children of his age. Physical examination revealed a premature penile development （stage 3 according to Tanner stages）1 and slight development of pubic hair （stage 3）, but without any development of laryngeal prominence. The patient had no focal neurological deficit.

Emphysema associated with acquired cutis laxa

To the editor： Cutis laxa is a rare connective tissue disorder which is manifested by loose, redundant and inelastic skins. Occasionally, pulmonary parenchyma will be involved, resulting in emphysema. Here we present two rare cases of emphysema associated with acquired cutis laxa.

Giant aortic arch aneurysm

To the editor： A 44-year-old man presented to our department with intermittent chest and back pain. He had a past medical history of having undergone aortic valve replacement and ascending aortic aneurysm repair （Bentall） in the same operation 12 years ago. The Bentall operation was performed due to the diagnosis of severe aortic valve regurgitation together with an aneurysm of the ascending aorta measuring 70 mm in diameter. The diameter of the remaining aortic arch had previously been measured as 40 nnn at the time.
