髋关节置换术患者实施快速康复外科护理的效果评价

目的探讨基于快速康复外科(FTS)理念的护理干预在行髋关节置换术(THA)患者中的应用效果。方法选择2021年1月—2022年6月宜春市第二人民医院骨外科行THA的患者52例为研究对象。按照组间基本特征具有可比性的原则分为观察组患者与对照组患... 目的探讨基于快速康复外科(FTS)理念的护理干预在行髋关节置换术(THA)患者中的应用效果。方法选择2021年1月—2022年6月宜春市第二人民医院骨外科行THA的患者52例为研究对象。按照组间基本特征具有可比性的原则分为观察组患者与对照组患者,每组26例,观察组患者实施基于FTS理念的护理干预,对照组患者实施常规护理,两组患者均持续随访6个月。对比两组患者术后康复、关节功能、生活质量及并发症发生率。结果观察组患者术后首次下床、术后住院时间均短于对照组患者,组间比较差异有统计学意义(P<0.05)。快速康复外科护理干预前,两组患者髋关节功能评分量表(Harris)中关节活动度、疼痛、功能性活动及畸形维度评分比较差异无统计学意义(P>0.05);干预后,观察组患者关节活动度、疼痛、功能性活动及畸形维度评分高于对照组患者,组间比较差异有统计学意义(P<0.05)。干预前,两组患者SF-36量表中的生理职能、生理机能、一般健康状况、躯体疼痛、社会功能、精力、精神健康及情感职能维度评分比较差异无统计学意义(P>0.05);干预后,观察组患者健康调查量表(SF-36)中的生理职能、生理机能、一般健康状况、躯体疼痛、社会功能、精力、精神健康及情感职能维度评分高于对照组患者,组间比较差异有统计学意义(P<0.05)。观察组患者并发症发生率低于对照组患者,组间比较差异有统计学意义(P<0.05)。结论基于FTS理念护理干预能够缩短THA患者术后康复时间,促进髋关节功能恢复,减少并发症的发生,提升生活质量。 展开更多

BACKGROUND Interleukin 10 receptor alpha subunit(IL10RA)dysfunction is the main cause of very early-onset inflammatory bowel disease(VEO-IBD)in East Asians.AIM To identify disease-causing gene mutations in four patients with VEO-IBD and verify functional changes related to the disease-causing mutations.METHODS From May 2016 to September 2020,four young patients with clinically diagnosed VEO-IBD were recruited.Before hospitalization,using targeted gene panel sequencing and trio-whole-exome sequencing(WES),three patients were found to harbor a IL10RA mutation(c.301C>T,p.R101W in one patient;c.537G>A,p.T179T in two patients),but WES results of the fourth patient were not conclusive.We performed whole-genome sequencing(WGS)on patients A and B and reanalyzed the data from patients C and D.Peripheral blood mononuclear cells(PBMCs)from patient D were isolated and stimulated with lipopolysaccharide(LPS),interleukin 10(IL-10),and LPS+IL-10.Serum IL-10 levels in four patients and tumor necrosis factor-α(TNF-α)in the cell supernatant were determined by enzyme-linked immunosorbent assay.Phosphorylation of signal transducer and activator of transcription 3(STAT3)at Tyr705 and Ser727 in PBMCs was determined by western blot analysis.RESULTS The four children in our study consisted of two males and two females.The age at disease onset ranged from 18 d to 9 mo.After hospitalization,a novel 333-bp deletion encompassing exon 1 of IL10RA was found in patients A and B using WGS and was found in patients C and D after reanalysis of their WES data.Patient D was homozygous for the 333 bp deletion.All four patients had elevated serum IL-10 levels.In vitro,IL-10-stimulated PBMCs from patient D failed to induce STAT3 phosphorylation at Tyr705 and only minimally suppressed TNF-αproduction induced by LPS.Phosphorylation at Ser727 in PBMCs was not affected by LPS or LPS+IL-10 in both healthy subjects and in patient D.CONCLUSION WGS revealed a novel 333-bp deletion of IL10RA in four patients with VEO-IBD,whereas the WES results were inconclusive. 展开更多

High-pressure synchrotron X-ray diffraction and infrared absorption spectroscopy have been employed to study the crystal chemistry and phase transitions in an[OH]-bearing carbonate,malachite Cu2(CO_(3))(OH)_(2),to determine the effect of[OH]on the stability of carbonate.We found that the crystal structure of malachite is stabilized by a high degree of[CuO_(6)]-octahedron distortion,as is manifested by large variations in Cu–O bond lengths resulting from oxygen atoms that connect to hydrogen at crystallographically different sites.External pressure offsets the effect of hydrogen bond,promotes[CuO_(6)]compression and regularization and accordingly[CO_(3)]rotation.Rotation of[CO_(3)]-triangles,in turn,assists in a conversion in the crystal orientation of the[CuO_(6)]structural unit.During compression to above~6 GPa,malachite begins to turn into the rosasite lattice,accompanied with a jump in density of 3.3%.Rosasite is characterized with a hardened lattice and preserves to the maximum pressure(18.2 GPa)of the present study.Phase transformation mechanism of malachite to rosasite is different from that of carbonates,with the latter being driven by an almost uniform compression of[MO_(6)]-octahedron(M=Ca,Cd,Mn,Fe,Zn,Mg,etc.)and rotation/translation of[CO_(3)]-triangle under pressure. 展开更多

BACKGROUND Infantile-onset inflammatory bowel disease(IO-IBD)occurs in very young children and causes severe clinical manifestations,which has poor responses to traditional inflammatory bowel disease(IBD)treatments.At present,there are no simple and reliable laboratory indicators for early screening IO-IBD patients,especially those in whom the disease is caused by monogenic diseases.AIM To search for valuable indicators for early identifying IO-IBD patients,especially those in whom the disease is caused by monogenic diseases.METHODS A retrospective analysis was performed in 73 patients with IO-IBD admitted to our hospital in the past 5 years.Based on the next-generation sequencing results,they were divided into a monogenic IBD group(M-IBD)and a non-monogenic IBD group(NM-IBD).Forty age-matched patients with allergic proctocolitis(AP)were included in a control group.The clinical manifestations and the inflammatory factors in peripheral blood were evaluated.Logistic regression analysis and receiver operating characteristic(ROC)curve analysis were used to identify the screening factors and cut-off values of IO-IBD as well as monogenic IO-IBD,respectively.RESULTS Among the 44 M-IBD patients,35 carried IL-10RA mutations,and the most common mutations were c.301C>T(p.R101W,30/70)and the c.537G>A(p.T179T,17/70).Patients with higher serum tumor necrosis factor(TNF)-αvalue were more likely to have IBD[odds ratio(OR)=1.25,95%confidence interval(CI):1.05-1.50,P=0.013],while higher serum albumin level was associated with lower risk of IBD(OR=0.86,95%CI:0.74-1.00,P=0.048).The cut-off values of TNF-αand albumin were 17.40 pg/mL(sensitivity:0.78;specificity:0.88)and 36.50 g/L(sensitivity:0.80;specificity:0.90),respectively.The increased ferritin level was indicative of a genetic mutation in IO-IBD patients.Its cut-off value was 28.20 ng/mL(sensitivity:0.93;specificity:0.92).When interleukin(IL)-10 level was higher than 33.05 pg/mL(sensitivity:1.00;specificity:0.84),or the onset age was earlier than 0.21 mo(sensitivity:0.82;specificity:0.94),the presence of diseasecausing mutations in IL-10RA in IO-IBD patients was strongly suggested.CONCLUSION Serum TNF-αand albumin level could differentiate IO-IBD patients from allergic proctocolitis patients,and serum ferritin and IL-10 levels are useful indicators for early diagnosing monogenic IO-IBD. 展开更多

BACKGROUND The global outbreak of human severe acute respiratory syndrome coronavirus(SARS-CoV)-2 infection represents an urgent need for readily available,accurate and rapid diagnostic tests.Nucleic acid testing of respiratory tract specimens for SARS-CoV-2 is the current gold standard for diagnosis of coronavirus disease 2019(COVID-19).However,the diagnostic accuracy of reverse transcription polymerase chain reaction(RT-PCR)tests for detecting SARS-CoV-2 nucleic acid may be lower than optimal.The detection of SARS-CoV-2-specific antibodies should be used as a serological non-invasive tool for the diagnosis and management of SARS-CoV-2 infection.AIM To investigate the diagnostic value of SARS-CoV-2 IgM/IgG and nucleic acid detection in COVID-19.METHODS We retrospectively analyzed 652 suspected COVID-19 patients,and 206 non-COVID-19 patients in Wuhan Integrated TCM and Western Medicine Hospital.Data on SARS-CoV-2 nucleic acid tests and serum antibody tests were collected to investigate the diagnostic value of nucleic acid RT-PCR test kits and immunoglobulin(Ig)M/IgG antibody test kits.The j2 test was used to compare differences between categorical variables.A 95%confidence interval(CI)was provided by the Wilson score method.All analyses were performed with IBM SPSS Statistics version 22.0(IBM Corp.,Armonk,NY,United States).RESULTS Of the 652 suspected COVID-19 patients,237(36.3%)had positive nucleic acid tests,311(47.7%)were positive for IgM,and 592(90.8%)were positive for IgG.There was a significant difference in the positive detection rate between the IgM and IgG test groups(P<0.001).Using the RT-PCR results as a reference,the specificity,sensitivity,and accuracy of IgM/IgG combined tests for SARS-CoV-2 infection were 98.5%,95.8%,and 97.1%,respectively.Of the 415 suspected COVID-19 patients with negative nucleic acid test results,366 had positive IgM/IgG tests with a positive detection rate of 88.2%.CONCLUSION Our data indicate that serological IgM/IgG antibody combined test had high sensitivity and specificity for the diagnosis of SARS-CoV-2 infection,and can be used in combination with RT-PCR for the diagnosis of SARS-CoV-2 infection. 展开更多

Tectonic activity and sediment supply have significantly affected the rifted basin fillings and hydrocarbon accumulations.The Huanghekou Sag in the southeastern Bohai Bay Basin is a hydrocarbon rich sag and was bounded by the Bonan Uplift to the north in the Paleogene,which strongly affected sediment disribution and hydrocarbon accumulation in this region.The sediment supply mechanism of the Paleogene parent rocks in this area has not been revealed yet.Therefore,the Huanghekou Sag is a natural laboratory to reveal the relationship between the tectonic activties and hydrcoarbon accumulations in a rifted basin.In this study,the tectonic evolution characteristics of the Bonan Uplift in different geological periods are revealed during the main fault depression periods.Then,the direction of the provenance and distribution of the depositional system of the Bonan Uplift are analyzed based on the sedimentary system and mineralogy analysis.Finally,the sediment supply in the Bonan Uplift in different geological periods has been systematically studied.The results indicated that sediment supply is the dominant factor of hydrocarbon accumulation in the Paleogene in the Huanghekou Sag.During the Paleocene-Middle Eocene,extensive denudation occurred in the Bonan Uplift,sediments were mainly sourced from the Bonan Uplift in the northern margin of the Huanghekou Sag.During the Late Eocene to Early Oligocene,the Bonan Uplift had a large angle rotation and subsidence due to the strong rifting of the Bozhong Sag in the north,and the sand supply capacity was seriously reduced.Due to the different sediment supply in different geological periods,high quality reservoir and cap conditions were formed in the Paleogene in the central and northern parts of the Huanghekou Sag,which is conducive to the accumulation and preservation of oil and gas. 展开更多

Antimicrobial resistance in the laying hen production industry has become a serious public health problem.The antimicrobial resistance and phylogenetic relationships of the common conditional pathogen Enterococcus along the laying hen production chain have not been systematically clarified.105 Enterococcus isolates were obtained from 115 environmental samples (air,dust,feces,fies,sewage,and soil) collected along the laying hen production chain (breeding chicken,chick,young chicken,and commercial laying hen).These Enterococcus isolates exhibited resistance to some clinically relevant antibiotics,such as tetracycline (92.4%),streptomycin (92.4%),and erythromycin (91.4%),and all strains had multidrug resistance phenotypes.Whole genome sequencing characterized 29 acquired antibiotic resistance genes (ARGs) that conferred resistance to 11 classes of antibiotics in 51pleuromutilin-resistant Enterococcus isolates,and lsa(E),which mediates resistance to pleuromutilins,always co-occurred with lnu(B).Alignments with the Mobile Genetic Elements database identified four transposons (Tn554,Tn558,Tn6261,and Tn6674) with several ARGs(erm(A),ant(9)-la,fex(A),and optrA) that mediated resistance to many clinically important antibiotics.Moreover,we identified two new transposons that carried ARGs in the Tn554 family designated as Tn7508 and Tn7492.A complementary approach based on conventional multi-locus sequence typing and whole genome single nucleotide polymorphism analysis showed that phylogenetically related pleuromutilin-resistant Enterococcus isolates were widely distributed in various environments on different production farms.Our results indicate that environmental contamination by antimicrobial-resistant Enterococcus requires greater attention,and they highlight the risk of pleuromutilin-resistant Enterococcus and ARGs disseminating along the laying hen production chain,thereby warranting effective disinfection. 展开更多

Chirality,one of the most fundamental characteristics of nature,plays an important role in different fields ranging from physics to astronomy,chemistry and biology[1].Materials with certain chirality usually show unique optical and photophysical properties.The chiral-induced spin selectivity(CISS)effect,which was first reported in 1999,is considered as one of the most important spin-related properties of chiral materials.The electron transportation through chiral materials depends on the electron spin.That is,the electrons with a certain spin state such as spin-up state can pass through the chiral materials more easily in one direction,while the transportation of electrons in the spin-down state will be blocked.Therefore,the distinct advantage of CISS effect opens up the possibility to control and manipulate the spin orientation of electrons without using external magnetic fields,which is promising for the fabrication of new spintronic devices[2]. 展开更多

Plasmids play a critical role in the dissemination of antimicrobial resistance genes(ARGs),however,a systematical understanding of ARGs originated from plasmids in swine production is currently lacking.Herein,quantitative polymerase chain reaction was applied to determine the prevalence of ten ARGs and the class1 integron gene intI1 of plasmid source in swinemanure from 44 farms in Sichuan,Hubei and Hebei provinces,China.All assayed ARGs were observed in plasmid DNA samples,and the average absolute abundance of aac(6’)-Ib-cr,blaNDM,blaCTX-M,optrA,ermB,floR,mcr-1,qnrS,tetM,sul1 and intI1 were 7.09,2.90,4.67,6.62,7.55,7.14,4.08,4.85,7.16,7.11 and 8.07 of 10 log copies/gram,respectively.IntI1 showed a high correlation(r>0.8,P<0.01)with the abundance of aac(6’)-Ib-cr and sul1 in swine manure.Moreover,the farm scale(i.e.,herd population)and geographical location were not found to be critical factors influencing the absolute abundance of ARGs of plasmid DNA in swine farms.However,the concentrations of florfenicol,Cu,Zn,Fe,total phosphorus(TP)and total potassium(TK)demonstrated a significant correlation with the abundance of several ARGs.Particularly,Cu and Zn had high correlations with optrA and blaCTX-M,respectively.Our results demonstrated that antibiotics,heavy metals and environmental nutrients are likely jointly contributing to the long-term persistence of ARGs in swine production.This study provides insights into the abundance and influencing factors of ARGs from swine manure,which is of significance for assessing and reducing the public health risks in livestock production. 展开更多

Indoxacarb and metaflumizone belong to a relatively new class of sodium channel blocker insecticides (SCBIs). Due to intensive use of indoxacarb, field-evolved indoxacarb resistance has been reported in several lepidopteran pests, including the dia- mondback moth Plutella xylostella, a serious pest of cruciferous crops. In particular, the BY 12 population of P. xylostella, collected from Baiyun, Guangdong Province of China in 2012, was 750-fold more resistant to indoxacarb and 70-fold more resistant to metaflu- mizone compared with the susceptible Roth strain. Comparison of complementary DNA sequences encoding the sodium channel genes of Roth and BY12 revealed two point mu- tations (F1845Y and V1848I) in the sixth segment of domain IV of the PxNav protein in the BY population. Both mutations are located within a highly conserved sequence region that is predicted to be involved in the binding sites of local anesthetics and SCBIs based on mammalian sodium channels. A significant correlation was observed among 10 field-collected populations between the mutant allele (Y 1845 or I 1848) frequencies (1.7% to 52.5%) and resistance levels to both indoxacarb (34- to 870-fold) and metaflumizone (1- to 70-fold). The two mutations were never found to co-exist in the same allele of PxNav, suggesting that they arose independently. This is the first time that sodium channel muta- tions have been associated with high levels of resistance to SCBIs. F 1845Y and V 1848I are molecular markers for resistance monitoring in the diamondback moth and possibly other insect pest species. 展开更多

This study aimed to evaluate the efficacy of Chinese herbal medicine(CHM)in patients with severe/critical coronavirus disease 2019(COVID-19).In this retrospective study,data were collected from 662 patients with severe/critical COVID-19 who were admitted to a designated hospital to treat patients with severe COVID-19 in Wuhan before March 20,2020.All patients were divided into an exposed group(CHM users)and a control group(non-users).After propensity score matching in a 1:1 ratio,156 CHM users were matched by propensity score to 156 non-users.No significant differences in seven baseline clinical variables were found between the two groups of patients.All-cause mortality was reported in 13 CHM users who died and 36 non-users who died.After multivariate adjustment,the mortality risk of CHM users was reduced by 82.2%(odds ratio 0.178,95%CI 0.076–0.418;P<0.001)compared with the non-users.Secondly,age(odds ratio 1.053,95%CI 1.023–1.084;P<0.001)and the proportion of severe/critical patients(odds ratio 0.063,95%CI 0.028–0.143;P<0.001)were the risk factors of mortality.These results show that the use of CHM may reduce the mortality of patients with severe/critical COVID-19. 展开更多

Rutile is a common Ti-bearing accessory mineral in high pressure (HP) metamorphic rocks of the western Tianshan. Distribution of rutile in the HP rocks varies from 0.5% in the greenschist to 30% in the rutile-bearing vein. Rutile can be subdivided into three groups based on the variation of trace elements: The first group has an averaged Zr content of 18 ppm-44 ppm and Hf content of 0.8 ppm-2.4 ppm, which correspond to occurrence of rutile from vein; the second group Zr of 59 ppm-63 ppm, Hf of 3.5 ppm-3.7 ppm; and the third group Zr of 150 ppm-160 ppm and Hf of 3.9 ppm, corresponding to rutile in the matrix of blueschist and eclogite, respectively. Rutile has been retrograded to ilmenite and titanite partly or completely, reducing the value of the ore. But rutile of HP rocks in the western Tianshan usually has the lowest content of uranium (〈1 ppm U), which might become an attractive raw material for the Ti industry. Therefore, rutile-bearing HP rocks in the western Tianshan as a mineral resource will be the focus of considerable attention. 展开更多
