喉罩通气方式对妇科腹腔镜手术患者的影响

目的比较妇科腹腔镜手术中使用喉罩通气道(laryngeal mask airway,LMA)条件下应用容量控制通气(volume controlled ventilation,VCV)和压力控制通气(pressure controlled ventilation,PCV)对呼吸力学、血流动力学、血气分析及气体交换... 目的比较妇科腹腔镜手术中使用喉罩通气道(laryngeal mask airway,LMA)条件下应用容量控制通气(volume controlled ventilation,VCV)和压力控制通气(pressure controlled ventilation,PCV)对呼吸力学、血流动力学、血气分析及气体交换的影响。方法选取2022年3月至2022年7月温州市人民医院收治的妇科腹腔镜手术患者60例作为研究对象,分为VCV组和PCV组,每组30例。两组均在LMA插入后5 min(T1)、15 min(T2)、气腹后15 min(T3)采集动脉血进行血气分析,监测记录呼吸力学参数。结果T1和T2时,VCV组气道峰压和平台压明显高于PCV组(P<0.05)。与VCV组相比,PCV组在T2和T3时动脉血氧分压(partial pressure of oxygen,PaO2)明显较高(P<0.05);PCV组的动脉血二氧化碳分压(partial pressure of carbon dioxide,PaCO_(2))在T1、T2时明显较低(P<0.05),T3时两组的PaCO_(2)无明显差异(P>0.05);PCV组中的呼气末二氧化碳分压(partial pressure of end-tidal carbon dioxide,PetCO_(2))浓度在T2、T3时明显较VCV组低(P<0.05)。结论VCV和PCV均适用于妇科腹腔镜手术,但PCV的气道压力低于VCV,选用PCV对预防肺的气道压力伤具有一定意义。 展开更多

In vitro inhibition of proliferation,migration and epithelial-mesenchymal transition of human lens epithelial cells by fasudil 被引量:5

AIM: To study the potential role of fasudil as a treatment for posterior capsular opacification(PCO) of the human crystalline lens.METHODS: Human lens epithelial cells(HLECs; line SRA01/04) was exposed to transforming growth factor-β2(TGF-β2) to induce the process of epithelial-mesenchymal transition(EMT). Fasudil was applied to the cell samples. Its effect on overall HLECs proliferation and migration was studied, as was its influence on EMT induction by TGF-β2 using cell migration assay, MTT colorimetric assay and Western blot assay.RESULTS: Fasudil inhibited the proliferation of SRA01/04. Its effect was time-and concentration-dependent. The migration of SRA01/04 cells was significantly reduced 24-72 h after fasudil treatment, and the half maximal inhibitory concentration(IC50) was 22.37 μmol/mL at 72 h. Reversal of the elongated, fibroblast-like shape changes induced by TGF-β2 in SRA01/04 cells was observed. Fasudil up-regulated the expression of Connexin43 protein and down-regulated the expression of α-SMA protein compared with the cells treated with TGF-β2. Furthermore, when exposed to fasudil, the phosphorylation of Rhoassociated protein kinase(Rock) and myosin light chain(MLC) could not be activated in the cell preparations.CONCLUSION: Fasudil suppresses the proliferation and migration of SRA01/04 cells, and inhibits the process of EMT induced by TGF-β2. These results suggest that fasudil may serve as a therapeutic agent for PCO. 展开更多

We investigate the non-reciprocal transmission properties of a three-layer structure filled with magneto–optical medium and normal medium. Based on the transfer matrix method, we deduce the total transmission coefficient for a one-dimensional(1 D) structure with anisotropic mediums. When two-side layers with magneto–optical medium loaded in opposite external magnetic field, the time-reversal symmetry of transmission properties will be broken. Our numerical results show that the non-reciprocal transmission properties are influenced by external magnetic fields, incident angle, and thickness of the normal medium layer. Since the non-reciprocal properties can be easily realized and adjusted by the simple structure, such a design has potential applications in integrated circulators and isolators. 展开更多

Neutrophil-to-Lymphocyte Ratio as a Novel Biomarker in Ischemic Stroke: A Literature Review

Background: Ischemic stroke accounts for approximately 80% of all cases of stroke and has the highest rates of morbidity,disability,mortality,and recurrence. The neutrophil-to-lymphocyte ratio (NLR) is a novel,feasible marker representing an integrated response to inflammation. The purpose of the current review is to evaluate all available evidence from clinical observational trials about the relationships between NLR and ischemic stroke. Methods: We searched PubMed,Embase,and CNKI for clinical observational trials on NLR published up to August 1,2018,and made a brief summary about the predictive value of the correlation between NLR and ischemic stroke. Results: A high NLR is associated with increased risk of developing stroke and is a new addition to the traditional risk factors of atrial fibrillation and carotid artery stenosis. NLR on admission is an independent predictor of short- and long-term mortality in patients with acute ischemic stroke. In patients receiving recombinant tissue plasminogen activator or endovascular treatments,high NLR was reported with higher mortality or major disability. Conclusion: NLR provides an easy,useful method to predict the incidence,severity,and prognosis of ischemic stroke;thus,it can emerge as a new potential target for anti-inflammatory treatment and development of therapeutic strategies. 展开更多
