体检人群代谢综合征及其不同组分高尿酸血症流行特征研究

目的:探讨体检人群中代谢综合征(metabolic syndrome,MS)及其不同组分高尿酸血症(hyperuricemia,HUA)流行特征。方法:纳入2021年6月~2023年3月在某医院健康管理中心的体检者作为研究对象。分析MS及其不同组分的HUA患病率,进一步将研究... 目的:探讨体检人群中代谢综合征(metabolic syndrome,MS)及其不同组分高尿酸血症(hyperuricemia,HUA)流行特征。方法:纳入2021年6月~2023年3月在某医院健康管理中心的体检者作为研究对象。分析MS及其不同组分的HUA患病率,进一步将研究对象按性别分层,评估不同MS组分数量人群的血清尿酸水平(serum uric acid,SUA)和HUA患病率及两性中具有最高HUA患病率的MS组分组合方式;logistic回归分析不同MS组分数量人群HUA的患病风险。结果:共有66520人纳入本次研究,MS组SUA水平(t=−82.947,P<0.001)和HUA患病率(χ2=3421.632,P<0.001)显著高于非MS组。腹型肥胖、高血压、高密度脂蛋白胆固醇(HDL-C)降低和甘油三酯(TG)增高者的SUA水平和HUA患病率显著高于正常者(P<0.001),而糖尿病患者SUA水平和HUA患病率存在性别差异,男性糖尿病患者SUA水平和HUA患病率显著低于血糖正常者,女性中这一结果相反。随MS组分数量增加,女性SUA水平和HUA患病率逐渐上升,而男性中,仅在MS1~MS4中观察到这样的趋势。男性HUA患病率最高的MS组分组合为腹型肥胖+高血压+HDL-C降低+TG增高(54.35%),女性为腹型肥胖+高血压+糖尿病+HDL-C降低+TG增高(41.46%)。Logistic回归分析显示:校正性别、年龄和民族后,女性HUA的患病风险随着MS组分数量的增加而增加,而在男性中,MS1~MS4的HUA患病风险不断上升。进一步校正体重指数、总胆固醇增高、低密度脂蛋白增高、冠心病后,结果仍然一致。结论:体检人群中MS及其各组分是HUA的危险因素,MS组分的不同组合与HUA的相关性不同,HUA的患病风险与MS异常组分数量具有相关性。 展开更多

Objective:To explore the epidemiological characteristics of hyperuricemia(HUA)in the metabolic syndrome(MS)and its different components in the physical examination population.Methods:Subjects who underwent medical check-ups at a hospital health management center from June 2021 to March 2023 were included in the study.To analyze the prevalence of HUA in MS and its different components,further,stratify the study population by gender and assess the serum uric acid(SUA)levels and prevalence of HUA in people with different numbers of MS components and the combination of MS components with the highest prevalence of HUA in both sexes.Logistic regression was used to analyze the risk of HUA in people with different numbers of MS components.Result:A total of 66,520 individuals were enrolled in the study.SUA levels(t=-82.947,P<0.001)and HUA prevalence(χ^(2)=3421.632,P<0.001)were significantly higher in the MS group than in the Non-MS group.SUA levels and prevalence of HUA were significantly higher in abdominal obesity,hypertension,decreased HDL-C and evaluated TG than in normal subjects(P<0.001),while there were gender differences in SUA levels and HUA prevalence in diabetic patients,with significantly lower SUA levels and HUA prevalence in men with diabetes than in those with normal blood glucose,an opposite result in women.SUA levels and HUA prevalence gradually increased with the increasing number of MS components in women,whereas in men,such a trend was only observed in MS1-MS4.The combination of MS components with the highest prevalence of HUA was abdominal obesity+hypertension+decreased HDL-C+evaluated TG(54.35%)in men and abdominal obesity+hypertension+diabetes+decreased HDL-C+evaluated TG(41.46%)in women.Logistic regression analysis showed that after adjusting for gender,age and ethnicity,the risk of HUA increased with the number of MS components in women,while in men,the risk of HUA increased continuously from MS1-MS4.Further adjustment for BMI,elevated TC,elevated LDL-C,and coronary artery disease,the results remained consistent.Conclusion:MS and its components are risk factors for HUA in the physical examination population,with different combinations of MS components having different correlations with HUA,and the risk of developing HUA correlates with the number of abnormal MS components. 展开更多

目的:研究刺槐素(Acacetin)对鞭毛蛋白诱导的小鼠骨髓巨噬细胞Nod样受体家族蛋白4(the nod-like receptor family card domaincontaining protein 4,NLRC4)活化的影响。方法:将小鼠骨髓巨噬细胞分为对照组、脂多糖(LPS)组、LPS+鞭毛蛋... 目的:研究刺槐素(Acacetin)对鞭毛蛋白诱导的小鼠骨髓巨噬细胞Nod样受体家族蛋白4(the nod-like receptor family card domaincontaining protein 4,NLRC4)活化的影响。方法:将小鼠骨髓巨噬细胞分为对照组、脂多糖(LPS)组、LPS+鞭毛蛋白组及LPS+Acacetin+鞭毛蛋白组,对照组加入完全培养基,LPS组用LPS(50 ng/mL)预处理3 h,LPS+鞭毛蛋白组用LPS(50 ng/mL)预处理3 h后加入鞭毛蛋白(10μmol/L)刺激0.5 h;LPS+刺槐素+鞭毛蛋白组用LPS(50 ng/mL)预处理3 h后再加入Acacetin(10μmol/L)处理0.5 h,最后加入鞭毛蛋白(10μmol/L)刺激0.5 h;采用WB检测细胞裂解液Pro-caspase-1和Pro-IL-1β的表达,上清液中caspase-1和IL-1β的蛋白表达;ELISA法检测上清液中IL-1β、IL-18和TNF-α的含量,LDH检测试剂盒检测细胞上清液中LDH的活性。结果:与对照组相比,LPS+鞭毛蛋白组细胞上清液中caspase-1、IL-1β蛋白表达明显增高,差异具有统计学意义(P<0.05),细胞裂解液中Pro-caspase-1、Pro-IL-1β蛋白表达比较,差异无统计学意义(P>0.05);与LPS+鞭毛蛋白组比较,LPS+Acacetin+鞭毛蛋白组细胞上清液caspase-1、IL-1β蛋白表达明显降低,差异具有统计学意义(P<0.05),细胞裂解液中Pro-caspase-1、Pro-IL-1β蛋白表达比较,差异无统计学意义(P>0.05)。ELISA结果显示,与对照组相比,LPS+鞭毛蛋白组细胞上清液中IL-1β、IL-18、TNF-α水平及LDH活性明显升高,差异均具有统计学意义(P<0.05);与LPS+鞭毛蛋白组比较,LPS+Acacetin+鞭毛蛋白组细胞上清液IL-1β释放水平明显降低,差异均具有统计学意义(P<0.05),IL-18、TNF-α的水平及LDH的活性降低,但差异无统计学意义(P>0.05)。结论:刺槐素可有效抑制鞭毛蛋白诱导的小鼠骨髓巨噬细胞NLRC4炎症小体的活化,减少细胞损伤。 展开更多

Objective:To investigate the effect of acacetin on flagellin induced NLRC4 inflammasome activation in mouse bone marrow-derived macrophages(BMDMs).Methods:Mouse BMDMs were divided into control group,LPS group,LPS+flagellin group and LPS+acacetin+flagellin group.All groups were added with complete medium,then primed with LPS(50 ng/mL)for 3 hrs except the control group,whereafter,LPS+flagellin group was treated with flagellin(10μmol/L)for 0.5 hr and LPS+acacetin+flagellin group was treated with acacetin(10μmol/L)for 0.5 hr following by flagellin(10μmol/L)for 0.5 hr.Pro-caspase-1,pro-IL-1βin cell lysate and caspase-1,IL-1βin supernatant were detected by Western blot(WB).IL-1β,IL-18 and TNF-αin supernatant were measured by enzyme-linked immunosorbent assay(ELISA).And the activity of LDH in supernatant was assessed by LDH test kit.Results:Compared with the control group,in LPS+flagellin group,the expression of caspase-1,IL-1βprotein in supernatant were significantly increased(all P-values<0.05),but the differences of the expression of pro-caspase-1 and pro-IL-1βprotein in cell lysate were not significant.Compared with LPS+flagellin group,in LPS+acacetin+flagellin group,the expression of caspase-1,IL-1βprotein in supernatant were significantly reduced(all P-values<0.05),while the differences of the expression of pro-caspase-1 and pro-IL-1βprotein in cell lysate were not significant.ELISA showed that compared with the control group,the levels of IL-1β,IL-18,and TNF-αand the activity of LDH in supernatant of LPS+flagellin group were significantly increased(all P-values<0.05).Compared with LPS+flagellin group,in LPS+Acacetin+flagellin group,the level of IL-1βin supernatant was significantly decreased(P<0.05),meanwhile,the decreases of the levels IL-18,TNF-αand the activity of LDH were not significant.Conclusions:We found that Acacetin can effectively inhibit flagellin induced NLRC4 inflammasome activation and reduce cell damage in mouse BMDMs. 展开更多
