Timing of surgery after neoadjuvant chemotherapy for gastric cancer: Impact on outcomes 被引量:13

AIM To evaluate whether the neoadjuvant chemotherapy(NAc T)-surgery interval time significantly impacts the pathological complete response(pc R) rate and longterm survival.METHODS One hundred and seventy-six patients with gastric cancer undergoing NAc T and a planned gastrectomy at the chinese p LA General hospital were selected from January 2011 to January 2017. Univariate and multivariable analyses were used to investigate the impact of NAc T-surgery interval time(< 4 wk, 4-6 wk, and > 6 wk) on pc R rate and overall survival(OS).RESULTS The NAc T-surgery interval time and clinician T stage were independent predictors of pc R. The interval time > 6 wk was associated with a 74% higher odds of pc R as compared with an interval time of 4-6 wk(p = 0.044), while the odds ratio(OR) of clinical T3 vs clinical T4 stage for pc R was 2.90(95%c I: 1.04-8.01, p = 0.041). In cox regression analysis of long-term survival, postneoadjuvant therapy pathological N(yp N) stage significantly impacted OS(N0 vs N3: h R = 0.16, 95%c I: 0.37-0.70, p = 0.015; N1 vs N3: h R = 0.14, 95%c I: 0.02-0.81, p = 0.029) and disease-free survival(DFS)(N0 vs N3: h R = 0.11, 95%c I: 0.24-0.52, p = 0.005; N1 vs N3: h R = 0.17, 95%c I: 0.02-0.71, p = 0.020). The surgical procedure also had a positive impact on OS and DFS. The hazard ratio of distal gastrectomy vs total gastrectomy was 0.12(95%c I: 0.33-0.42, p = 0.001) for OS, and 0.13(95%c I: 0.36-0.44, p = 0.001) for DFS.CONCLUSION The NAc T-surgery interval time is associated with pc R but has no impact on survival, and an interval time > 6 wk has a relatively high odds of pc R. 展开更多

AIM: To elucidate the role of Rab23 in hepatocellular carcinoma (HCC) by assessing the expression of Rab23 in HCC tissue and in HCC cell lines. METHODS: Primary tumors (n = 100) were stained with Rab23 antibodies using immunohistochemistry and in situ hybridization in tissue microarrays. Relationships between gene expression and pathology parameters were analysed. The biological significance of Rab23 in Hep-3B cells was examined by knocking down Rab23 gene expression. We designed a pair of double- stranded RNAs against human rab23 and transfected siRNA into Hep-3B cells. Rab23 expression in these cells was examined using RT-PCR and Western blots. We investigated cell growth by MTT assays and fluorescence- activated cell sorting. RESULTS: High cytoplasmic and nuclear expression of Rab23 was found in 38 of 71 (53.5%) and in 49 of 68 HCC patients (72%) respectively, which correlated with tumor size. HCC cell lines expressed Rab23. In Hep3B cells, siRNA for Rab23 decreased Rab23 mRNA by 4.5-fold and protein expression by 2-fold. Survival rates at 24 and 48 h for Hep-3B cells transfected with siRNA were lower and about 30% Hep-3B cells were apoptotic. Knocking down rab23 suppressed Hep3B cell growth, suggesting that rab23 could play an important role in Hep3B cell growth. CONCLUSION: Rab23 is overexpressed and/or activatedin HCC. Rab23 may be both a HCC predictor and a target for treating HCC. 展开更多

AIM: To examine the effect of the potential interaction between KIF1 B variants(rs17401966 and rs3748578) and environmental factors on the risk of hepatocellular carcinoma(HCC) in a high-risk region in China.METHODS: Three hundred and six patients with HCC and 306 hospital-based control participants residing in the Shunde region of Guangdong Province, China were enrolled. Clinical characteristics were collected by reviewing the complete medical histories from the patient archives, and epidemiological data were collected using a questionnaire and clinical examination. Two single nucleotide polymorphisms(SNPs) of KIF1B(rs17401966 and rs3748578) were chosen for the current study. All subjects were genotypedusing a Taq Man real-time polymerase chain reaction. Multiplicative and additive logistic regression models were used to evaluate various gene-environment interactions.RESULTS: Smoking, frequent consumption of raw freshwater fish, hepatitis B virus(HBV) infection, and a family history of HCC were important risk factors for HCC in this population. Chronic infection with HBV was the most important environmental risk factor for HCC [odds ratio(OR) = 12.02; 95% confidence interval(95%CI): 6.02-24.00]. No significant association was found between the KIF1 B variants alone and the risk of HCC. Nevertheless, a significant additive effect modification was observed between rs17401966 and alcohol consumption(P for additive interaction = 0.0382). Compared with non-drinkers carrying either the AG or GG genotype of rs17401966, individuals classified as alcohol consumers with the AA genotype of rs17401966 had a significantly increased risk of HCC(OR = 2.36; 95%CI: 1.49-3.74).CONCLUSION: The gene-environment interaction between the KIF1 B rs17401966 variant and alcohol consumption may contribute to the development of HCC in Chinese individuals. 展开更多

METHODS: Human hepatocellular carcinoma cells Huh7 were used. Cell viability was analyzed by 3-(4,5-Dimethylthiazol-2-yl)-2,5-diphenyl tetrazolium bromide (MTT) assay. The expressions of Gli-1 and Bcl-2 family members were detected by RT-PCR and Western blot. Apoptosis was detected by Flow cytometry using propidium iodide,measured by Hoechst 33258 staining using Advanced Fluorescence Microscopy and caspase-3 enzymatic assay. Cell growth was analyzed after treatment with Gli-1 siRNA and 5-? uorouracil (5-Fu). RESULTS: Inhibition of Gli-1 mRNA in Huh7 cells through Gli-1 siRNA reduced cell viability. Gli-1 siRNA treatment also induced apoptosis by three criteria,increase in the sub-G1 cell cycle fraction,nuclear condensation,a morphologic change typical of apoptosis,and activation of caspase-3. Gli-1 siRNA was also able to down-regulate Bcl-2. However,Gli-1 siRNA resulted in no significant changes in Bcl-xl,Bax,Bad,and Bid. Furthermore,Gli-1 siRNA increased the cytotoxic effect of 5-Fu on Huh7 cell. CONCLUSION: Down-regulation of Bcl-2 plays an important role in apoptosis induced by Gli-1 siRNA in HCC cells. Combination Gli-1 siRNA with chemotherapeutic drug could represent a more promising strategy against HCC. The effects of the strategies need further investigation in vivo and may have potential clinical application. 展开更多

Serum HER2 as a predictive biomarker for tissue HER2 status and prognosis in patients with gastric cancer 被引量:4

AIM To investigate the association between serum human epidermal growth factor receptor 2(HER2) extracellular domain(ECD) and tissue HER2 status, and the prognostic value of serum HER2 ECD in patients with gastric cancer.METHODS A total of 239 patients with gastric cancer were enrolled from December 2012 to June 2013. Serum HER2 ECD was determined by chemiluminescent assay, and tissue HER2 status was evaluated by immunohistochemistry and fluorescence in situ hybridization assay. A receiver operating characteristic(ROC) curve was plotted to identify the optimal cut-off value for serum HER2 ECD assay for predicting survival in gastric cancer patients.RESULTS Serum HER2 ECD was significantly correlated with tissue HER2 status(P < 0.001), tumor size(P < 0.001), and intestinal type of gastric cancer(P =0.021). Serum HER2 ECD levels differed significantly between patients with HER2-positive tissue expression and those with HER2-negative tissue expression. ROC analysis yielded an area under the curve value of 0.79(95%CI: 0.71-0.87, P < 0.001), with a sensitivity and specificity of 0.54(95%CI: 0.37-0.70) and 0.93(95%CI: 0.88-0.96), respectively. With a cut-off value of 24.75 ng/m L, high serum HER2 ECD had a negative impact on overall survival of the patients(HR: 1.93, 95%CI: 1.32-4.38, P = 0.006). CONCLUSION Serum HER2 ECD could be a highly specific surrogate biomarker for tissue HER2 status in gastric cancer. Optimal cut-off criteria for predicting survival should be established. 展开更多

Perforating and nonperforating indications in repeated surgeries for Crohn's disease

BACKGROUND Despite advances in medical therapy for Crohn’s disease(CD),most patients with CD require repeated resection surgeries.AIM To analyze the perforating and nonperforating indications of repeated CD operations and identify the anastomosis characteristics for postoperative CD.METHODS We retrospectively reviewed 386 patients who underwent at least one resection for CD between 2003 and 2013.Clinical characteristics of each surgery were collected.Univariate and multivariate analyses were performed to determine risk factors for recurrence.RESULTS The indication for reoperation in CD tends to be the same as that for primary operation,i.e.,perforating disease tends to represent as perforating disease and nonperforating as nonperforating.Concordance was found between the first surgery and second surgery in terms of the indication for the operation(P=0.006),and the indication for the third surgery was also correlated with that for the second surgery(P=0.033).Even if the correlation of surgical indications between repeated operations,the rate of perforating indication for the second and third surgeries was significantly higher than that of the first surgery.In addition,the presence of perforating CD was a predictor of recurrence for both the first and second surgeries.Moreover,anastomotic lesions were the most common sites of recurrence after the operation.Based on the importance of anastomosis,anastomosis might be a new type of disease location for the classification of postoperative CD.CONCLUSION CD not only has stable characteristics but also progresses chronically.Perforation is a progressive surgical indication for Crohn’s disease.For CD after surgery,anastomosis may be a new classification of disease location. 展开更多

目的:揭示双季杂交稻系统中硅(Si)养分补充对水稻养分利用率与对土壤养分有效性影响的相应关系及其生态机制。创新点:发现一定量的Si养分补充可提高稻田土壤氮(N)和磷(P)养分的有效性,并促进水稻根系对养分的吸收,从而提高水稻产量及肥... 目的:揭示双季杂交稻系统中硅(Si)养分补充对水稻养分利用率与对土壤养分有效性影响的相应关系及其生态机制。创新点:发现一定量的Si养分补充可提高稻田土壤氮(N)和磷(P)养分的有效性,并促进水稻根系对养分的吸收,从而提高水稻产量及肥料利用率。其核心生态机制是补充Si养分可显著增加稻田土壤的微生物总量,从而促进土壤养分同化固定,提高土壤中N和P的矿化以及易矿化的土壤微生物量氮(MBN)和土壤微生物量磷(MBP)的储备。方法:2017和2018连续两年,在双季稻作区设计了系列田间试验,统一常规养分管理,于早稻和晚稻种植期间设置五个有效Si用量梯度处理(即0(对照)、7.8、15.6、23.4和31.2 kg/ha Si),分析收获后的水稻生长性状(株高、籽粒产量、植株生物量等),水稻根、茎、叶和籽粒中N和P的含量,土壤有效态N和P的含量以及土壤微生物量碳(MBC)、MBN和MBP的含量,最后统计分析Si的供应与对水稻N和P养分利用率、土壤养分有效性、MBC、MBN和MBP的影响及其相互关系。结论:Si养分补充可增加双季杂交稻系统土壤微生物总量,促进土壤养分同化固定,提高土壤养分的有效性以及易矿化的有机养分的储备,使得土壤养分易于被水稻根系吸收利用,从而提高水稻肥料利用率,促进水稻的生长发育,提高水稻产量。其中Si施用量为23.4 kg/ha时双季水稻产量及N和P肥利用率均达到最大值,此时土壤有效态氮与总氮比(AN/TN)、有效态磷与总磷比(AP/TP)以及MBN和MBP也均达到最大值。上述结果表明,通过Si养分补充可适当削减双季杂交稻系统因过多化学肥料投入带来的面源污染问题。 展开更多

Closed-form solution to optimal coherent combining is derived. Given coherent combining, the approximate SCP solution is presented. Finally, an alternating iterative structure is developed. Simulation results and analysis show that, given the symbol error rate and data rate, the proposed alternating iterative structure achieves signal-to-noise ratio gains over existing schemes in maximum ratio combining(MRC) plus matched filter,MRC plus antenna selection, and distributed space-time block coding due to the use of SCP and iterative structure. 展开更多
