燥湿苦参汤联合NB-UVB治疗寻常性银屑病及对Th17和Treg及其相关因子表达的影响 被引量:13

目的探讨燥湿苦参汤联合窄谱中波紫外线(NB-UVB)治疗寻常性银屑病的临床疗效及其对Th17、Treg细胞相关的转录、分泌因子的影响。方法选取2014年6月-2015年12月该院收治的寻常型银屑病患者126例。按照就诊顺序随机分为中医对照组、NB-UV... 目的探讨燥湿苦参汤联合窄谱中波紫外线(NB-UVB)治疗寻常性银屑病的临床疗效及其对Th17、Treg细胞相关的转录、分泌因子的影响。方法选取2014年6月-2015年12月该院收治的寻常型银屑病患者126例。按照就诊顺序随机分为中医对照组、NB-UVB对照组和研究组,每组42例。中医对照组内服燥湿苦参汤,NB-UVB对照组进行NB-UVB照射治疗,研究组内服燥湿苦参汤加用NB-UVB局部照射。另选择40例体检正常者为健康对照组。3组均治疗12周,评价其综合疗效,对比治疗前后的外周血Th17细胞比例及其特异性转录因子—孤儿受体转录因子(RORγt mRNA)、分泌因子—白细胞介素17(IL-17),Treg细胞的比例及其特异性转录因子—叉头状转录因子(Foxp3 mRNA)、分泌因子—转化生长因子(TGF-β)的表达水平。结果 (1)治疗12周后,3组的严重程度评分(PASI)均下降,且研究组评分低于中医对照组与NB-UVB对照组,总有效率也高于中医对照组与NB-UVB对照组,差异有统计学意义(P <0.05)。(2)与健康对照组比较,3组治疗前外周血Th17细胞比例及RORγt mRNA与IL-17均升高,而Treg细胞的比例及Foxp3 mRNA与TGF-β降低,差异有统计学意义(P <0.05)。治疗后,3组Th17细胞的相关因子表达均较治疗前下降,Treg细胞相关因子表达较治疗前升高,差异有统计学意义(P <0.05)。结论燥湿苦参汤联合NB-UVB局部照射可抑制Th17细胞及其相关因子的表达,恢复Treg/Th17细胞的动态平衡,增强Treg细胞的免疫抑制功能,从而降低外周血及皮损处的炎症细胞因子,达到改善寻常性银屑病临床症状的理想疗效。 展开更多

The method of nonlinear finite element reliability analysis (FERA) of slope stability using the technique of slip surface stress analysis (SSA) is studied. The limit state function that can consider the direction of slip surface is given, and the formula-tions of FERA based on incremental tangent stiffness method and modified Aitken accelerating algorithm are developed. The limited step length iteration method (LSLIM) is adopted to calculate the reliability index. The nonlinear FERA code using the SSA technique is developed and the main flow chart is illustrated. Numerical examples are used to demonstrate the efficiency and robustness of this method. It is found that the accelerating convergence algorithm proposed in this study proves to be very efficient for it can reduce the iteration number greatly, and LSLIM is also efficient for it can assure the convergence of the iteration of the reliability index. 展开更多

Testicular endothelial cells have been found to play an important role in spermatogenesis and fertility,but their mechanism is obscure.Exosomes released by various cells are recognized as cell–cell communication mediators during the initiation and progression of many diseases.Therefore,the current study aimed to investigate the protein and miRNA components of human testicular endothelial cell-derived exosomes(HTEC-Exos)and to explore their potential effects on spermatogenesis.In this study,HTEC-Exos were first isolated by the ultracentrifugation method,and then identified by nanoparticle tracking analysis,transmission electron microscopy(TEM),and western blotting.The characteristics of HTEC-Exos were examined by liquid chromatography–mass spectrometry and microRNA(miRNA)chip analysis.Bioinformatics analysis was performed to explore the potential role of the exosomal content on spermatogenesis.A total of 945 proteins were identified,11 of which were closely related to spermatogenesis.A total of 2578 miRNAs were identified.Among them,30 miRNAs demonstrated potential associations with male reproductive disorders,such as azoospermia,and spermatogenesis disorders.In particular,11 out of these 30 miRNAs have been proven to be involved in spermatogenesis based on available evidence.This study provides a global view of the proteins and miRNAs from HTEC-Exos,suggesting that HTEC-Exos may function as potential effectors during the process of spermatogenesis. 展开更多

In engineering practice,the stability of a slope is often analyzed as a 2D problem assuming a plane-strain condition,which may lead to significant errors.In this paper,a comprehensive study is carried out to compare the results of 2D and 3D slope stability analyses,using the strength reduction method for deterministic analysis and the random field approach for probabilistic analysis,respectively.The results of this comparison study confirm that in the deterministic stability evaluation,the 2D analysis tends to obtain a smaller factor of safety than does its 3D counterpart.In the probabilistic evaluation that considers the spatial variability of soil properties,the 2D analysis tends to yield a larger probability of failure than its 3D counterpart.A significant feature of the 3D probabilistic slope stability analysis is the presence of multiple local failures distributed along the slope longitudinal direction.This paper provides insights regarding the degree of errors in modeling a 3D slope as a 2D problem,which can be regarded as a complement to the previous 3D slope stability analyses. 展开更多
