Changes in CD4+, CD8+, Th17 and IgG, IgM, and IgA levels. <strong>Methods:</strong> A total of 118 NSCLC patients who attended the Oncology Department of the Affiliated Hospital of Chengde Medical College from September 2018 to September 2021 were selected as the research objects, and the patients were analyzed at different time points (before chemotherapy, after the first chemotherapy, and after the second chemotherapy). The effects of NKG2D, IL-12, IL-15, IL-18, DC cells, TNF-A, IFN-r, CD4+, CD8+ Th17, IgG, IgM and IgA levels in peripheral blood at different time points (before chemotherapy, after the first chemotherapy and after the second chemotherapy) were analyzed. The changes of NKG2D, IL-12, IL-15, IL-18, DC cells, TNF-A, IFN-r and the levels of CD4+, CD8+ Th17, IgG, IgM and IgA in peripheral blood were compared at each time point. <strong>Results:</strong> NKG2D, IL-12, IL-15, IL-18, TNF-a, IFN-r gradually decreased before chemotherapy, one week after chemotherapy, and two weeks after chemotherapy, the difference was statistically significant, but DC cells were not significant Variety. CD4+ and CD8+ both increased significantly, and the levels of Th17, IgG, IgM, and IgA gradually decreased. <strong>Conclusion:</strong> In the course of chemotherapy, all immune factors except DC cells were significantly decreased compared with those before chemotherapy, and the decrease of immune factors except DC cells was positively correlated with the length of chemotherapy cycle. Neutropenia with fever is a special group of patients. Due to low immune function, inflammation-related clinical symptoms and signs are often not obvious, and pathogenic bacteria and infection focus are not clear. Fever may be the only sign of infection. If appropriate antimicrobial treatment is not given in time, infection-related mortality is high. In our study, we aimed to optimize the dosage regimen of Micafungin in children with febrile neutropenic against Candida spp. by Mote Carlo Simulation (MCS). Pharmacokinetic parameters and microbiological data of Micafungin were collected. Then we used MCS to calculate Probability of Target Attainment (PTA) and Cumulative Fraction of Response (CFR). With dosages of 0.5 mg/kg, 1 mg/kg, 1.5 mg/kg, 2 mg/kg, 3 mg/kg, and 4 mg/kg in oral group and dosages of 100 mg, and 200 mg in intravenous administration, all have different degree of antifungal effect. But when the dosage regimen was 50 mg IV, the therapeutic effect of Micafungin against Candida spp. was not good. 展开更多

The blood levels of interleukin-12 (IL-12), interleukin-15 (IL-15), interleukin-18 (IL-18), dendritic cells (DC cells), tumor necrosis factor A (TNF-a) and the levels of immune interferon (IFN-r) and NK cell activating receptor (NKG2D) in blood before chemotherapy, after the first chemotherapy and after the second chemotherapy were analyzed. <strong>Results:</strong> Except for the viability of DC cells and DC cells, all other immune factor groups showed statistical differences. <strong>Conclusion: </strong>Chemotherapy will have a negative effect on all immune factors except DC cells. The effect of immune factors will be weakened according to the increase of the chemotherapy cycle. Methods: Doxorubicin (1, 2, 4, 6, 10 ug/ml) was co-cultured with H9C2 cells for 6, 12 and 24 hours. The morphological changes of cells were observed, and the cell inhibition rates of different time and drug concentration were calculated. Results: Doxorubicin could inhibit the activity of cardiomyocytes in a dose-dependent manner from 1 to 10 ug/ml. Conclusion: A certain dose of doxorubicin has a toxic effect on cardiomyocytes and can cause cardiomyocyte necrosis and apoptosis. 展开更多

Evaluation of Dosages Regimen of Fluconazole in Patients of Candidemia with Gender by PK/PD and Mote Carlo Simulation

Candidemia is one of the four most common nosocomial blood infections and is the most common hospital-acquired fungemia in a recent multi-institutional study from the US. The mortality of Candidemia can be up to 50%. Fluconazole is a triazole derivative widely used for the treatment of invasive candidiasis. It was recommended as first-line drugs for the treatment and prevention of mycoses. In our study, we aimed to optimise the dosage of fluconazole with gender against Candida spp. based on pharmacokinetic/pharmacodynamics (PK/PD) analysis. We collected the published data about pharmacokinetic parameters of fluconazole and the MIC distribution of Candida spp. on fluconazole. We decided to evaluate the gender between males and females with the pharmacokinetics of fluconazole. Using probability of target attainment (PTA) and cumulative fraction of response (CFR) as indexes, crystal ball software 11.1.2.4 was used for Monte Carlo simulation of different dosage regimens of different males and females. For C. albicans, C. tropicalis and C. lusitaniae, when doses of regimen are 100 mg IV, 200 mg IV and MIC was lower than 1 g/ml, PTA was higher than 90%. For C. tropicalis, each dosing regimen and MIC was less than 2 g/ml. PTA was higher than 90%. As C. glabrata, C. parapsilosis, C. krusei, C. guilliermondii for PTA with more than 90%, MIC of fluconazole 200 mg were less than 32 g/ml, 64 g/ml and 64 g/ml, respectively. For the different dosage regimens 100 mg IV and 200 mg IV of fluconazole for Candida spp., it is desirable that fluconazole dosage regimens take into account both the gender of the patient. 展开更多

The difference between each factor of each point was statistically significant (all P < 0.05). Pearson correlation analysis showed that there was no correlation between peripheral blood DC cells of patients at different time points and other factors. <strong>Conclusion:</strong> The decrease of other immune cytokines except DC cells in peripheral blood of patients with NSCLC after chemotherapy may be one of the mechanisms by which the patient’s immune function is suppressed. With research on the carrier phase synchronization and symbol synchronization algorithm of demodulation module, a synchronization circuit system is designed for GPS software receiver based on field programmable gate array (FPGA), and a series of experiment is done on the hardware platform. The result shows the all-digital synchronization and demodulation of GPS intermediate frequency (IF) signal can be realized and applied in embedded real-time GPS software receiver system. It is verified that the decision-directed joint tracking algorithm of carrier phase and symbol timing for received signals from GPS is reasonable. In addition, the loop works steadily and can be used for receiving GPS signals using synchronous demodulation. The synchronization circuit for GPS software receiver designed based on FPGA has the features of low cost, miniaturization, low power and realtime. Surely, it will become one of the development directions for GPS and even GNSS embedded real-time software receiver. 展开更多
