腹腔灌注治疗在晚期胃癌腹膜转移患者中的应用效果 被引量:2

目的:探讨腹腔灌注治疗对胃癌腹膜转移患者生存的影响。方法:回顾性分析2014年1月至2020年12月哈尔滨医科大学附属肿瘤医院收治的353例晚期胃癌合并腹膜转移患者临床资料。根据是否腹腔灌注给药分为灌注治疗组和未灌注治疗组,并对患者... 目的:探讨腹腔灌注治疗对胃癌腹膜转移患者生存的影响。方法:回顾性分析2014年1月至2020年12月哈尔滨医科大学附属肿瘤医院收治的353例晚期胃癌合并腹膜转移患者临床资料。根据是否腹腔灌注给药分为灌注治疗组和未灌注治疗组,并对患者进行生存随访。Kaplan-Meier法绘制生存曲线,Cox风险比例模型分析影响患者预后的因素,根据病灶部位、分化程度等进行亚组分析。结果:灌注治疗组和未灌注治疗组中位总生存(overall survival,OS)期为9.50个月(95%CI:7.39~11.61)和7.83个月(95%CI:5.74~9.92,P=0.022)。体质量指数(BMI)>25及伴有腹水的胃癌腹膜转移患者更易从腹腔灌注中获益。多因素分析显示:腹腔积液(HR=1.52,95%CI:1.18~1.94)、全身系统治疗(HR=0.43,95%CI:0.33~0.56)、手术(HR=0.52,95%CI:0.39~0.70)、腹腔灌注治疗(HR=0.65,95%CI:0.52~0.82)是影响患者生存的因素。结论:胃癌伴有腹膜转移患者可进行腹腔灌注治疗,与未灌注治疗患者相比,其生存期明显延长。Cox多因素分析显示无腹水、接受手术治疗、接受全身系统治疗和腹腔灌注治疗的患者预后更佳。分层分析显示BMI>25及伴有腹水的胃癌腹膜转移患者更易从腹腔灌注治疗中获益。 展开更多

Numerical Study on the Suitability of Passive Solar Heating Technology Based on Differentiated Thermal Comfort Demand

Indoor thermal comfort and passive solar heating technologies have been extensively studied.However,few studies have explored the suitability of passive solar heating technologies based on differentiated thermal comfort demands.This work took the rural dwellings in Northwest China as the research object.First,the current indoor and outdoor thermal environment in winter and the mechanism of residents’differentiated demand for indoor thermal comfort were obtained through tests,questionnaires,and statistical analysis.Second,a comprehensive passive optimized design of existing buildings was conducted,and the validity of the optimized combination scheme was explored using DesignBuilder software.Finally,the suitability of passive solar heating technology for each region in Northwest China was analyzed based on residents’differentiated demand for indoor thermal comfort.The regions were then classified according to the suitability of the technology for these.The results showed that the indoor heating energy consumption was high and the indoor thermal environment was not ideal,yet the solar energy resources were abundant.Indoor comfort temperature indexes that match the functional rooms and usage periods were proposed.For the buildings with the optimized combination scheme,the average indoor temperature was increased significantly and the temperature fluctuation was decreased dramatically.Most regions in Northwest China were suitable for the development of passive solar heating technology.Based on the obtained suitability of the technology for the regions of Northwest China,these were classified into most suitable,more suitable,less suitable,and unsuitable regions. 展开更多

A new cytotoxic salannin-class limonoid alkaloid, azadiramide A(1),was isolated from seeds of Azadirachta indica A. Juss. Its structure was elucidated by extensive analysis of spectroscopic data and quantum chemical calculation. Compound 1 is a rare salannin-class limonoid alkaloid. To our best knowledge, only two compounds belonging to this type were so far found. It showed inhibitory activity against human breast cancer MDA-MB-231 cell line with IC_(50) value of 2.70±0.63 μmol/L. 展开更多

A photoinduced reaction of potassium alkyltrifluoroborates,sulfur dioxide,and para-quinone methides under visible light irradiation at room temperature is developed,giving rise to diarylmethyl alkylsulfones in moderate to good yields.This reaction works well under photocatalysis with a broad substrate scope by using DABCO·(SO_(2))_(2)as the source of sulfur dioxide.Mechanistic study shows that this transformation is initiated by alkyl radicals generated in situ from potassium alkyltrifluoroborates in the presence of photocatalyst.The subsequent insertion of sulfur dioxide and radical 1,6-addition of para-quinone methides with alkylsulfonyl radical intermediates afford the corresponding diarylmethyl alkylsulfones. 展开更多

Four new limonoid-type nortriterpenoids, 1-detigloyl-1-O-methacryloylsalannin(1), 28-deoxo-2,3-dihydronimbolide(2), 12-acetoxy-3-O-acetyl-7-O-tigloylvilasinin(3) and 12-acetoxy-3-Oacetyl-7-O-methacryloylvilasinin(4), ... Four new limonoid-type nortriterpenoids, 1-detigloyl-1-O-methacryloylsalannin(1), 28-deoxo-2,3-dihydronimbolide(2), 12-acetoxy-3-O-acetyl-7-O-tigloylvilasinin(3) and 12-acetoxy-3-Oacetyl-7-O-methacryloylvilasinin(4), along with five known ones, were isolated from seeds of Azadirachta indica A. Juss. Their structures were elucidated by various spectroscopic methods, including UV, IR, MS, NMR, X-ray crystallography, quantum chemical calculation, as well as by comparison of their spectroscopic data with those reported. In the in vitro cytotoxic assay, 2 showed inhibitory activity against human breast cancer MDA-MB-231 cell line with IC_(50) value of 7.68±1.74 μmol/L, and 5 inhibited growth of human cervical cancer Hela cell line, melanoma A375 cell line and promyelocytic leukemia HL-60 cell line, with IC_(50) 12.00±2.08, 17.44±2.11, and 13.95±5.74 μmol/L, respectively. 展开更多
