Background:The Global Leadership Initiative on Malnutrition(GLIM)criteria were published to build a global consensus on nutritional diagnosis.Reduced muscle mass is a phenotypic criterion with strong evidence to suppo...Background:The Global Leadership Initiative on Malnutrition(GLIM)criteria were published to build a global consensus on nutritional diagnosis.Reduced muscle mass is a phenotypic criterion with strong evidence to support its inclusion in the GLIM consensus criteria.However,there is no consensus regarding how to accurately measure and define reduced muscle mass in clinical settings.This study aimed to investigate the optimal reference values of skeletal muscle mass index for diagnosing sarcopenia and GLIM-defined malnutrition,as well as the prevalence of GLIM-defined malnutrition in hospitalized cirrhotic patients.Methods:This retrospective study was conducted on 1002 adult patients with liver cirrhosis between January 1,2018,and February 28,2022,at Beijing You-An Hospital,Capital Medical University.Adult patients with a clinical diagnosis of liver cirrhosis and who underwent an abdominal computed tomography(CT)examination during hospitalization were included in the study.These patients were randomly divided into a modeling group(cohort 1,667 patients)and a validation group(cohort 2,335 patients).In cohort 1,optimal cut-off values of skeletal muscle index at the third lumbar skeletal muscle index(L3-SMI)were determined using receiver operating characteristic analyses against in-hospital mortality in different gender groups.Next,patients in cohort 2 were screened for nutritional risk using the Nutritional Risk Screening 2002(NRS-2002),and malnutrition was diagnosed by GLIM criteria.Additionally,the reference values of reduced muscle mass in GLIM criteria were derived from the L3-SMI values from cohort 1.Multivariate logistic regression analysis was used to analyze the association between GLIM-defined malnutrition and clinical outcomes.Results:The optimal cut-off values of L3-SMI were 39.50 cm 2/m 2 for male patients and 33.06 cm 2/m 2 for female patients.Based on the cut-off values,31.63%(68/215)of the male patients and 23.3%(28/120)of the female patients had CT-determined sarcopenia in cohort 2.The prevalence of GLIM-defined malnutrition in cirrhotic patients was 34.3%(115/335)and GLIM-defined malnutrition was an independent risk factor for in-hospital mortality in patients with liver cirrhosis(Wald=6.347,P=0.012).Conclusions:This study provided reference values for skeletal muscle mass index and the prevalence of GLIM-defined malnutrition in hospitalized patients with liver cirrhosis.These reference values will contribute to applying the GLIM criteria in cirrhotic patients.展开更多
Background:Hyperperfusion syndrome(HPS)is a rare but potentially a life-threatening complication after carotid artery angioplasty and stenting(CAS).Staged CAS has been an alternative to prevent HPS.Materials and metho...Background:Hyperperfusion syndrome(HPS)is a rare but potentially a life-threatening complication after carotid artery angioplasty and stenting(CAS).Staged CAS has been an alternative to prevent HPS.Materials and methods:44 of 908 patients with high-grade internal carotid artery stenosis or near occlusion were at risk of HPS because of poor collateral flow and impaired cerebral blood flow(CBF).They were treated with first(stage 1),followed by a full CAS(stage 2)1 month later.Their 30-day outcomes were tabulated and analysed.Results:During follow-up,1 of the 44(2.2%)patients developed HPS immediately,3(7%)had postprocedural HPS(ie,transcranial Doppler(TCD)>120%)without clinical symptoms and 3(7%)required stenting at stage 1 for carotid dissections.After stage 1,there were significant improvement between the preprocedural and postprocedural CBF(0.98±0.06 vs 0.85±0.05,p<0.05),mean transit time(MTT;1.05±0.05 vs 1.15±0.05,p<0.05),time to peak(TTP;1.04±0.06 vs 1.20±0.06,p<0.05)on CT perfusion(CTP),and CBF(66.41±7.41 vs 44.44±6.43,p<0.05)on TCD.After stage 2,improvement was seen in CBF(1.01±0.07 vs 0.98±0.06,p<0.05),MTT(1.01±0.05 vs 1.05±0.05,p<0.05),TTP(0.99±0.06 vs 1.04±0.06,p<0.05)on CTP and CBF(66.41±7.41 vs 93.78±18.81,p<0.05)on TCD.2 had postoperative increase of middle cerebral artery mean flow velocity of 120%after stage 2 without clinical symptoms.Conclusion:Staged carotid artery stenting probably decreased the chance of developing HPS in this group of selected patients.Although requiring a 2-step intervention,staged CAS may be a safe and effective alternative.展开更多

Many tools focus on two-dimensional space to display pathways in metabolic network. However, the usage of three-dimensional visualization may help to understand better the intricate topology of metabolic and regulatory networks. Methods: We manually curated large amount of experimental data (including pathways, reactions and metabolites) from literature related with different types of engineered E. coli and then utilized a novel technology of three dimensional visualization to develop a comprehensive metabolic network named SynBioEcoll. Results: SynBioEoli contains 740 biosynthetic pathways, 3,889 metabolic reactions, 2,255 chemical compounds manually curated from about 11,000 metabolism publications related with different types of engineered E. coil Furthermore, SynBioEcoli integrates with various informatics techniques. Conclusions: SynBioEcoli could be regarded as a comprehensive knowledgebase of engineered E. coli and represents the next generation cellular metabolism network visualization technology. Meiosis is essential for fertility in sexually reproducing species and this sophisticated process has been extensively studied.Notwithstanding these efforts,key factors involved in meiosis have not been fully characte...Meiosis is essential for fertility in sexually reproducing species and this sophisticated process has been extensively studied.Notwithstanding these efforts,key factors involved in meiosis have not been fully characterized.In this study,we investigate the regulatory roles of zinc finger protein 541(ZFP541)and its interacting protein potassium channel tetramerization domain containing 19(KCTD19)in spermatogenesis.ZFP541 is expressed from leptotene to the round spermatid stage,while the expression of KCTD19 is initiated in pachytene.Depletion of Zfp541 or Kctd19 leads to infertility in male mice and delays progression from early to mid/late pachynema.In addition,Zfp541^(-/-)spermatocytes show abnormal programmed DNA double-strand break repair,impaired crossover formation and resolution,and asynapsis of the XY chromosomes.ZFP541 interacts with KCTD19,histone deacetylase 1/2(HDAC1/2),and deoxynucleotidyl transferase terminal-interacting protein 1(DNTTIP1).Moreover,ZFP541 binds to and activates the expression of genes involved in meiosis and post-meiosis including Kctd19;in turn,KCTD19 promotes the transcriptional activation activity of ZFP541.Taken together,our studies reveal that the ZFP541/KCTD19 signaling complex,acting as a key transcription regulator,plays an indispensable role in male fertility by regulating pachytene progression.展开更多
Multispeed transmissions can enhance the dynamics and economic performance of electric vehicles(EVs),but the coordinated control of the drive motor and gear shift mechanism during gear shifting is still a difficult ch...Multispeed transmissions can enhance the dynamics and economic performance of electric vehicles(EVs),but the coordinated control of the drive motor and gear shift mechanism during gear shifting is still a difficult challenge because gear shifting may cause discomfort to the occupants.To improve the swiftness of gear shifting,this paper proposes a coordinated shift control method based on the dynamic tooth alignment(DTA)algorithm for nonsynchronizer automated mechanical transmissions(NSAMTs)of EVs.After the speed difference between the sleeve(SL)and target dog gear is reduced to a certain value by speed synchronization,angle synchronization is adopted to synchronize the SL quickly to the target tooth slofs angular position predicted by the DTA.A two-speed planetary NS AMT is taken as an example to carry out comparative simulations and bench experiments.Results show that gear shifting duration and maximum jerk are reduced under the shift control with the proposed method,which proves the effectiveness of the proposed coordinated shift control method with DTA.展开更多
