目的:观察强脉冲光(IPL)与红蓝光分别联合果酸治疗寻常性痤疮的临床疗效,并进行对比分析。方法:选择2016年6月-2016年8月笔者科室就诊的40例面部寻常性痤疮患者,随机分为两组。常规组:20例,给予红蓝光联合果酸治疗;研究组:20例,给予强... 目的:观察强脉冲光(IPL)与红蓝光分别联合果酸治疗寻常性痤疮的临床疗效,并进行对比分析。方法:选择2016年6月-2016年8月笔者科室就诊的40例面部寻常性痤疮患者,随机分为两组。常规组:20例,给予红蓝光联合果酸治疗;研究组:20例,给予强脉冲光联合果酸治疗。对比两组治疗后皮损减退、色素沉着、并发症及血清中IL-8、IL-17水平情况。结果:治疗2周后,研究组皮损减退总有效率35.00%,显著高于常规组5.00%,差异有统计学意义(χ~2=3.906,P=0.048);治疗6周后,研究组皮损减退总有效率55.00%,显著高于常规组20.00%,差异有统计学意义(χ~2=5.227,P=0.022);治疗8周后,研究组皮损减退总有效率85.00%,明显高于常规组45.00%,差异有统计学意义(χ~2=7.033,P=0.008)。治疗2周后,研究组色素沉着总有效率40.00%,显著高于常规组10.00%,差异有统计学意义(χ~2=4.800,P=0.029);治疗6周后,研究组色素沉着总有效率60.00%,显著高于常规组25.00%,差异有统计学意义(χ~2=5.013,P=0.025);治疗8周后,研究组色素沉着总有效率85.00%,显著高于常规组50.00%,差异有统计学意义(χ~2=5.584,P=0.018)。治疗8周后,研究组IL-8水平(8.76±1.26)显著低于常规组(10.96±1.62),差异有统计学意义(t=4.794,P<0.05);研究组IL-17水平(13.69±2.63)显著低于常规组(17.62±3.21),差异有统计学意义(t=4.235,P<0.05);研究组总并发症发生率为10.00%,常规组总并发症发生率为20.00%,研究组并发症发生率低于常规组,但两组患者并发症发生率无显著性差异(χ~2=0.7843,P=0.3758)。结论:寻常性痤疮患者给予强脉冲光联合果酸治疗,其疗效较好,不良反应较少。 展开更多

报告泛发性肥厚性扁平苔藓1例。患者男,76岁,全身泛发紫红色丘疹、斑块伴瘙痒3月余。皮肤科检查:躯干、四肢泛发紫红色增殖性肥厚性斑块,部分呈疣状外观,面部、头皮、手掌弥漫性紫红色斑疹,口腔颊黏膜可见Wickham纹,龟头可见一暗紫红色... 报告泛发性肥厚性扁平苔藓1例。患者男,76岁,全身泛发紫红色丘疹、斑块伴瘙痒3月余。皮肤科检查:躯干、四肢泛发紫红色增殖性肥厚性斑块,部分呈疣状外观,面部、头皮、手掌弥漫性紫红色斑疹,口腔颊黏膜可见Wickham纹,龟头可见一暗紫红色斑,双足底皮肤增厚、皲裂。右大腿皮损组织病理符合扁平苔藓。诊断:泛发性肥厚性扁平苔藓。患者既往规律抗结核治疗7个月余,且免疫指标异常,考虑皮疹可能与抗结核药物及免疫异常有关,调整药物治疗后皮疹好转。 展开更多

Background Diffusion tensor imaging (DTI) is a noninvasive imaging technique for the assessment of the integrity of cerebral tissues. This study was undertaken to assess the changes of diffusion indices of hippocampal formation (HF) in patients with medial temporal lobe epilepsy (MTLE). Methods Fourteen patients with MTLE and 14 healthy subjects were evaluated. Mean diffusivity (MD) and fractional anisotropy (FA) from the symmetrical-voxel sampling regions of the anterior HF were calculated in all subjects. The MD and FA values were compared across the groups. Results No significant differences of MD and FA values were noted between right and left HF in the controls. In the patient group, MD significantly increased in the HF ipsilateral to the lesioned side [(9.27±1.09)×10^-4 mm^2/s], compared with the values in the contralateral HF [(8.20±0.59)×10^-4 min^2]s] (t = 4.479, P = 0.001) and healthy subjects [(7.58±0.51)×10^-4 mm^2/s] (P 〈 0.001), but no significant differences were found in FA. When compared with the controls, patients had a significantly higher MD in the contralateral HF (P 〈 0.05), but the difference in FA was not statistically significant. Conclusions DTI could detect hippocampal abnormality in patients with MTLE. This technique may be helpful for preoperative evaluation of such patients. 展开更多

Objective To review the applications of magnetic resonance imaging (MRI) techniques in assessing treatment response to gamma knife radiosurgery for brain tumors. Data sources Published articles about assessing treatment response to gamma knife radiosurgery for brain tumors were selected using PubMed. The search terms were "MRr', "gamma knife" and "brain tumors". Study selection Articles regarding the MRI techniques using for early assessment of treatment response of gamma knife were selected. Results MRI techniques, especially diffusion weighted imaging, perfusion weighted imaging, magnetic resonance spectroscopy, are useful for early assessment of treatment response of gamma knife by detecting the hemodynamic, metabolic, and cellular alterations. Moreover, they can also provide important information on prognosis. Conclusions Diffusion weighted imaging, perfusion weighted imaging and magnetic resonance spectroscopy can provide early assessment of treatment response of gamma knife for brain tumors, and also information of tumor progression or recurrence earlier than conventional MRI. But there are still many questions to be answered which should be based on the development and advancement of MRI and related disciplines. 展开更多
