Association between operative position and postoperative nausea and vomiting in patients undergoing laparoscopic sleeve gastrectomy

BACKGROUND Bariatric surgery is one of the most effective ways to treat morbid obesity,and postoperative nausea and vomiting(PONV)is one of the common complications after bariatric surgery.At present,the mechanism of the high incidence of PONV after weight-loss surgery has not been clearly explained,and this study aims to investigate the effect of surgical position on PONV in patients undergoing bariatric surgery.AIM To explore the effect of the operative position during bariatric surgery on PONV.METHODS Data from obese patients,who underwent laparoscopic sleeve gastrectomy(LSG)in the authors’hospital between June 2020 and February 2022 were divided into 2 groups and retrospectively analyzed.Multivariable logistic regression analysis and the t-test were used to study the influence of operative position on PONV.RESULTS There were 15 cases of PONV in the supine split-leg group(incidence rate,50%)and 11 in the supine group(incidence rate,36.7%)(P=0.297).The mean operative duration in the supine split-leg group was 168.23±46.24 minutes and 140.60±32.256 minutes in the supine group(P<0.05).Multivariate analysis revealed that operative position was not an independent risk factor for PONV(odds ratio=1.192,95%confidence interval:0.376-3.778,P=0.766).CONCLUSION Operative position during LSG may affect PONV;however,the difference in the incidence of PONV was not statistically significant.Operative position should be carefully considered for obese patients before surgery. 展开更多

AIM: To investigate the prevalence of and risk factors for non-alcoholic fatty liver disease(NAFLD) in a Chinese population.METHODS: A total of 1948 adults from China was followed for 8 years. A cross-sectional study was performed to investigate the prevalence of NAFLD at baseline, and then the participants were followed for 8 years to investigate risk factors for the development of NAFLD.RESULTS: A total of 1948 participants were enrolled at baseline, of whom 691 were diagnosed with NAFLD. During the 8-year follow-up, 337 baseline NAFLD-free participants developed NAFLD. They had a greaterincrease in body mass index(BMI), serum uric acid, fasting plasma glucose, very low-density lipoprotein cholesterol and a considerable decrease in high-density lipoprotein cholesterol. 123 participants who had NAFLD at baseline lost NAFLD during the 8-year follow-up period. They had a greater decrease in BMI, fasting plasma glucose, triglycerides, total cholesterol, low-density lipoprotein cholesterol, alanine aminotransferase, aspartate aminotransferase, and γ-glutamyl transpeptidase.CONCLUSION: NAFLD is prevalent in Chinese population with a rapidly increasing tendency. It can be reversed when patients lose their weight, control their hyperlipidemia and hyperglycemia, and reduce the liver enzyme levels. 展开更多

目的:探讨人参皂苷Rg1注射剂联合肌苷片和维生素B_(1)对原发性视网膜色素变性的血清脑源性神经营养因子(BDNF)、垂体腺苷酸环化酶激活多肽(PACAP)影响及临床疗效。方法:纳入2019-08/2022-03河北北方学院附属第二医院眼科收治的50例100... 目的:探讨人参皂苷Rg1注射剂联合肌苷片和维生素B_(1)对原发性视网膜色素变性的血清脑源性神经营养因子(BDNF)、垂体腺苷酸环化酶激活多肽(PACAP)影响及临床疗效。方法:纳入2019-08/2022-03河北北方学院附属第二医院眼科收治的50例100眼原发性视网膜色素变性患者作为研究对象,按随机数字表分为研究组和对照组,各50眼。对照组患者给予肌苷片和维生素B_(1)治疗,研究组患者给予人参皂苷Rg1注射剂联合肌苷片和维生素B_(1)治疗。比较治疗前后两组血清BDNF和PACAP表达、视网膜电图、光谱域光学相干断层扫描(SD-OCT)检查黄斑中心凹为圆点直径1mm范围视网膜厚度(RT)、视野平均缺失(MD)及临床疗效及安全性指标。结果:治疗前,两组MD比较无差异(t=1.670,P=0.098)。治疗后,研究组MD显著低于对照组(t=3.628,P<0.01);治疗前,两组黄斑中心凹为圆点直径1mm范围RT比较无差异(t=0.108,P=0.914)。治疗后,研究组黄斑中心凹为圆点直径1mm范围RT显著高于对照组(t=6.125,P<0.01);治疗前,两组各项视网膜电图暗适应的结果比较无差异(均P>0.05)。治疗后,研究组各项暗视网膜电图暗适应的结果改善均显著优于对照组(均P<0.01)。治疗前,两组各项视网膜电图明适应的结果比较无差异(均P>0.05)。治疗后,研究组各项视网膜电图明适应的结果改善显著优于对照组(均P<0.01)。治疗前,两组BDNF、PACAP比较无差异(均P>0.05);治疗后,研究组BDNF、PACAP高于对照组(均P<0.01)。治疗后,两组均未见任何的不良反应。结论:原发性视网膜色素变性患者采用人参皂苷治疗可通过调节BDNF和PACAP的表达,改善患者视网膜功能,促进病情转归,且安全性高。 展开更多

Enhancing light absorption for organic solar cells using front ITO nanograting and back ultrathin Al layer

To address the discrepancy between carrier collection and light absorption of organic solar cells caused by the limited carrier mobility and optical absorption coefficient for the normally employed organic photoactive layers,a light management structure composed of a front indium tin oxide(ITO)nanograting and ultrathin Al layer inserted in between the photoactive layer and the electron transport layer(ETL)is introduced.Owing to the antireflection and light scattering induced by the ITO nanograting and the suppression of light absorption in the ETL by the inserted Al layer,the light absorption of the photoactive layer is significantly enhanced in a spectral range from 400 nm to 650 nm that also covers the main energy region of solar irradiation for the normally employed active materials such as the P3HT:PC_(61)BM blend.The simulation results indicate that comparing with the control device with a planar configuration of ITO/PEDOT:PSS/P3HT:PC_(61)BM(80-nm thick)/ZnO/Al,the short-circuit current density and power conversion efficiency of the optimized light management structure can be improved by 32.86%and 34.46%.Moreover,good omnidirectional light management is observed for the proposed device structure.Owing to the fact that the light management structure possesses the simple structure and excellent performance,the exploration of such a structure can be believed to be significant in fabricating the thin film-based optoelectronic devices. 展开更多

目的:探讨血清淀粉样蛋白A(SAA)在感染性眼内炎诊断中的临床应用价值。方法:收集2016-06/2019-03我院就诊的270例患者,其中116例感染性眼内炎患者作为研究组,154例非感染性患者作为对照组。采用胶体金免疫层析法检测SAA水平,用受试者工... 目的:探讨血清淀粉样蛋白A(SAA)在感染性眼内炎诊断中的临床应用价值。方法:收集2016-06/2019-03我院就诊的270例患者,其中116例感染性眼内炎患者作为研究组,154例非感染性患者作为对照组。采用胶体金免疫层析法检测SAA水平,用受试者工作特征(ROC)曲线分析诊断效能。结果:感染性眼内炎患者和对照组SAA值中位数分别为14.98、2.56mg/L,两组比较有差异(P<0.001);CRP值和WBC值中位数两组比较均有差异(P<0.001)。SAA、CRP和WBC计数诊断感染性眼内炎的ROC曲线下面积分别为0.772、0.638、0.618,SAA检测取Youden指数最大值所对应的最佳临界值为6.975mg/L,其灵敏度为63.79%,特异度为84.42%。结论:SAA联合CRP和WBC计数有助于提高感染性眼内炎的诊断效能。SAA可为感染性眼内炎的辅助诊断提供有用的参考信息。 展开更多

rTg4510 mice are transgenic mice expressing P301L mutant tau and have been developed as an animal model of tauopathies including Alzheimer’s disease(AD).Besides cognitive impairments,rTg4510 mice also show abnormal h... rTg4510 mice are transgenic mice expressing P301L mutant tau and have been developed as an animal model of tauopathies including Alzheimer’s disease(AD).Besides cognitive impairments,rTg4510 mice also show abnormal hyperactivity behavior.Cornel iridoid glycoside(CIG)is an active ingredient extracted from Cornus officinalis,a traditional Chinese herb.The purpose of the present study was to investigate the effects of CIG on the emotional disorders such as hyperactivity,and related mechanisms.The emotional hyperactivity was detected by locomotor activity test and Y maze test.Immunofluorescent and immunohistochemical analyses were conducted to measure neuron loss and phosphorylated tau.Western blotting was used to detect the expression of related proteins.The results showed that intragastric administration of CIG for 3 months decreased the hyperactivity phenotype,prevented neuronal loss,reduced tau hyperphosphorylation and aggregation in the amygdala of rTg4510 mice.Meanwhile,CIG alleviated the synaptic dysfunction by increasing the expression of N-methyl-D-aspartate receptors(NMDARs)subunits GluN1 and GluN2A andαamino-3-hydroxy-5-methyl-4-isoxazole propionic acid receptor(AMPAR)subunits GluA1 and GluA2,and increased the level of phosphorylated Ca2+/calmodulin dependent protein kinase IIα(p-CaMK IIα)in the brain of rTg4510 mice.In conclusion,CIG may have potential to treat the emotional disorders in tauopathies such as AD through reducing tau pathology and improving synaptic dysfunction. 展开更多

Background: Endometriosis (EMs) is a common gynecological disorder characterized by endometrial-like tissue outside the uterus. Hypoxia induces the expression of many important downstream genes to regulate the implantation, survival, and maintenance ofectopic endometriotic lesions. Transtbrming growth factor-beta I (TGF-β1) plays a major role in the etiology of EMs. We aimed to determine whether TGF-β1 affects EMs development and progression and its related mechanisms in hypoxic conditions. Methods: Endometrial tissue was obtained from women with or without EMs undergoing surgery from October, 2015 to October, 2016. Endometrial cells were cultured and then exposed to hypoxia and TGF-β1 or TGF-β1 inhibitors. The messenger RNA (mRNA) and protein expression levels ofTGF-β1, vascular endothelial growth fhctor (VEGF), and hypoxia-inducible fhctor-Ic~ (HIF-β1) were measured. A DuaI-Luciferase Reporter Assay was used to examine the effect ofTGF-[31 and hypoxia on a VEGF promoter construct. Student's t-test was pertbrmed/br comparison among groups (one-sided or two-sided) and a value ofP 〈 0.05 was considered statistically significant. Results: TGF-β1, VEGF, HIF-β1 mRNA, and protein expression were significantly higher in EMs tissue than that in normal endometrial tissue (t = 2.16, P = 0.042). EMs primary cultured cells exposed to hypoxia expressed 43.8% higher VEGF mRNA and protein (t = 6.84, P - 0.023). VEGF mRNA levels increased 12.5% in response to TGF-β1, whereas the combined treatment of hypoxia/TGF-β1 resulted in a much higher production (87.5% increases) of VEGF. The luciferase activity of the VEGF promoter construct was increased in the presence of either TGF-β1 (2.6-fold, t = 6.08, P = 0.032) or hypoxia (11.2-fold, t = 32.70, P 〈 0.001 ), whereas the simultaneous presence of both stimuli resulted in a significant cooperative effect ( 18.5-fold, t = 33.50, P 〈 0.001 ). Conclusions: The data support the hypothesis that TGF-β1 is involved in the pathogenesis of EMs through regulating VEGF expression. An additive effect of TGF-[31 and hypoxia is taking place at the transcriptional level. 展开更多

While a high-fat diet(HFD) is assumed to be related to fat-mediated oxidative stress decreasing antioxidant enzyme activity, probiotics are believed to have positive effects on the regulation of HFD-induced obesity as well as lipid metabolism, energy homeostasis, and anti-oxidation. Because Bacillus subtilis B10 has beneficial effects on the abnormal lipid metabolism and the oxidative stress in HFD-induced obese mice, ICR mice were randomly assigned into an HFD group and the HFD was supplemented with 0.1%(w/w) Bacillus subtilis B10(HFD+B10 group). Thereafter, 30-d treatments were run, and then hepatic lipid level and antioxidant status were measured. The expression of genes related to lipid metabolism and oxidative stress in the liver was determined by reverse-transcription quantitative polymerase chain reaction(RT-q PCR). We found that HFD-induced obese mice treated with B10 showed a decrease in weight gain, serum glucose activity as well as hepatic triglyceride(TG), glutamic oxaloacetic transaminase(GOT), and glutamic pyruvic transaminase(GPT) activities. In addition, the gene expressions of antioxidant genes, glutathione reductase(GR), xanthine oxidase(XO), heat-shock protein 90(Hsp90), and lipid synthesis gene 3β-hydroxysteroid-?24 reductase(DHCR24) in the HFD+B10 group were down-regulated, suggesting alleviation of oxidative stress, while the lipolysis gene 3-hydroxy-3-methylglutaryl-Co A synthase 2(HMGCS2), energy metabolism gene peroxisome proliferator-activated receptor α(PPARα) and the gene encoding tumor-suppressor protein p53 were up-regulated. The regulatory and positive effect of dietary supplementation of probiotic B10 suggests that it has a beneficial effect on the homeostasis of the lipid metabolism and on alleviating oxidative stress in HFD-induced obese mice. 展开更多

Yttria-stabilized zirconia( YSZ) coatings were deposited by low pressure plasma spray( LPPS) in 1.0× 10^4 Pa,1.5 × 10^4 Pa,and 2.5 × 10^4 Pa. Both in-flight particle diagnostic detected by DPV-2000 system and ANSYS-FLUENT software were used to study the connection between the parameters of flying particles and the coating formation,which might help to recognize the relationship between the operation parameters and the coatings quality. The results of simulation showed that particles in a lower spray pressure could achieve a higher velocity. The particle velocity was around 380 m/s at a distance of 35 cm from the nozzle at 1.0 × 10^4 Pa while only 300 m/s at 2.5 × 10^4 Pa in actual measurement.The results showed that the velocity of particles increased with decreasing the spray pressure,which might enhance the flattening rate of coatings and thereby decreased the porosity. The deposited YSZ coating with the lowest porosity can be gained under 1.0 × 10^4 Pa condition. 展开更多

The resilient modulus,accumulated plastic strain,peak shear stress,and critical shear stress are the elastoplastic behaviors of frozen sand–concrete interfaces under cyclic shear loading.They reflect the bearing capacity of buildings(e.g.highspeed railways)in both seasonal frozen and permafrost regions.This study describes a series of direct shear experiments conducted on frozen sand–concrete interfaces.The results indicated that the elastoplastic behaviors of frozen sand–concrete interfaces,including the resilient modulus,accumulated plastic strain,and shear strength,are influenced by the boundary conditions(constant normal loading and constant normal height),initial normal stress,negative temperature,and cyclic-loading amplitude.The resilient modulus was significantly correlated with the initial normal stress and negative temperature,but not with the cyclic-loading amplitude and loading cycles.The accumulated plastic shear strain increased when the initial normal stress and cyclic-loading amplitude increased and the temperature decreased.Moreover,the accumulated plastic shear strain increment decreased when the loading cycles increased.The accumulated direction also varied with changes in the initial normal stress,negative temperature,and cyclic-loading amplitude.The peak shear stress of the frozen sand–concrete interface was affected by the initial normal stress,negative temperature,cyclic-loading amplitude,and boundary conditions.Nevertheless,a correlation was observed between the critical shear stress and the initial normal stress and boundary conditions.The peak shear stress was higher,and the critical shear stress was lower under the constant normal height boundary condition.Based on the results,it appears that the properties of frozen sand–concrete interfaces,including plastic deformation properties and stress strength properties,are influenced by cyclic shear stress.These results provide valuable information for the investigation of constitutive models of frozen soil–structure interfaces. 展开更多

Objective:Alzheimer’s disease(AD)is along with cognitive decline due to amyloid-β(Aβ)plaques,tau hyperphospho rylation,and neuron loss.Shenqi Xingnao Granules(SQXN),a traditional Chinese medicine,significantly ameliorated the cognitive function and daily living abilities of patients with AD.However,till date,no study has investigated the mechanism of action of SQXN on AD.The present study aimed to verify the effects of SQXN treatment on cognitive impairments and AD-like pathologies in APP/PS1 mice.Methods:Four-month-old APP/PS1 transgenic(Tg)mice were randomly divided into a model group and SQXN-treated(3.5,7,14 g/kg per day)groups.Learning-memory abilities were determined by Morris water maze and object recognition test.All mice were sacrificed and the brain samples were collected after 75 d.The soluble Aβcontents were detected by Elisa kit;The levels of expression of NeuN,APP,phosphorylated tau and related protein were measured by Western blotting;The inflammation factors were detected by the proinflammatory panel kit.Results:Four-month-old APP/PS1 mice were administered SQXN by oral gavage for 2.5 months.Using the Morris water maze tests and Novel object recognition,we found that SQXN restored behavioral deficits in the experimental group of Tg mice when compared with the controls.SQXN also inhibited neuronal loss(NeuN marker).SQXN treatment decreased soluble Aβ42 through inhibiting the expression of sAPPβand BACE-1 without regulating full-length amyloid precursor protein(FL APP).Insulin degrading enzyme(IDE),the Aβdegrading enzyme,were increased by SQXN.In addition,SQXN reduced hyperphosphorylated tau protein levels and prevented excessive activation of p-GSK-3βin the brain of APP/PS1 mice.Compared with APP/PS1 transgenic negative mice,IFN-γ,IL-1β,IL-2,IL-4,IL-5,IL-6,IL-12 p70,KC/GRO and TNF-αwere not obviously changed in the brain of 6.5-month-old APP/PS1 transgenic(Tg)mice.However,SQXN could inhibited the expression of IL-2.Conclusion:These results demonstrate that SQXN ameliorates the cognitive impairments in APP/PS1 mice.The possible mechanisms involve its inhibition of neuronal loss,soluble Aβdeposition,tau hyperphosphorylation and inflammation. 展开更多

The shear strength properties of the frozen sand–structure interface are critical for evaluating the serviceability of pile foundations in frozen ground.The shear characteristics of the frozen sand–concrete interface were studied with two boundary conditions(constant normal load(CNL)and constant normal height(CNH)),at three normal stresses(100,200,and 300 k Pa),and at three temperatures(-2,-5,and-8℃).A detailed comparative analysis was performed to explore the principal factors affecting the shear/normal-shear displacement.The results showed that the shear behavior of the frozen sand–concrete interface under CNL was similar to that under CNH.The shear stress–shear displacement exhibited strain softening.The temperature and normal stress were the major influences on normal properties.The lower the temperature and the higher the normal stress,the greater was the elastic shear modulus.The peak shear stress and critical shear stress exhibited a dependence on normal stress.An exponential growth in the peak shear stress was observed as the temperature decreased.Critical shear stress was dependent on temperature.The value and percentage of peak ice-cementation in peak shear stress was affected by temperature and normal stress. 展开更多
