目的通过对胃食管反流病(gastroesophageal reflux disease,GERD)患者食管高分辨率测压及24 h食管p H-阻抗联合监测结果分析,以探讨GERD患者食管动力及酸暴露特点.方法选取2017-07/2018-07就诊于石河子大学第一附属医院消化内科门诊及... 目的通过对胃食管反流病(gastroesophageal reflux disease,GERD)患者食管高分辨率测压及24 h食管p H-阻抗联合监测结果分析,以探讨GERD患者食管动力及酸暴露特点.方法选取2017-07/2018-07就诊于石河子大学第一附属医院消化内科门诊及住院部具有典型症状(反酸、烧心)、根据蒙特利尔共识意见可诊断为GERD的患者59例,行胃镜、食管高分辨率测压及24 h食管p H-阻抗联合监测等检查;根据胃镜是否诊断反流性食管炎(reflux esophagitis, RE)组(27例,男/女:12/15)和非糜烂性反流病(nonerosive reflux disease, NERD)组(32例,男/女:14/18),比较两组患者食管高分辨测压及24 h食管pH-阻抗监测结果的差异.结果食管高分辨率测压结果中, RE组患者下食管括约肌静息压明显低于NERD组(P<0.05),上食管括约肌静息压及上下食管括约肌残余压比较差异无统计学意义(P>0.05), RE组患者远端波波幅低于NERD组(P <0.05),蠕动波持续时间、起始速度、远端收缩延迟时间两组间差异无统计学意义(P>0.05);分析24 h食管pH-Z监测结果, RE组DeMeester评分明显高于NERD组(P<0.05),总计酸反流时间低于NERD组(P <0.05),立卧位反流次数、总计反流次数、不同物理性质之间差异无统计学意义(P>0.05).结论不同GERD患者食管动力及酸反流存在一定的差异,RE患者食管动力障碍及酸反流较NERD更明显. 展开更多

The prevalence of,and related factors to,stress urinary incontinence (SUI) among perimenopausal Chinese women and its impact on daily life among those women with sexual desire problem in Hubei province were investigated.In this study,1519 perimenopausal women aged 40 to 65 years were selected from three urban communities in the Wuhan area,and two impoverished,mountainous communities in Hubei province,and followed from April to October 2014.Detailed information about demographic characteristics,menstruation,pregnancy,sexual life and chronic diseases was collected.A cross-sectional survey was carried out following information collection by Chi-square test and multiple logistic regression analysis.Univariate and multivariate logistic regression analysis demonstrated that the potential factors associated with developing SUI were old age (OR=3.4,95% CI:1.92-6.04),vaginal delivery (OR=0.623,95% CI:0.45-0.87),low income (OR=0.063,95% CI:0.40-0.92),atrophic vaginitis (OR=1.4,95% CI:1.03-1.80),pelvic organ prolapse (OR=2.81,95% CI:1.36-5.80),chronic pelvic pain (OR=2.17,95% CI:1.90-4.03),constipation (OR=1.44,95% CI:1.07-1.93) and incontinence of feces (OR=3.32,95% CI:2.03-5.43).Moreover,the ratio of SUI (33.2%) was higher than the ratio of urgency urinary incontinence (24.1 %) or the ratio of mixed urinary incontinence (17.4%),and SUI had a greater impact on daily life among women with decreased sexual desire.In conclusion,SUI is a common disorder affecting over one third of the women surveyed,and has a severe impact on the daily life of perimenopausal women with declined sexual desire.Age,mode of delivery,and monthly income are major risk factors involved in the development of SUI. 展开更多

Neutrophil peptide 1 belongs to a family of peptides involved in innate immunity. Continuous intramuscular injection of neutrophil peptide 1 can promote the regeneration of peripheral nerves, but clinical application in this manner is not convenient. To this end, the effects of a single intraoperative administration of neutrophil peptide 1 on peripheral nerve regeneration were experimentally observed. A rat model of sciatic nerve crush injury was established using the clamp method. After model establishment, a normal saline group and a neutrophil peptide 1 group were injected with a single dose of normal saline or 10 μg/mL neutrophil peptide 1, respectively. A sham group, without sciatic nerve crush was also prepared as a control. Sciatic nerve function tests, neuroelectrophysiological tests, and hematoxylin-eosin staining showed that the nerve conduction velocity, sciatic functional index, and tibialis anterior muscle fiber cross-sectional area were better in the neutrophil peptide 1 group than in the normal saline group at 4 weeks after surgery. At 4 and 8 weeks after surgery, there were no differences in the wet weight of the tibialis anterior muscle between the neutrophil peptide 1 and saline groups. Histological staining of the sciatic nerve showed no significant differences in the number of myelinated nerve fibers or the axon cross-sectional area between the neutrophil peptide 1 and normal saline groups. The above data confirmed that a single dose of neutrophil peptide 1 during surgery can promote the recovery of neurological function 4 weeks after sciatic nerve injury. Peripheral nerves have a limited capacity for self-repair and those that are severely damaged or have significant defects are challenging to repair. Investigating the pathophysiology of peripheral nerve repair is important for the clinical treatment of peripheral nerve repair and regeneration. In this study, rat models of right sciatic nerve injury were established by a clamping method. Protein chip assay was performed to quantify the levels of neurotrophic, inflammation-related, chemotaxis-related and cell generation-related factors in the sciatic nerve within 7 days after injury. The results revealed that the expression levels of neurotrophic factors(ciliary neurotrophic factor) and inflammationrelated factors(intercellular cell adhesion molecule-1, interferon γ, interleukin-1α, interleukin-2, interleukin-4, interleukin-6, monocyte chemoattractant protein-1, prolactin R, receptor of advanced glycation end products and tumor necrosis factor-α), chemotaxis-related factors(cytokine-induced neutrophil chemoattractant-1, L-selectin and platelet-derived growth factor-AA) and cell generation-related factors(granulocyte-macrophage colony-stimulating factor) followed different trajectories. These findings will help clarify the pathophysiology of sciatic nerve injury repair and develop clinical treatments of peripheral nerve injury. Objective:To study the influence of targeted inhibition of Notch1 gene on the killing effects of Paclitaxel on triple-negative breast cancer cells.Methods:The triple-negative [estrogen receptor(ER)/progesterone receptor(PR)/human epidermal growth factor receptor 2(Her2)] breast cancer cell line MDA-MB-231 and ER/PR/HER-2-positive breast cancer cell line MCF-7 were cultured,transfected with Notch1-si RNA-overexpression plasmid and blank plasmid,and treated with different concentrations of paclitaxel,and then the cell proliferation activity and apoptosis rate as well as the m RNA expression of Caspase-3,Caspase-9 and Bcl-2 were determined.Results:Paclitaxel could decrease the MDA-MB-231 and MCF-7 cell proliferation activity as well as Bcl-2 mRNA expression,and increase MDA-MB-231 and MCF-7 cell apoptosis rate as well as Caspase-3 and Caspase-9 mRNA expression in dosedependent manners;with the same dose of paclitaxel treatment,the inhibitory effects on MDAMB-231 cell proliferation activity and Bcl-2 m RNA expression as well as the promoting effects on MDA-MB-231 cell apoptosis and mR NA expression of Caspase-3 and Caspase-9 were weaker than those on MCF-7 cell;after 0.5 μM paclitaxel combined with Notch1-siRNA treatment,MDA-MB-231 cell proliferation activity and Bcl-2 mRNA expression were significantly lower than those after 0.5 μM paclitaxel combined with control plasmid treatment while cell apoptosis rate and mR NA expression of Caspase-3 and Caspase-9 were higher than those after 0.5 μM paclitaxel combined with control plasmid treatment.Conclusions:Targeted inhibition of Notch1 gene may enhance the killing effects of paclitaxel on triple-negative breast cancer cells by up-regulating the expression of Caspase-3 and Caspase-9 and inhibiting the expression of Bcl-2. 展开更多

Telescopes with large aspherical primary mirrors collect more light and are therefore sought after by astronomers. Instead of using a single large one-piece mirror, smaller segments can be assembled into a useable telescopic primary. Because the segments must fit together to create the effect of a single mirror, segmented optics present unique challenges to the fabrication and testing that are absent in monolithic optics. A dispersed fringe sensor (DFS) using a broadband point source is an efficient method for cophasing and is also highly automated and robust. Unlike the widely adopted Shack- Hartmann Wavefront sensor and curvature wavefront sensor with edge sensors for calibration of relative pistons, DFS can estimate the piston between segments by only using the spectrum formed by the transmissive grating's dispersion, and therefore can replace the edge sensors, which are difficult to calibrate. We introduce the theory of the DFS and Dispersed Hartmann Sensor (DHS) for further utilization of the coarse phasing method of DFS. According to the theory, we bring out the preliminary system design of the cophasing experimental system based on DFS and DHS which is now established in our institute. Our previous studies have confirmed that during nerve transposition repair to injured peripheral nerves, the regenerated nerve fibers of motor neurons in the anterior horn of the spinal cord can effectively repair distal nerve and target muscle tissue and restore muscle motor function. To observe the effect of nerve regeneration and motor function recovery after several types of nerve transposition for median nerve defect(2 mm), 30 Sprague-Dawley rats were randomly divided into sham operation group, epineurial neurorrhaphy group, musculocutaneous nerve transposition group, medial pectoral nerve transposition group, and radial nerve muscular branch transposition group. Three months after nerve repair, the wrist flexion test was used to evaluate the recovery of wrist flexion after regeneration of median nerve in the affected limbs of rats. The number of myelinated nerve fibers, the thickness of myelin sheath, the diameter of axons and the cross-sectional area of axons in the proximal and distal segments of the repaired nerves were measured by osmic acid staining. The ratio of newly produced distal myelinated nerve fibers to the number of proximal myelinated nerve fibers was calculated. Wet weights of the flexor digitorum superficialis muscles were measured. Muscle fiber morphology was detected using hematoxylin-eosin staining. The cross-sectional area of muscle fibers was calculated to assess the recovery of muscles. Results showed that wrist flexion function was restored, and the nerve grew into the distal effector in all three nerve transposition groups and the epineurial neurorrhaphy group. There were differences in the number of myelinated nerve fibers in each group. The magnification of proximal to distal nerves was 1.80, 3.00, 2.50, and 3.12 in epineurial neurorrhaphy group, musculocutaneous nerve transposition group, medial pectoral nerve transposition group, and radial nerve muscular branch transposition group, respectively. Nevertheless, axon diameters of new nerve fibers, cross-sectional areas of axons, thicknesses of myelin sheath, wet weights of flexor digitorum superficialis muscle and cross-sectional areas of muscle fibers of all three groups of donor nerves from different anterior horn motor neurons after nerve transposition were similar to those in the epineurial neurorrhaphy group. Our findings indicate that donor nerve translocation from different anterior horn motor neurons can effectively repair the target organs innervated by the median nerve. The corresponding spinal anterior horn motor neurons obtain functional reinnervation and achieve some degree of motor function in the affected limbs. 展开更多

Background: Although emerging evidence points to benefits from Tai Chi Chuan (TCC) in improving immune system function, its effects on cellular immune responses remain under-studied. The objective of this study was to evaluate the effects of TCC training on cellular immunity in non-small cell lung cancer patients. Methods: A 2-group randomized trial design in which post-surgical, non-small cell lung cancer survivors were randomly assigned to a TCC training group (n = 16) or a control group (n = 16). The participants in the TCC group completed a 16-week intervention. The main immune response outcome measures assayed included the ratio of T-helper cells/T-suppressor cells (CD4+:CD8+ ratio) and complement regulatory proteins status (CRPs; CD55 and CD59). Using repeated measures ANOVA, the data were analyzed for the participants who completed the study (n = 27). Results: At 16 weeks, the TCC participants showed a significantly lower increment in the expression of CD55 (p 〈 0.05) as compared to the control group. No significant between-group differences were found in the CD4+:CD8+ ratio or CD59 expression. There were also no significant correlations among the changes in CRPs or T lymphocyte subpopulations, either. Conclusion: A 16-week TCC intervention caused no alterations in CD4+:CD8+ ratio, but significantly attenuated CD55 expression among post- surgical non-small cell lung cancer survivors. 展开更多

During peripheral nerve transposition repair, if the diameter difference between transposed nerves is large or multiple distal nerves must be repaired at the same time, traditional epineurial neurorrhaphy has the problem of high tension at the suture site, which may even lead to the failure of nerve suture. We investigated whether a small gap bio-sleeve suture with different inner diameters at both ends can be used to repair a 2-mm tibial nerve defect by proximal transposition of the common peroneal nerve in rats and compared the results with the repair seen after epineurial neurorrhaphy. Three months after surgery, neurological function, nerve regeneration, and recovery of nerve innervation muscle were assessed using the tibial nerve function index, neuroelectrophysiological testing, muscle biomechanics and wet weight measurement, osmic acid staining, and hematoxylin-eosin staining. There was no obvious inflammatory reaction and neuroma formation in the tibial nerve after repair by the small gap bio-sleeve suture with different inner diameters at both ends. The conduction velocity, muscle strength, wet muscle weight, cross-sectional area of muscle fibers, and the number of new myelinated nerve fibers in the biosleeve suture group were similar to those in the epineurial neurorrhaphy group. Our findings indicate that small gap bio-sleeve suture with different inner diameters at both ends can achieve surgical suture between nerves of different diameters and promote regeneration and functional recovery of injured peripheral nerves. 展开更多

Identification of four differentially expressed genes associated with acute and chronic spinal cord injury based on bioinformatics data 被引量:1

Complex pathological changes occur during the development of spinal cord injury(SCI),and determining the underlying molecular events that occur during SCI is necessary for the development of promising molecular targets and therapeutic strategies.This study was designed to explore differentially expressed genes(DEGs)associated with the acute and chronic stages of SCI using bioinformatics analysis.Gene expression profiles(GSE45006,GSE93249,and GSE45550)were downloaded from the Gene Expression Omnibus database.SCI-associated DEGs from rat samples were identified,and Gene Ontology and the Kyoto Encyclopedia of Genes and Genomes pathway enrichment analyses were performed.In addition,a protein-protein interaction network was constructed.Approximately 66 DEGs were identified in GSE45550 between 3–14 days after SCI,whereas 2418 DEGs were identified in GSE450061–56 days after SCI.Moreover,1263,195,and 75 overlapping DEGs were identified between these two expression profiles,3,7/8,and 14 days after SCI,respectively.Additionally,16 overlapping DEGs were obtained in GSE450061–14 days after SCI,including Pank1,Hn1,Tmem150c,Rgd1309676,Lpl,Mdh1,Nnt,Loc100912219,Large1,Baiap2,Slc24a2,Fundc2,Mrps14,Slc16a7,Obfc1,and Alpk3.Importantly,3882 overlapping DEGs were identified in GSE932491–6 months after SCI,including 3316 protein-coding genes and 567 long non-coding RNA genes.A comparative analysis between GSE93249 and GSE45006 resulted in the enrichment of 1135 overlapping DEGs.The significant functions of these 1135 genes were correlated with the response to the immune effector process,the innate immune response,and cytokine production.Moreover,the biological processes and KEGG pathways of the overlapping DEGs were significantly enriched in immune system-related pathways,osteoclast differentiation,the nuclear factor-κB signaling pathway,and the chemokine signaling pathway.Finally,an analysis of the overlapping DEGs associated with both acute and chronic SCI,assessed using the expression profiles GSE93249 and GSE45006,identified four overlapping DEGs:Slc16a7,Alpk3,Lpl and Nnt.These findings may be useful for revealing the biological processes associated with SCI and the development of targeted intervention strategies. 展开更多

Background:To observe the effect of perioperative acupuncture on postoperative pain in patients undergoing standard sternotomy.Methods:57 patients who received standard median sternotomy were randomly divided into acupuncture group(30 cases)and control group(27 cases).The control group was treated with routine operation and nursing,and the acupuncture group was treated with acupuncture for 10 days on the basis of perioperative period.The pain levels of the two groups before and after operation were compared.Results:The pain level of the acupuncture group was significantly lower than that of the control group on the 2nd,10th,and 30th days after operation(P<0.05),and the short-term pain levels in the two groups were statistically significant(P<0.05).Conclusion:The perioperative acupuncture treatment has a significant analgesic effect on short-term pain for patients undergoing anterior midline thoracotomy,and it is helpful for patients to recover quickly after surgery. 展开更多

Multiple regeneration of axonal buds has been shown to exist during the repair of peripheral nerve injury, which confirms a certain repair potential of the injured peripheral nerve. Therefore, a systematic nerve transposition repair technique has been proposed to treat severe peripheral nerve injury. During nerve transposition repair, the regenerated nerve fibers of motor neurons in the anterior horn of the spinal cord can effectively grow into the repaired distal nerve and target muscle tissues, which is conducive to the recovery of motor function. The aim of this study was to explore regeneration and nerve functional recovery after repairing a long-segment peripheral nerve defect by transposition of different donor nerves. A long-segment(2 mm) ulnar nerve defect in Sprague-Dawley rats was repaired by transposition of the musculocutaneous nerve, medial pectoral nerve, muscular branches of the radial nerve and anterior interosseous nerve(pronator quadratus muscle branch). In situ repair of the ulnar nerve was considered as a control. Three months later, wrist flexion function, nerve regeneration and innervation muscle recovery in rats were assessed using neuroelectrophysiological testing, osmic acid staining and hematoxylin-eosin staining, respectively. Our findings indicate that repair of a long-segment ulnar nerve defect with different donor nerve transpositions can reinnervate axonal function of motor neurons in the anterior horn of spinal cord and restore the function of affected limbs to a certain extent. 展开更多

A telescope with a larger primary mirror can collect much more light and resolve objects much better than one with a smaller mirror, and so the larger version is always pursued by astronomers and astronomical technicians. Instead of using a monolithic primary mirror, more and more large telescopes, which are currently being planned or in construction, have adopted a segmented primary mirror design. Therefore, how to sense and phase such a primary mirror is a key issue for the future of extremely large optical/infrared telescopes. The Dispersed Fringe Sensor (DFS), or Dispersed Hartmann Sensor (DHS), is a non-contact method using broadband point light sources and it can estimate the piston by the two-directional spectrum formed by the transmissive grating's dispersion and lenslet array. Thus it can implement the combination of co-focusing by Shack-Hartmann technology and phasing by dispersed fringe sensing technologies such as the template-mapping method and the Hartmann method. We introduce the successful design, construction and alignment of our dis- persed Hartmann sensor together with its design principles and simulations. We also conduct many successful real phasing tests and phasing corrections in the visible waveband using our existing indoor segmented mirror optics platform. Objective:To study the effectiveness of Qingre Yangyin Humo prescription in preventing radioaction esophagitis and its effect on the serum radiation damage indexes.Methods:Patients with advanced esophageal cancer who received radiotherapy in the First Affiliated Hospital of Bengbu Medical College between June 2015 and December 2016 were selected as the research subjects and randomly divided into two groups, the intervention group received Qingre Yangyin Humo prescription during radiotherapy, and the control group received regular intervention during radiotherapy. The incidence of radiation esophagitis within 90 days after radiotherapy was observed, and serum inflammation indexes, oxidative stress indexes and pulmonary fibrosis index were determined before radiotherapy as well as 2 weeks and 4 weeks after radiotherapy.Results: After radiotherapy, serum TNF-α, IL-1β, IL-6, IL-8, MDA, 8-OHdG, MMP9, TGF-β1 and ACE levels of both groups were significantly higher than those before radiotherapy while GPX1, T-SOD and Mn-SOD levels were significantly lower than those before radiotherapy, and serum TNF-α, IL-1β, IL-6, IL-8, MDA, 8-OHdG, MMP9, TGF-β1 and ACE levels of intervention group were significantly lower than those of control group while GPX1, T-SOD and Mn-SOD levels were significantly higher than those of control group.Conclusion:Qingre Yangyin Humo prescription can prevent the occurrence of radiation esophagitis and reduce radiation damage during radiotherapy. 展开更多

The in vivo behaviors of doxorubicin (DOX)-loaded dextran-poly(3-acrylamidophenylboronic acid) (Dextran- PAPBA) nanoparticles (NPs) were studied. The DOX-loaded NPs had a narrowly distributed diameter of ca. 74 nm and mainly accumulated in liver of tumor-bearing mice after intravenous injection as demonstrated by in vivo real-time near infrared fluorescent imaging. The DOX contents in various tissues were quantified and consisted well with the results of fluorescent imaging. The biodistribution pattern of DOX-loaded NPs encourages us to investigate their liver tumor treatment by using an orthotopically implanted liver tumor model, revealing that the DOX-loaded NPs formulation had better antitumor effect than free DOX. 展开更多

The Hedgehog(Hh)signaling pathway is one o f the major regulators of embryonic development and tissue homeostasis in multicellular organisms.However,the role of this pathway in the silkworm,especially in the silkworm midgut,remains poorly understood.Here,we report that Bombyx mori Hedgehog(BmHh)is expressed in most tissues of silkworm larvae and that its functions are well-conserved throughout evolution.We further demonstrate that the messenger RNA o f four Hh signaling components,BmHh ligand,BmPtch receptor,signal transducer BmSmo and transcription factor BmCi,are all upregulated following Escherichia coli or Bacillus thuringiensis infection,indicating the activation o f the Hh pathway.Simultaneously,midgut cell proliferation is strongly promoted.Conversely,the repression of Hh signal transduction with double-stranded RNA or cyclopamine inhibits the expression o f BmHh and BmCi and reduces cell proliferation.Overall,these findings provide new insights into the Hh signaling pathway in the silkworm,B.mori. 展开更多
