老年患者出院后跌倒发生率及影响因素的调查研究

目的了解老年人出院后1年内累积跌倒发生率及其影响因素。方法2019年5月至2020年8月,对重庆市一所三级甲等医院的出院老年患者进行前瞻性调查研究。采用中文版跌倒风险自评量表、衰弱评估量表(FRAIL scale)和9项患者健康问卷(Patient He... 目的了解老年人出院后1年内累积跌倒发生率及其影响因素。方法2019年5月至2020年8月,对重庆市一所三级甲等医院的出院老年患者进行前瞻性调查研究。采用中文版跌倒风险自评量表、衰弱评估量表(FRAIL scale)和9项患者健康问卷(Patient Health Questionnaire-9,PHQ-9)、Barthel指数评定量表(Bar-thel index,BI)调查出院老年人跌倒风险、衰弱、抑郁、自理能力状况并在其出院后电话追踪1年。应用累积发生率函数估计老年人出院后1年内跌倒的累积发生率,利用竞争风险模型中部分分布风险函数探讨跌倒的危险因素。结果共有1077例老年患者参与此研究,出院后1、6、12个月跌倒累计发生率分别为4.45%、9.03%和10.80%。出院时伴有抑郁和衰弱的老年人在出院后1、6、12个月的跌倒累计发生率分别26.19%、49.93%、58.53%和21.59%、41.67%、48.73%,远高于没有抑郁和衰弱的老年人(P<0.05)。出院时伴有抑郁、衰弱、Barthel指数得分61~80分、住院时间>20 d、随访时间内再次入院、由他人照料和跌倒风险是影响老人出院后1年内跌倒的因素。结论住院老年人出院后1年内跌倒发生率随出院时间的延长具有累积效应,且受多种因素影响,未来构建预出院老年人跌倒预防策略时应关注这些影响因素,并重点关注老年人抑郁和衰弱。 展开更多

The traditional CdS buffer layers in flexible CZTSSe solar cells lead to light absorption losses and environmental pollution problems. Therefore, the study of Cd-free buffer layer is very important for the realization of environmentally friendly and efficient CZTSSe solar cells. The Zn1-xMgxO(ZnMgO) and Zn1-xSnxO(ZnSnO) alternate buffer layers are studied in this study using the simulation package solar cell capacitance simulator(SCAPS-1D) numerical simulation model, and the theoretical analysis is further verified by the results of the experiments. We simulate the performance of CZTSSe/ZnXO(X = Mg/Sn) heterojunction devices with different Mg/(Zn+Mg) and Sn/(Zn+Sn) ratios and analyze the intrinsic mechanism of the effect of conduction band offsets(CBO) on the device performance. The simulation results show that the CZTSSe/ZnXO(X = Mg/Sn) devices achieve optimal performance with a small “spike” band or “flat” band at Mg and Sn doping concentrations of 0.1 and 0.2, respectively. To investigate the potential of Zn_(0.9)Mg_(0.1O) and Zn_(0.8)Sn_(0.2)O as alternative buffer layers, carrier concentrations and thicknesses are analyzed. The simulation demonstrates that the Zn0.9Mg0.1O device with low carrier concentration has a high resistivity, serious carrier recombination, and a greater impact on performance from thickness variation. Numerical simulations and experimental results show the potential of the ZnSnO buffer layer as an alternative to toxic CdS, and the ZnMgO layer has the limitation as a substitute buffer layer. This paper provides the theoretical basis and experimental proof for further searching for a suitable flexible CZTSSe Cd-free buffer layer. 展开更多

背景与目的小细胞肺癌(small cell lung cancer, SCLC)是细胞分化程度低,恶性程度高,生长速度快,早期容易发生转移的恶性肿瘤。目前SCLC患者的临床治疗以化疗为主,但是在治疗6个月-9个月后极易发生获得性耐药而复发。因此,构建有效的临... 背景与目的小细胞肺癌(small cell lung cancer, SCLC)是细胞分化程度低,恶性程度高,生长速度快,早期容易发生转移的恶性肿瘤。目前SCLC患者的临床治疗以化疗为主,但是在治疗6个月-9个月后极易发生获得性耐药而复发。因此,构建有效的临床前SCLC动物模型具有重要的临床价值。人源性肿瘤异种移植动物模型(patient-derived xenotransplantation, PDX)能够较好地保留原发肿瘤的特性,是比较理想的临床前动物模型。本研究旨在构建中国人来源的SCLC PDX动物模型,并诱导构建化疗耐药的PDX模型,为研究SCLC耐药机制及个体化治疗提供实验模型。方法取临床SCLC患者的新鲜手术切除标本或穿刺标本,移植至重度免疫缺陷小鼠NOD-PrkdcscidIL2rgtm1/Bcgen(B-NSGTM)皮下,HE染色及免疫组化对比移植肿瘤组织与患者肿瘤组织的病理学一致性。给予可稳定传代的每一代PDX模型小鼠腹腔注射8个周期的化疗药物(顺铂8 mg/kg+依托泊苷5mg/kg),定期监测荷瘤小鼠体质量和肿瘤体积,对长至1,000 mm^3大小的肿瘤进行传代移植。结果本研究收集并移植9例SCLC肿瘤标本,成功构建可多次传代SCLC PDX模型3例并成功诱导其耐药模型,模型较好地保留了原发肿瘤的特征。结论成功构建了SCLC PDX模型及其耐药模型,建模成功率为33%。为后续研究人的SCLC耐药机制、临床药物筛选以及个体化治疗提供了实验平台。 展开更多

[Objectives]To promote the development of the ramie industry in Dazhou City of Sichuan Province and provide a material basis for the breeding of new ramie varieties.[Methods]The Institute of Bast Fiber Crops of Dazhou Academy of Agricultural Sciences performed a screening test of excellent ramie germplasm resources from 2017 to 2019 to compare the growth,resistance,raw fiber yield and fiber fineness of the experimental materials.[Results]All the experimental materials showed strong growth potential,drought resistance and disease resistance.There were 8 kinds of resources with a fiber fineness greater than 2000 m/g,of which 3 kinds of resource materials had a fiber fineness greater than 2300 m/g;12 kinds of resources has the raw fiber yield exceeding the control and 10 kinds of resources had the raw fiber yield≥2000 kg/ha;3 kinds of resources met the requirements of the high-yield and high-quality indicators(fiber fineness exceeding 2000 m/g and raw fiber yield≥2000 kg/ha),they were BD0718,BD1614 and BYL2.[Conclusions]These high-quality ramie resources can provide a rich resource base for the breeding of new ramie varieties. 展开更多

Hydroxyapatite(HAP)is a common bio-adsorbent,which performance depends heavily upon its morphology and microporous structure.In this study,a novel synthesis strategy was proposed for hierarchical porous HAP microspheres by a simple"one-pot"hydrothermal reaction.In the strategy,L-glutamic acid serves as soft template to modulate the morphology and inner crystalline of HAP.To evaluate the application potential,doping Ni^(2+) on hierarchical porous HAP microspheres gives metal chelated affinity adsorbents.The prepared adsorbents show a perfect spherical shape,particles size of 96.6μm,relatively specific surface area of 48.5 m^(2)·g^(-1) and hierarchical pores(mesopores:4 nm and macropores:53 nm).By the adsorption evaluation,it reveals that the Ni^(2+)-HAP adsorbents have high adsorption capacities of275.11 and 97.55 m^(2)·g^(-1) for hemoglobin and bovine serum albumin,respectively,which is comparable to other similar adsorbent.Therefore,this work provides a promising method for high-efficiency hydroxyapatite microspheres for proteins purification. 展开更多

“有志不在年高”:我国企业职场成功老龄化诱发因素及多元驱动路径研究

随着我国人口老龄化程度不断加深,企业劳动力老龄化将成为职场常态。如何促进企业年长员工实现职场成功老龄化成为人力资源管理的热点,对提升组织绩效,开发老龄员工人力资本价值具有重要意义。本文扎根于企业案例,首先将内容分析法应用... 随着我国人口老龄化程度不断加深,企业劳动力老龄化将成为职场常态。如何促进企业年长员工实现职场成功老龄化成为人力资源管理的热点,对提升组织绩效,开发老龄员工人力资本价值具有重要意义。本文扎根于企业案例,首先将内容分析法应用于本土企业管理情境分析,其次以职场成功老龄化为结果变量,个体因素、企业因素及家庭社会因素为前因变量,运用模糊集定性比较分析法探索我国企业职场成功老龄化的前因条件组合与多元驱动路径。研究发现:(1)个体因素(主动选择及优化和补偿、投入心理资本、强化个人能力)、企业因素(工作环境、工作设计、教育培训)和家庭社会因素(家庭工作平衡、给予后代支持)是诱发职场成功老龄化的前因条件,其中主动选择及优化和补偿、投入心理资本和工作设计是必要条件;(2)7种驱动实现企业职场成功老龄化的条件组态可划分为三类路径,分别是“个体-组织强响应协同驱动路径”“组织强主导-个体中响应驱动路径”和“个体-组织弱响应互动驱动路径”,其中工作设计是三类路径中唯一共同核心条件,次核心条件为工作环境、主动选择及优化和补偿、投入心理资本。强化个人能力、教育培训、家庭工作平衡、给予后代支持等其他前因条件作为边缘条件存在或缺失,起到的促进作用有限。最后,本文基于模糊集定性比较分析结论提出促进实现企业职场成功老龄化的建议。 展开更多

Intrauterine adhesion(IUA) is a common cause of uterine infertility and one of the most severe clinical features is endometrial fibrosis namely endometrial scarring for which there are few cures currently. Blocked angiogenesis is the main pathological change in the scarred endometrium. The fibroblast growth factor 2(b FGF), a member of FGF family, is usually applied to promote healing of refractory ulcer and contributes to angiogenesis of tissues. In this study, the sustained-release system of b FGF100 μg was administrated around scarred endometrium guiding by ultrasound every 4 weeks in 18 patients(2–4 times). Results showed that after treatment, the menstrual blood volume, endometrial thickness and the scarred endometrial area were improved.Histological study showed blood vessel density increased obviously. Three patients(3/18) achieved pregnancy over 20 gestational weeks. Therefore, administrating the b FGF surrounding scarred endometrium may provide a new therapeutic approach for the patients with endometrial fibrosis. 展开更多

Labor-saving preparation of highly active electrocatalysts for alcohols oxidation, especially for the ethylene glycol and glycerol electrooxidation is of great importance for the development of fuel cells. Herein, mesocellular graphene foam (MGF) constructed by ultrathin nanosheets were prepared using lamellar MCM-22 zeolite as template and then ultra-small Pd NPs were facile grew on it via the stabilizer-free synthesis. Detailed characterizations showed that the obtained Pd/MGF had large surface area, hierarchical porous architecture, semi-graphitic framework and dispersed Pd NPs an chori ng. Electrochemical measurements dem on strated that Pd/MGF possessed the higher catalytic activity (1.7-2.9 fold higher) and stability for the different alcohols electrooxidation, especially for the ethylene glycol and glycerol electrooxidation in the alkaline solution, than the commercial Pd/C (10 wt.%) catalyst. Reaction kinetics analysis revealed the expanded diffusion-controlled process of Pd/MGF as compared to Pd/C. These findings promised a potential electrocatalyst for direct alcohol fuel cells (DAFC), especially for the direct ethylene glycol or glycerol fuel cells with high energy density. 展开更多

Single atom catalyst is of great importance for the oxygen reduction reaction(ORR).However,facile preparation of single atom catalyst without using well-designed precursors or labor-intensive acid leaching remains an urgent challenge.Herein,a simple pyrolysis of Fe3+-loaded mesoporous phenolic resin(mPF)-melamine precursor is used to prepare the single atom iron-anchored N-doped mesoporous graphitic carbon nanospheres(Fe/N-MGN).Investigation of the synthesis reveals the appropriate Fe-assisted catalysis effect and mPF template effect,which not only spurs the highly graphitic porous framework of Fe/N-MGN with plentiful pyridinic N/graphitic N,but also assures the dispersed single atom Fe anchoring without elaborated procedures.As a result,the as-synthesized Fe/N-MGN demonstrates high catalytic activity,good durability and excellent methanol tolerance for ORR.This work promises a facile method to regulate the graphitic carbon growth and single atom Fe loading for the highly efficient electrocatalysis. 展开更多

In this paper,we consider the recovery of block sparse signals,whose nonzero entries appear in blocks (or clusters)rather than spread arbitrarily throughout the signal,from incomplete linear measurements.A high order sufficient condition based on block RIP is obtained to guarantee the stable recovery of all block sparse signals in the presence of noise,and robust recovery when signals are not exactly block sparse via mixed l2/l1 minimization.Moreover,a concrete example is established to ensure the condition is sharp.The significance of the results presented in this paper lies in the fact that recovery may be possible under more general conditions by exploiting the block structure of the sparsity pattern instead of the conventional sparsity pattern. 展开更多
