BACKGROUND Gastric cystica profunda(GCP)represents a rare condition characterized by cystic dilation of gastric glands within the mucosal and/or submucosal layers.GCP is often linked to,or may progress into,early gastric cancer(EGC).AIM To provide a comprehensive evaluation of the endoscopic features of GCP while assessing the efficacy of endoscopic treatment,thereby offering guidance for diagnosis and treatment.METHODS This retrospective study involved 104 patients with GCP who underwent endoscopic resection.Alongside demographic and clinical data,regular patient followups were conducted to assess local recurrence.RESULTS Among the 104 patients diagnosed with GCP who underwent endoscopic resection,12.5%had a history of previous gastric procedures.The primary site predominantly affected was the cardia(38.5%,n=40).GCP commonly exhibited intraluminal growth(99%),regular presentation(74.0%),and ulcerative mucosa(61.5%).The leading endoscopic feature was the mucosal lesion type(59.6%,n=62).The average maximum diameter was 20.9±15.3 mm,with mucosal involvement in 60.6%(n=63).Procedures lasted 73.9±57.5 min,achieving complete resection in 91.3%(n=95).Recurrence(4.8%)was managed via either surgical intervention(n=1)or through endoscopic resection(n=4).Final pathology confirmed that 59.6%of GCP cases were associated with EGC.Univariate analysis indicated that elderly males were more susceptible to GCP associated with EGC.Conversely,multivariate analysis identified lesion morphology and endoscopic features as significant risk factors.Survival analysis demonstrated no statistically significant difference in recurrence between GCP with and without EGC(P=0.72).CONCLUSION The findings suggested that endoscopic resection might serve as an effective and minimally invasive treatment for GCP with or without EGC. 展开更多

Effects of dense planting patterns on photosynthetic traits of different vertical layers and yield of wheat under different nitrogen rates

A two-year field experiment was conducted to measure the effects of densification methods on photosynthesis and yield of densely planted wheat.Inter-plant and inter-row distances were used to define ratefixed pattern(RR)and row-fixed pattern(RS)density treatments.Meanwhile,four nitrogen(N)rates(0,144,192,and 240 kg N ha-1,termed N0,N144,N192,and N240)were applied with three densities(225,292.5,and 360×10^(4)plants ha^(-1),termed D225,D292.5,and D360).The wheat canopy was clipped into three equal vertical layers(top,middle,and bottom layers),and their chlorophyll density(Ch D)and photosynthetically active radiation interception(FIPAR)were measured.Results showed that the response of Ch D and FIPAR to N rate,density,and pattern varied with different layers.N rate,density,and pattern had significant interaction effects on Ch D.The maximum values of whole-canopy Ch D in the two seasons appeared in N240 combined with D292.5 and D360 under RR,respectively.Across two growing seasons,FIPAR values of RR were higher than those of RS by 29.37%for the top layer and 5.68%for the middle layer,while lower than those of RS by 20.62%for the bottom layer on average.With a low N supply(N0),grain yield was not significantly affected by density for both patterns.At N240,increasing density significantly increased yield under RR,but D360 of RS significantly decreased yield by 3.72%and 9.00%versus D225 in two seasons,respectively.With an appropriate and sufficient N application,RR increased the yield of densely planted wheat more than RS.Additionally,the maximum yield in two seasons appeared in the combination of D360 with N144 or N192 rather than of D225 with N240 under both patterns,suggesting that dense planting combined with an appropriate N-reduction application is feasible to increase photosynthesis capacity and yield. 展开更多

Nocardiosis manifests as an opportunistic infection,primarily affecting individuals who are immunocompromised and susceptible to the infection.We present a case study of one patient with nephrotic syndrome and membranous nephropathy,who underwent treatment with prednisone and cyclosporine in 2016.In early 2017,the patient was diagnosed with a"fungal infection"and discontinued the use of cyclosporine.After one month of anti-infection therapy,a cranial magnetic resonance imaging scan showed multiple abscesses in the right temporal region.The diagnosis of nocardiosis was confirmed based on the presence of metastatic abscess masses,multiple lung and brain lesions,and a positive culture of Nocardia in the drainage.We changed the anti-infection therapy to a combination of trimethoprim-sulfamethoxazole(TMP-SMX),minocycline,and voriconazole.However,the patient experienced a sudden cardiac arrest and subsequently recovered after cardiopulmonary resuscitation.During the five-month follow-up period following the discharge,the patient displayed an enhanced nutritional status and stable renal function.The focal infection ultimately resolved during the subsequent three years.Neuro-infection caused by Nocardia should be considered in immunocompromised patients,and TMP-SMX is the preferred initial therapy;however,because of the high mortality rate,a long-term combination therapy with imipenem,cefotaxime,amikacin,and TMP-SMX is suggested. 展开更多

BACKGROUND Postoperative pain management and cognitive function preservation are crucial for patients undergoing thoracoscopic surgery for lung cancer(LC).This is achieved using either a thoracic paravertebral block(TPVB)or sufentanil(SUF)-based multimodal analgesia.However,the efficacy and impact of their combined use on postoperative pain and postoperative cognitive dysfunction(POCD)remain unclear.AIM To explore the analgesic effect and the influence on POCD of TPVB combined with SUF-based multimodal analgesia in patients undergoing thoracoscopic radical resection for LC to help optimize postoperative pain management and improve patient outcomes.METHODS This retrospective analysis included 107 patients undergoing thoracoscopic radical resection for LC at The Affiliated Cancer Hospital of Zhengzhou University and Henan Cancer Hospital between May 2021 and January 2023.Patients receiving SUF-based multimodal analgesia(n=50)and patients receiving TPVB+SUF-based multimodal analgesia(n=57)were assigned to the control group and TPVB group,respectively.We compared the Ramsay Sedation Scale and visual analog scale(VAS)scores at rest and with cough between the two groups at 2,12,and 24 h after surgery.Serum levels of epinephrine(E),angio-tensin Ⅱ(Ang Ⅱ),norepinephrine(NE),superoxide dismutase(SOD),vascular endothelial growth factor(VEGF),transforming growth factor-β1(TGF-β1),tumor necrosis factor-α(TNF-α),and S-100 calcium-binding proteinβ(S-100β)were measured before and 24 h after surgery.The Mini-Mental State Examination(MMSE)was administered 1 day before surgery and at 3 and 5 days after surgery,and the occurrence of POCD was monitored for 5 days after surgery.Adverse reactions were also recorded.RESULTS There were no significant time point,between-group,and interaction effects in Ramsay sedation scores between the two groups(P>0.05).Significantly,there were notable time point effects,between-group differences,and interaction effects observed in VAS scores both at rest and with cough(P<0.05).The VAS scores at rest and with cough at 12 and 24 h after surgery were lower than those at 2 h after surgery and gradually decreased as postoperative time increased(P<0.05).The TPVB group had lower VAS scores than the control group at 2,12,and 24 h after surgery(P<0.05).The MMSE scores at postoperative days 1 and 3 were markedly higher in the TPVB group than in the control group(P<0.05).The incidence of POCD was significantly lower in the TPVB group than in the control group within 5 days after surgery(P<0.05).Both groups had elevated serum E,Ang Ⅱ,and NE and decreased serum SOD levels at 24 h after surgery compared with the preoperative levels,with better indices in the TPVB group(P<0.05).Marked elevations in serum levels of VEGF,TGF-β1,TNF-α,and S-100β were observed in both groups at 24 h after surgery,with lower levels in the TPVB group than in the control group(P<0.05).CONCLUSION TPVB combined with SUF-based multimodal analgesia further relieves pain in patients undergoing thoracoscopic radical surgery for LC,enhances analgesic effects,reduces postoperative stress response,and inhibits postoperative increases in serum VEGF,TGF-β1,TNF-α,and S-100β levels.This scheme also reduced POCD and had a high safety profile. 展开更多

BACKGROUND Calcifying fibrous tumors(CFTs)are rare mesenchymal lesions that can occur in various sites throughout the body,including the tubular gastrointestinal(GI)tract.AIM To analyze the clinical findings of 36 patients with GI tract CFTs to provide guidance for diagnosis and treatment.METHODS This retrospective study included 36 patients diagnosed with CFTs of the GI tract.We collected demographic and clinical information and conducted regular follow-ups to assess for local recurrence.RESULTS The stomach was the most commonly involved site,accounting for 72.2%of the 36 CFTs.Endoscopic mucosal resection(n=1,2.8%),endoscopic submucosal dissection(n=14,38.9%),endoscopic full-thickness resection(n=16,44.4%),and submucosal tunneling endoscopic resection(n=5,13.9%)were used to resect calcifying fibrous tumors.Overall,34(94.4%)CFTs underwent complete endoscopic resections with a mean procedure time of 39.8±29.8 min.The average maximum diameter of the tumors was 10.6±4.3 cm.No complications,such as bleeding or perforation,occurred during an average hospital stay of 2.9±1.2 d.In addition,two patients developed new growth of CFTs near the primary tumor sites,and none of the patients developed distant metastases during the follow-up period.CONCLUSION GI tract CFTs are rare and typically benign tumors that can be effectively managed with endoscopic procedures. 展开更多

In the present work, a novel analytical method was proposed for the determination of toluene diisocyanate (TDI) in syntheticrubber track by ion chromatography (IC) coupled with an ultraviolet detector setting at 212 nm. TDI can be hydrolyzed to toluene diamine (TDA) which can be separated by cation-exchange IC easily. The optimum IC separation was performed on an IonPac CS12A column (150 mm ×4.0 mm) using 20 mmol L^-1 sodium sulfate, 10 mmol L^-1 sulfuric acid and 10% acetonitrile as eluent. It was found that a higher signal response of TDA could be obtained under alkaline condition. A suppressor was used to change the acidic eluent into alkaline one. 0.8 mol L^-1 potassium hydroxide was chosen as the optimum regeneration eluent. With the added suppressor and regenerant, signal response was magnified by about 16 times and lower limit of detection (LOD, 0.13 μg L^-1) was obtained. Within-day relative standard deviation (R.S.D.) was less than 3.6%. The recoveries of TDI spiked in synthetic-rubber track samoles were 96.4-110.6%. 展开更多

Observing and timing a group of millisecond pulsars with high rotational stability enables the direct detection of gravitational waves(GWs).The GW signals can be identified from the spatial correlations encoded in the times-of-arrival of widely spaced pulsar-pairs.The Chinese Pulsar Timing Array(CPTA)is a collaboration aiming at the direct GW detection with observations carried out using Chinese radio telescopes.This short article serves as a“table of contents”for a forthcoming series of papers related to the CPTA Data Release 1(CPTA DR1)which uses observations from the Five-hundred-meter Aperture Spherical radio Telescope.Here,after summarizing the time span and accuracy of CPTA DR1,we report the key results of our statistical inference finding a correlated signal with amplitude logA_(c)=-14.4_(-2.8)^(+1.0)for spectral index in the range ofα∈[-1.8,1.5]assuming a GW background(GWB)induced quadrupolar correlation.The search for the Hellings–Downs(HD)correlation curve is also presented,where some evidence for the HD correlation has been found that a 4.6σstatistical significance is achieved using the discrete frequency method around the frequency of 14 n Hz.We expect that the future International Pulsar Timing Array data analysis and the next CPTA data release will be more sensitive to the n Hz GWB,which could verify the current results. 展开更多

目的探讨生大黄水煎液灌肠联合哌拉西林他唑巴坦治疗深Ⅱ度烧伤并脓毒症的临床疗效。方法选取医院2022年1月至12月收治的深Ⅱ度烧伤并脓毒症患者62例,按治疗方案的不同分为对照组(36例)和观察组(26例)。两组患者均给予补充血容量、营养... 目的探讨生大黄水煎液灌肠联合哌拉西林他唑巴坦治疗深Ⅱ度烧伤并脓毒症的临床疗效。方法选取医院2022年1月至12月收治的深Ⅱ度烧伤并脓毒症患者62例,按治疗方案的不同分为对照组(36例)和观察组(26例)。两组患者均给予补充血容量、营养支持、增强免疫等常规综合辅助治疗,同时予注射用哌拉西林钠他唑巴坦钠静脉滴注。观察组患者加用生大黄水煎液灌肠。两组均连续治疗7 d。结果观察组总有效率为61.54%,明显高于对照组的41.67%(P<0.05)。治疗7 d后,两组患者急性生理学和慢性健康状况评价Ⅱ评分、序贯器官衰竭评分及降钙素原、C反应蛋白、肿瘤坏死因子-α、白细胞介素6水平均明显降低(P<0.05),且观察组明显低于对照组(P<0.05),免疫球蛋白(Ig)M、T淋巴细胞亚群(CD_(4)^(+)、CD_(8)^(+))水平及CD_(4)^(+)/CD_(8)^(+)均明显升高(P<0.05),且除CD_(8)^(+)外,观察组其余指标均明显高于对照组(P<0.05)。两组不良反应发生率相当(P>0.05)。结论生大黄水煎液灌肠联合哌拉西林他唑巴坦治疗深Ⅱ度烧伤并脓毒症,可有效缓解患者的临床症状,保护各器官系统功能,减轻炎性反应,改善免疫功能。 展开更多

心肌梗死在外科手术治疗中常伴随着心肌缺血再灌注损伤(MI/RI),影响术后患者的正常生活。丹红注射液(DHI)已在临床中被广泛用来治疗冠心病和心绞痛等疾病,但其对MI/RI的治疗效果仍需进一步研究。本研究旨在探讨DHI抗MI/RI的药效作用成... 心肌梗死在外科手术治疗中常伴随着心肌缺血再灌注损伤(MI/RI),影响术后患者的正常生活。丹红注射液(DHI)已在临床中被广泛用来治疗冠心病和心绞痛等疾病,但其对MI/RI的治疗效果仍需进一步研究。本研究旨在探讨DHI抗MI/RI的药效作用成分及其作用机制。通过化学分离的方法,从DHI中得到其中的初生代谢物组分(PM)和次生代谢物组分(SM),通过建立大鼠心肌缺血再灌注损伤模型并分别给药,验证DHI、PM和SM的药效作用。实验结果表明DHI、PM和SM能够纠正扩张的心脏结构,还可以下调补体C2的表达以减轻炎症反应,通过上调环氧合酶(COX)的表达减少活性氧(ROS)的产生以及通过抑制脂肪酸代谢和刺激糖代谢恢复正常能量供应来改善心脏功能。此外,DHI和SM还可以通过下调Ca^(2+)/钙调素依赖性蛋白激酶II(CaMKII)的表达来减轻因钙超载引起的炎症反应和氧化应激。这些发现可能为中药注射剂的质量控制和安全性评价提供依据,并为心血管疾病的预防和治疗提供新思路。 展开更多

The Five-hundred-meter Aperture Spherical radio Telescope(FAST)has been running for several years.A new ultra-wide bandwidth(UWB)receiver,simultaneously covering 500-3300 MHz,has been mounted in the FAST feed cabin and has passed a series of observational tests.The whole UWB band is separated into four independent bands.Each band has 1,048,576 channels in total,resulting in a spectral resolution of 1 kHz.At 500-3300 MHz,the antenna gain is around 14.3-7.7 K Jy^(-1),the aperture efficiency is around 0.56-0.30,the system temperature is around 88-130 K,and the half-power beamwidth is around 7.6'-1.6'.The measured standard deviation of pointing accuracy is better than~7.9"when zenith angle is within 26.4°.The sensitivity and stability of the UWB receiver are confirmed to satisfy expectations through spectral observations,e.g.,HI and OH.The FAST UWB receiver has already demonstrated good performance in capturing sensitive observations for various scientific goals. 展开更多

Chinese guideline has proven effective in the fight against coronavirus disease 2019(COVID-19)during the global spread of the epidemic.Traditional Chinese medicine(TCM)is widely recognized for its effectiveness in alleviating symptoms,inhibiting disease progress,reducing mortality,and improving the cure rate of patients with COVID-19.During the pandemic,“three medicines and three formulas”(TMTFs)stood out from hundreds of others in registered clinical studies,which became highly recommended as TCM for COVID-19 treatment.The TMTFs not only effectively relieve the clinical symptoms of fever,cough,fatigue,and phlegm,but also substantially shorten the time of nucleic acid negative conversion,improve lung computed tomography imaging features and inflammation,ameliorate clinical biochemical indicators,and reduce sequelae.Their potential pharmacological mechanisms are mainly related to the crosstalk between viral toxicity,endothelial damage,cytokine storm,immune response,and microthrombus.In brief,the clinical effects and the potential mechanisms of TMTFs on COVID-19 were systematically analyzed and summarized covering the entirety of disease development,including virus invasion and replication,immune response and cytokine storm,and acute respiratory distress syndrome and multiple organ dysfunction syndrome.This review provides a theoretical basis and reference for the in-depth understanding of the positive role of TMTFs in COVID-19 treatment. 展开更多

This paper presents the preliminary simulation results of the prototype 40-element phased array feed(PAF)array for the Five-hundred-meter Aperture Spherical radio Telescope(FAST)and the measurement of a manufactured single Vivaldi element.The simulation results include the characteristic of a standalone Vivaldi element.The full-wave simulation of the explicit PAF array is carried out,from which the performance of the embedded elements and the mutual coupling among the elements could be accurately inferred.The performance of the combination of the PAF and the main reflector of FAST is calculated.The results imply that the PAF combined with the FAST antenna could operate over a much wider bandwidth from 650 to 1450 MHz compared with the standalone element.The antenna temperature remains almost the same for zenith angles from 0 to 40°.From the simulation results,the PAF shows good potential to improve the performance of FAST.A measurement of a manufactured Vivaldi element is also presented. 展开更多

掺杂金属纳米团簇的光学性质因为其在生物探针和光催化等方向的应用受到广泛关注.MAu_(24)(SR)_(18)团簇的光致发光和激发态过程复杂,其机制尚不清楚.本文利用超快光谱探究了两种掺杂金属纳米团簇MAu_(24)(SR)_(18)(M=Pd,Pt,SR为苯乙硫... 掺杂金属纳米团簇的光学性质因为其在生物探针和光催化等方向的应用受到广泛关注.MAu_(24)(SR)_(18)团簇的光致发光和激发态过程复杂,其机制尚不清楚.本文利用超快光谱探究了两种掺杂金属纳米团簇MAu_(24)(SR)_(18)(M=Pd,Pt,SR为苯乙硫醇)的激子和相干振动动力学.与未掺杂的Au_(25)(SR)_(18)团簇相比,Pd和Pt掺杂导致激子寿命显著降低几个数量级.研究发现PtAu_(24)(5 ps)和PdAu_(24)(30 ps)超短的激子寿命来源于超小的能隙(Eg=0.3eV).在两个掺杂金属团簇中,都观测到显著的来自金核的相干振荡(2.4 THz),表明这些相干振荡在超短激子寿命下依然能稳定存在.理解掺杂金属纳米团簇的激子和振动动力学将为设计这些类似分子的纳米材料提供新思路. 展开更多

Non-rigid registration of point clouds is still far from stable,especially for the largely deformed one.Sparse initial correspondences are often adopted to facilitate the process.However,there are few studies on how to build them automatically.Therefore,in this paper,we propose a robust method to compute such priors automatically,where a global and local combined strategy is adopted.These priors in different degrees of deformation are obtained by the locally geometrical-consistent point matches from the globally structural-consistent region correspondences.To further utilize the matches,this paper also proposes a novel registration method based on the Coherent Point Drift framework.This method takes both the spatial proximity and local structural consistency of the priors as supervision of the registration process and thus obtains a robust alignment for clouds with significantly different deformations.Qualitative and quantitative experiments demonstrate the advantages of the proposed method. 展开更多
