METHODS: Patients with superficial lesions no larger than 1.0 cm found during h...AIM: To evaluate the feasibility of a new computed virtual chromoendoscopy (CVC) device (M i-scan) in the diagnosis of gastric neoplasia. METHODS: Patients with superficial lesions no larger than 1.0 cm found during high definition endoscopy were included. Those with advanced or obviously protruded or depressed lesions, lesions larger than 1.0 cm and/or lesions which were not amenable to observation by zoom function were excluded. The endoscopist was required to give the real-time descriptions of surface pit patterns of the lesions, based on surface pattern classification of enhanced magnification endoscopy. According to previous reports, types Ⅰ-Ⅲ represent nonneoplastic lesions, and types Ⅳ-Ⅴ represent neoplastic lesions. Diagnosis with M i-scan and biopsy was performed before histopathological diagnosis. Magnified images of gastric lesions with and without enhancement were collected for further analysis. The diagnostic yield of real-time M i-scan and effects on magnification image quality by tone enhancement (TE), surface enhancement (SE) and color enhancement (CE) were calculated. The selected images were sent to another endoscopist. The endoscopist rated the image quality of each lesion at 3 levels. Ratings of image quality were based on visualization of pit pattern, vessel and demarcation line. RESULTS: One hundred and eighty-three patients were recruited. Five patients were excluded for advanced gastric lesions, 1 patient was excluded for poor preparation and 2 patients were excluded for superficial lesions larger than 1.0 cm; 132 patients were excluded for no lesions found by high definition endoscopy. In the end, 43 patients with 43 lesions were included. Histopathology revealed 10 inflammation, 14 atrophy, 10 metaplasia, 1 low grade dysplasia (LGD), 5 high grade dysplasia (HGD) and 3 cancers. For 7 lesions classified into type Ⅰ, histopathology revealed 6 atrophy and 1 metaplasia; for 10 lesions classified into type Ⅱ, histopathology revealed 2 inflammation, 7 atrophy and 1 metaplasia; for 10 lesions classified into type Ⅲ, histopathology revealed 1 inflammation, 8 metaplasia and 1 LGD; for 9 lesions classified into type Ⅳ, histopathology revealed 4 inflammation, 1 atrophy and 4 HGD; for 7 lesions classified into type Ⅴ, histopathology revealed 3 inflammation, 1 HGD and 3 cancers. A total of 172 still images, including 43 images by white light (MWL) and 129 images by M i-scan (43 with TE, 43 with SE and 43 with CE), were selected and sent to the endoscopist who did the analysis. General image quality of M i-scan with TE and SE was significantly better than that of MWL (TE, 4.55 ± 1.07; SE, 4.30 ± 1.02; MWL, 3.25 ± 0.99; P < 0.001). Visualization of pit pattern was significantly improved by M i-scan with SE (1.93 ± 0.25 vs 1.50 ± 0.50, P < 0.001). Microvessel visualization was significantly improved by M i-scan with TE (1.23 ± 0.78 vs 0.76 ± 0.73, P < 0.001). Demarcation line visualization was improved by M i-scan with both TE and SE (TE, 1.75 ± 0.52; SE, 1.56 ± 0.59; MWL, 0.98 ± 0.44; P < 0.001). M i-scan with CE did not show any significant improvements of image quality in general or in the 3 key parameters. Although M i-scan with TE and SE slightly increased the diagnostic yield of MWL, there was no significant difference (P > 0.1). CONCLUSION: Although digital enhancement improves the image quality of magnification endoscopy, its value in improving the diagnostic yield seems to be limited.展开更多

Subsequently, we down-regulated and upregulated Smoc2 expression using siR NA and lentivirus transfection assay, respectively. Then, we identified the effect of Smoc2 on cell proliferation and cell cycle using CCK-8 and flow cytometry, respectively. The common cell growth signaling influenced by Smoc2 was detected by western blot assay. RESULTS The expression of Smoc2 was significantly higher in HCC tissues compared with CNL tissues. Overexpression of Smoc2 promoted HCC cell proliferation and cell cycle progression. Down-regulation of Smoc2 led to inhibition of cell proliferation and cell cycle progression. Smoc2 had positive effect on ERK and AKT signaling.CONCLUSION Smoc2 promotes the proliferation of HCC cells through accelerating cell cycle progression and might act as an anti-cancer therapeutic target in the future.展开更多

Tests showed decreased levels of hem...Here, we report a rare case of primary gastrointestinal amyloidosis in a stable condition after being followed up for three years. The patient was admitted to the hospital in 2014. Tests showed decreased levels of hemoglobin and ferritin. Transoral and transanal enteroscopy showed multiple nodular protuberances in the esophagus, ileum, colon and rectum. Endoscopic ultrasonography indicated the nodular protuberances stemmed from the submucosa and partially invaded the intrinsic myometrium. Pathological examinations found multiple small nodules in the submucosa and dyed structures, which were positive for special Congo red dyeing. After treatment with oral iron supplements, the levels of hemoglobin and ferritin became normal. It is concluded that the patient represents a case of primary gastrointestinal amyloidosis with multiple nodular protuberances in the digestive tract with controllable moderate abdominal discomfort and anemia and a benign course. Enteroscopy and endoscopic ultrasonography play an important role in the diagnosis of primary gastrointestinal amyloidosis.展开更多

The Navier-Stokes equations are used to consider the inertia term along the flow direction. A novel contour integral method is used to solve the complex Airy function. The boundary conditions of linear gradient flow distribution for finite problems are determined. The vorticity function, the pressure function, and the turbulent velocity profiles are provided, and the stability of particle trajectories is studied. An Lx-function form of the third derivative circulation is used to to simplify the solution. Theoretical results are compared with the experimental measurements with satisfactory agreement.展开更多

AIM: To evaluate gut microbial dysbiosis in two visceral hypersensitive models in comparison with irritable bowel syndrome(IBS) patients and to explore the extent to which these models capture the dysbiosis of IBS pat...AIM: To evaluate gut microbial dysbiosis in two visceral hypersensitive models in comparison with irritable bowel syndrome(IBS) patients and to explore the extent to which these models capture the dysbiosis of IBS patients. METHODS: Visceral hypersensitivity was developed using the maternal separation(MS) rat model and post-inflammatory rat model. The visceral sensitivity of the model groups and control group was evaluated using the abdominal withdraw reflex score and electromyography in response to graded colorectal distention. The 16 S ribosomal RNA gene from fecal samples was pyrosequenced and analyzed. The correlation between dysbiosis in the microbiota and visceral hypersensitivity was calculated. Positive findings were compared to sequencing data from a published human IBS cohort.RESULTS: Dysbiosis triggered by neonatal maternal separation was lasting but not static. Both MS and postinflammatory rat fecal microbiota deviated from that of the control rats to an extent that was larger than the co-housing effect. Two short chain fatty acid producing genera, Fusobacterium and Clostridium XI, were shared by the human IBS cohort and by the maternal separation rats and post-inflammatory rats, respectively, to different extents. Fusobacterium was significantly increased in the MS group, and its abundance positively correlated with the degree of visceral hypersensitivity. Porphyromonadaceae was a protective biomarker for both the rat control group and healthy human controls. CONCLUSION: The dysbiosis MS rat model and the post-inflammatory rat model captured some of the dysbiosis features of IBS patients. Fusobacterium, Clostridium XI and Porphyromonadaceae were identified as targets for future mechanistic research.展开更多
AIM:To compare the endomicroscopic image quality of integrated confocal laser endomicroscopy(iCLE)and sedation efficacy of propofolvs midazolam plus fentanyl(M/F).METHODS:Consecutive outpatients undergoing iCLE were p...AIM:To compare the endomicroscopic image quality of integrated confocal laser endomicroscopy(iCLE)and sedation efficacy of propofolvs midazolam plus fentanyl(M/F).METHODS:Consecutive outpatients undergoing iCLE were prospectively recruited and randomized to the propofol group(P group)or M/F group.The patient,performing endoscopist and endoscopic assistant were blinded to the randomization.The quality of endomicro-scopic images and anesthetic efficacy outcomes were blindly evaluated after iCLE examination.RESULTS:There were significantly more good quality endomicroscopic images in the propofol group than in the M/F group(72.75%vs 52.89%,P<0.001).The diagnostic accuracy for upper gastrointestinal mucosal lesions using confocal laser endomicroscopy favors the P group,although this did not reach statistical significance.Adverse events and patient assessment were not significantly different for M/F vs propofol except for more frequent intraprocedural recall with M/F.Procedure duration and sedation times were significantly longer in the M/F group,while the scores of endoscopist,anesthetist and assistant assessment were all significantly better in the P group.CONCLUSION:Sedation with propofol might increase the proportion of good quality endomicroscopic images,and may result in improved procedural efficacy and diagnostic accuracy during iCLE examination.展开更多

Accurate description of the evolution of mechanical properties of soils can help to reveal the internal behavior of pore water. Based on the idea of using the fractional order to reflect mechanical properties of soils, a fractional creep model is proposed by introducing a variable-order fractional operator, and realized on a series of creep responses in soft soils. A comparative analysis illustrates that the evolution of mechanical properties, shown through the simulated results, exactly corresponds to the motion of pore water and the solid skeleton. This demonstrates that the proposed variable-order fractional model can be employed to characterize the evolution of mechanical properties of and the pore water motion in soft soils during creep. It is observed that the fractional order from the proposed model is related to the dissipation rate of pore water pressure.展开更多

BACKGROUND Major societies provide differing guidance on management of Barrett’s esophagus(BE),making standardization challenging.AIM To evaluate the preferred diagnosis and management practices of BE among Asian end...BACKGROUND Major societies provide differing guidance on management of Barrett’s esophagus(BE),making standardization challenging.AIM To evaluate the preferred diagnosis and management practices of BE among Asian endoscopists.METHODS Endoscopists from across Asia were invited to participate in an online questionnaire comprising eleven questions regarding diagnosis,surveillance and management of BE.RESULTS Five hundred sixty-nine of 1016(56.0%)respondents completed the survey,with most respondents from Japan(n=310,54.5%)and China(n=129,22.7%).Overall,the preferred endoscopic landmark of the esophagogastric junction was squamocolumnar junction(42.0%).Distal palisade vessels was preferred in Japan(59.0%vs 10.0%,P<0.001)while outside Japan,squamo-columnar junction was preferred(59.5%vs 27.4%,P<0.001).Only 16.3%of respondents used Prague C and M criteria all the time.It was never used by 46.1%of Japanese,whereas 84.2%outside Japan,endoscopists used it to varying extents(P<0.001).Most Asian endoscopists(70.8%)would survey long-segment BE without dysplasia every two years.Adherence to Seattle protocol was poor with only 6.3%always performing it.73.2%of Japanese never did it,compared to 19.3%outside Japan(P<0.001).The most preferred(74.0%)treatment of non-dysplastic BE was proton pump inhibitor only when the patient was symptomatic or had esophagitis.For BE with low-grade dysplasia,6-monthly surveillance was preferred in 61.9%within Japan vs 47.9%outside Japan(P<0.001).CONCLUSION Diagnosis and management of BE varied within Asia,with stark contrast between Japan and outside Japan.Most Asian endoscopists chose squamo-columnar junction to be the landmark for esophagogastric junction,which is incorrect.Most also did not consistently use Prague criteria,and Seattle protocol.Lack of standardization,education and research are possible reasons.展开更多
AIM: To investigate the potential differences between topography-guided(TG) and wavefront-optimized(WFO) laser in situ keratomileusis(LASIK) for the treatment of myopia.METHODS: A systematic literature search was perf...AIM: To investigate the potential differences between topography-guided(TG) and wavefront-optimized(WFO) laser in situ keratomileusis(LASIK) for the treatment of myopia.METHODS: A systematic literature search was performed to determine relevant trials comparing LASIK with TG and WFO from the time of library construction to August 2020, and The PubMed, Cochrane, Web of Science, EMBASE and Chinese databases(i.e. CNKI, CBM, WAN FANG and VIP) were accessed. The data on visual acuity, refractive status and wavefront aberration were retrieved and evaluated from three to six months after surgery. STATA(version 14.0) software was used for statistical analysis. A cumulative Meta-analysis was simultaneously performed.RESULTS: Eleven studies with a total of 1425 eyes were incorporated. No statistically significant differences were evident between TG and WFO ablation in the proportion of eyes achieving an uncorrected distance visual acuity(UCVA) of 20/20 or better(P=0.377), gaining one line or more(P=0.05), postoperative cylinder(P=0.40), vertical coma(P=0.593) and horizontal coma(P=0.957). After TG ablation, the proportion of the patients’ eyes of which postoperative refraction is within ±0.5 diopter of the target refraction was significantly higher than that undergoes WFO(P=0.003). As opposed to the WFO group, manifest refraction spherical equivalent(MRSE;P=0.000) was lower, and UCVA(P=0.005) was better in the TG group. The higher-order aberrations(HOAs;P=0.000), spherical aberration(P=0.000) and coma(P=0.000) were significantly lower in TG group. The cumulative Meta-analysis illustrated that the proportion of eyes achieving UCVA of 20/20 or better, postoperative refraction within ±0.5 diopter, and MRSE has steady between the two groups.CONCLUSION: Both TG-LASIK and WFO-LASIK are safe, effective, and predictable for correcting myopia. TG-LASIK may produce fewer aberration and is more precise than WFO-LASIK.展开更多

Plasmonic metal electrodes with subwavelength nanostructures are promising for enhancing light harvesting in photovoltaics.However,the nonradiative damping of surface plasmon polaritons(SPPs)during coupling with sunli...Plasmonic metal electrodes with subwavelength nanostructures are promising for enhancing light harvesting in photovoltaics.However,the nonradiative damping of surface plasmon polaritons(SPPs)during coupling with sunlight results in the conversion of the excited hot-electrons to heat,which limits the absorption of light and generation of photocurrent.Herein,an energy recycling strategy driven by hotelectron emission for recycling the SPP energy trapped in the plasmonic electrodes is proposed.A transparent silver-based plasmonic metal electrode(A-PME)with a periodic hexagonal nanopore array is constructed,which is combined with a luminescent organic emitter for radiative recombination of the injected hot-electrons.Owing to the suppressed SPP energy loss via broadband hot-electron emission,the A-PME achieves an optimized optical transmission with an average transmittance of over 80%from 380 to 1200 nm.Moreover,the indium-tin-oxide-free organic solar cells yield an enhanced light harvestingwith a power conversion efficiency of 16.1%.展开更多
The need for bladder reconstruction and side effects of cystoplasty have spawned the demand for the development of alternative material substitutes.Biomaterials such as submucosa of small intestine(SIS)have been widel...The need for bladder reconstruction and side effects of cystoplasty have spawned the demand for the development of alternative material substitutes.Biomaterials such as submucosa of small intestine(SIS)have been widely used as patches for bladder repair,but the outcomes are not fully satisfactory.To capture stem cells in situ has been considered as a promising strategy to speed up the process of re-cellularization and functionalization.In this study,we have developed an anti-CD29 antibody-conjugated SIS scaffold(AC-SIS)which is capable of specifically capturing urine-derived stem cells(USCs)in situ for tissue repair and regeneration.The scaffold has exhibited effective capture capacity and sound biocompatibility.In vivo experiment proved that the AC-SIS scaffold could promote rapid endothelium healing and smooth muscle regeneration.The endogenous stem cell capturing scaffolds has thereby provided a new revenue for developing effective and safer bladder patches.展开更多

文摘AIM: To evaluate the feasibility of a new computed virtual chromoendoscopy (CVC) device (M i-scan) in the diagnosis of gastric neoplasia. METHODS: Patients with superficial lesions no larger than 1.0 cm found during high definition endoscopy were included. Those with advanced or obviously protruded or depressed lesions, lesions larger than 1.0 cm and/or lesions which were not amenable to observation by zoom function were excluded. The endoscopist was required to give the real-time descriptions of surface pit patterns of the lesions, based on surface pattern classification of enhanced magnification endoscopy. According to previous reports, types Ⅰ-Ⅲ represent nonneoplastic lesions, and types Ⅳ-Ⅴ represent neoplastic lesions. Diagnosis with M i-scan and biopsy was performed before histopathological diagnosis. Magnified images of gastric lesions with and without enhancement were collected for further analysis. The diagnostic yield of real-time M i-scan and effects on magnification image quality by tone enhancement (TE), surface enhancement (SE) and color enhancement (CE) were calculated. The selected images were sent to another endoscopist. The endoscopist rated the image quality of each lesion at 3 levels. Ratings of image quality were based on visualization of pit pattern, vessel and demarcation line. RESULTS: One hundred and eighty-three patients were recruited. Five patients were excluded for advanced gastric lesions, 1 patient was excluded for poor preparation and 2 patients were excluded for superficial lesions larger than 1.0 cm; 132 patients were excluded for no lesions found by high definition endoscopy. In the end, 43 patients with 43 lesions were included. Histopathology revealed 10 inflammation, 14 atrophy, 10 metaplasia, 1 low grade dysplasia (LGD), 5 high grade dysplasia (HGD) and 3 cancers. For 7 lesions classified into type Ⅰ, histopathology revealed 6 atrophy and 1 metaplasia; for 10 lesions classified into type Ⅱ, histopathology revealed 2 inflammation, 7 atrophy and 1 metaplasia; for 10 lesions classified into type Ⅲ, histopathology revealed 1 inflammation, 8 metaplasia and 1 LGD; for 9 lesions classified into type Ⅳ, histopathology revealed 4 inflammation, 1 atrophy and 4 HGD; for 7 lesions classified into type Ⅴ, histopathology revealed 3 inflammation, 1 HGD and 3 cancers. A total of 172 still images, including 43 images by white light (MWL) and 129 images by M i-scan (43 with TE, 43 with SE and 43 with CE), were selected and sent to the endoscopist who did the analysis. General image quality of M i-scan with TE and SE was significantly better than that of MWL (TE, 4.55 ± 1.07; SE, 4.30 ± 1.02; MWL, 3.25 ± 0.99; P < 0.001). Visualization of pit pattern was significantly improved by M i-scan with SE (1.93 ± 0.25 vs 1.50 ± 0.50, P < 0.001). Microvessel visualization was significantly improved by M i-scan with TE (1.23 ± 0.78 vs 0.76 ± 0.73, P < 0.001). Demarcation line visualization was improved by M i-scan with both TE and SE (TE, 1.75 ± 0.52; SE, 1.56 ± 0.59; MWL, 0.98 ± 0.44; P < 0.001). M i-scan with CE did not show any significant improvements of image quality in general or in the 3 key parameters. Although M i-scan with TE and SE slightly increased the diagnostic yield of MWL, there was no significant difference (P > 0.1). CONCLUSION: Although digital enhancement improves the image quality of magnification endoscopy, its value in improving the diagnostic yield seems to be limited.

文摘AIM To determine the influence of Smoc2 on hepatocellular carcinoma(HCC) cell proliferation and to find a possible new therapeutic target for preventing HCC progression.METHODS We detected expression of Smoc2 in HCC tissues and corresponding non-tumor liver(CNL) tissues using PCR, western blot, and immunohistochemistry methods. Subsequently, we down-regulated and upregulated Smoc2 expression using siR NA and lentivirus transfection assay, respectively. Then, we identified the effect of Smoc2 on cell proliferation and cell cycle using CCK-8 and flow cytometry, respectively. The common cell growth signaling influenced by Smoc2 was detected by western blot assay. RESULTS The expression of Smoc2 was significantly higher in HCC tissues compared with CNL tissues. Overexpression of Smoc2 promoted HCC cell proliferation and cell cycle progression. Down-regulation of Smoc2 led to inhibition of cell proliferation and cell cycle progression. Smoc2 had positive effect on ERK and AKT signaling.CONCLUSION Smoc2 promotes the proliferation of HCC cells through accelerating cell cycle progression and might act as an anti-cancer therapeutic target in the future.
文摘Here, we report a rare case of primary gastrointestinal amyloidosis in a stable condition after being followed up for three years. The patient was admitted to the hospital in 2014. Tests showed decreased levels of hemoglobin and ferritin. Transoral and transanal enteroscopy showed multiple nodular protuberances in the esophagus, ileum, colon and rectum. Endoscopic ultrasonography indicated the nodular protuberances stemmed from the submucosa and partially invaded the intrinsic myometrium. Pathological examinations found multiple small nodules in the submucosa and dyed structures, which were positive for special Congo red dyeing. After treatment with oral iron supplements, the levels of hemoglobin and ferritin became normal. It is concluded that the patient represents a case of primary gastrointestinal amyloidosis with multiple nodular protuberances in the digestive tract with controllable moderate abdominal discomfort and anemia and a benign course. Enteroscopy and endoscopic ultrasonography play an important role in the diagnosis of primary gastrointestinal amyloidosis.

文摘This paper uses the Oseen transformation to solve the differential equations governing motion of the vertical linear gradient flow distribution close to a wall surface. The Navier-Stokes equations are used to consider the inertia term along the flow direction. A novel contour integral method is used to solve the complex Airy function. The boundary conditions of linear gradient flow distribution for finite problems are determined. The vorticity function, the pressure function, and the turbulent velocity profiles are provided, and the stability of particle trajectories is studied. An Lx-function form of the third derivative circulation is used to to simplify the solution. Theoretical results are compared with the experimental measurements with satisfactory agreement.

文摘Culture-loaded words refer to words with specific cultural connotations that can express an abstract or a specific concept,which may be related to religious beliefs or social customs,but do not exist in other languages and cultures.Therefore,culture-loaded words bring some difficulties to translation work.Huang Di Nei Jing(《黄帝内经》Huangdi’s Internal Classic)is the foundation of traditional Chinese medicine(TCM)theory,and is listed as the first of the four classics of TCM.It contains a large number of cultureloaded words,which embody the ancient Chinese traditional culture.The translation of culture-loaded words is a difficult but crucial point in the translation of Huangdi’s Internal Classic and directly relates to the quality of the translation of Huangdi’s Internal Classic as a whole.Taking the two English versions of Maoshing Ni and Li Zhaoguo as examples,this work identifies the culture-loaded words appearing in the chapter“Yin Yang Ying Xiang Da Lun”(阴阳应象大论Comprehensive Discourse on Phenomena Corresponding to the Yin and Yang).This work studies the strategies and translation process of culture-loaded words in Huangdi’s Internal Classic,with a view to contribute to the English translation of TCM classics.

文摘AIM: To evaluate gut microbial dysbiosis in two visceral hypersensitive models in comparison with irritable bowel syndrome(IBS) patients and to explore the extent to which these models capture the dysbiosis of IBS patients. METHODS: Visceral hypersensitivity was developed using the maternal separation(MS) rat model and post-inflammatory rat model. The visceral sensitivity of the model groups and control group was evaluated using the abdominal withdraw reflex score and electromyography in response to graded colorectal distention. The 16 S ribosomal RNA gene from fecal samples was pyrosequenced and analyzed. The correlation between dysbiosis in the microbiota and visceral hypersensitivity was calculated. Positive findings were compared to sequencing data from a published human IBS cohort.RESULTS: Dysbiosis triggered by neonatal maternal separation was lasting but not static. Both MS and postinflammatory rat fecal microbiota deviated from that of the control rats to an extent that was larger than the co-housing effect. Two short chain fatty acid producing genera, Fusobacterium and Clostridium XI, were shared by the human IBS cohort and by the maternal separation rats and post-inflammatory rats, respectively, to different extents. Fusobacterium was significantly increased in the MS group, and its abundance positively correlated with the degree of visceral hypersensitivity. Porphyromonadaceae was a protective biomarker for both the rat control group and healthy human controls. CONCLUSION: The dysbiosis MS rat model and the post-inflammatory rat model captured some of the dysbiosis features of IBS patients. Fusobacterium, Clostridium XI and Porphyromonadaceae were identified as targets for future mechanistic research.

文摘AIM:To compare the endomicroscopic image quality of integrated confocal laser endomicroscopy(iCLE)and sedation efficacy of propofolvs midazolam plus fentanyl(M/F).METHODS:Consecutive outpatients undergoing iCLE were prospectively recruited and randomized to the propofol group(P group)or M/F group.The patient,performing endoscopist and endoscopic assistant were blinded to the randomization.The quality of endomicro-scopic images and anesthetic efficacy outcomes were blindly evaluated after iCLE examination.RESULTS:There were significantly more good quality endomicroscopic images in the propofol group than in the M/F group(72.75%vs 52.89%,P<0.001).The diagnostic accuracy for upper gastrointestinal mucosal lesions using confocal laser endomicroscopy favors the P group,although this did not reach statistical significance.Adverse events and patient assessment were not significantly different for M/F vs propofol except for more frequent intraprocedural recall with M/F.Procedure duration and sedation times were significantly longer in the M/F group,while the scores of endoscopist,anesthetist and assistant assessment were all significantly better in the P group.CONCLUSION:Sedation with propofol might increase the proportion of good quality endomicroscopic images,and may result in improved procedural efficacy and diagnostic accuracy during iCLE examination.

文摘The motion of pore water directly influences mechanical properties of soils, which are variable during creep. Accurate description of the evolution of mechanical properties of soils can help to reveal the internal behavior of pore water. Based on the idea of using the fractional order to reflect mechanical properties of soils, a fractional creep model is proposed by introducing a variable-order fractional operator, and realized on a series of creep responses in soft soils. A comparative analysis illustrates that the evolution of mechanical properties, shown through the simulated results, exactly corresponds to the motion of pore water and the solid skeleton. This demonstrates that the proposed variable-order fractional model can be employed to characterize the evolution of mechanical properties of and the pore water motion in soft soils during creep. It is observed that the fractional order from the proposed model is related to the dissipation rate of pore water pressure.

文摘Diabetic erectile dysfunction(DED)is a common complication of diabetes mellitus,significantly impairing the quality of life of patients.The conventional clinical treatment still has limitations.Stem cells(SCs),as a type of cells with multidirectional or directional differentiation capability and sustainable selfrenewal potential,are widely used in regenerative medicine and tissue engineering.With the continuous update of regenerative medicine theory and the success of animal experiments,SCs as a treatment for male erectile dysfunction,especially DED,have attracted widespread attention because of curable possibility.This review focus on the current progress in the clinical application of SC treatment for DED.Moreover,we summarize the development prospects of SCs in the field of DMED therapy.
文摘Intraperitoneal foreign bodies such as retained surgical instruments can cause intestinal obstruction.However,intestinal obstruction due to transmural migration of foreign bodies has rarely been reported.Here,we report a case of intestinal obstruction due to a clinical thermometer which migrated from the bladder into the abdominal cavity.A 45-year-old man was admitted to our hospital with a one-year history of recurrent lower abdominal cramps.Two days before admission,the abdominal cramps aggravated.Intestinal obstruction was confirmed with upright abdominal radiography and computerized tomography scan which showed dilation of the small intestines and a thermometer in the abdominal cavity.Then laparotomy was performed.A scar was observed at the fundus of the bladder and a ther-mometer was adhering to the small bowels and mesentery which resulted in intestinal obstruction.Abdominal cramps were eliminated and defecation and flatus recovered soon after removal of the thermometer.
文摘BACKGROUND Major societies provide differing guidance on management of Barrett’s esophagus(BE),making standardization challenging.AIM To evaluate the preferred diagnosis and management practices of BE among Asian endoscopists.METHODS Endoscopists from across Asia were invited to participate in an online questionnaire comprising eleven questions regarding diagnosis,surveillance and management of BE.RESULTS Five hundred sixty-nine of 1016(56.0%)respondents completed the survey,with most respondents from Japan(n=310,54.5%)and China(n=129,22.7%).Overall,the preferred endoscopic landmark of the esophagogastric junction was squamocolumnar junction(42.0%).Distal palisade vessels was preferred in Japan(59.0%vs 10.0%,P<0.001)while outside Japan,squamo-columnar junction was preferred(59.5%vs 27.4%,P<0.001).Only 16.3%of respondents used Prague C and M criteria all the time.It was never used by 46.1%of Japanese,whereas 84.2%outside Japan,endoscopists used it to varying extents(P<0.001).Most Asian endoscopists(70.8%)would survey long-segment BE without dysplasia every two years.Adherence to Seattle protocol was poor with only 6.3%always performing it.73.2%of Japanese never did it,compared to 19.3%outside Japan(P<0.001).The most preferred(74.0%)treatment of non-dysplastic BE was proton pump inhibitor only when the patient was symptomatic or had esophagitis.For BE with low-grade dysplasia,6-monthly surveillance was preferred in 61.9%within Japan vs 47.9%outside Japan(P<0.001).CONCLUSION Diagnosis and management of BE varied within Asia,with stark contrast between Japan and outside Japan.Most Asian endoscopists chose squamo-columnar junction to be the landmark for esophagogastric junction,which is incorrect.Most also did not consistently use Prague criteria,and Seattle protocol.Lack of standardization,education and research are possible reasons.
文摘AIM: To investigate the potential differences between topography-guided(TG) and wavefront-optimized(WFO) laser in situ keratomileusis(LASIK) for the treatment of myopia.METHODS: A systematic literature search was performed to determine relevant trials comparing LASIK with TG and WFO from the time of library construction to August 2020, and The PubMed, Cochrane, Web of Science, EMBASE and Chinese databases(i.e. CNKI, CBM, WAN FANG and VIP) were accessed. The data on visual acuity, refractive status and wavefront aberration were retrieved and evaluated from three to six months after surgery. STATA(version 14.0) software was used for statistical analysis. A cumulative Meta-analysis was simultaneously performed.RESULTS: Eleven studies with a total of 1425 eyes were incorporated. No statistically significant differences were evident between TG and WFO ablation in the proportion of eyes achieving an uncorrected distance visual acuity(UCVA) of 20/20 or better(P=0.377), gaining one line or more(P=0.05), postoperative cylinder(P=0.40), vertical coma(P=0.593) and horizontal coma(P=0.957). After TG ablation, the proportion of the patients’ eyes of which postoperative refraction is within ±0.5 diopter of the target refraction was significantly higher than that undergoes WFO(P=0.003). As opposed to the WFO group, manifest refraction spherical equivalent(MRSE;P=0.000) was lower, and UCVA(P=0.005) was better in the TG group. The higher-order aberrations(HOAs;P=0.000), spherical aberration(P=0.000) and coma(P=0.000) were significantly lower in TG group. The cumulative Meta-analysis illustrated that the proportion of eyes achieving UCVA of 20/20 or better, postoperative refraction within ±0.5 diopter, and MRSE has steady between the two groups.CONCLUSION: Both TG-LASIK and WFO-LASIK are safe, effective, and predictable for correcting myopia. TG-LASIK may produce fewer aberration and is more precise than WFO-LASIK.

文摘A series of novel quinolinyl imidazolidin-2-imine nickel complexes with different substituents on the imidazolidin-2-imine ligand were synthesized and characterized.The complexes in the presence of methylaluminoxane(MAO)as a cocatalyst catalyzed the copolymerization of norbornene(N)and styrene(S)or para-chlorostyrene(CS)with high activity(up to 1070 kg·mol^-1·h^-1).The installation of sterically bulky substituents on the imidazolidine-2-imine ligand was effective for the increase of the molecular weight and the comonomer content,affording high molecular weight copolymers with tunable CS content(0.57 mol%-11.7 mol%),in which the existence of CI group can provide reaction site for the further functionalization of copolymers as well as the synthesis of graft or cross-linked polymers.The linear relationship between the comonomer content and the glass transition temperature of the copolymers and the monomer reactivity ratios in the copolymerization indicated the formation of the expected functionalized cyclic olefin copolymers(COC).

文摘Plasmonic metal electrodes with subwavelength nanostructures are promising for enhancing light harvesting in photovoltaics.However,the nonradiative damping of surface plasmon polaritons(SPPs)during coupling with sunlight results in the conversion of the excited hot-electrons to heat,which limits the absorption of light and generation of photocurrent.Herein,an energy recycling strategy driven by hotelectron emission for recycling the SPP energy trapped in the plasmonic electrodes is proposed.A transparent silver-based plasmonic metal electrode(A-PME)with a periodic hexagonal nanopore array is constructed,which is combined with a luminescent organic emitter for radiative recombination of the injected hot-electrons.Owing to the suppressed SPP energy loss via broadband hot-electron emission,the A-PME achieves an optimized optical transmission with an average transmittance of over 80%from 380 to 1200 nm.Moreover,the indium-tin-oxide-free organic solar cells yield an enhanced light harvestingwith a power conversion efficiency of 16.1%.

文摘The need for bladder reconstruction and side effects of cystoplasty have spawned the demand for the development of alternative material substitutes.Biomaterials such as submucosa of small intestine(SIS)have been widely used as patches for bladder repair,but the outcomes are not fully satisfactory.To capture stem cells in situ has been considered as a promising strategy to speed up the process of re-cellularization and functionalization.In this study,we have developed an anti-CD29 antibody-conjugated SIS scaffold(AC-SIS)which is capable of specifically capturing urine-derived stem cells(USCs)in situ for tissue repair and regeneration.The scaffold has exhibited effective capture capacity and sound biocompatibility.In vivo experiment proved that the AC-SIS scaffold could promote rapid endothelium healing and smooth muscle regeneration.The endogenous stem cell capturing scaffolds has thereby provided a new revenue for developing effective and safer bladder patches.
