体位管理在内镜下胃肿瘤全层切除术中的影响 被引量:2

目的分析术中体位管理对胃黏膜下巨大肿瘤患者在内镜下全层切除术(EFTR)中的影响。方法回顾性分析2017年12月-2019年7月在该院消化内科被诊断为胃黏膜下巨大肿瘤(肿瘤直径≥2.0 cm)并接受EFTR的患者。术中调整患者体位以避免肿瘤被胃腔... 目的分析术中体位管理对胃黏膜下巨大肿瘤患者在内镜下全层切除术(EFTR)中的影响。方法回顾性分析2017年12月-2019年7月在该院消化内科被诊断为胃黏膜下巨大肿瘤(肿瘤直径≥2.0 cm)并接受EFTR的患者。术中调整患者体位以避免肿瘤被胃腔内液体淹没,并分析患者的术中体位与肿瘤位置的关系以及对EFTR术后效果的影响。结果共37例患者纳入研究,术中体位包括:左侧卧位、仰卧位、俯卧位和右侧卧位。根据肿瘤的位置,采取对应的术中体位使肿瘤处于高位,术中视野暴露良好,镜下止血、缝合以及其他操作不受胃腔内液体的影响。所有患者均成功实施EFTR,未见大出血、肿瘤掉入腹腔、显性腹膜炎征象。结论EFTR治疗胃黏膜下巨大肿瘤时,使肿瘤处于高位的术中体位管理应受到重视。通过术前CT确认肿瘤位置或内镜下注水后观察手术视野,均可为患者体位的选择提供依据。该体位管理简单易行,不增加医疗费用,具有降低内镜下操作难度、增加手术安全性和缩短手术时间的优势。 展开更多

Myeloid sarcomas (MS) preceding acute myeloid leukemia (AML) are rare, which presenting as acute spinal cord compression is even rare. Here we report two new cases of myeloid sarcoma patients, whose outcomes were different. Twenty-seven patients with spinal MS preceding AML have been reported to date, including the two cases presented in this article. Surgical decompression was performed in 25 of the 27 patients, and 23 of these received additional anti-AML therapy. Considering our patients and the published cases in the literature we suggest that immunohistochemical study plays an essential role in arriving at a correct diagnosis of MS, and that emergency surgery to resect spinal MS is an available treatment to make neural function recovery, and that the disease must be treated with intensive chemotherapy similar to that used to treat AML as soon as possible after resection or irradiation of the tumor. 展开更多

Starting from optical nihility media (ONM),we design several intriguing devices with transformation optics method in two dimensions,such as a wave splitter,a concave lens,a field rotator,a concentrator,and an invisibility cloak.Though the extreme anisotropic property of ONM hinders the fabrication of these devices.We demonstrate that those devices could be effectively realized by simplified materials with Fabry-Pérot resonances (FPs) at discrete frequencies.Moreover,we propose a reduced version of simplified materials with FPs to construct a concentrator and a rotator,which is feasible in experimental fabrications.The simulations of total scattering cross-sections confirm their functionalities. 展开更多

With the rapid development of various portable electronic devices,lithium ion battery electrode materials with high energy and power density,long cycle life and low cost were pursued.Vanadium-based oxides/sulfides were considered as the ideal next-generation electrode materials due to their high capacity,abundant reserves and low cost.However,the inherent low conductivity and ion diffusion coefficient limit their practical applications in lithium ion batteries.In recent years,vanadium-based electrode materials have been designed into various nanostructures through a variety of nanofabrication processes to overcome the electrochemical performance bottleneck caused by the above disadvantages due to the new properties of nanomaterials that cannot be achieved at the solid level.However,how to obtain high-performance vanadium-based electrode nanomaterials with controllable morphology and structure through low-cost and environmentally friendly processes is still a huge challenge.In this paper,the basic structure,modified morphologies and synthesis methods of vanadium-based electrode materials for lithium ion batteries were reviewed.In addition,the disadvantages,new challenges and future development direction of vanadium electrode materials were also discussed. 展开更多
