In recent years,the national higher education reform has proposed the concept of curriculum ideological and political education as a major element.This integration into the standardized training of general practice residents(commonly referred to as general resident training)is an unavoidable trend in the teaching field.Based on the actual situation of our unit at The First Affiliated Hospital of Xi’an Medical University,this paper emphasizes the necessity of incorporating ideological and political education into the general resident training curriculum,despite the absence of pertinent practical experience in how to effectively incorporate such courses into comprehensive residential training.In our hospital,we must assess the ideological and political education in the curriculum of general resident training,proposing a scientific,reasonable ideological and political education concept system for general practice courses,integrating ideological and political education with general practice resident training,and strengthening the team of ideological and political education physicians to teach them to carry out such education,this would foster enthusiasm and initiative.Strengthening admission education and training for general practice resident training,as well as regularly hosting symposiums to share ideas and understand the mission of doctors can provide effective experience for ideological and political education in general resident training courses. 展开更多

目的探讨≥75岁非瓣膜性心房颤动(NVAF)患者体循环栓塞(SE)的相关危险因素,为临床中降低NVAF患者SE风险提供依据。方法病例对照研究。收集2018年10月至2020年10月于新疆医科大学第一附属医院住院的≥75岁NVAF患者,根据NVAF后发生SE情况... 目的探讨≥75岁非瓣膜性心房颤动(NVAF)患者体循环栓塞(SE)的相关危险因素,为临床中降低NVAF患者SE风险提供依据。方法病例对照研究。收集2018年10月至2020年10月于新疆医科大学第一附属医院住院的≥75岁NVAF患者,根据NVAF后发生SE情况分为无SE组(1127例)和SE组(433例)。应用多因素logistic回归对NVAF患者SE相关因素及未抗凝患者相关危险因素进行分析。结果多因素回归分析表明房颤病史≥5年的OR值为2.75(95%CI 1.98~3.82,P<0.01),脂蛋白(a)>300 g/L的SE风险高于脂蛋白(a)≤300 g/L(OR=2.07,95%CI 1.50~2.84,P<0.01),载脂蛋白B>1.2 g/L的OR值为1.91(95%CI 1.25~2.93,P=0.003),左心室射血分数(LVEF)30%~49%的SE风险较高(OR=2.45,95%CI 1.63~3.69,P<0.01),左心房内径>40 mm的OR值为1.54(95%CI 1.16~2.07,P=0.003),CHA2DS2-VASc≥3分的OR值为15.14(95%CI 2.05~112.13,P=0.01)。载脂蛋白AI>1.6 g/L是SE的保护性因素(OR=0.28,95%CI 0.15~0.51,P<0.01)。结论房颤病史≥5年、脂蛋白(a)>300 g/L、载脂蛋白B升高、左心房内径>40 mm、LVEF 30%~49%、CHA2DS2-VASc评分、HAS-BLED评分是NVAF患者SE的独立相关危险因素。其中载脂蛋白AI>1.6 g/L与NVAF患者SE风险呈负相关。 展开更多

目的回顾性总结单中心心脏再同步治疗(CRT)相关并发症概况为临床对CRT患者的管理提供相关经验。方法本研究为回顾性研究,纳入2009年6月至2021年6月在新疆医科大学第一附属医院心血管内科植入CRT的心力衰竭(心衰)患者,按照植入装置类型... 目的回顾性总结单中心心脏再同步治疗(CRT)相关并发症概况为临床对CRT患者的管理提供相关经验。方法本研究为回顾性研究,纳入2009年6月至2021年6月在新疆医科大学第一附属医院心血管内科植入CRT的心力衰竭(心衰)患者,按照植入装置类型分为心脏再同步治疗除颤器(CRT-D)组和心脏再同步治疗起搏器(CRT-P)组,分析患者术中、术后发生左心室导线植入失败、膈肌刺激、冠状静脉系统损伤等相关并发症的情况及原因。结果共纳入469例患者,其中男359例(359/469,76.55%),年龄(62.31±11.69)岁,术前左心室射血分数为34.76%±7.96%,术前QRS时限为(161.82±30.57)ms。左心室导线植入成功率为95.1%(446/499)。术中膈肌刺激2例(0.43%);急性左心衰竭3例(6.40%);冠状静脉夹层15例(3.19%);囊袋血肿4例(0.85%);导线脱位10例(2.13%),其中急性脱位2例(20%)、亚急性脱位4例(40%)、迟发性脱位4例(40%)。结论CRT术中发生相关并发症的风险较高,同时也要警惕和排除发生相关术后并发症的可能。 展开更多

Simulation of FY-2D infrared brightness temperature and sensitivity analysis to the errors of WRF simulated cloud variables

This study simulated FY-2 D satellite infrared brightness images based on the WRF and RTTOV models. The effects of prediction errors in WRF micro-and macroscale cloud variables on FY-2 D infrared brightness temperature accuracy were analyzed. The principle findings were as follows. In the T+0–48 h simulation time, the root mean square errors of the simulated brightness temperatures were within the range 10–27 K, i.e., better than the range of 20–40 K achieved previously. In the T+0–24 h simulation time, the correlation coefficients between the simulated and measured brightness temperatures for all four channels were >0.5. The simulation performance of water channel IR3 was stable and the best. The four types of cloud microphysical scheme considered all showed that the simulated values of brightness temperature in clouds were too high and that the distributions of cloud systems were incomplete, especially in typhoon areas. The performance of the THOM scheme was considered best, followed in descending order by the WSM6, WDM6, and LIN schemes. Compared with observed values, the maximum deviation appeared in the range 253–273 K for all schemes. On the microscale, the snow water mixing ratio of the THOM scheme was much bigger than that of the other schemes. Improving the production efficiency or increasing the availability of solid water in the cloud microphysical scheme would provide slight benefit for brightness temperature simulations. On the macroscale, the cloud amount obtained by the scheme used in this study was small. Improving the diagnostic scheme for cloud amount, especially high-level cloud, could improve the accuracy of brightness temperature simulations. These results could provide an intuitive reference for forecasters and constitute technical support for the creation of simulated brightness temperature images for the FY-4 satellite. 展开更多
