【应用背景】随着数据的爆炸式增长、算法的不断改进以及计算能力的快速发展,机器学习在教育、金融、制造和医疗等领域均得到了广泛应用。在眼健康领域,机器学习也已经在疾病诊断、疾病分级、医学检查和疾病早期筛查等许多任务上实现了... 【应用背景】随着数据的爆炸式增长、算法的不断改进以及计算能力的快速发展,机器学习在教育、金融、制造和医疗等领域均得到了广泛应用。在眼健康领域,机器学习也已经在疾病诊断、疾病分级、医学检查和疾病早期筛查等许多任务上实现了初步应用。【方法】本文通过对眼健康领域国内外相关文献的调研,从眼科疾病类别、就诊阶段、数据类型及技术类型4个不同维度对领域应用进行了梳理与分析,并对接下来的研究做出相应的展望。【结果】基于调研分析的结果可以看出,在眼健康领域中,机器学习技术主要以图像数据为主,围绕疾病诊断与分级展开。同时在疾病早期筛查和疾病风险预测等处于疾病发展早期阶段的任务上也取得了不错的表现。【结论】通过将机器学习技术应用到眼科诊疗过程的各个阶段,有望降低眼科医生负担、提升眼科医生工作效率、帮助控制患者病情发展、提升患者生活质量并降低患者治疗的经济成本和时间成本。 展开更多

Objective:The open-label,phase II RATIONALE-209 study evaluated tislelizumab(anti-programmed cell death protein 1 antibody)as a tissue-agnostic monotherapy for microsatellite instability-high(MSI-H)/mismatch repair-deficient(dMMR)tumors.Methods:Adults with previously treated,locally advanced unresectable or metastatic MSI-H/dMMR solid tumors were enrolled.Patients received tislelizumab 200 mg intravenously every 3 weeks.Objective response rate(ORR;primary endpoint),duration of response(DoR),and progression-free survival(PFS)were assessed by independent review committee(Response Evaluation Criteria in Solid Tumors v1.1).Results:Eighty patients were enrolled and treated;75(93.8%)patients had measurable disease at baseline.Most had metastatic disease and received at least one prior therapy for advanced/metastatic disease(n=79;98.8%).At primary analysis(data cutoff July 8,2021;median follow-up 15.2 months),overall ORR[46.7%;95%confidence interval(95%CI),35.1−58.6;one-sided P<0.0001]and ORR across tumor-specific subgroups[colorectal(n=46):39.1%(95%CI,25.1–54.6);gastric/gastroesophageal junction(n=9):55.6%(95%CI,21.2−86.3);others(n=20):60.0%(95%CI,36.1−80.9)]were significantly greater with tislelizumab vs.a prespecified historical control ORR of 10%;five(6.7%)patients had complete responses.Median DoR,PFS,and overall survival were not reached with long-term follow-up(data cutoff December 5,2022;median follow-up 28.9 months).Tislelizumab was well tolerated with no unexpected safety signals.Treatment-related adverse events(TRAEs)of grade≥3 occurred in 53.8%of patients;7.5%of patients discontinued treatment due to TRAEs.Conclusions:Tislelizumab demonstrated a significant ORR improvement in patients with previously treated,locally advanced unresectable or metastatic MSI-H/dMMR tumors and was generally well tolerated. 展开更多

Objective:To analyze the effects of repetitive transcranial magnetic stimulation combined with motor control training on the treatment of stroke-induced hemiplegia,specifically focusing on the impact on patients’balance function and gait.Methods:Fifty-two cases of hemiplegic stroke patients were randomly divided into two groups,26 in the control group and 26 in the observation group,using computer-generated random grouping.All participants underwent conventional treatment and rehabilitation training.In addition to these,the control group received repetitive transcranial magnetic pseudo-stimulation therapy+motor control training,while the observation group received repetitive transcranial magnetic stimulation therapy+motor control training.The balance function and gait parameters of both groups were compared before and after the interventions and assessed the satisfaction of the interventions in both groups.Results:Before the invention,there were no significant differences in balance function scores and each gait parameter between the two groups(P>0.05).However,after the intervention,the observation group showed higher balance function scores compared to the control group(P<0.05).The observation group also exhibited higher step speed and step frequency,longer step length,and a higher overall satisfaction level with the intervention compared to the control group(P<0.05).Conclusion:The combination of repetitive transcranial magnetic stimulation and motor control training in the treatment of stroke-induced hemiplegia has demonstrated positive effects.It not only improves the patient’s balance function and gait but also contributes to overall physical rehabilitation. 展开更多

消费升级的产业结构升级效应研究 被引量:3

中国既是生产大国,也是消费大国,在此基础上实现供需结构高水平动态平衡具有重要的理论和现实意义。因此,通过理论模型阐释消费升级对产业结构升级的影响及机制,并基于2000—2017年的省级面板数据,构建双重固定效应模型进行实证分析。... 中国既是生产大国,也是消费大国,在此基础上实现供需结构高水平动态平衡具有重要的理论和现实意义。因此,通过理论模型阐释消费升级对产业结构升级的影响及机制,并基于2000—2017年的省级面板数据,构建双重固定效应模型进行实证分析。研究发现,消费升级不仅促进产业结构整体升级,还推动制造业和服务业内部结构优化。经过工具变量法和系统GMM检验,结论仍然稳健。这种促进作用主要由需求牵引效应实现,也通过引致技术创新和培育人力资本等机制发挥正向推动作用;但消费升级也会抑制投资需求,并对资本密集的制造业部门产生负向影响,总体上正向促进作用占主导地位。在建设强大国内消费市场的同时,也要引导投资流向知识和技术密集型的高端制造业部门,增强消费对经济发展的基础性作用和投资对优化供给结构的关键作用。异质性分析表明,经济发达省份以及东部地区消费升级的产业结构升级效应更为明显。进一步分析发现,消费结构变动和消费规模扩大均是产业结构升级的驱动因素。经过测算,目前中国居民消费率仍低于大国最优水平,降低居民消费偏离程度,能进一步发挥消费升级对产业结构升级的促进作用,增强国内大循环内生动力和可靠性。研究结论为实现需求引导下的产业结构升级提供了理论和经验上的支持,也为增强消费基础性作用提供了决策参考。 展开更多

目的 分析儿童肝移植术后经济负担,为开展相关卫生经济评估提供数据支持,为优化我国器官移植医疗资源配置提供依据。方法 回顾性收集2017—2021年首都医科大学附属北京友谊医院儿童肝移植受者的基本信息和缴费数据分析直接医疗成本,并... 目的 分析儿童肝移植术后经济负担,为开展相关卫生经济评估提供数据支持,为优化我国器官移植医疗资源配置提供依据。方法 回顾性收集2017—2021年首都医科大学附属北京友谊医院儿童肝移植受者的基本信息和缴费数据分析直接医疗成本,并采用便利抽样法对门诊及住院随访的儿童肝移植受者的监护人进行问卷调查,通过问卷获得的信息和人力资本法计算直接非医疗成本和间接成本,同时调查患儿家庭主观经济负担感受。分析2017—2021年直接医疗成本变化趋势、构成及相关影响因素。结果 共纳入683例患儿,问卷调查部分共收到有效问卷234份。2017—2021年儿童肝移植术后直接医疗成本呈先增高后降低的趋势,其中药费占比最高,门诊患者药费占比45.27%~57.33%。2021年人均疾病经济负担17 870.12元(95%CI:16 370.87~19 369.37),其中直接医疗成本13 321.65元(95%CI:11 818.46~14 824.85),直接非医疗成本2 248.05元(95%CI:2 169.86~2 326.24),间接成本2 300.42元(95%CI:2 253.43~2 347.40)。疾病经济负担与移植术后时长呈负相关。从全社会的角度分析,特病医保患儿较其他保险类型支出更多(P <0.05)。结论儿童肝移植术后家庭和社会的经济负担较重,药费是重要组成部分。期待国家持续优化医保资源配置,通过免疫抑制药物集中采购等政策减轻儿童肝移植受者的疾病经济负担。 展开更多

巩固拓展脱贫攻坚成果是建设农业强国的底线任务。大量研究聚焦于消费帮扶的脱贫效应,但对脱贫农户返贫预防效应关注较少。基于中国家庭金融调查数据,借助家庭发展韧性指标测度脱贫农户返贫风险,考察消费帮扶政策对脱贫农户家庭发展韧... 巩固拓展脱贫攻坚成果是建设农业强国的底线任务。大量研究聚焦于消费帮扶的脱贫效应,但对脱贫农户返贫预防效应关注较少。基于中国家庭金融调查数据,借助家庭发展韧性指标测度脱贫农户返贫风险,考察消费帮扶政策对脱贫农户家庭发展韧性的影响及其作用机制。研究发现,消费帮扶显著提升了家庭发展韧性,且该结果在经过一系列稳健性测试后仍然成立。作用机制检验结果表明,消费帮扶通过缓解外部约束(促进农业产业发展、扩大社会网络拓展),以及改变内生动力(激发内生发展动能,避免“福利依赖”),提高了脱贫农户家庭发展韧性。因此,要借助消费帮扶建立的联农带农机制,推动农业产业兴旺,拓展社会关系网络,激发内生发展动力,筑牢脱贫农户发展根基。 展开更多

以春玉米为供试作物,于2021—2022年在甘肃省定西市进行不同深度施肥田间试验,以双垄沟覆膜种植为基础,设置3个氮肥施用深度:5 cm(D5)、15 cm(D15)和25 cm(D25),以不施氮(N0)处理为对照,共4个处理,分析不同氮肥施用深度对农田土壤养分... 以春玉米为供试作物,于2021—2022年在甘肃省定西市进行不同深度施肥田间试验,以双垄沟覆膜种植为基础,设置3个氮肥施用深度:5 cm(D5)、15 cm(D15)和25 cm(D25),以不施氮(N0)处理为对照,共4个处理,分析不同氮肥施用深度对农田土壤养分含量、干物质积累、水分利用及玉米产量的影响,为优化施肥模式和进一步提升其节水增产效能提供理论依据。结果表明,随施肥深度的增加,农田0~40 cm土层土壤养分含量逐渐增大,D25处理较D15和D5处理的土壤有机碳、全氮、速效磷和速效钾含量分别平均提高8.67%、2.23%、22.11%和15.18%;氮肥深施可提高生育后期养分供给能力,玉米干物质积累量显著提高,D25处理播后105 d~收获期干物质积累量分别较D15和D5提高6.53%和25.30%;2个试验年份农田耗水量和水分利用效率均随着施肥深度的增加逐渐增大,D25处理分别较D15和D5平均提高10.09%和10.08%;D25处理玉米生物产量和经济产量较D5处理分别平均提高27.20%和34.93%。在西北旱作区,氮肥25 cm深施模式能为玉米提供更好的养分供给,是提高覆膜旱作农田玉米干物质积累量及产量的有效措施。 展开更多

A composite separator of SiC/PVDF-HFP was synthesized for lithium-ion batteries with high thermal and mechanical stabilities.Benefiting from the nanoscale,high hardness,and melting point of SiC,SiC/PVDFHFP with highly uniform microstructure was obtained.This polarization caused by barrier penetration was significantly restrained.Due to the Si-F bond between SiC and PVDF-HFP,the structural stability has been obviously enhanced,which could suppress the growth of lithium(Li) dendrite.Furthermore,some 3D reticulated Si nanowires are found on the surface of Li anode,which also greatly inhibit Li dendrites and result in irregular flakes of Li metal.Especially,the shrinkage of 6% SiC/PVDF-HFP at 150℃ is only 5%,which is notably lower than those of PVDF-HFP and Celgard2500.The commercial LiFePO_(4) cell assembled with 6% SiC/PVDF-HFP possesses a specific capacity of 157.8 mA h g^(-1) and coulomb efficiency of 98% at 80℃.In addition,the tensile strength and modulus of 6% SiC/PVDF-HFP could reach 14.6 and 562 MPa,respectively.And a small deformation(1000 nm) and strong deformation recovery are obtained under a high additional load(2.3 mN).Compared with PVDF-HFP and Celgard2500,the symmetric Li cell assembled with 6% SiC/PVDF-HFP has not polarized after 900 cycles due to its excellent mechanical stabilities.This strategy provides a feasible solution for the composite separator of high-safety batteries with a high temperature and impact resistance. 展开更多

Objective:The spectrum and risk of cancer in relatives of BRCA1/2 pathogenic variant carriers in the Chinese population have not been established.Methods:A family history of cancer in 9903 unselected breast cancer patients was retrospectively analyzed.BRCA1/2 status was determined for all patients and relative risks(RRs)were calculated to evaluate cancer risk in relatives of the patients.Results:The incidences of breast cancer in female relatives of BRCA1 carriers,BRCA2 carriers,and non-carriers were 33.0%,32.2%,and 7.7%,respectively.The corresponding incidences of ovarian cancer were 11.5%,2.4%,and 0.5%,respectively.The incidences of pancreatic cancer in male relatives of BRCA1 carriers,BRCA2 carriers,and non-carriers were 1.4%,2.7%,and 0.6%,respectively.The corresponding incidences of prostate cancer were 1.0%,2.1%,and 0.4%,respectively.The risks of breast and ovarian cancers in female relatives of BRCA1 and BRCA2 carriers were significantly higher than female relatives of non-carriers(BRCA1:RR=4.29,P<0.001 and RR=21.95,P<0.001;BRCA2:RR=4.19,P<0.001 and RR=4.65,P<0.001,respectively).Additionally,higher risks of pancreatic and prostate cancers were noted in male relatives of BRCA2 carriers than non-carriers(RR=4.34,P=0.001 and RR=4.86,P=0.001,respectively).Conclusions:Female relatives of BRCA1 and BRCA2 carriers are at increased risk for breast and ovarian cancers,and male relatives of BRCA2 carriers are at increased risk for pancreatic and prostate cancers. 展开更多

Infrared bubbles provide a unique opportunity to study the interactions between massive stars and surrounding material.We conduct a multi-wavelength study on the environment and star formation around an infrared bubble N13.Three dust clumps and two molecular clumps are identified around N 13,which are all distributed on the layer.Young stellar objects(YSOs)are carefully searched using infrared colors and YSO candidates of WISE and Gaia DR2,and three Class I/II YSOs are found in N 13.In addition,four O-type stars identified in N 13 are probably the exciting stars.The dynamical and fragmentation ages of N 13 are 0.32-0.35 and 1.37-2.80 Myr respectively,which suggest that the radiation-driven implosion model may be dominant in N 13.By comparing the small-size bubble N 13(R~1.9 pc)and the larger-size bubble G15.684-0.29(R~15.7 pc)we found that star formation activity is more active in the large-size bubble.Brief comparisons of ten bubbles show that small-size bubbles have a small ratio of kinetic age versus the fragmentation time.Triggering star formation may be more active in bubbles with larger ratio between kinetic and fragmentation ages.Furthermore,the collect and collapse mechanism may play the dominant role in the large-size ones. 展开更多
