Epileptic brain network mechanisms and neuroimaging techniques for the brain network

Epilepsy can be defined as a dysfunction of the brain network,and each type of epilepsy involves different brain-network changes that are implicated diffe rently in the control and propagation of interictal or ictal discharges.Gaining more detailed information on brain network alterations can help us to further understand the mechanisms of epilepsy and pave the way for brain network-based precise therapeutic approaches in clinical practice.An increasing number of advanced neuroimaging techniques and electrophysiological techniques such as diffusion tensor imaging-based fiber tra ctography,diffusion kurtosis imaging-based fiber tractography,fiber ball imagingbased tra ctography,electroencephalography,functional magnetic resonance imaging,magnetoencephalography,positron emission tomography,molecular imaging,and functional ultrasound imaging have been extensively used to delineate epileptic networks.In this review,we summarize the relevant neuroimaging and neuroelectrophysiological techniques for assessing structural and functional brain networks in patients with epilepsy,and extensively analyze the imaging mechanisms,advantages,limitations,and clinical application ranges of each technique.A greater focus on emerging advanced technologies,new data analysis software,a combination of multiple techniques,and the construction of personalized virtual epilepsy models can provide a theoretical basis to better understand the brain network mechanisms of epilepsy and make surgical decisions. 展开更多

The Chinese tree shrew(Tupaia belangeri chinensis)has emerged as a promising model for investigating adrenal steroid synthesis,but it is unclear whether the same cells produce steroid hormones and whether their production is regulated in the same way as in humans.Here,we comprehensively mapped the cell types and pathways of steroid metabolism in the adrenal gland of Chinese tree shrews using single-cell RNA sequencing,spatial transcriptome analysis,mass spectrometry,and immunohistochemistry.We compared the transcriptomes of various adrenal cell types across tree shrews,humans,macaques,and mice.Results showed that tree shrew adrenal glands expressed many of the same key enzymes for steroid synthesis as humans,including CYP11B2,CYP11B1,CYB5A,and CHGA.Biochemical analysis confirmed the production of aldosterone,cortisol,and dehydroepiandrosterone but not dehydroepiandrosterone sulfate in the tree shrew adrenal glands.Furthermore,genes in adrenal cell types in tree shrews were correlated with genetic risk factors for polycystic ovary syndrome,primary aldosteronism,hypertension,and related disorders in humans based on genome-wide association studies.Overall,this study suggests that the adrenal glands of Chinese tree shrews may consist of closely related cell populations with functional similarity to those of the human adrenal gland.Our comprehensive results(publicly available at http://gxmujyzmolab.cn:16245/scAGMap/)should facilitate the advancement of this animal model for the investigation of adrenal gland disorders. 展开更多

BACKGROUND Multilocular thymic cyst(MTC)is a rare mediastinal lesion which is considered to occur in the process of acquired inflammation.It is usually characterized by well-defined cystic density and is filled with transparent liquid.CASE SUMMARY We report on a 39-year-old male with a cystic-solid mass in the anterior mediastinum.Computer tomography(CT)imaging showed that the mass was irregular with unclear boundaries.After injection of contrast agent,there was a slight enhancement of stripes and nodules.According to CT findings,it was diagnosed as thymic cancer.CONCLUSION After surgery,MTC accompanied by bleeding and infection was confirmed by pathological examination.The main lesson of this case was that malignant thymic tumor and MTC of the anterior mediastinum sometimes exhibit similar CT findings.Caution is necessary in clinical work to avoid misdiagnosis. 展开更多

本试验旨在研究云上黑山羊常用16种粗饲料的瘤胃降解特性,并建立不同营养成分有效降解率与营养成分含量间的预测模型。选用5头体重为(65±0.5)kg、装有永久性瘘管的去势云上黑山羊,运用尼龙袋法测定16种粗饲料的干物质(DM)、粗蛋白... 本试验旨在研究云上黑山羊常用16种粗饲料的瘤胃降解特性,并建立不同营养成分有效降解率与营养成分含量间的预测模型。选用5头体重为(65±0.5)kg、装有永久性瘘管的去势云上黑山羊,运用尼龙袋法测定16种粗饲料的干物质(DM)、粗蛋白质(CP)、中性洗涤纤维(NDF)和酸性洗涤纤维(ADF)瘤胃降解率及降解参数。结果表明:1)在72 h时,DM降解率最高的为北部地区的苜蓿,达90.54%,并且其在各个时间点的DM降解率均显著高于其他15种粗饲料(P<0.05);DM降解率最低的为北部地区的麦糠,仅为43.95%。在72 h时,CP降解率最高的为北部地区的苜蓿,达96.41%;最低的为北部地区的麦糠,仅为53.50%。北部地区的苜蓿在各个时间点的NDF和ADF降解率均高于其他粗饲料。2)DM有效降解率最高的为南部地区的玉米青贮,CP有效降解率最高的为北部地区的蚕豆糠,ADF和NDF有效降解率最高的均为北部地区的苜蓿。3)DM、CP、NDF和ADF的瘤胃降解率均受饲料常规营养成分含量的影响,DM有效降解率与CP含量呈正相关,与NDF和ADF含量呈负相关;CP有效降解率与CP含量呈正相关,与NDF和ADF含量呈负相关;NDF和ADF有效降解率与ADF含量呈负相关。综上可知,北部地区苜蓿的DM、CP、ADF和NDF降解特性较优,可作为云上黑山羊优质粗饲料,北部和中部地区的麦糠降解特性较差,需与营养价值较高的粗饲料配合饲喂云上黑山羊。 展开更多

针对某光学舱所采用的泡沫铝夹层防护结构在破片冲击下的抗冲击性能问题,采用Monte-Carlo方法创建了泡沫铝结构的二维细观模型,在常规态型近场动力学理论中引入了Mises屈服准则和线性各向同性强化模型,建立了近场动力学塑性本构的数值... 针对某光学舱所采用的泡沫铝夹层防护结构在破片冲击下的抗冲击性能问题,采用Monte-Carlo方法创建了泡沫铝结构的二维细观模型,在常规态型近场动力学理论中引入了Mises屈服准则和线性各向同性强化模型,建立了近场动力学塑性本构的数值计算框架。基于近场动力学计算程序模拟了低速冲击作用下泡沫铝夹层结构的塑性变形以及有机玻璃背板的裂纹扩展形态,分析了泡沫铝芯材孔隙率对该夹层结构抗冲击性能和损伤模式的影响规律。结果表明:泡沫铝夹层结构良好的塑性变形能力是其发挥缓冲与防护作用的主要因素,并且在一定范围内,泡沫铝芯材孔隙率越高,则夹层结构具有更好的抗冲击性能;当泡沫铝孔隙率从0.4提升到0.7时,泡沫铝对冲击物的动能吸收率从90%提高到99%;模拟结果与实验结果具有较好的一致性,验证了模拟结果的准确性和分析结论的有效性。通过数值模拟,预测了有机玻璃背板的裂纹扩展形态,发现提高泡沫铝的孔隙率能获得更好的防护效果。 展开更多

目的对比、分析近5年内循证医学教学评估工具的特点,为进一步设计与优化课程评价方式提供参考依据。方法计算机检索PubMed、Web of Science、中国生物医学文献数据库、中国知网、万方数据库。检索时限为2018年1月1日至2022年9月16日。... 目的对比、分析近5年内循证医学教学评估工具的特点,为进一步设计与优化课程评价方式提供参考依据。方法计算机检索PubMed、Web of Science、中国生物医学文献数据库、中国知网、万方数据库。检索时限为2018年1月1日至2022年9月16日。分别由2位研究人员筛选文献,提取工具的应用对象、聚焦主题、心理测量学指标、条目内容等,对现有工具进行全面性分析。结果最终纳入16篇文献(涉及14个工具),均具有一定的可靠性。其中,11个工具关注调查对象对循证医学相关知识和(或)技能的掌握水平(条目数在6~24之间),多涉及对循证医学的综合性认知、常用术语的了解程度、关键概念的判断、共享和应用循证实践等,评估标准多采用是非判断、自我评价、反馈内容的客观评估等。7个工具聚焦调查对象对开展循证实践的真实态度(条目数在3~21之间),评估标准多设置主观性意向选项;4个工具了解调查对象对循证实践关键环节的实施频次、时间分配、分享等(条目数在6~18之间),评估标准多为频率程度的主观选择。结论循证医学课程评估可考虑采用相关知识/技能及其态度和行为等问卷作为教学评估辅助工具,但仍需进一步根据授课对象的教学特点及目标,优化条目内容和比重,研发循证医学课程教学过程监测评估工具。 展开更多

Laser–plasma instability(LPI)is one of the main obstacles to achieving predictable and reproducible fusion at high gain through laser-driven inertial confinement fusion(ICF).In this paper,for the first time,we show analytically and confirm with three-dimensional particle-incell simulations that angular incoherence provides suppression of the instability growth rate that is additional to and much stronger than that provided by the well-known temporal and spatial incoherence usually used in ICF studies.For the model used in our calculations,the maximum field ratio between the stimulated Raman scattering and the driving pulses drops from 0.2 for a Laguerre–Gaussian pulse with a single nonzero topological charge to 0.05 for a super light spring with an angular momentum spread and random relative phases.In particular,angular incoherence does not introduce extra undesirable hot electrons.This provides a novel method for suppressing LPI by using light with an angular momentum spread and paves the way towards a low-LPI laser system for inertial fusion energy with a super light spring of incoherence in all dimensions of time,space,and angle,and may open the door to the use of longer-wavelength lasers for inertial fusion energy. 展开更多

BACKGROUND Prevotella oris-induced meningitis and Prevotella oris-induced meningitis concomitant with spinal canal infection are extremely rare.To the best of our knowledge,only 1 case of Prevotella oris-induced central system infection has been reported.This is the second report on meningitis combined with spinal canal infection due to Prevotella oris.CASE SUMMARY We report a case of a 9-year-old boy suffering from meningitis and spinal canal infection.The patient presented to the neurosurgery department with lumbosacral pain for 1 mo and headache and vomiting for 1 d.He had been treated with cephalosporin and nonsteroidal anti-inflammatory drugs for fever,otalgia and pharyngalgia in a local hospital 2 mo prior to this admission.During hospitalization,magnetic resonance imaging suggested meningitis and L3-S1 lumbosacral dural sac infection.The cerebrospinal fluid and blood cultures were negative,but the cerebrospinal fluid specimen indicated the presence of Prevotella oris by metagenomic next-generation sequencing.Previous cases of Prevotella oris infection were retrieved from PubMed to characterize the clinicopathological features and identify the prognostic factors and related antimicrobial treatment of infection due to Prevotella oris.CONCLUSION This report shed light on the characteristics of Prevotella oris infection and highlighted the role of metagenomic next-generation sequencing in pathogen detection. 展开更多

All-inorganic CsPbI_(3) perovskite has attracted wide attention due to its desirable optical bandgap(Eg:∼1.7 eV)as well as high chemical stability.Nevertheless,the photovoltaic performance of CsPbI_(3) perovskite solar cells(PSCs)was limited by severe nonradiative charge recombination due to high defect density at the grain boundary and surface of perovskitefilms.To address this issue,a pyrrolidinium iodide(PyI)molecule was introduced to modify the surface and grain boundary of CsPbI_(3) perovskitefilms to passivate defects,which improves the quality of CsPbI_(3) perovskitefilms as well as induces the generation of a quasi-2D Py_(2)CsPb_(2)I_(7) capping layer between per-ovskite layer and hole transport layer.Such quasi-2D Py_(2)CsPb_(2)I_(7) capping layer optimizes interface contact between CsPbI_(3) perovskite layer and hole transport layer and blocks the electron transfer from CsPbI_(3) perovskite photoactive layer to the hole transport layer.As a result,the performance of CsPbI_(3) PSCs is well improved to 17.87%for power conversion efficiency(PCE)with an ultra-high fill factor(FF)of 0.84.In addition,the PyI mole-cule modified CsPbI_(3) perovskite devices exhibit excellent stability,which remains its initial PCE almost unchanged after aging for 35 days under the dry air atmosphere(temperature:20℃–30℃,control relative humid-ity(RH):<10%). 展开更多

Background Somatic symptom disorder(SSD)commonly presents in general hospital settings,posing challenges for healthcare professionals lacking specialised psychiatric training.The Neuro-11 Neurosis Scale(Neuro-11)offers promise in screening and evaluating psychosomatic symptoms,comprising 11 concise items across three dimensions:somatic symptoms,negative emotions and adverse events.Prior research has validated the scale’s reliability,validity and theoretical framework in somatoform disorders,indicating its potential as a valuable tool for SSD screening in general hospitals.Aims This study aimed to establish the reliability,validity and threshold of the Neuro-11 by comparing it with standard questionnaires commonly used in general hospitals for assessing SSD.Through this comparative analysis,we aimed to validate the effectiveness and precision of the Neuro-11,enhancing its utility in clinical settings.Methods Between November 2020 and December 2021,data were collected from 731 patients receiving outpatient and inpatient care at Shenzhen People’s Hospital in China for various physical discomforts.The patients completed multiple questionnaires,including the Neuro-11,Short Form 36 Health Survey,Patient Health Questionnaire 15 items,Hamilton Anxiety Scale and Hamilton Depression Scale.Psychiatry-trained clinicians conducted structured interviews and clinical examinations to establish a gold standard diagnosis of SSD.Results The Neuro-11 demonstrated strong content reliability and structural consistency,correlating significantly with internationally recognised and widely used questionnaires.Despite its brevity,the Neuro-11 exhibited significant correlations with other questionnaires.A test-retest analysis yielded a correlation coefficient of 1.00,Spearman-Brown coefficient of 0.64 and Cronbach’sαcoefficient of 0.72,indicating robust content reliability and internal consistency.Confirmatory factor analysis confirmed the validity of the three-dimensional structure(p<0.001,comparative fit index=0.94,Tucker-Lewis index=0.92,root mean square error of approximation=0.06,standardised root mean square residual=0.04).The threshold of the Neuro-11 is set at 10 points based on the maximum Youden’s index from the receiver operating characteristic curve analysis.In terms of diagnostic efficacy,the Neuro-11 has an area under the curve of 0.67. 展开更多

BACKGROUND Severely elevated intracranial pressure due to various reasons,such as decreased cerebral perfusion,can lead to devastating neurological outcomes,such as brain herniation.Decompression craniectomy is a life-saving procedure that is commonly performed for such a critical situation,but the changes in cerebral microvessels after brain herniation and decompression are unclear.Ultrafast power Doppler imaging(uPDI)is a new microvascular imaging technology that utilizes high frame rate plane/diverging wave transmission and advanced clutter filters.uPDI significantly improves Doppler sensitivity and can detect microvessels,which are usually invisible using traditional ultrasound Doppler imaging.CASE SUMMARY In this report,uPDI was used for the first time to observe the brain blood flow of a hypoperfusion area in a 4-year-old girl who underwent decompression craniectomy due to refractory intracranial hypertension(ICP)after malignant brain tumor surgery.B-mode imaging was used to verify the increased densities of the cerebral cortex and basal ganglia that were observed by computed tomography.CONCLUSION uPDI showed the local blood supplies and anatomical structures of the patient after decompressive craniectomy.uPDI is potentially a more intuitive and noninvasive method for evaluating the effects of severe ICP on cerebral microvessels. 展开更多

Investigating correlations between radiomic and genomic profiling in breast cancer(BC)molecular subtypes is crucial for understanding disease mechanisms and providing personalized treatment.We present a well-designed radiogenomic framework image–gene–gene set(IMAGGS),which detects multi-way associations in BC subtypes by integrating radiomic and genomic features.Our dataset consists of 721 patients,each of whom has 12 ultrasound(US)images captured from different angles and gene mutation data.To better characterize tumor traits,12 multi-angle US images are fused using two distinct strategies.Then,we analyze complex many-to-many associations between phenotypic and genotypic features using a machine learning algorithm,deviating from the prevalent one-to-one relationship pattern observed in previous studies.Key radiomic and genomic features are screened using these associations.In addition,gene set enrichment analysis is performed to investigate the joint effects of gene sets and delve deeper into the biological functions of BC subtypes.We further validate the feasibility of IMAGGS in a glioblastoma multiforme dataset to demonstrate the scalability of IMAGGS across different modalities and diseases.Taken together,IMAGGS provides a comprehensive characterization for diseases by associating imaging,genes,and gene sets,paving the way for biological interpretation of radiomics and development of targeted therapy. 展开更多

背景与目的小细胞肺癌(small cell lung cancer,SCLC)是一种以增殖迅速、易早期远处转移和获得性治疗耐药为特征的高侵袭性恶性肿瘤,临床预后极差。研究发现干细胞标志物CD44与肿瘤复发转移及治疗抵抗有关,而有关CD44在SCLC患者中的表... 背景与目的小细胞肺癌(small cell lung cancer,SCLC)是一种以增殖迅速、易早期远处转移和获得性治疗耐药为特征的高侵袭性恶性肿瘤,临床预后极差。研究发现干细胞标志物CD44与肿瘤复发转移及治疗抵抗有关,而有关CD44在SCLC患者中的表达及其临床预后意义研究不多。本研究拟通过检测SCLC患者肿瘤组织和血清中CD44的表达水平,初步探索其与患者临床特征的关系并评估其判断预后的临床价值。方法将本院47例初治SCLC患者肿瘤组织和血清标本配对,分别应用免疫组化法(immunohistochemistry,IHC)和酶联免疫吸附试验方法(enzyme-linked immunosorbent assay,ELISA)对CD44进行检测,分析其与患者临床特征及预后的关系。结果SCLC患者肿瘤组织和血清中均可检测到干细胞标志物CD44的表达,体能状态(performance status,PS)2分的患者肿瘤组织CD44阳性率明显高于PS 0分-1分的患者(85.71%vs 30%,P=0.017),根据化疗疗效对患者进行分组,疾病进展组肿瘤组织CD44免疫组化得分及血清CD44浓度均明显高于疾病控制组且差异均达到统计学意义(P=0.006,P=0.034),单因素分析肿瘤组织CD44阳性患者无进展生存期(progression-free survival,PFS)较CD44阴性患者PFS明显缩短(5.23个月vs 9.03个月,P=0.036)。结论初治SCLC患者肿瘤组织中CD44的表达水平与PFS相关,CD44在SCLC中的临床意义值得进一步深入研究。 展开更多
