We retrospectively evaluated the efficacy and safety of the combination drug piperacillin/tazobactam (PIPC/TAZ) in comparison with those of fourth-generation cephalosporins (4th Cephs) as initial empirical treatment i...We retrospectively evaluated the efficacy and safety of the combination drug piperacillin/tazobactam (PIPC/TAZ) in comparison with those of fourth-generation cephalosporins (4th Cephs) as initial empirical treatment in hematological malignancies patients with febrile neutropenia (FN). Among 200 patients assessed in this study, 49 had received PIPC/TAZ and 151 4th Cephs. Patient background characteristics were comparable between the two treatment groups. The overall efficacy rate in those receiving 4th Cephs and PIPC/TAZ was 57.0% (86/151 patients) and 59.2% (29/49 patients), respectively, with no significant difference detected between the two treatment regimens (P = 0.78). Treat-ment did not need to be discontinued or interrupted due to development of adverse drug reactions in any of the patients. Therefore in this study the efficacy and safety of PIPC/TAZ as initial antimicrobial treatment for FN in patients with hematological malignancies were not inferior to those of 4th Cephs. Based on the preliminary data obtained in this study, we propose to conduct a multicenter, prospective, controlled study to compare PIPC/TAZ versus CFPM given as empirical antimicrobial treatment against FN in patients with hematological malignancies.展开更多

There have been few experimental studies to visualize cryogenic foil cavitation. Therefore a new cryogenic cavitation tunnel of blowdown type was built. The foil shape is “piano-convex”. This profile was chosen because of simplicity, but also of being similar to the one for a rocket inducer impeller. Working fluids were water at room temperature, hot water and liquid nitrogen. In case of Angle of Attack (AOA)=8°, periodical cavity departure was observed in the experiments of both water at 90℃ and nitrogen at -190℃ under the same velocity 10 m/sec and the same cavitation number 0.7. The frequencies were observed to be 110 and 90 Hz, respectively, and almost coincided with those of vortex shedding from the foil. Temperature depression due to the thermodynamic effect was confirmed in both experiment and simulation especially in the cryogenic cavitation.展开更多

Liquid phase is treated with Eulerian framework as a mixture containing minute cavitating bubbles. Vapor phase consists of various sizes of minute vapor bubbles, which is distributed to classes based on their mass. The change of bubble number density for each class was solved by considering the change of bubble mass due to phase change as well as generation of new bubbles due to heterogeneous nucleation. In this method the mass of bubbles is treated as an independent variable, in other word, a new coordinate, and dependant variables are solved in Eulerian framework for spatial coordinates and bubble-mass coordinate. The present method is applied to a cavitating flow in a convergent-divergent nozzle, and the two-phase flow with bubble size distribution and phase change was successfully predicted.展开更多
Edible birds’nests(EBNs)have traditionally been produced in Southeast Asia.Indonesian farmers construct buildings for swiftlets and harvest their nests.EBN farming does not directly degrade forest resources and is th...Edible birds’nests(EBNs)have traditionally been produced in Southeast Asia.Indonesian farmers construct buildings for swiftlets and harvest their nests.EBN farming does not directly degrade forest resources and is therefore considered a sustainable means of production,whereas the expansion of other agricultural activities often relies on the degradation of natural resources.This study examines the relationship between natural resources and agricultural livelihoods,focusing on Indonesian EBN farmers.Using survey data that we collected in 2017,combined with satellite information on the extent of the forest in Central Kalimantan,Indonesia,we measured production efficiency and identified the natural and social factors that enhance production performance.The results show that a forest extent ranging between 2,000 and 6,000 meters from nesting building is positively associated with the production efficiency of EBN farming,perhaps because extensive forest could help swiftlets to collect food and build nests.Conversely,while EBN farming is a sustainable and profitable option,the initial costs of constructing buildings to house swiftlets may deter farmers from participating in the process.展开更多

Two hydro...In order to explain criteria for periodical shedding of the cloud cavitation, flow patterns of cavitation around a piano-convex hydrofoil were observed using a cryogenic cavitation tunnel of a blowdown type. Two hydrofoils of similarity of 20 and 60 mm in chord length with two test sections of 20 and 60 mm in width were prepared. Working fluids were water at ambient temperature, hot water and liquid nitrogen. The parameter range was varied between 0.3 and 1.4 for cavitation number, 9 and 17 m/sec for inlet flow velocity, and -8° and 8° for the flow incidence angle, respectively. At incidence angle 8°, that is, the convex surface being suction surface, periodical shedding of the whole cloud cavitation was observed on the convex surface under the specific condition with cavitation number and inlet flow velocity, respectively, 0.5, 9 m/sec for liquid nitrogen at 192℃ and 1.4, 11 m/sec for water at 88℃, whereas under the supercavitation condition, it was not observable. Periodical shedding of cloud cavitation occurs only in the case that there are both the adverse pressure gradient and the slow flow region on the hydrofoil.展开更多
