BACKGROUND Laryngeal contact granuloma(LCG)is difficult to treat and frequently associated with high persistence and recurrence,despite the availability of both surgical and pharmacological treatment options.An approp...BACKGROUND Laryngeal contact granuloma(LCG)is difficult to treat and frequently associated with high persistence and recurrence,despite the availability of both surgical and pharmacological treatment options.An appropriate strategy is therefore needed to help patients with multiple recurrences of LCG to potentially avoid unnecessary surgery.CASE SUMMARY We describe the case of a 34-year-old male patient with recurrent LCG in which a good response was achieved through successful management of laryngopharyngeal reflux disease using a combination pharmacotherapeutic regimen consisting of anti-reflux therapy,pepsin secretion inhibition,bile acid neutralization,and lifestyle modifications.This patient underwent surgery to excise the granuloma,then relapsed,underwent a second surgery,which was followed by a second recurrence.The granuloma then disappeared after 9 mo of combined treatment with ilaprazole enteric-coated capsules(10 mg qd),mosapride tablets(5 mg tid)and compound digestive enzyme capsules(2 tablets).The drug regimen was discontinued after one year,and no recurrence of the lesion has been reported during the one-year follow-up period.CONCLUSION We report a combination of pharmacotherapeutics and lifestyle modifications for the management of laryngopharyngeal reflux disease to address recurring LCG.展开更多

Background Management of patients with diabetic foot ulcer is often burdensome,complex,and arduous.In addition,it can be challenging for clinicians and researchers to understand the surgical management of diabetic foo...Background Management of patients with diabetic foot ulcer is often burdensome,complex,and arduous.In addition,it can be challenging for clinicians and researchers to understand the surgical management of diabetic foot ulcer from the existing studies because of the extensive literature on this topic.Methods Bibliometric analysis was conducted using CiteSpace,and data were retrieved from the Web of Science Core Collection.Results A total of 1,475 publications were retrieved.The“United States Department of Health and Human Services(HHS,USA)”and“National Institutes of Health(NIH,USA)”were the most important funders.The most active authors were Armstrong DG and Lipsky BA.The most active institutions were the University of Washington and University of Arizona.Developed countries in the USA and England contributed the most to the literature,and the publications were clustered into 15 topics.The emerging topic trend was the cluster label for“diabetic foot osteomyelitis”and“multidisciplinary setting”.Conclusions This study provides researchers and clinicians with important information on the cooperation of authors,institutions,and countries,intellectual structure,knowledge flow,and emerging topic trend,to help them with their subject cognition,study visits,study abroad,research direction selection,and grant applications.展开更多
Human interaction with animals has been implicated as a primary risk factor for several high impact zoonoses,including many bat-origin viral diseases.However the animal-to-human spillover events that lead to emerging ...Human interaction with animals has been implicated as a primary risk factor for several high impact zoonoses,including many bat-origin viral diseases.However the animal-to-human spillover events that lead to emerging diseases are rarely observed or clinically examined,and the link between specific interactions and spillover risk is poorly understood.To investigate this phenomenon,we conducted biological-behavioral surveillance among rural residents in Yunnan,Guangxi,and Guangdong districts of Southern China,where we have identified a number of SARS-related coronaviruses in bats.Serum samples were tested for four bat-borne coronaviruses using newly developed enzyme-linked immunosorbent assays(ELISA).Survey data were used to characterize associations between human-animal contact and bat coronavirus spillover risk.A total of 1,596 residents were enrolled in the study from 2015 to 2017.Nine participants(0.6%)tested positive for bat coronaviruses.265(17%)participants reported severe acute respiratory infections(SARI)and/or influenza-like illness(ILI)symptoms in the past year,which were associated with poultry,carnivore,rodent/shrew,or bat contact,with variability by family income and district of residence.This study provides serological evidence of bat coronavirus spillover in rural communities in Southern China.The low seroprevalence observed in this study suggests that bat coronavirus spillover is a rare event.Nonetheless,this study highlights associations between human-animal interaction and zoonotic spillover risk.These findings can be used to support targeted biological behavioral surveillance in high-risk geographic areas in order to reduce the risk of zoonotic disease emergence.展开更多
