BACKGROUND Eccrine porocarcinoma(EPC)is a rare skin tumor that mainly affects the elderly population.Tumors often present with slow growth and a good prognosis.EPCs are usually distinguished from other skin tumors usi...BACKGROUND Eccrine porocarcinoma(EPC)is a rare skin tumor that mainly affects the elderly population.Tumors often present with slow growth and a good prognosis.EPCs are usually distinguished from other skin tumors using histopathology and immunohistochemistry.However,surgical management alone may be inadequate if the tumor has metastasized.However,currently,surgical resection is the most commonly used treatment modality.CASE SUMMARY A seventy-four-year-old woman presented with a slow-growing nodule in her left temporal area,with no obvious itching or pain,for more than four months.Histopathological examination showed small columnar and short spindle-shaped cells;thus,basal cell carcinoma was suspected.However,immunohistochemical analysis revealed the expression of cytokeratin 5/6,p63 protein,p16 protein,and Ki-67 antigen(40%),and EPC was taken into consideration.The skin biopsy was repeated,and hematoxylin and eosin staining revealed ductal differentiation in some cells.Finally,the patient was diagnosed with EPC,and Mohs micrographic surgery was performed.We adapted follow-up visits in a year and not found any recurrence of nodules.CONCLUSION This case report emphasizes the diagnosis and differentiation of EPC.展开更多
AIM To investigate the efficacy and safety of transcutaneouselectroacupuncture(TEA) to alleviate postoperative ileus(POI) after gastrectomy.METHODS From April 2014 to February 2017, 63 gastric cancer patients were rec...AIM To investigate the efficacy and safety of transcutaneouselectroacupuncture(TEA) to alleviate postoperative ileus(POI) after gastrectomy.METHODS From April 2014 to February 2017, 63 gastric cancer patients were recruited from the Second Affiliated Hospital of Zhejiang University School of Medicine, Hangzhou, China. After gastrectomy, the patients were randomly allocated to the TEA(n = 33) or control(n = 30) group. The patients in the TEA group received 1 h TEA on Neiguan(ST36) and Zusanli(PC6) twice daily in the morning and afternoon until they passed flatus. The main outcomes were hours to the first flatus or bowel movement, time to nasogastric tube removal, time to liquid and semi-liquid diet, and hospital stay. The secondary outcomes included postoperative symptom assessment and complications.RESULTS Time to first flatus in the TEA group was significantly shorter than in the control group(73.19 ± 15.61 vs 82.82 ± 20.25 h, P = 0.038), especially for open gastrectomy(76.53 ± 14.29 vs 87.23 ± 20.75 h, P = 0.048). Bowel sounds on day 2 in the TEA group were significantly greater than in the control group(2.30 ± 2.61/min vs 1.05 ± 1.26/min, P = 0.017). Time to nasogastric tube removal in the TEA group was earlier than in the control group(4.22 ± 1.01 vs 4.97 ± 1.67 d, P = 0.049), as well as the time to liquid diet(5.0 ± 1.34 vs 5.83 ± 2.10 d, P = 0.039). Hospital stay in the TEA group was significantly shorter than in the control group(8.06 ± 1.75 vs 9.40 ± 3.09 d, P = 0.041). No significant differences in postoperative symptom assessment and complications were found between the groups. There were no severe adverse events related to TEA.CONCLUSION TEA accelerated bowel movements and alleviated POI after open gastrectomy and shortened hospital stay.展开更多

BACKGROUND MET fusion is a key driver mutation,but it is rare in gastric cancer(GC).Several MET(hepatocyte growth factor receptor)inhibitors have been approved for the treatment of MET-positive patients,but the tumor ...BACKGROUND MET fusion is a key driver mutation,but it is rare in gastric cancer(GC).Several MET(hepatocyte growth factor receptor)inhibitors have been approved for the treatment of MET-positive patients,but the tumor response is heterogeneous.With the development of next-generation sequencing,diverse MET fusion partner genes have been identified.We herein report a fusion variant involving KIF5BMET in GC.CASE SUMMARY After thoracoscopic inferior lobectomy plus lymph node dissection under general anesthesia,a“tumor within a tumor”was found in the lung tumor tissue of a 64-year-old non-smoking male patient.Combining the medical history and the results of enzyme labeling,the focal area was considered to be GC.To seek potential therapeutic regimens,an intergenic region between KIF5B and MET fusion was identified.This fusion contains a MET kinase domain and coil-coiled domains encoded by KIF5B exons 1-25,which might drive the oncogenesis.CONCLUSION Our finding could extend the spectrum and genomic landscape of MET fusions in GC and favor the development of personalized therapy.展开更多
The effect of heat treatment and deep cryogenic treatment on microstructural evolution of low carbon martensitic bearing steel was investigated.The experimental results showed that the lath martensite was obtained by ...The effect of heat treatment and deep cryogenic treatment on microstructural evolution of low carbon martensitic bearing steel was investigated.The experimental results showed that the lath martensite was obtained by quenching and a few twins as substructures formed in some martensitic laths.The rudiment of sub-interfaces of martensitic lath was formed in the highdensity dislocation regions after deep cryogenic treatment;meanwhile,the number of twins increased,especially in the highdensity dislocation regions.This phenomenon is due to the increase in internal stress caused by cryogenic treatment.After tempering,the rudiment of sub-interface further evolved into the martensitic lath boundary,and thus the original martensitic laths were refined.The twins formed by cryogenic treatment did not disappear after tempering.In addition,small quantities of annealing twins formed in tempering process.Martensitic laths morphology and substructures in different stages of the heat and deep cryogenic treatment were observed by tninsmission electron microscopy.展开更多
The microstructure evolution and the pitting corrosion resistance of a supermartensitic stainless steel after deep cryogenic treatment process were clarified through X-ray diffraction,field emission scanning electron ...The microstructure evolution and the pitting corrosion resistance of a supermartensitic stainless steel after deep cryogenic treatment process were clarified through X-ray diffraction,field emission scanning electron microscopy,transmission electron microscopy(TEM)and electrochemical methods.The results showed that the microstructure of supermartensitic stainless steel mainly consisted of reversed austenite,tempered martensite,and M_(23)C_(6)carbides after tempering.The deep cryogenic treatment promoted the refinement of the martensite laths and the precipitation of the carbides in comparison with the traditional process.TEM analysis indicated that the segregation of Si atoms at the boundary was found at the interface between carbide and martensite.The pitting corrosion potential of the specimens subjected to deep cryogenic treatment decreased with the elevated tempering temperature,and the lowest pitting corrosion potential was found at the tempering temperature of 650℃.The sensitivity of the pitting corrosion potential was attributed to the precipitation of M_(23)C_(6)carbides and Si atoms segregation.Si atoms segregation engendered the formation of Cr-depleted zone near M_(23)C_(6)and impeded the recovery of Cr-depleted zone.展开更多
