Sequence Analysis of TNFRSF13b, Encoding TACI, in a Patient with Very Early Onset Inflammatory Bowel Disease: a Case Report

Very early onset inflammatory bowel disease (VEO-IBD), IBD diagnosed before 6 years of age, frequently presents with increased severity, aggressive progression, and often poor response to conventional treatments. Although the cause of IBD is generally considered to be intestinal immune dysfunction induced by polygenic mutations and environment and other factors, VEO-IBD has a stronger genetic susceptibility specifically the neonatal- or infantile-onset IBD. Herein we report compound heterozygous mutations in the tumor necrosis factor receptor superfamily member 13b (TNFRSF13B) gene in a 3-year-old male that was admitted to our hospital with lasted jaundice, repeated fever and diarrhea in May 2014 at 2-month-old. He was diagnosed with VEO-IBD based on clinical, laboratory and histopathological examination. However, he was unresponsive to the conventional therapy, including the nutritional support therapy, antibiotic and immunosuppressive treatment, and surgical release of neonatal intestinal obstruction. Novel compound heterozygous mutations, c.[365G>A];[452C>T](p.[R122Q];[P151L]), were discovered in TNFRSF13B, encoding TACI, for this patient.

Therapeutic Effect Observation of Tiotropium Bromide in the Treatment of Overlap Syndrome

Objective:To study the M receptor blocker on inhalation in patients with overlap syndrome(chronic obstructive pulmonary disease and Obstructive sleep apnea syndrome)curative effect analysis.Methods:25 patients with overlap syndrome as the experimental group,chronic obstructive pulmonary disease patients(30)as control group,patients with overlap syndrome use inhaled tiotropium powder treat 30 days,to observe the changes of pulmonary function,polysomnography,and other indicators after treatment.Results:Overlap syndrome were treated by tiotropium bromide inhalation powder,has improved the pulmonary function,the sleep apnea index and lowest nocturnal oxygen saturation after treatment.Conclusion:tiotropium bromide has a preferable effective in treatment of overlap syndrome,COPD and OSAHS are interacting with each other.

Experience of Nursing and Pipeline Maintenance before and after Implantation ECMO in Patients with Acute Respiratory Distress Syndrome(A Case)

Objective:experience in post-implantation care and pipeline maintenance in patients with acute respiratory distress syndrome.Methods:2020Admitted on 27 July1 Cases of severe pneumonia were transferred to severe ventilator on August 6 and to critical ECMO on August 10.Results:after active treatment,especially for the late stage of ECMO pipeline,the vital signs of the patients improved obviously and the condition gradually stabilized.Conclusion:The nursing care before and after the establishment of ECMO pipeline in patients with acute respiratory distress syndrome,the matters needing attention and the working experience after the establishment,It can provide a reference for the treatment of severe infectious diseases in the future.

Specific Targeting MRI of Chitosan Oligosaccharide Modified Fe_(3)O_(4) Nanoprobe on Macrophage and the Inhibition of Macrophage Foam­ing Induced by ox-LDL

Atherosclerosis(AS)is a primary cause of morbidity and mortality all over the world.Molecular imaging techniques can enable early localization and diagnosis of atherosclerosis plaques.Recent newly developed chitooligosaccharides(CSO)is considered to be capable of target mannose receptors on the surface of macrophages and to inhibit foam cell formation.Here we present a targeting magnetic resonance imaging(MRI)nanoprobe,which was successfully constructed with polyacrylic acid(PAA)modified nanometer iron oxide(Fe_(3)O_(4))as the core,and coating with CSO molecules,possessing the abilities of targeted MRI and specifically inhibition of the formation of foamy macrophages in the atherosclerotic process.The experimental results showed that the distributions of PAA-Fe_(3)O_(4) and CSO-PAA-Fe_(3)O_(4) were uniform and the corresponding sizes were about 5.93 nm and 8.15 nm,respectively.The Fourier transform infrared spectra(FTIR)testified the CSO was coupled with PAA-Fe_(3)O_(4) successfully.After coupled with CSO,the r1 of PAA-Fe_(3)O_(4) was increased from 5.317 mM s-1 to 6.147 mM s-1,indicating their potential as MRI contrast agent.Oil Red O staining and total cholesterols(TC)determination showed that CSO-PAA-Fe_(3)O_(4) could significantly inhibit the foaming process of RAW264.7 cells induced by oxidatively modified low density lipoprotein(ox-LDL).In vitro cellular MRI displayed that,compared with PAA-Fe_(3)O_(4),CSO-PAA-Fe_(3)O_(4) could lower the T1 relaxation time of RAW264.7 cells better.In summary,construction of CSO-PAA-Fe_(3)O_(4) nanoprobe in this study could realize the targeted MRI of macrophages and inhibition of ox-LDL induced macrophage foaming process.This will provide a new avenue in the diagnosis and treatment of AS.

Thoracic Endovascular Aortic Repair for Type B Aortic Dissection

The incidence of aortic dissection(AD)is estimated to be 1 in 100,000 per year.The onset and progression of AD are rapid.Failure to receive appropriate treatment might lead to death in a short time.Even following active treatment,patients might develop low cardiac output syndrome,severe infection,and hemorrhage,which lead to death.Interventional therapy is a surgical method that has been widely used in Stanford type B AD recently.It is characterized by minimal invasiveness,low incidence of postoperative complications,and low cost.This article will review the interventional treatments for AD and will guide the selection of treatment options.

Discussion on Nursing Mode of Ecmo in Treating Severe Patients

Objective:To evaluate the nursing effect of Ecmo treatment for severe patients.Methods:66 patients treated with extracorporeal membrane pulmonary oxygenation were included in the experimental data.From August 2018 to August 2019,the patients were divided into experimental group and reference group by random digital table method,each group was 33 cases.Routine nursing and targeted nursing were performed to compare the complications of the two groups.Results:(1)The correlation index of extracorporeal membrane pulmonary oxygenation treatment before treatment was consistent,P>0.05,the oxygen saturation and oxygen partial pressure of 2 h,4 h after treatment in the experimental group were higher than those in the reference group,compared with the reference group,the carbon dioxide partial pressure of 2 h,4 h after treatment in the experimental group was lower,showing statistical significance of data test(PP>0.05).(2)The incidence of infection,bleeding,coagulation,embolism and hypotension in the experimental group(12.12%)was lower than that in the reference group(45.45%),showing statistical significance(P<0.05).Conclusions:the specific nursing effect of extracorporeal membrane pulmonary oxygenation in severe patients can effectively improve the success rate of treatment,and the possibility of complications during treatment is low.

Anti-inflammatory Effect of Heat-sensitive Moxibustion via the NF-κB Signaling Pathway on Cerebral Ischemia/Reperfusion Injury in Rats

Ischemic stroke is universally acknowledged as a common cause of long-term disability or even death. Suspended moxibustion, an indirect form of moxibustion, is when moxibustion is placed superficially over the skin without being in contact with it. Some researchers have used this method to treat stroke patients, but strong evidence of its therapeutic effectiveness is lacking. However, the effect of traditional suspended moxibustion has recently been improved with the development of heat-sensitive suspended moxibustion. Our previous studies showed that moxibustion for 35 min provided a more effective treatment strategy than moxibustion for 15 min, and moxibustion by 35 min with tail temperature increase had a better outcome than that without, however, the mechanism underlying the effect is not clear. In this study, we treated the stroke rats with moxibustion by 35min and divided them into non-heat sensitive moxibustion(NHSM) group and heat sensitive moxibustion (HSM) group according to difference in the tail temperature increase, then we compared the effect and investigated the mechanisms between NHSM and HSM. We found that HSM significantly decreased tail-flick latency, increased neurological function score, decreased infarct volume, reduced inflammatory cells, decreased the expression of inflammatory factor ICAM-1 and reduced the expression of NF-κB p65 and p-IKKα/β in rats with focal cerebral ischemia/reperfusion injury. Our experimental findings revealed that HSM exerted its anti-inflammatory and neuroprotective effects from MCAO-induced injury by decreasing the expression of the NF-κB signaling pathway.

Nursing Care of Patients With Transcatheter Closure of Atrial Septal Defect via Femoral Vein

Objective:To explore the operative nursing coordination method for the treatment of congenital atrial septal defect(ASD)by transcatheter closure of atrial septal defect via femoral vein.It provides useful experience for the treatment of congenital heart disease.Methods a total of 12 patients undergoing minimally invasive atrial septal defect closure via femoral vein from January 2017 to November 2017 in our department of cardiac surgery were selected as the subjects.All patients received transesophageal ultrasound guided ASD occlusion by femoral vein.The operation and nursing contents include preoperative care,the cooperation of the itinerant nurses,the coordination of the instrument nurses and the postoperative nursing.Results the operation of 12 patients in this group was successful.The diameter of the occluder was 17.1+4.5 mm during the operation.The time of tracheal intubation was 2.4+0.7 h,from the femoral vein to the sheath tube time was 38.7+9.4 min,the occupancy of ICU was 12.5+2.6 h after the operation.The average time of hospitalization was 4.5+1.8 D.There were 2 cases of shunt 1mm immediately after operation.After 24h reexamination,the shunt disappeared,the heart murmur disappeared in the rest of the patients.No residual shunt and other complications occurred.

Therapeutic Effect of Common Goldenrop Decoction Combined With Ramuli Cinnamomi Decoction in the Treatment of Patients with Insomnia during the Period of the Day from 11 PM to 3 AM

Objective:To observe the clinical efficacy of common goldenrop decoction combined with ramuli cinnamomi decoction in the treatment of patients with insomnia during the period of the day from 11 PM to 3 AM.Methods:80 patients with insomnia during the period of the day from 11 PM to 3 AM were randomly divided into control group and intervention group.The control group was treated with 1 mg of estazolam tablets at 9 PM every night;while the intervention group was given common goldenrop decoction combined with ramuli cinnamomi decoction based on the estazolam tablets.After 2 weeks of treatment and after 2 weeks of withdrawal,the improvement in sleep was observed.Results:After 2 weeks of treatment,the efficacy of the intervention group(97.5%)was significantly higher than that of the control group(75.0%).The difference was statistically significant.After 2 weeks of withdrawal,the intervention group still had an effective rate of 87.5%,which was significantly higher than that of the control group(55.0%).Conclusion:common goldenrop decoction combined with ramuli cinnamomi decoction can improve the short-term and long-term sleep quality of patients with insomnia during the period of the day from 11 PM to 3 AM.

Clinical Application of Percutaneous Transluminal Angioplasty and Stent Implantation in Acute Lower Extremity Deep Venous Thrombosis

Purpose: To analyze the application of percutaneous transluminal angioplasty and stenting in acute deep venous thrombosis of lower extremities. Methods: 70 patients were divided into two groups according to the presence or absence of percutaneous transluminal angioplasty and stenting. Results: The mean circumferential diameter difference between the affected limbs and the healthy limbs and the knees at 15 cm was statistically significant. The cure rate and effective rate of the research group were higher than those of the control group (P<0.05). Conclusion: Percutaneous transluminal angioplasty and stenting are of high value in acute lower extremity deep venous thrombosis.

Investigation and Analysis on Constitution and Syndrome Types of High- Normal Blood Pressure People in Hebei Area in the Year of 2017

Purpose: To investigate the distribution of population and syndrome patterns in high-normal blood pressure people in Hebei area. Methods: A total of 453 people who met the inclusion criteria were investigated using a high-normal blood pressure human mass survey questionnaire. Results: of the 453 constitution types of high-normal blood pressure patients in Hebei area, 184 were the constitution of yin-yang harmony, accounting for 40.62% of the total;73 were the constitution of yang asthenia, accounting for 16.11% of the total;59 were the constitution of damp-heat, accounting for 13.02% of the total;52 were the constitution of qi asthenia, accounting for 11.48% of the total;8 were the allergic constitution, accounting for 1.77% of the total. In addition, of their syndrome types, the syndrome of liver-fire hyperactivity were accounting for 30.24%;the syndrome of excessive phlegm-dampness were accounting for 26.71%;the syndrome of yin-deficiency and yang-predominance were accounting for 23.18%;the syndrome of deficiency of both yin and yang were accounting for 19.87%. Conclusion: Among the high-normal blood pressure people in Hebei area, the proportion of the constitution of yin-yang harmony is the highest, followed by the constitution of yang asthenia, the constitution of damp-heat, and the constitution of qi asthenia, with the lowest percentage being the allergic constitution. In addition, when it comes to the syndrome of them, the proportion of the syndrome of liver-fire hyperactivity is the highest, followed by the syndrome of yin-deficiency and yang-predominance, and the lowest is the syndrome of deficiency of both yin and yang.

Effect of Warm Acupuncture on “Dinghui Acupoint” and “Heart Acupoint” in Traditional Mongolian Medicine on Behaviors and Hypothalamic Inflammatory Cytokines in Rats with Chronic Fatigue Syndrome

Objective: To observe the effects of warm acupuncture on “Dinghui Acupoint” and “Heart Acupoint” in Traditional Mongolian Medicine on behavior and hypothalamic inflammatory cytokines IL-1β, IL-6 and IFN-r in Rats with Chronic Fatigue Syndrome. Methods: SD rats were randomly divided into normal group, model group, warm acupuncture group and moxibustion positive control group. The latter three groups of rats were used to establish a model of rats with chronic fatigue syndrome (CFS) using a combination of physical fatigue and mental fatigue. When establishing the model of warm acupuncture group,“Dinghui Acupoint” and “Heart Acupoint” intervention was carried out;when establishing the model of moxibustion positive control group,“Zusanli Acupoint” intervention was carried out on both sides. Behavioral observations (body weight, exhaustive swimming time, tail suspension experiment, water maze) were performed before and after modeling. The hypothalamic inflammatory cytokines IL-1β, IL-6 and IFN-r were detected by ELISA method after warm acupuncture and moxibustion intervention. Results: After 21 days of modeling, the body weight of the rats in each group was significantly lower than that in the normal group, and there was a significant difference (P<0.01);Compared with the model group, the weight of the rats in the warm acupuncture group increased significantly, and there was a significant difference (P<0.01);Compared with the model group, the exhaustive swimming time of the rats in the warm acupuncture group was significantly prolonged, and there was a significant difference (P<0.01);Compared with the moxibustion group, the exhaustion time of the rats in the warm acupuncture group was relatively prolonged, and there was a significant difference (P<0.05);Compared with the normal group, the tail suspension time of the model group was significantly prolonged, and there was a significant difference (P<0.05);Compared with the model group, there was a significant difference in the duration of the suspension of the warm acupuncture group and the moxibustion group (P<0.01);Compared with the normal group, the total distance of the water maze test was shorter in the model group, and there was a significant difference (P<0.01);Compared with the model group, both the warm acupuncture group and the moxibustion group were prolonged, and there was a significant difference (P<0.05);Compared with the moxibustion group, the distance between the rats in the warm acupuncture group was relatively longer, but there was no significant difference (P>0.05);Compared with the normal group, IL-1β, IL-6 and IFN-r increased significantly in the model group and there was a significant difference (P<0.05);Compared with the model group, IL- 1β and IL-6 in the warm acupuncture group was significantly decreased (P<0.05), and the IL-6 in the moxibustion group was significantly different (P<0.05);Compared with the model group, there was no significant difference between the IFN-r group and the moxibustion group (P>0.05);Compared with the moxibustion group, the levels of IL-1β, IL-6 and IFN-r were not significantly different (P>0.05). Conclusion: Warm acupuncture on “Dinghui Acupoint” and “Heart Acupoint” in Traditional Mongolian Medicine has the ability to improve the body’s defense and self-healing ability, improve chronic fatigue syndrome (CFS), and thus play a preventive role. The results of this research indicate that the warm acupuncture group and the moxibustion group have the same effect.
