Clinical Experience Sharing Of 218 Cases without Hemorrhage after Procedure for Prolapse and Hemorrhoids

Objective:To summarize the experience without hemorrhage after PPH of 218 patients with mixed hemorrhoid.Methods:PPH(Procedure for prolapse and hemorrhoids)was performed routinely.During operation,it is paid attention that the hemorrhoid artery should be completely mutilated,the anastomat should be squeezed before and after the anastomosis,the anastomosis should be carefully examined,the hemostasis should be completely performed,and relevant hemostasis measures should be taken after the operation.Results:None of the 218 patients had postoperative hemorrhage.Conclusion:Relevant measures can be taken after operation to prevent the occurrence of postoperative hemorrhage.

Objective Assessment in Continual Medical Education (CME) Medical Objective Assessment in System Control

Existing examinations in postgraduate education and continuing medical education (CME) are not perfect. Modern assessment does not reflect disadvantages of older responders, for whom more time for reply is needed. Specialists with wide clinical experience may choose more than one correct answer in alternative questions. Reduced ability to remember in older people restricts examination without additional sources of information. We offer an individualised system for testing doctors. It provides personalised choice of examination questions using multiple choice questions with weight characteristics and absence of distractors, interactive cooperation in case of negative answers and the final decision of an expert in relation to the person tested. A special algorithm is proposed for typical questions that combines the advantages of known approaches to testing. The questioning system is complex for the creators of tests, but is more convenient and objective than existing ones for medical doctors.

Micro-traumatic Experiences and Psychotherapeutic Treatment

It’s becoming even more widely recognized from neurosciences, epigenetics, and clinical research on observation of infant-caregiver interaction that daily cumulated micro-traumatic experiences cause damages not only to one’s mental health and identity, but also to immune system, leading to metabolic, eating, sleeping, affective, behavioural, cognitive and linguistic, and social disorders in adults as well as in children and infants. Relational Psychoanalytic treatment argues that the therapeutic change is related to expanding levels of consciousness and exploring new ways of being in the world. Clinical examples are provided.

Impact of Connexins on Atherogenesis: A Brief Review

Connexins family in humans consists of 21 highly conserved proteins that are responsible for contact formation between cells. On the cell surface, connexins form hemichannels, or connexons. Two hemichannels brought together form a gap junction, a form of intercellular contact that allows for direct transfer of material and signals between the adjacent cells. Gap junctions serve for transporting ions and other soluble, low molecular weight molecules therefore synchronizing the microenvironment of the contacting cells and maintaining cell and tissue homeostasis. Impairment of gap junctions is associated with different pathological conditions. Importantly, it has been described in atherosclerosis, which causes local cellular dysfunction in the arterial wall tissues followed by the development of atherosclerotic plaque. There are 3 main connexins expressed in human cardiovascular system: Cx37, Cx40, and Cx43. Alterations in the arterial wall cells observed in atherosclerosis include changes in the expression pattern of the main connexins and impairment of intercellular contacts and communication. According to the currently available data, Cx37 and Cx40 have anti-atherogenic and vasculoprotective properties, while Cx43 appears to be more pro-atherogenic. However, the effects of connexins are cell type-dependent and in many cases, remain to be studied in detail. In this review, we summarize the available knowledge on connexins of the arterial wall cells involved in atherosclerosis development.

Effect of Locking Compression Plate on the Treatment of Limb Fracture and the Effect of Operative Success Rate and Postoperative Recovery Time

Objective:To analyze the effect of locking compression plate on the success rate of operation and the time of postoperative recovery.Methods:120 patients with limb fractures from March 2018 to March 2020 were randomly divided into control group(60 cases)and observation group(60 cases).The control group was treated with plate screw internal fixation,The observation group used locking compression plate internal fixation,Compare the effect of treatment,the success rate of operation and the time of postoperative recovery.results:compared the effective rate of the two groups,the observation group(93.33%)was significantly higher than the control group(75.00%).Compared with the two groups,the success rate of operation and the time of postoperative recovery,the observed composition power was higher than that of the control group,and the postoperative recovery time was lower than that of the control group,P<0.05.Conclusion:The use of locking compression plate for the treatment of limb fracture can significantly increase the probability of successful operation,shorten the recovery time after operation,the overall curative effect is ideal,and the clinical popularization value is high.

Urticaria Successfully Treated with Mindfulness and Nutraceutical Supplements

The study is to describe a patient with urticarial lesions which was successfully treated with mindfulness and nutraceuticals.A 20-year-old female patient with an unmarked past medical history presented with diffuse pruritic urticarial lesions associated with breathless and severe fatigue,lasting for 5 months.The lesions covered all body,except palms and plants.Hospitalized for three days,she received dexamethasone,promethazine,and subcutaneous adrenalin and discharged with fexofenadine,prednisone,and ranitidine.Mindfulness and relaxation practice were instructed and some nutraceuticals.After two months,she returned asymptomatic,without any new episodes of urticaria or fatigue.Most of the chronic urticaria patients are treated with antihistamines and/or steroids.This case illustrates an alternative holistic approach,combining mindfulness and relaxation practices with nutraceuticals.This alternative therapeutical strategy should be explored by back to back comparison to the conventional pharmaceutical approach.

Photodynamic Therapy in the Treatment of Cancer: A Review

The search for non-invasive or minimally invasive approaches for the treatment of cancer has led to the development of different therapeutic regimes and one such regime is photodynamic therapy(PDT).PDT is a non-thermal treatment based on the synergy of three elements:the administration of a photosensitizer drug;light at a precise wavelength;and the presence of oxygen.When these three components are combined,they lead to the formation of reactive oxygen species(ROS),resulting in a complex cascade of events and subsequent cell death Studies revealed that PDT can prolong survival in patients with inoperable cancers and significantly improve the quality of life.With a number of recent technological improvements,PDT has the potential to become integrated into the mainstream strategy for cancer treatment.In this review,we have addressed the most important biological and physicochemical aspects of PDT,summarized its clinical status and provided an outlook for its potential future development.We also discussed the factors that hamper the exploration of this effective therapy and what should be changed to render it a more effective and more widely available option for patients.

Understanding of Traditional Chinese Medicine and Western Medicine on Swimming Shoulder

Swimming shoulder refers to shoulder pain syndrome which is caused by repeated impinges on shoulder joints among swimmers.It is one of the most common causes of shoulder pain among swimmers with high incidence.Swimming shoulder can affect the systematization of swimmers’training and the improvement of their training level and performance,which is a difficult problem for many swimmers,coaches,medical staff and sports medical doctors.Therefore,attention should be paid to the treatment of swimming shoulder in daily diagnosis and treatment.

Expression of CDC20, TOP2A in Esophageal Phosphorus Carcinoma and Its Relationship with Survival of Esophageal Carcinoma

Objective:To study the expression of CDC20,TOP2A in esophageal squamous cell carcinoma and its relationship with survival of esophageal carcinoma.Methods:65 patients with esophageal squamous cell carcinoma from January 2016 to June 2018 were selected by computer random selection.All patients were treated with radical operation.The CDC20,TOP2A expression of the patients was examined.At the same time,the relationship between 3-year survival rate and CDC20,TOP2A was analyzed by follow-up investigation.Results:the CDC20,TOP2A expression level of cancer tissue group was higher than that of adjacent tissue group and normal tissue group(P<0.05),and the CDC20,TOP2A expression level of adjacent tissue group was higher than that of normal tissue group(P<0.05).There was no significant difference in sex,age,tumor size,tumor location and CDC20,TOP2A expression level in patients with esophageal squamous cell carcinoma,P>0.05;there were differences between groups(P P>0.05)and positive proportion in TNM stage,lymphatic metastasis,invasion factor and CDC20,TOP2A expression level;there were differences in tumor differentiation,5-year survival rate and CDC20,TOP2A expression level(P P>0.05),and showed inverse proportion relationship.Conclusion:metastasis,recurrence and prognosis of esophageal squamous cell carcinoma are related to the level of CDC20,TOP2A expression.These two indexes can effectively evaluate the pathological situation of esophageal cancer and provide an important reference for the prognosis of esophageal squamous cell carcinoma patients.

The Observation of Blood Biochemistry and HSP72 Expression Changing Rules in Peripheral Blood of Flying-saucer-athletes under High Temperature Training

Objective:To study the effect of training under high temperature on blood and heat shock protein 72(HSP72)expression of flying saucer athletes.Methods:The numbers of training group and control group are 30.Twenty-four flying saucer athletes in Zhejiang province were selected and randomly divided into training group and control group,15 in each group.Peripheral venous blood of each group was taken before and after the experiment.The HSP72 content of lymphocyte,blood routine and biochemical indexes were measured respectively.Results:There was significant difference in white blood cell in training group count before and after the experiment(P<0.05).The expression of HSP72 and the white blood cell in training group was higher than that control group(P<0.05).Conclusions:Organism could be affected and turned to stress state due to training under high temperature.It is necessary to provide the safeguard for the flying saucer athletes under high temperature in order to prevent the damage.

Detection of Puerarin from Pueraria Mirifica Tuber and Its Formulated Cosmetic Products

Pueraria mirifica(PM)has traditionally been used to relieve postmenopausal symptoms.Recently,its extract has been developed into various cosmetic products to promote skin rejuvenation and youthfulness.This study investigated the phytochemicals of PM tuber and compared between the tuber flesh and its outer peel.Puerarin which is one of the major isoflavones and being considered as the marker compound was used to determine the presence of PM extract in local cosmetic products.Puerarin could be ionized by a mass spectrometer at both negative and positive modes.The peak ionized at the negative mode showed to have a narrower peak width(0.2 min)and higher signal-to-noise ratio(30)for pueararin(1 mg/L).The results also found PM extract contained many C-and O-glycosylated isoflavones,especially from its peel extract.This explains the peel extract showed to have four times higher antiradical activity than those of flesh extract.Puerarin from the cosmetic products was recovered via successive methanolic sonication and followed by liquid-liquid extraction using ethyl acetate.Puerarin was successfully partitioned from the highly complex chemical mixture of cosmetic products with the recovery ranged from 89.1%to 115%.Hence,isoflavones was found to be higher at the outer peels than its tuber flesh.A simple and reliable method has been developed to analyse the presence of PM extract in cosmetic products based on the detection of puerarin after successive extraction via methanolic sonication and ethyl acetate partition.

Dance Movement Therapy: A Promising Lifestyle Intervention in the Management of Chronic Obstructive Pulmonary Disease

Chronic Obstructive Pulmonary Disease(COPD),a progressive multicomponent malady with high morbidity and mortality,is an important public health challenge,throughout the world.Despite remarkable progress in its diagnostic and therapeutic modalities,significant number of patients,especially the elderly,continue to suffer from distressing dyspnoea and harrowing extra-pulmonary manifestations limiting their daily activities,with resultant exercise intolerance.Dance Movement Therapy(DMT),a pleasurable and feasible exercise,has been found to be equally efficacious when compared to routinely prescribed physical exercises.Moreover,it has aesthetic expression,attractive to both genders irrespective of age,tempting to those with disabilities,and a fruitful tool in developing self-confidence.During the current stressful situation,caused by COVID-19 pandemic,the COPD patients,notably those over 70 years,are particularly vulnerable to intensification of symptoms and some of them may experience serious disorders of mental illness.Home isolation,social distancing,limiting outdoor activities and prohibiting participation in group exercises,though being appropriate prophylactic measures,are likely to add to already existing physical inactivity and heighten stress and depression,with deleterious effects on overall well-being.Solo dancing,while restricted to home,is a highly accessible,doable,sustainable and well rewarding alternative.

Analysis on the Special Prescriptions from the Book of Chengfang Qieyong

The kinds of special prescriptions in the book of Chengfang Qieyong cover a wide range of subjects such as internal medicine,gynecology,pediatrics and surgery,etc.These prescriptions have gathered the original discussions of doctors of all ages,condensed the author's own in-depth insights,and displayed the profound connotation of traditional Chinese medicine prescriptions or clinical efficacy characteristics.Guided by the function and categories of the original monograph,26 prescriptions were integrated into seven categories,i.e.treating Qi and regulating Blood,nourishing and eliminating,exterior dispersing and dispelling cold,compromising and purging fire,drying dampness and expelling phlegm,fetal birth and baby,and carbuncle.Based on the characteristics of clinical efficacy,the connotation of legislative prescriptions,the modification of prescriptions and medicines,and the degree of decoction and prescription,etc,this paper analyzes the connotation of traditional Chinese medicine of these special prescriptions,and provides important reference for the research of modern prescriptions,the expansion of clinical application and the development of new drugs of traditional Chinese medicine.

Chinese Prescription Kangen-karyu against Metabolic Syndrome:Successful Treatment of Three Patients

Metabolic syndrome is the cluster of diseases,which is manifested by central obesity,impaired glucose tolerance,lipodystrophy,and high blood pressure.These metabolic syndrome-related traits significantly increase the risk of type 2 diabetes,adverse cardiac events,stroke,and hepatic steatosis.In the past decade,several organizations have proposed different diagnostic criteria.The use of traditional Chinese medicine to treat metabolic syndrome has received increasing attention due to its wide availability.In this paper,we report a case of three patients with metabolic syndrome with improved administration of 7.5 g of Kangen-karyu extract per day for 6 months.We present and discuss evidence supporting the possibility of using Kangenkaryu for metabolic syndrome.

Treatment and Nursing of a 90% Bullous Epidermal Necrolysis Drug Eruption Complicated with Diabetes, Hypoproteinemia and Bilateral Pneumonia

Bullous epidermal necrolysis drug eruption is mainly caused by drug allergy, also known as toxic epidermal necrolysis, TEN, first reported by Lyell A in 1956, also known as Lyell syndrome, is the most serious type of drug eruption, the fatality rate is about 25%-50%[1]. The disease is characterized by acute onset, obvious systemic toxic symptoms, and flaccid blisters of varying sizes on the skin of the whole body. At the beginning of the disease, the skin rash is dark red or dark red. It quickly fuses into flakes and develops into the whole body. There are flaccid blisters and epidermolysis in the lesions, which are slightly rubbed or broken, and the tenderness is obvious and accompanied by a large amount of exudation. Analysis of Cognitive Investigation and Intervention of COVID-19 in Different Populations

Objective:To understand the rate of cognition and protection of COVID-19 in a community in Hubei province.Methods:The related knowledge,epidemic situation and self-protective measures of COVID-19 were investigated by mobile phone questionnaire among 836 people in a community in Hubei province.Results:The rate of the awareness of COVID-19’s relevant knowledge and the ways to obtain the epidemic situation varied among different age groups.After multi-channel intervention in the community,the rate of the awareness of COVID-19’s relevant knowledge increased from 75.0%to 96.2%,and the implementation rate of personal protection increased from 59.6%to 92.6%.Conclusion:It is of great significance to improve residents’awareness and protection consciousness and block the spread of COVID-19 in the community through multi-channel intervention.

Correlative Factors Research of Tibial Shaft Fracture with Ipsilateral Posterior Malleolar Crack Fractures

Background:Missed diagnosis rate of spiral tibia shaft fracture with posterior malleolus crack fracture(PMCF)is high in the clinical.However,the mechanism and related factors of fracture are still unclear.Moreover,PMCF has been observed in other types of tibial shaft fractures.Objective:To explore the correlative factors of tibial shaft fracture with ipsilateral PMCF,decrease the rate of clinical missed diagnosis,strengthen the effective fixation of PMCF,and reduce the incidence of traumatic arthritis.Methods:From September 2014 to May 2019,we collected 137 tibiofibular fracture.Only 68 cases involved in ankle joint CT examination and were retrospectively analyzed.The patients were divided into posterior malleolus group(30 cases)and non-posterior malleolus group(38 cases)according to whether come up PMCF or not.The posterior malleolus group contained 24 males and 6 females,27-77(47.57±11.79)years old,the non-posterior malleolus group contained 23 males and 15 females,18-85(48.71±13.84)years old.The gender,age,location,fibula fracture and tibial shaft fracture classification were observed for univariate and multivariate analysis.Results:The probability of PMCF was higher with right tibial shaft fracture(OR=3.6995%CI:1.13-12.08 P<0.05);the probability of PMCF following distal fibular fracture was higher than that without fibular fracture(OR=11.3695%CI:1.72-75.05 P<0.05);the probability of PMCF with type A tibial shaft fracture was higher than type C(OR=4.8295%CI:1.19-19.58 P<0.05).Conclusion:Right type A tibial shaft fracture accompanied by distal fibular fracture are very important factors related to PMCF,which needs highly attention to avoid clinical missed diagnosis.

This article aims to discuss the psychological consequences of the COVID-19 lockdown for patients being treated for psychological disorders and preparing for childbirth.Two clinical examples are used for illustration.
