Patients with Chronic Obstructive Pulmonary Disease(COPD)often experience difficulty breathing,decreased exercise tolerance and respiratory function as the disease progresses.Pulmonary rehabilitation training can impr...Patients with Chronic Obstructive Pulmonary Disease(COPD)often experience difficulty breathing,decreased exercise tolerance and respiratory function as the disease progresses.Pulmonary rehabilitation training can improve respiratory symptoms,increase exercise tolerance and quality of life through exercise stimulation.However,after the end of pulmonary rehabilitation training,the patient's movement behavior is not easy to maintain.Cognitive behavioral therapy(CBT)can effectively enhance cognition,strengthen the motivation of"regular motor behavior",and maintain motor behavior,which can be used as a maintenance strategy for pulmonary rehabilitation training.Objective:To study the effect of cognitive behavioral therapy on the rehabilitation effect and the changing stage of"regular motor behavior"in COPD patients.Methods:30 patients consulted in the Rehabilitation Department of the Third Veterans Hospital of Sichuan Province were interviewed by telephone,and the exercise status and rehabilitation status were recorded and analyzed.From 30 patients,severe and extremely severe(GOLD grade III-IV)in motivation period of"regular exercise behavior"(not starting regular exercise but had exercise thoughts)were randomly divided into two groups.Pulmonary rehabilitation group(CBT-PR group,n=15)conducted one-to-one CBT intervention for 30-40 minutes to discuss the reasons for regular exercise and teach exercise precautions and principles using exercise education form;conventional pulmonary rehabilitation group(conventional PR group,n=15)conducted intensive pulmonary rehabilitation education without consultation and discussion.Both groups underwent 4 weeks of outpatient rehabilitation training,And the regular exercise ability was detected in the first and third months after training(6-Minutes Walking Distance,6-WMD;Short Physical Performance Battery,SPPB),(International Physical Activity Questionnaire,IPAQ),(St.George's Respiratory Scale,St.George's Respiratory Questionnaire,SGRQ),change.展开更多
This study aimed to investigate the impact of artificial intelligence-assisted intensity-modulated radiotherapy(IMRT)plan optimization on the radiation doses received by the rectum and bladder as well as radiation-ind...This study aimed to investigate the impact of artificial intelligence-assisted intensity-modulated radiotherapy(IMRT)plan optimization on the radiation doses received by the rectum and bladder as well as radiation-induced injuries in patients with locally advanced cervical cancer.A total of 100 patients with locally advanced cervical cancer were enrolled and divided into a conventional IMRT group and an artificial intelligence-assisted IMRT group.The results showed that in terms of the radiation doses to the rectum and bladder,all dosimetric parameters(such as mean dose,maximum dose,and volume-dose parameters,etc.)in the artificial intelligence-assisted group were significantly lower than those in the conventional group(p<0.05).Regarding radiation-induced injuries,the incidences and severities of both acute and late radiation-induced proctitis and cystitis in the artificial intelligence-assisted group were lower than those in the conventional group(p<0.05).These findings suggest that artificial intelligence-assisted IMRT plan optimization can effectively reduce the radiation doses to the rectum and bladder and decrease radiation-induced injuries in patients with locally advanced cervical cancer,which is expected to provide a more precise and safer treatment strategy for radiotherapy of locally advanced cervical cancer.However,this study has limitations such as a limited sample size and being a single-center study.Future research with multi-center,large-sample,and more in-depth investigations is needed.展开更多

This study aims to explore the causes of medical staff turnover in county public hospitals from the perspective of developmental psychology and put forward effective countermeasures.Through qualitative research method...This study aims to explore the causes of medical staff turnover in county public hospitals from the perspective of developmental psychology and put forward effective countermeasures.Through qualitative research method,a semi-structured in-depth interview was conducted with 23 medical staff in Liannan County Hospital to analyze the current situation and influencing factors of their loss.The study found that difficult working environment,lack of career development opportunities,poor sociocultural adaptation and inconsistency between personal values and organizational goals were the main reasons leading to the turnover of medical staff.In response to these problems,the research puts forward countermeasures and suggestions to improve the working environment,provide career development platform,strengthen social and cultural support,and promote the integration of individual values with organizational goals.The results of this study provide a new perspective for understanding and solving the problem of medical staff turnover in county public hospitals,and provide a reference for relevant policy formulation and hospital management practice.展开更多
This study aimed to explore the characteristics of serum metabolomics in patients with non-alcoholic fatty liver disease(NAFLD)and its correlation with the severity of the disease based on gas chromatography-mass spec...This study aimed to explore the characteristics of serum metabolomics in patients with non-alcoholic fatty liver disease(NAFLD)and its correlation with the severity of the disease based on gas chromatography-mass spectrometry(GC-MS).A total of 200 NAFLD patients and 100 healthy controls were included.Serum samples were collected and analyzed by GC-MS,and the data were processed using multivariate statistical methods such as principal component analysis(PCA)and partial least squares-discriminant analysis(PLS-DA).The results showed that a total of 50 differential metabolites were identified,including fatty acids such as palmitic acid and oleic acid,amino acids such as glutamine and glycine,and organic acids such as lactate and citrate.The level of palmitic acid was increased while the level of glutamine was decreased in the patient group.Moreover,some of the metabolites were significantly correlated with the severity of the disease.For example,palmitic acid was positively correlated with the severity of NAFLD,while glutamine was negatively correlated.This study indicates that serum metabolomics can effectively distinguish NAFLD patients from healthy individuals,and the characteristic metabolites play a crucial role in the pathogenesis of NAFLD.Furthermore,they have the potential to serve as biomarkers for disease diagnosis and monitoring,providing a basis for a deeper understanding of NAFLD and the development of novel diagnostic and therapeutic strategies.展开更多
This research focuses on the automatic detection and grading of microaneurysms in fundus images of diabetic retinopathy using artificial intelligence deep learning algorithms.By integrating multi-source fundus image d...This research focuses on the automatic detection and grading of microaneurysms in fundus images of diabetic retinopathy using artificial intelligence deep learning algorithms.By integrating multi-source fundus image data and undergoing a rigorous preprocessing workflow,a hybrid deep learning model architecture combining a modified U-Net and a residual neural network was adopted for the study.The experimental results show that the model achieved an accuracy of[X]%in microaneurysm detection,with a recall rate of[Y]%and a precision rate of[Z]%.In terms of grading diabetic retinopathy,the Cohen’s kappa coefficient for agreement with clinical grading was[K],and there were specific sensitivities and specificities for each grade.Compared with traditional methods,this model has significant advantages in processing speed and result consistency.However,it also has limitations such as insufficient data diversity,difficulties for the algorithm in detecting microaneurysms in severely hemorrhagic images,and high computational costs.The results of this research are of great significance for the early screening and clinical diagnosis decision support of diabetic retinopathy.In the future,it is necessary to further optimize the data and algorithms and promote clinical integration and telemedicine applications.展开更多

文摘This study aims to explore career planning and management strategies for healthcare talent based on life cycle theory,in order to enhance retention rates and job satisfaction among healthcare professionals.Through qualitative research methods combined with a case study of Liannan County Hospital,the study analyzes the needs and challenges faced by healthcare talent at different career stages.The findings reveal major issues confronted by healthcare talent during career planning,including compensation and benefits,career development opportunities,work environment,and organizational support.The study proposes a series of targeted management strategies,such as individualized career development paths,continuous professional training,and support for work-life balance,to promote long-term retention and career growth of healthcare talent.The conclusion emphasizes that healthcare organizations should consider the life cycle needs of healthcare talent in career planning and management to achieve optimal talent allocation and sustainable hospital development.
