温中和胃汤联合耳穴压豆对脾胃虚寒型慢性胃炎患者治疗疗效及对血清VIP、MTL水平的影响

目的 温中和胃汤联合耳穴压豆对脾胃虚寒型慢性胃炎(CG)患者的治疗疗效及对血清VIP、MTL水平的影响。方法 选取2021年1月—2022年5月该院接收的脾胃虚寒型CG患者90例,采用随机数字表法分为观察组47例和对照组43例。所有患者均行常规治疗... 目的 温中和胃汤联合耳穴压豆对脾胃虚寒型慢性胃炎(CG)患者的治疗疗效及对血清VIP、MTL水平的影响。方法 选取2021年1月—2022年5月该院接收的脾胃虚寒型CG患者90例,采用随机数字表法分为观察组47例和对照组43例。所有患者均行常规治疗,同时对照组给予耳穴压豆治疗,观察组在对照组基础上联合温中和胃汤治疗。对比两组的治疗疗效、中医证候评分、不良反应发生情况。治疗前后,检测两组患者外周血胃肠功能指标[胃动素(MTL)、血管活性肽(VIP)]以及炎症指标[白细胞介素-1β(IL-1β)以及白细胞介素-6(IL-6)]表达水平。在患者停药3个月后,对其进行为期半年的跟踪随访调查,记录其复发情况。结果 观察组、对照组治疗总有效率分别为93.62%、76.74%,不良反应总共发生率分别为17.02%、9.30%,6个月内复发率分别为0.00%、13.95%。,观察组治疗总有效率较高、6个月复发率较低(P<0.05),两组不良反应发生率无差异(P>0.05)。治疗后中医证候对比,观察组胃隐痛、喜温喜按、劳累/受凉后症状加重和泛吐清水评分低于对照组(P<0.05),两组神疲倦怠、舌淡胖评分无差异(P>0.05)。治疗后,观察组血清MTL高于对照组(P<0.05),VIP、IL-1β低于对照组(P<0.05),两组IL-6浓度无差异(P>0.05)。结论 温中和胃汤联合耳穴压豆可有效提高脾胃虚寒型CG治疗疗效,降低VIP和IL-1β表达,同时提高MTL表达水平,降低短期内复发率。 展开更多

目的探究和营清脉饮联合羟苯磺酸钙治疗非增殖期糖尿病视网膜病变(NPDR)的疗效及对炎症细胞因子的影响。方法选择2020年1月—2021年5月上海中医药大学附属龙华医院眼科收治的73例NPDR患者为研究对象,随机分为中药组(37例)和对照组(36例... 目的探究和营清脉饮联合羟苯磺酸钙治疗非增殖期糖尿病视网膜病变(NPDR)的疗效及对炎症细胞因子的影响。方法选择2020年1月—2021年5月上海中医药大学附属龙华医院眼科收治的73例NPDR患者为研究对象,随机分为中药组(37例)和对照组(36例)。中药组给予和营清脉饮和羟苯磺酸钙治疗,对照组给予羟苯磺酸钙,连续治疗12周。观察治疗前后两组患者的糖尿病视网膜病变严重程度评分(DRSS)、黄斑中心凹厚度、最佳矫正视力、视敏度、血清炎症细胞因子(白细胞介素-6、白细胞介素-8、肿瘤坏死因子-α)及安全指标。结果治疗12周后,中药组有效率为86%,显著高于对照组的38%(P<0.01)。治疗后中药组DRSS评分较治疗前明显降低,且中药组低于对照组(P<0.05)。治疗后中药组黄斑中心凹厚度、最佳矫正视力、视敏度较治疗前明显改善,且中药组优于对照组(P<0.05)。治疗后中药组白细胞介素-6、白细胞介素-8、肿瘤坏死因子-α较治疗前明显降低,且中药组低于对照组(P<0.05)。在治疗过程中未发现明显不良反应。结论和营清脉饮联合羟苯磺酸钙能够改善NPDR患者的眼底症状,缓解黄斑水肿,提高视敏度,有助于促进视力恢复,并且能降低白细胞介素-6、白细胞介素-8、肿瘤坏死因子-α,无不良反应。 展开更多

目的探究疏肝和胃汤联合雷贝拉唑治疗肝胃不和型反流性食管炎的临床疗效及对肠道菌群的影响。方法选取医院2021年1月—2022年12月收治的115例肝胃不和型反流性食管炎患者,随机分为西药组(55例)和疏肝和胃组(60例),西药组予口服雷贝拉唑... 目的探究疏肝和胃汤联合雷贝拉唑治疗肝胃不和型反流性食管炎的临床疗效及对肠道菌群的影响。方法选取医院2021年1月—2022年12月收治的115例肝胃不和型反流性食管炎患者,随机分为西药组(55例)和疏肝和胃组(60例),西药组予口服雷贝拉唑钠肠溶胶囊,疏肝和胃组在西药组基础上加用疏肝和胃汤加减治疗,均治疗8周。分别于治疗前后评价两组患者中医症状积分(反酸、胃胀、胸骨后灼痛、倦怠乏力、大便黏腻),胃镜下观察患者的胃液反流情况(24 h反流次数、最长反流时间、胃液反流分级),采集粪便检测肠道菌群种属和数量,并评价临床疗效,观察治疗期间出现的不良反应情况。结果(1)中医症状评分:治疗后两组较治疗前均降低(P<0.05),疏肝和胃组患者在胃胀、胸骨后灼痛、倦怠乏力、大便黏腻方面较西药组低(P<0.05),差异均有统计学意义。反酸方面两组治疗后差异无统计学意义(P>0.05)。(2)胃液反流情况:治疗后两组24 h食管内胃液反流次数、最长反流时间及胃镜下食管内胃液反流程度分级均较治疗前改善(P<0.05),且疏肝和胃组优于西药组(P<0.05),差异均有统计学意义。(3)临床疗效:疏肝和胃组临床总有效率为95.00%(57/60),高于西药组的78.18%(43/55)(Z=2.542,P=0.011),差异有统计学意义。(4)肠道菌群:治疗后疏肝和胃组乳酸菌、双歧杆菌数量较治疗前升高(P<0.05),肠球菌、肠杆菌数量减少(P<0.05);西药组治疗后乳酸菌、双歧杆菌数量减少(P<0.05),肠杆菌、肠球菌较治疗前差异无统计学意义(P>0.05)。与西药组比较,疏肝和胃组治疗后乳酸菌、双歧杆菌数量升高(P<0.05),肠球菌、肠杆菌数量减少(P<0.05),差异均有统计学意义。(5)不良反应:两组治疗过程中均未发生严重的不良反应事件,主要不良反应为腹胀腹泻、恶心呕吐、大便次数增多、口干、头痛,西药组不良反应总发生率为21.82%(12/55),疏肝和胃组为16.67%(10/60),两组比较差异无统计学意义(χ^(2)=3.379,P=0.066)。结论疏肝和胃汤联合西药治疗肝胃不和型反流性食管炎,能够改善患者反酸、胃胀等症状,抑制胃液反流,调节肠道菌群,效果均优于单纯西药治疗,且安全性较好。 展开更多

目的清金化痰益肺汤合保和丸对支气管肺炎的应用价值以及对肺功能、免疫功能、一氧化氮(Fractional ex-haled nitric oxide,FeNO)、血清嗜酸粒细胞(Eosinophil,EOS)、降钙素原(Procalcitonin,PCT)及C反应蛋白(C-reactive protein,CRP)... 目的清金化痰益肺汤合保和丸对支气管肺炎的应用价值以及对肺功能、免疫功能、一氧化氮(Fractional ex-haled nitric oxide,FeNO)、血清嗜酸粒细胞(Eosinophil,EOS)、降钙素原(Procalcitonin,PCT)及C反应蛋白(C-reactive protein,CRP)水平的影响。方法选取医院收治的支气管肺炎患儿100例,并将患儿随机分为对照组和观察组,每组50例。对照组均给予常规西药对症治疗,观察组在对照组方案的基础上给予清金化痰益肺汤合保和丸,两组疗程均为1周,对比两组临床治疗效果、肺功能、免疫功能、呼出气FeNO、EOS、PCT及CRP水平。结果两组患者治疗后,观察组总有效率为94.00%(47/50),显著高于对照组(76.00%,38/50)(P<0.05);对照组及观察组经治疗后一秒呼气最大容积(Forced expiratory volume in one second,FEV_(1))、用力肺活量(Forced vital capacity,FVC)及FEV_(1)/FVC水平均显著提升(P<0.05),同时观察组改善显著优于对照组(P<0.05);对照组及观察组经治疗后免疫球蛋白G(Immunoglobulin G,IgG)、免疫球蛋白A(Immunoglobulin A,IgA)、免疫球蛋白M(Immunoglobulin A,IgM)水平均较治疗前提升显著(P<0.05),同时观察组改善显著优于对照组(P<0.05);对照组及观察组经治疗后FeNO、EOS、PCT及CRP水平均较治疗前降低显著(P<0.05),同时观察组改善显著优于对照组(P<0.05)。结论清金化痰益肺汤合保和丸能显著提高小儿支气管肺炎的临床治疗效果,改善患儿的肺功能及免疫功能,减轻相关炎性反应,具有临床研究意义。 展开更多

目的 系统评价肝癌经动脉化疗栓塞术(transcatheter arterial chemoembolization, TACE)后影响复发的危险因素,为预防TACE术后复发提供科学依据。方法 检索PubMed、EmBase、Cochrane Library、Web of Science、中国知网、万方数据库、... 目的 系统评价肝癌经动脉化疗栓塞术(transcatheter arterial chemoembolization, TACE)后影响复发的危险因素,为预防TACE术后复发提供科学依据。方法 检索PubMed、EmBase、Cochrane Library、Web of Science、中国知网、万方数据库、维普、中国生物医学文献数据库自建库至2023年2月1日发表的有关肝癌TACE术后复发危险因素的文献报道。由2名研究者根据纳入和排除标准进行文献筛选、数据提取和质量评价。使用软件Rev Man 5.3对纳入文献进行Meta分析和发表偏倚评估。结果 通过筛选最终共纳入9篇高质量队列研究进行Meta分析。分析结果显示差异有统计学意义的危险因素包括甲胎蛋白(OR=3.05,95%CI:1.11~8.38,P=0.03)、白蛋白(OR=5.79,95%CI:2.46~13.62,P<0.0001)、肿瘤分化程度(OR=2.64,95%CI:1.00~6.92,P=0.05)、肿瘤数量(OR=3.76,95%CI:1.71~8.27,P=0.001)、肿瘤ADC值(OR=0.01,95%CI:0.00~0.09,P<0.0001)。结论 肿瘤直径、甲胎蛋白、白蛋白、肿瘤分化程度、肿瘤数量和肿瘤ADC值是TACE术后复发的危险因素。 展开更多

Pre-modern Chinese crafts,such as iron smelting,cookery,medicine,and the production of vehicles,bows,and arrows indicate the traditional Chinese view of technology as being organic,holistic,and comprehensive.This view of technology is guided by the concept of he(和)and employs the means and methods of he,thus achieving the purport of he.In Chinese,the character he(和)possesses positive connotations.It originated from the meaning of"to season;to add flavoring to"(调和)and that of flavors being"perfectly harmonious"(和美).From this sensory experience,he gradually extended to the abstract levels of materiality,humanity,sociality,"order"(wei位),and“power,situation,force"(shi势).Finally,he was elevated to the supreme level of"qi of great harmony"(taihe zhi qi太和之气),which is comparable to the concept of dao(道).The philosophy of he has exerted wide impact on such areas as technology,art,national character,cultural psychology,and behavior patterns,and has become an integral part of China's inherent culture.The paradoxes and deviations of he hold their own profound philosophical implications that merit further exploration.As humanity confronts significant challenges,such as how we can coexist with others,with technology,and with nature,the ancient Eastern wisdom embodied in he continues to possess practical characteristics and value. 展开更多

By measuring the variation of the P-and S-wave velocities of tight sandstone samples under water saturation,it was confirmed that with the decrease in water saturation,the P-wave velocity first decreased and then increased.The variation in velocity was influenced by the sandstone’s porosity.The commonly used Gassmann equation based on fluid substitution theory was studied.Comparing the calculated results with the measured data,it was found that the Gassmann equation agreed well with the measured data at high water saturation,but it could not explain the bending phenomenon of P-wave velocity at low saturation.This indicated that these equations could not accurately describe the relationship between fluid content and rock acoustic velocity.The reasons for this phenomenon were discussed through Taylor’s expansion.The coefficients of the fitting formula were calculated and verified by fitting the measured acoustic velocity changes of the cores.The relationship between P-wave velocity and saturation was discussed,which provides experimental support for calculating saturation using seismic and acoustic logging data. 展开更多

Frothers facilitate the reduction of bubbles size by preventing bubbles coalescence and produce more stable froths.The collision probability of the bubbles and particles substantially increases by decreasing bubble size.For the same volume system,fewer bubbles result from a distribution of large-sized bubbles,and more bubbles result from a distribution of small-sized bubbles.In this research,fundamental two-phase frother characterization parameters were aimed to link with three-phase coal and talc flotation behavior.For this purpose,the effect of single and dual frother systems on inhibiting bubble coalescence was investigated with methyl isobutyl carbinol(MIBC),isooctanol(2 ethyl hexanol),pine oil,and Dowfroth 250.Based on the results of single frothers,isooctanol at the lowest critical coalescence concentration(CCC)value of 6×10^(−6) achieved the smallest bubbles with Sauter mean diameter of 0.80 mm.By blending Dowfroth 250 and pine oil,the bubbles size decreased significantly,reaching 0.45 mm.While the highest recoveries in coal flotation were obtained in single and frother blends where the bubbles size was measured as the smallest in two-phase system,and such a relationship was not found for talc flotation. 展开更多

The influences of canopy temperature measuring on the derived crop water stress index

Crop water stress index(CWSI)is widely used for efficient irrigation management.Precise canopy temperature(T_(c))measurement is necessary to derive a reliable CWSI.The objective of this research was to investigate the influences of atmospheric conditions,settled height,view angle of infrared thermography,and investigating time of temperature measuring on the performance of the CWSI.Three irrigation treatments were used to create different soil water conditions during the 2020-2021 and 2021-2022 winter wheat-growing seasons.The CWSI was calculated using the CWSI-E(an empirical approach)and CWSI-T(a theoretical approach)based on the T_(c).Weather conditions were recorded continuously throughout the experimental period.The results showed that atmospheric conditions influenced the estimation of the CWSI;when the vapor pressure deficit(VPD)was>2000 Pa,the estimated CWSI was related to soil water conditions.The height of the installed infrared thermograph influenced the T_(c)values,and the differences among the T_(c)values measured at height of 3,5,and 10 m was smaller in the afternoon than in the morning.However,the lens of the thermometer facing south recorded a higher T_(c)than those facing east or north,especially at a low height,indicating that the direction of the thermometer had a significant influence on T_(c).There was a large variation in CWSI derived at different times of the day,and the midday measurements(12:00-15:00)were the most reliable for estimating CWSI.Negative linear relationships were found between the transpiration rate and CWSI-E(R^(2)of 0.3646-0.5725)and CWSI-T(R^(2)of 0.5407-0.7213).The relations between fraction of available soil water(FASW)with CWSI-T was higher than that with CWSI-E,indicating CWSI-T was more accurate for predicting crop water status.In addition,The R^(2)between CWSI-T and FASW at 14:00 was higher than that at other times,indicating that 14:00 was the optimal time for using the CWSI for crop water status monitoring.Relative higher yield of winter wheat was obtained with average seasonal values of CWSI-E and CWSI-T around 0.23 and 0.25-0.26,respectively.The CWSI-E values were more easily influenced by meteorological factors and the timing of the measurements,and using the theoretical approach to derive the CWSI was recommended for precise irrigation water management. 展开更多

Vertical distribution characteristics of soil saturated hydraulic conductivity under different land use patterns in the Mu Us sandy land

Background,aim,and scope Soil saturated hydraulic conductivity(K_(s))is a key parameter in the hydrological cycle of soil;however,we have very limited understanding of K_(s)characteristics and the factors that inf luence this key parameter in the Mu Us sandy land(MUSL).Quantifying the impact of changes in land use in the Mu Us sandy land on K_(s)will provide a key foundation for understanding the regional water cycle,but will also provide a scientific basis for the governance of the MUSL.Materials and methods In this study,we determined K_(s)and the basic physical and chemical properties of soil(i.e.,organic matter,bulk density,and soil particle composition)within the first 100 cm layer of four different land use patterns(farmland,tree,shrub,and grassland)in the MUSL.The vertical variation of K_(s)and the factors that influence this key parameter were analyzed and a transfer function for estimating K_(s)was established based on a multiple stepwise regression model.Results The K_(s)of farmland,tree,and shrub increased gradually with soil depth while that of grassland remained unchanged.The K_(s)of the four patterns of land use were moderately variable;mean K_(s)values were ranked as follows:grassland(1.38 mm·min^(-1))<tree(1.76 mm·min^(-1))<farmland(1.82 mm·min^(-1))<shrub(3.30 mm·min^(-1)).The correlation between K_(s)and organic matter,bulk density,and soil particle composition,varied across different land use patterns.A multiple stepwise regression model showed that silt,coarse sand,bulk density,and organic matter,were key predictive factors for the K_(s)of farmland,tree,shrub,and grassland,in the MUSL.Discussion The vertical distribution trend for K_(s)in farmland is known to be predominantly influenced by cultivation,fertilization,and other factors.The general aim is to improve the water-holding capacity of shallow soil on farmland(0-30 cm in depth)to conserve water and nutrients;research has shown that the K_(s)of farmland increases with soil depth.The root growth of tree and shrub in sandy land exerts mechanical force on the soil due to biophysical processes involving rhizospheres,thus leading to a significant change in K_(s).We found that shallow high-density fine roots increased the volume of soil pores and eliminated large pores,thus resulting in a reduction in shallow K_(s).Therefore,the K_(s)of tree and shrub increased with soil depth.Analysis also showed that the K_(s)of grassland did not change significantly and exhibited the lowest mean value when compared to other land use patterns.This finding was predominantly due to the shallow root system of grasslands and because this land use pattern is not subject to human activities such as cultivation and fertilization;consequently,there was no significant change in K_(s)with depth;grassland also had the lowest mean K_(s).We also established a transfer function for K_(s)for different land use patterns in the MUSL.However,the predictive factors for K_(s)in different land use patterns are known to be affected by soil cultivation methods,vegetation restoration modes,the distribution of soil moisture,and other factors,thus resulting in key differences.Therefore,when using the transfer function to predict K_(s)in other areas,it will be necessary to perform parameter calibration and further verification.Conclusions In the MUSL,the K_(s)of farmland,tree,and shrub gradually increased with soil depth;however,the K_(s)of grassland showed no significant variation in terms of vertical distribution.The mean K_(s)values of different land use patterns were ranked as follows:shrub>farmland>tree>grassland;all land use patterns showed moderate levels of variability.The K_(s)for different land use patterns exhibited differing degrees of correlation with soil physical and chemical properties;of these,clay,silt,sand,bulk density,and organic matter,were identified as important variables for predicting K_(s)in farmland,tree,shrub,and grassland,respectively.Recommendations and perspectives In this study,we used a stepwise multiple regression model to establish a transfer function prediction model for K_(s)for different land use patterns;this model possessed high estimation accuracy.The ability to predict K_(s)in the MUSL is very important in terms of the conservation of water and nutrients. 展开更多

Hydrogen peroxide(H_(2)O_(2))has gained widespread attention as a versatile oxidant and a mild disin-fectant.Here,an electrostatic self-assembly method is applied to couple ZnSe quantum dots(QDs)with a flower-like covalent organic framework(COF)to form a step-scheme(S-scheme)photocata-lyst for H_(2)O_(2)production.The as-prepared S-scheme photocatalyst exhibits a broad light absorption range with an edge at 810 nm owing to the synergistic effect between the ZnSe QDs and COF.The S-scheme charge-carrier transfer mechanism is validated by performing Fermi level calculations and in-situ X-ray photoelectron and femtosecond transient absorption spectroscopies.Photolumi-nescence,time-resolved photoluminescence,photocurrent response,electrochemical impedance spectroscopy,and electron paramagnetic resonance results show that the S-scheme heterojunction not only promotes charge carrier separation but also boosts the redox ability,resulting in enhanced photocatalytic performance.Remarkably,a 10%-ZnSe QD/COF has excellent photocatalytic H_(2)O_(2)-production activity,and the optimal S-scheme composite with ethanol as the hole scavenger yields a H_(2)O_(2)-production rate of 1895 mol g^(-1)h-1.This study presents an example of a high-performance organic/inorganic S-scheme photocatalyst for H_(2)O_(2)production. 展开更多

Deep learning model based on PET/CT and combination with Cox proportional hazard model for predicting progression of lung invasive adenocarcinoma after surgery

Objective To observe the efficacy of deep learning(DL)model based on PET/CT and its combination with Cox proportional hazard model for predicting progressive disease(PD)of lung invasive adenocarcinoma within 5 years after surgery.Methods The clinical,PET/CT and 5-year follow-up data of 250 patients with lung invasive adenocarcinoma were retrospectively analyzed.According to PD or not,the patients were divided into the PD group(n=71)and non-PD group(n=179).The basic data and PET/CT findings were compared between groups,among which the quantitative variables being significant different between groups were transformed to categorical variables using receiver operating characteristic(ROC)curve and corresponding cut-off value.Multivariant Cox proportional hazard model was used to select independent predicting factors of PD of lung invasive adenocarcinoma within 5 years after surgery.The patients were divided into training,validation and test sets at the ratio of 6∶2∶2,and PET/CT data in training set and validation set were used to train model and tuning parameters to build the PET/CT DL model,and the combination model was built in serial connection of DL model and the predictive factors.In test set,the efficacy of each model for predicting PD of lung invasive adenocarcinoma within 5 years after surgery was assessed and compared using the area under the curve(AUC).Results Patients'gender and smoking status,as well as the long diameter,SUV max and SUV mean of lesions measured on PET images,the long diameter,short diameter and type of lesions showed on CT were statistically different between groups(all P<0.05).Smoking(HR=1.787[1.053,3.031],P=0.031)and lesion SUV max>4.15(HR=5.249[1.062,25.945],P=0.042)were both predictors of PD of lung invasive adenocarcinoma within 5 years after surgery.In test set,the AUC of PET/CT DL model for predicting PD was 0.847,of the combination model was 0.890,of the latter was higher than of the former(P=0.036).Conclusion DL model based on PET/CT had high efficacy for predicting PD of lung invasive adenocarcinoma within 5 years after surgery.Combining with Cox proportional hazard model could further improve its predicting efficacy. 展开更多

With the rise and development of major types of platforms,the competition for resources has become extremely fierce,and the market share of C2C platforms has been seriously threatened by the loss of resources.Therefore,building and maintaining buyers’satisfaction and loyalty to C2C platforms is critical to the survival and sustainability of C2C platforms in China.However,the current knowledge on how platform satisfaction and loyalty are constructed in the C2C e-commerce environment is incomplete.In this study,seller-based satisfaction and platform-based satisfaction are constructed separately.We further distinguish seller-based transaction satisfaction into economic and social satisfaction and explore their antecedents and consequences.To test our research hypotheses,we conduct a survey and collect data from a real online market(Taobao website).The results show that seller-based transaction satisfaction positively affects platform-based overall satisfaction and loyalty,and that perceived product quality,perceived assurance,and perceived price fairness all have a significant effect on economic satisfaction,whereas perceived relationship quality and perceived empathy significantly influence social satisfaction.These findings help us understand the literature related to customer satisfaction in the context of C2C in China and provide inspiration for online sellers and platforms. 展开更多
