重症急性胰腺炎并发持续炎症-免疫抑制-分解代谢综合征的列线图预测模型构建与验证

目的在重症急性胰腺炎(severe acute pancreatitis,SAP)是否并发持续炎症-免疫抑制-分解代谢综合征(PICS)中探讨中性粒细胞与淋巴细胞计数比值(NLR)联合纤维蛋白降解产物(FDP)与D-二聚体比值等指标的临床预测效能,并构建预测模型。方法... 目的在重症急性胰腺炎(severe acute pancreatitis,SAP)是否并发持续炎症-免疫抑制-分解代谢综合征(PICS)中探讨中性粒细胞与淋巴细胞计数比值(NLR)联合纤维蛋白降解产物(FDP)与D-二聚体比值等指标的临床预测效能,并构建预测模型。方法回顾性收集2018年5月至2023年5月安徽医科大学第一附属医院重症医学科、急诊医学科、消化内科入住的133例SAP患者的临床资料。根据PICS诊断标准将其分为SAP并发PICS组(60例)和SAP非并发PICS组(73例),对两组间NLR、FDP与D-二聚体比值及常见临床指标进行回顾性分析;比较两组患者临床特征、重症监护病房(ICU)病死率及随访1年后生存率。采用Lasso回归及多因素Logistic回归筛选出独立危险因素,构建列线图预测模型。采用受试者工作特征(ROC)曲线、校准曲线对模型进行内部验证;采用临床决策曲线分析(DCA)评估模型的临床实用性。结果与SAP非并发PICS组比较,SAP并发PICS组的改良Marshall评分、NLR、血小板计数与淋巴细胞计数比值(PLR)及FDP与D-二聚体比值均明显升高(P<0.05);SAP并发PICS组合并血流感染及腹腔感染比例明显高于SAP非并发PICS组(P<0.05);通过多因素Logistic回归分析发现,NLR、FDP与D-二聚体比值是SAP并发PICS的独立危险因素(OR分别为0.790、0.131,P均<0.05)。Lasso回归筛选的预测变量得到三因素逻辑回归模型,预测变量包括FDP/D-二聚体、NLR、C反应蛋白(CRP)/前白蛋白(OR分别为1.981、1.048、4.726,P均<0.05)。将上述因素进行模型拟合,经bootstrap内部验证列线图模型曲线下面积(AUC)为0.948(95%CI 0.909~0.980)。校准曲线接近参考曲线,且DCA显示预测模型具有良好临床实用性。结论基于NLR、FDP与D-二聚体比值、CRP/前白蛋白构建SAP患者并发PICS风险列线图预测模型具备良好区分度、校准度和实用性。在SAP早期对是否并发PICS进行评估,若并发PICS,可能提示患者预后不良。 展开更多

脓毒症并发PICS的早期临床特征及相关影响因素分析 被引量:9

【目的】分析脓毒症并发持续炎症-免疫抑制-分解代谢综合征(PICS)的早期临床特征及相关影响因素,为早期防治脓毒症并发PICS提供参考依据。【方法】回顾性收集2013年1月至2019年9月在广东省中医院重症医学科(ICU)住院且住院天数>14 d... 【目的】分析脓毒症并发持续炎症-免疫抑制-分解代谢综合征(PICS)的早期临床特征及相关影响因素,为早期防治脓毒症并发PICS提供参考依据。【方法】回顾性收集2013年1月至2019年9月在广东省中医院重症医学科(ICU)住院且住院天数>14 d的216例脓毒症患者的临床资料,比较脓毒症并发PICS组(108例)与非并发PICS组(108例)的一般情况、急性生理与慢性健康状况评分系统(APACHEⅡ)评分、实验室指标及预后转归等的差异,分析脓毒症并发PICS的早期中医证候特点,采用Logistic回归分析探索脓毒症并发PICS的相关影响因素。【结果】(1)与非PICS组比较,脓毒症并发PICS组患者早期合并急性肾损伤(AKI)比例、APACHEⅡ评分、降钙素原(PCT)和超敏C反应蛋白(hs-CRP)水平均更高,CD4+/CD8+T淋巴细胞比值更低,以及表现为更低的前体蛋白(PP)水平,更高的肌酐(Cr)水平和更低的肾小球滤过率(eGFR),差异均有统计学意义(P<0.05或P<0.01)。(2)PICS组90 d病死率为79.6%(86/108),非PICS组为51.9%(56/108),PICS组90 d病死率明显高于非PICS组,差异有统计学意义(P<0.01)。另外,与非PICS组比较,脓毒症并发PICS组的总住院天数和ICU住院天数均更长,差异均有统计学意义(P<0.05或P<0.01)。(3)PICS组早期虚实夹杂证占66.7%(72/108),实证占29.6%(32/108),虚证占3.7%(4/108);非PICS组早期虚实夹杂证占50.0%(54/108),实证占43.5%(47/108),虚证占6.5%(7/108)。组间证型分布比较,差异有统计学意义(P<0.05)。(4)APACHEⅡ评分、hs-CRP、PP是脓毒症并发PICS的相关影响因素(P<0.05或P<0.01)。进一步绘制ROC曲线,APACHEⅡ评分>26.5分、hs-CRP>85.45 mg/L、PP<101.5 mg/L,对于早期预警脓毒症并发PICS可能具有一定的参考价值。【结论】APACHEⅡ评分、hs-CRP和前体蛋白水平可作为脓毒症并发PICS的早期预警因素;脓毒症并发PICS预后差,扶正补虚或可作为当前的治疗策略之一。 展开更多

目的探索年龄联合辅助性T细胞(CD4^(+))与杀伤性T细胞(CD8^(+))比值对多发伤并发持续炎症-免疫抑制-分解代谢综合征(PICS)的预测价值。方法回顾性收集2017年1月至2020年12月入住联勤保障部队第九二〇医院急诊ICU的163例多发伤患者为研... 目的探索年龄联合辅助性T细胞(CD4^(+))与杀伤性T细胞(CD8^(+))比值对多发伤并发持续炎症-免疫抑制-分解代谢综合征(PICS)的预测价值。方法回顾性收集2017年1月至2020年12月入住联勤保障部队第九二〇医院急诊ICU的163例多发伤患者为研究对象,根据是否并发PICS将其分为多发伤并发PICS组(PICS组)及多发伤未并发PICS组(非PICS组),比较分析两组入院当天相关临床资料差异,筛选差异有统计学意义和可能影响预后的变量进行多因素Logistic分析,探索多发伤并发PICS的早期独立影响因素,并进一步绘制受试者工作特征(ROC)曲线评估其预测价值。结果本研究共纳入163例入住ICU多发伤患者,其中PICS组44例(27%),非PICS组119例(73%)。PICS组年龄[岁,49.5(43.0,60.5)vs.42.0(28.0,52.0)]、急性生理与慢性健康状况评估系统Ⅱ(APACHEⅡ)评分[分,15.00(9.00,18.75)vs.9.00(5.00,12.00)]及序贯器官衰竭评分(SOFA)[分,5.00(3.00,7.75)vs.3.00(2.00,5.00)]高于非PICS组,差异有统计学意义(P<0.05),而两组性别比例、体重指数(BMI)、体温、创伤严重程度(ISS)评分、失血性休克比例、受伤机制及受伤部位差异无统计学意义(P>0.05)。PICS组淋巴细胞计数[×109/L,0.70(0.44,1.00)vs.0.91(0.60,1.36)]、白蛋白(g/L,27.99±6.31 vs.32.60±7.30)、碱剩余[mmol/L,-3.10(-5.38,-0.73)vs.-0.70(-3.70,2.40)]、CD4^(+)(%,32.80±13.47 vs.37.98±10.73)及CD4^(+)/CD8^(+)比值[%,1.11(0.71,1.84)vs.1.46(1.12,2.08)]低于非PICS组,差异有统计学意义(P<0.05);而葡萄糖[mmol/L,8.40(6.75,10.95)vs.7.60(6.50,8.80)]、降钙素原[ng/m L,2.29(0.35,7.77)vs.0.64(0.28,3.65)]、C-反应蛋白[mg/L,67.70(15.25,101.63)vs.32.90(12.60,68.90)]及血乳酸[mmol/L,2.35(1.43,3.90)vs.1.60(1.20,2.90)]高于非PICS组,差异有统计学意义(P<0.05),两组中性粒细胞计数、中性粒细胞/淋巴细胞比值、血小板计数、动脉血氧分压、动脉血二氧化碳分压、血酸碱度、凝血酶原时间、凝血酶时间、活化部分凝血活酶时间、CD3;及CD8^(+)差异无统计学意义(P>0.05)。多因素Logistic回归分析提示,年龄及CD4^(+)/CD8^(+)是多发伤并发PICS的独立影响因素(OR=1.044、0.487,P<0.05);ROC曲线分析提示,年龄、CD4^(+)/CD8^(+)及年龄联合CD4^(+)/CD8^(+)预测多发伤并发PICS均有一定价值(AUC分别为0.689、0.653、0.753,P<0.05),其中年龄联合CD4^(+)/CD8^(+)的预测效果最佳。结论年龄联合CD4^(+)/CD8^(+)是多发伤并发PICS的良好预测指标。 展开更多

Sleep Quality for Patients Receiving Noninvasive Positive Pressure Ventilation and Nasal High-Flow Oxygen Therapy in an ICU: Two Case Studies 被引量:1

Aim: The purpose of this case study was to examine the sleep quality of patients receiving noninvasive positive pressure ventilation (NPPV) or nasal high-flow oxygen therapy (NHF) in an intensive care unit and to investigate what types of nursing support are offered to such patients. Methods: We examined one patient each for NPPV and NHF. Polysomnography (PSG), review of the patient charts, and semi-structured interviews were used to collect the data for analysis. Results: Patients treated with NPPV or NHF demonstrated a noticeable reduction in deep sleep, with most of their sleep being shallow. Their sleep patterns varied greatly from those of healthy individuals. These results suggest that, in addition to experiencing extremely fragmented sleep, sleep in these patients was more likely to be interrupted by nursing interventions, such as during auscultation of breath sounds. Furthermore, it was revealed that “anxiety or discomfort that accompanies the mask or air pressure” in patients treated with NPPV and “discomfort that accompanies the nasal cannula or NHF circuit” in patients treated with NHF may be primary causes of disrupted sleep. Our results suggest a need for nursing care aimed at improving sleep quality in patients treated with NPPV or NHF. 展开更多

Silicon photonic platforms offer relevance to large markets in many applications,such as optical phased arrays,photonic neural networks,programmable photonic integrated circuits,and quantum computation devices.As one of the basic tuning devices,the thermo-optic phase shifter(TOPS)plays an important role in all these applications.A TOPS with the merits of easy fabrication,low power consumption,small thermal time constant,low insertion loss,small footprint,and low crosstalk,is needed to improve the performance and lower the cost of the above applications.To meet these demands,various TOPS have been proposed and experimentally demonstrated on different foundry platforms In this paper,we review the state-of-the-art of TOPS,including metal heater,doped silicon,silicide,with silicon substrate undercut for heat insulation,folded waveguide structure,and multi-pass waveguide structure.We further compare these TOPSs and propose the directions of the future developments on TOPS. 展开更多

Long-Term Consistent Recalibration of VIRR Solar Reflectance Data Record for Fengyun Polar-Orbiting Satellites

The visible infrared radiometer(VIRR)is the first instrument with longest measurements equipped on the Fengyun(FY)polar-orbiting satellites.Through re-processing of the historic VIRR measurements,long-term(over 20 yr)global data can be integrated from multiple participating VIRRs on a consistent radiometric scale,which are valuable to climate and climate change studies.Due to lack of an onboard calibration system for VIRR,the reflective solar bands must be vicariously calibrated.This study applied the multi-site vicarious approach for consistent calibration of the VIRR visible(VIS)and near-infrared(NIR)data,and produced calibration coefficients for five VIRRs on FY-1 C/D and FY-3 A/B/C.The data quality was then evaluated with observations.The reflectance predicted by using the radiative transfer model over multiple invariant desert and ocean targets was used to derive the calibration slope via a weighted fitting scheme,in which the weights are the inverse of the variance from a radiative transfer simulation evaluated with reference to Aqua moderate resolution imaging spectroradiometer(MODIS).The sensor-specific calibration coefficients were derived on a daily basis by using piecewise polynomials.The calibration reference of the VIRR solar band record was further traced to the Aqua MODIS Collection 6.1 reference calibration with a systematic correction derived from the Libya4 desert.The VIRR record was compared with the Aqua MODIS C6.1 calibration over the polar region based on simultaneous nadir overpass observations.The lifetime relative difference for each sensor are within 3.3%and 4.5%for channels 1 and 2.Invariant deserts were also employed to evaluate the stability and consistency of the VIRR record.In general,the means of the directional and spectral corrected reflectance for each sensor are within 1%of the 20-yr average,implying that the VIRR reflectance of the invariant targets is consistent to within 1%among the sensors for channels 1 and 2.The VIRR data thus derived are reliable. 展开更多
